The Zulekha Healthcare Group found its roots in the year 1961 when our founder Dr. Zulekha Daud arrived in
Sharjah. From a young medical graduate to a practising physician serving patients from all walks of life, she
soon became a household name. After years of dedicated service, the year 1992 saw the establishment of
Zulekha Hospital in Sharjah. It started off as a 30 bed set up with basic facilities for Gynaecology, Obstetrics,
Surgery, Medicine and Paediatrics. Today the Zulekha Healthcare Group has expanded and includes one more
hospital in Dubai, three medical centres, one diagnostic centre and three pharmacies. Today we provide
specialized treatment in over 30 disciplines ranging from Gynaecology to Interventional Cardiology. From
humble beginnings to what we are today, it has been a journey of commitment and dedication. And as we look

ADMISSION You can get admitted to Zulekha Hospital in 2 ways:
Routine Planned Admissions & Emergency Admissions
ROUTINE PLANNED ADMISSIONAt the time of admission you have to ensure the following
DO’S• Please come to the hospital with relative / attendant for
convenience of completing the admission formalities
• Attendants contact details and information should be given
to the Nursing Station• Please ensure that you follow all the instructions given by
your treating doctor• Please bring your prescribed medication list (if any)
• Bring all the necessary documents for delivery (Valid
Passport copies of Husband & Wife and Attested Marriage
Certificate) and previous medical records if any
• Please carry the required deposit amount
• The IP department / Admission cell will be informed of your
admission before hand • Bring Insurance cards and other documents if any
• Your Insurance cards will be retained and returned to you
on dischargeDON’TSIt is advisable not to carry the following to the hospital:
Ornaments and Valuables Eatables and flowers • Do not self administer any medications
• To ensure patient safety it is advisable not to leave the room
unaccompanied and without informing the Nurse incharge.
NOTEAny valuables brought to the hospital would be the
responsibility of the patient. Any valuables if present have to be handed over to the
Nursing personnel.During visits to the hospital, parents should avoid bringing
children below 12 years.The hospital reserves the right to declare a patient as
absconding in the event of patient leaving without permission.
We would like you to register yourself with Zulekha Hospital
when you are coming for the first time. A Patient Identification
Number (PIN) will be issued to you on registration. This number
will be required to be produced by you, every time you visit the
hospital for easy and effective traceability of previous medical
records.
Kindly note:
All patients who are insured or those who have their company
pay for their Medical expenses (corporate patients) are
requested to clarify all details before admission with the
Admission cell / IP department.
ADMISSION RULES
• Admissions are decided by your treating doctor
• Admission to the rooms is subject to the availability of room
/ bed
• Bed charges are inclusive of meals, which are for the
PATIENT ONLY
• Specialist doctor on-call consultation charges are levied for
any emergency call during their off duty hours
• For children below 7 months - no food is provided, but bed
charges are fully levied
• All charges are subject to revision without prior notice

MEALS
Your diet will be prepared, in consultation with the dietician
and your doctor. Therefore, the menu will need to be finalized
by 10.30 a.m. for lunch and 6.30 p.m. for dinner. Outside food
and utensils are not permitted inside the hospital. Relatives
are requested to utilize the cafeteria for all their food
requirements. Any orders for guest meals for your relativs
or friends can be availed on informing the nurse by
10:30 a.m. & 6:30 p.m. for lunch & dinner respectively at
an extra charge. The in-house cafeteria can be contacted
on the Ext. Nos. (ZH-Shj - 265) (ZH Dxb - 734)
CAFETERIA
An in-house cafeteria is available on the ground floor
(ZH Sharjah) which is functional from 7am - 10pm and on
the first floor (ZH Dubai) from 8am -10pm.
BEDDING
A couch is made available for your relative. A pillow, bed sheet
& blanket can be issued on request to the staff nurse.
(Other than sharing / Day care room)
Note: Only one attendant is allowed to stay with the
patient during the night.
PRAYER ROOMS
Dubai - separate prayer rooms are available for ladies and gents
on the first floor.
Sharjah - separate prayer rooms are available for ladies and
gents within the hospital premises.
TELEPHONES
All rooms are provided with telephone facilities for local calls only.
Emergency calls on mobile phones can be made through our
Operator, any such calls will be on extra cost.
INTERNET FACILITY (ZULEKHA HOSPITAL - DUBAI)
Internet facilities are available at prefixed areas, coupon
for which can be availed from pharmacy cash counters.
In an emergency, you can get admitted to our Hospital any time as per Hospital policy. In case you have an insurance cover, an approval for the admission shall be sought immediately. In case the approval for any reason cannot be sought immediately, you would be requested to sign an Emergency Admission form. You can get admitted as a self-paying patient and the amount deposited shall be refunded subject to approval from the Insurance Company.
WHAT HAPPENS AT THE ADMISSION CELL • It is the policy of the Hospital to collect the full payment as advance for any Admission package / procedure• Your relative shall be directed to the Admission Cell / IP Department / Cashier to do the necessary payments before admission • In case there is any refusal of cover from the Insurance Company, you will be advised accordingly and you will have to bear the cost of the treatment
DURING YOUR STAY WITH US Visiting Hours 1. Visiting Hours are 9 a.m. till 11p.m. 2. The ICU / NICU /SBU visiting hours are from 11 am -12 Noon & 6pm - 7pm, (one person at a time) ICU/NICU/SBU Visitor Provisions • Only one relative to be available in the Hospital at all times

