I am pleased to present the LMICS Strategic Plan 2013-2018.

The Strategic Plan 2013-2018 guides LMICS activities, with the Strategic Objectives providing the focus for the Governance Group and three Advisory Groups. The purpose of the Governance and Advisory Groups is strategic and defined, charged with the task of ensuring that LMICS’ activities align with and deliver results that support the Objectives of the Strategic Plan. The LMICS secretariat structure has also been reviewed in order to support the Plan, with the appointment of regionally based CSIFs.

The Strategic Plan 2013-2018 sets a strong emphasis on engaging consumers, including carers, with consumers represented on the Governance and Advisory Groups as well as health service representation from each of the sectors of providers across the LMR.

Over the next five years LMICS will focus on activities that deliver on a whole of region approach, supporting cancer care providers to deliver services that are sustainable and consistently of a high standard.

I look forward to the exciting times ahead as we implement the direction and objectives in the LMICS Strategic Plan 2013-2018.

The LMR covers about 25% of Victoria and, in 2011, had an estimated resident population of 320,000 persons which is 5.7% of the population of Victoria.

LMR has a small percentage of the state’s population spread across a large percentage of the state’s area. This is the key challenge to delivering cancer services across our region.

Incidence is a simple measure of demand for cancer services. It is clear from Figure 1: that the incidence of cancer in the region is increasing. One would expect from this trend, the increasing size of the population, and the ageing of the population, that the incidence of cancer will continue to increase in the next few years. Furthermore, as the incidence of cancer increases, and survival rates increase, so too will the prevalence of cancer increase. This means that there will be an increase in the number of people who are living with cancer and need supportive and survivorship care.

The incidence of cancer is the number of new cases diagnosed in a given region and a given year. The incidence data used in this section is the most recent data provided by VCR.

Table 2: shows the distribution of incidence of cancer in LMR in 2012 by tumour stream. The pattern is not dissimilar from the broader picture in Victoria.

The Governance Group commenced work on the Strategic Plan in early 2013 and signed off on a draft in June 2013.

The draft was shared with key stakeholders and their input was sought and incorporated during late 2013.

The revised Plan was approved in principle with the newly elected Governance Group in October 2013 and was endorsed at the December 2013 Governance Group meeting.

Vision:Excellence in cancer care for our community

Our Vision is for excellent cancer care across the LMR. So no matter where cancer care is delivered it will be the same standard of high level care.

Role: To support development of an integrated network of cancer and support services

Our Role defines what LMICS will do to deliver on the Vision. We will work with health services across the region to provide high quality, connected services, to our community.

Principles:• We consider the needs of consumers, including carers, in all we do

• We work collaboratively with consumers and cancer care providers

• We adopt a region wide approach to improving cancer care

• We take an evidence based approach to our work

• We seek to develop improvements in cancer care that are sustainable

Our Principles outline the way in which we will work, providing a clear basis for decision making and a strong focus on collaboration and engagement with consumers, carers and cancer care providers.

The Strategic Plan identifies five strategic objectives to assist LMICS to deliver the vision of Excellence in cancer care for our community. The strategic objectives provide the framework for LMICS to deliver on its role in assisting the health services in the region, by facilitating and supporting development of cancer care services.

The strategic objectives are:

1. Adopting a region wide approach to cancer care

2. Reducing unwanted variation in cancer care

3. Improving access to cancer care

4. Improving support for cancer survivors

5. Engaging consumers of cancer care services

These strategic objectives, and their related strategies, provide a defined focus for LMICS activities and clearly deliver on the agreed Vision and Role of this Strategic Plan.

1. Adopting a Region Wide Approach to Cancer Care Cancer care has many different pathways, and for those affected by cancer in the LMR, the treatments are often provided by different services at different locations. Our challenge is to take a region wide approach to cancer care provision, for health services across the region to collaborate in order to provide the most effective and efficient service for our community.

LMICS will facilitate communication and collaboration with cancer care providers to make improvements in common need areas across the region.

1.1 Investigate ways to leverage the development of the BRICC to have a positive impact on cancer care delivery across the region

1.2 Encourage and facilitate collaboration and information sharing across LMR cancer care providers

1.3 Investigate workforce planning needs across LMR and develop a region wide approach to address these needs

2. Reducing Unwanted Variation in Cancer Care Variations in cancer care can lead to disparities in survival rates for those diagnosed with cancer, resulting in a reduced length and quality of life for cancer survivors. Our aim is to assist cancer care providers to deliver high standard, standardised cancer care across the region. The areas in which disparities can occur include access to MDT meetings, supportive care, care coordination, and cancer care practice itself.

3. Improving Access to Cancer CareThe LMR has a small percentage of the state’s population (5.7%) spread across a large percentage of the state’s area (25%). This creates a challenge to delivering cancer services. The region has few oncologists, specialised cancer nurses and allied health professionals with expertise in cancer care. In addition, clinicians with this expertise tend to be concentrated in Bendigo.

LMICS will work with cancer care providers to improve access and ensure that cancer care is delivered safely in the most appropriate locations.

3.1 Improve access to acute cancer care for LMR service users, including consideration of telehealth solutions

3.2 Improve access to supportive care for LMR service users

4. Improving Support for Cancer SurvivorsCancer can be considered a chronic disease that requires ongoing monitoring and management. Once cancer survivors cease treatment they still require support that is particular to their cancer.

LMICS will work with cancer care providers and the broader community to enhance the understanding of cancer as a chronic disease and to improve access to services which support cancer survivors.

4.1 Educate sub-acute, primary health and community health service providers on the concept of cancer as a chronic disease

4.2 Investigate ways to meet LMR cancer survivors’ ongoing needs, utilising chronic disease models of care

4.3 Assist LMR cancer care providers to tailor services in order to meet local survivorship needs

5. Engaging Consumers of Cancer Care Services The experience of those affected by cancer should play an important role in shaping the future direction of cancer services. Whilst there are challenges associated with engaging consumers in a region as large as LMR, there is great benefit in having this stakeholder group advising on cancer care service provision in the LMR.

LMICS will ensure consumer representation at all governance levels and promote the consumer experience as a driver for service improvement.

5.1 Appoint consumers as representatives to LMICS Governance, Advisory Groups and steering committees for major projects
