Variability of inspired oxygen concen-tration with nasal cannulas
The paper by Dr EA Bazuaye and others(August 1992;47:609-1 1) on inspired oxygenconcentrations from nasal cannulas("prongs") is welcome. The potentialhazards of nasal oxygen are under-appreciated, as we found when asking juniormedical staff how they use oxygen duringexacerbations of chronic ventilatory failure.'This inquiry followed a particularly spec-tacular example of "prong poisoning," wherenasal oxygen at 1 litre per minute delivered anFio2 conservatively estimated at 40%.'Nearly half the doctors we asked thoughtnasal oxygen was as good as or better than aVenturi mask in these circumstances, andonly one appreciated that nasal cannulasdeliver an FIo2 which can rise as respiratoryfailure worsens and ventilation falls. Our casesupports the suggestion of Dr Bazuaye andcolleagues that nasal cannulas may be morehazardous during acute exacerbations of ven-tilatory failure, and sustains their statementthat nasal cannulas are "unsatisfactory ifprecise control ofinspired oxygen is desired."

Randomised clinical trial of chestdrainage systems
We should like to comment on the article byDr ANJ Graham and others (June1992;47461-2). The study sample consistedof only postoperative cases, though the auth-ors have extended the recommendation of aPortex drain to trauma and emergencieswhere the risk of tube blockage is higher andlow pressure negative suction is invariablyrequired.' In selecting their sample the auth-ors have excluded patients who would requiresuction, but in the discussion they haveanalysed the requirement of suction in twogroups. This would be a major confoundingfactor in the study. The reasons for reductionin "time to sitting" and in morbidity with aPortex drain, although drainage with bottleswas adequate, are not discussed. As thedrainage systems were not "blind" and therewas variable encouragement for mobilisationby the nursing stafl, there is scope for a bias formobilisation ofthe two study groups and thusin the observations. The major factors in theearly mobility of patients and dischargewould be the type of underlying andassociated illness and complications. Con-
sidering the requirement of suction and thepossibility that Portex tubes can fall out, theauthors' recommendations need furtherevaluation.

AUTHORS' REPLY We thank Dr Samiak forshowing interest in our paper. His assertionthat chest drains inserted after thoracictrauma invariably require suction is not, infact, supported by the reference he cites. Inour experience this is necessary only in casesof persistent pneumothorax, usually becauseof a massive air leak.' Pleural suction is notreadily available at the roadside or battlefieldand the advantages that drainage bags haveover underwater seals in these circumstancesare obvious. This is supported by recentmilitary experience in wartime conditions.2
Patients were not entered into the trial if itwas known that postoperative pleural suctionwould be essential. Randomised patients whosubsequently required suction were retained inthe study and the results were analysedaccording to the intended treatment. This isan accepted method for reducing bias in thistype of clinical trial.3There are inherent difficulties in carrying
out a trial comparing such different methodsof treatment. It was not possible to perform ablind study. Nevertheless, we believe that it isimportant to attempt to assess different tech-niques in a randomised fashion rather thanbasing management on uninformed opinionand speculation. The fact that drainage bagswere found to be safe when used after electivesurgery, which is in effect a form of thoracictrauma, should give clinicians the confidenceto use them, when required, in emergencyconditions.

Barcelona's asthma epidemics
With regard to Dr C Picado's "For Discus-sion" paper (March 1992;47:197-200), wepropose that exogenous 1 5-lipoxygenaseenzyme from soybean modulates the severityof the asthma response in soybean inducedasthma, since soybean is the world's richestknown source of lipoxygenase enzyme and iscausal in epidemic asthma with the unusualclinical features described by Dr Picado.
Plant lipoxygenases are common in natureand could readily be aerosolised during thebulk handling of dry soybean. After antigenchallenge a product of the 15-lipoxygenase
pathway of arachidonic acid metabolism-namely, 15(S)-hydroxy-5,8, 1-cis-13-trans-eicosatetraenoic acid (1 5-HETE)-isobserved in large amounts in bronchoalveolarlavage fluid from patients with chronic stableasthma. Eosinophil suspensions incubatedwith ionophore and granulocytes whenstimulated in the presence of 15-HETEproduce lipoxin A4.' A characteristic ofasthma is the presence of eosinophils inbronchial exudates. Lee et al have foundlipoxin A4 in the bronchoalveolar lavage fluidm human pulmonary disease.2 Lipoxin A4elicits constriction of the guinea pig lungstrip, is chemotactic and chemokinetic inresponse to leucocytes, and may furtherregulate arachidonic acid release. Lai et alshowed that preinhalation of 15-HETEincreased the early asthma response in atopicindividuals by 39%, with no late response,after stimulation by antigen.3 It is thereforelikely that the exacerbation of the asthmaticresponse seen after the inhalation of soybeandust is due to the excessive production oflipoxins or related products. We submit thatthe magnitude of that response is due to theexogenous enzyme. The clinical features inthe Barcelona epidemics that support thishypothesis are: sudden onset, severity, andrapid recovery; augmentation of the earlyasthma response and no late phase response;and a lack of clinical reproducibility withstandard methods of antigen challenge.Dr Picado's paper provides the clinical
observations necessary to describe a novelhuman disease mechanism whereby exogen-ous lipoxygenase enzymes may gain access tohuman substrates and modulate patho-physiological responses.
