Differences from self-assessment:

•Themes 3 and 4 scored less than in self-assessment •Themes 5 and 6 scored better than in self-assessment

What we found:

•No major differences between scores ofself-assessment and hospital visits •Good things happening in all 7 areas

•Theme 1: Information needs the most work!