MEDICATION
• The hospital runs a 24-hour Pharmacy
• As a rule Medicine from outside are not permitted
• No self administration of medication is allowed as per Hospital policy.
BLOOD BANK
Blood will be charged as per the charges of the MOH/DHA.
USAGE OF MOBILE PHONES
We would appreciate minimum use of mobile phones as it disturbs
others and also interferes with equipments. We request our patients
/ relatives to keep their mobiles in silent mode when they are in the
hospital and also to switch off the mobile before entering the ICU /
NICU / SBU / LR.
In case of need for charging the mobile, charging stations are
available at a designated location in the ward.
CONSENT FORM
At the time of admission you would be asked to sign consent forms as
per the instructions of the treating Doctor. Your treating doctor will
explain to you the details of the procedure. You are advised to clear all
your doubts with him / her and then sign the form. There can be more
than one consent form for a procedure.
This is a statutory requirement and is necessary to be signed
before any procedure.
NO SMOKING POLICY
Zulekha Hospital strictly follows No Smoking Policy as per
the UAE Laws. Kindly co-operate by not smoking during your
stay with us in the Hospital premises.
Kindly note that the following department's personnel would be
meeting you on an everyday basis if required apart from
the doctors and the nurses -
• Laboratory services: For sample collection.
• Physiotherapist: For your physiotherapy requirements.
• Dietician: For your nutritional requirements.
• House keeping supervisor: To check the cleanliness of your room.
• Food Service: To serve you food.
• Customer Care: To ensure your stay with us is made as
comfortable as possible.
• Management representatives: Goodwill visit

• Once the doctor has advised for your discharge, the ward
nurse & IP CRE would require a minimum of 4 hours to
ensure your Discharge Medication, Entries & Billing
formalities are complete.• We would really appreciate if you could fill in the Feedback
Form provided by the ward Customer Care / Nurse and IP
staff and deposit them in the suggestion box before leaving
or hand it over to the IP department. This will help us to
evaluate ourselves better and serve you in a more efficient
way. Your identity will not be disclosed to any hospital staff.
• In case of any complaint/serious issue, you are requested
to bring it to the notice of the Customer Relations Manager/
Hospital Administrators / IP - CRE. • At the time of discharge you will be provided with discharge
summary medications and investigation reports.
• Sick Leave Documents will be handed over as per the doctor’s
instruction and according to MOH / DHA Rules and regulations.
FORMALITIES Post discharge, you may have to process a lot of papers with the
insurance companies or with your own organization. It will be prudent
if you complete all the formalities at the time of discharge. Your ward
Nurse will give you a discharge summary at the time of discharge.
If you should need any additional assistance in this regard, you may
please contact the IP CRE / Nurse in-charge. Please note that history
once recorded cannot be changed.

We generate two types of bills viz. Interim Bills and Final Bill.
Your interim bill will be handed over to you every day and the
final bill will be handed over at the time of discharge. Billing
counseling is given to the patient and relatives at time ofadmissions by the IP Staff / Admission cell. Assistance will
be provided at every stage in case the same is required by
the patients.In case of insurance coverage, any excess charges/uncovered
charges limits will be informed to you for paymentOTHER USEFUL INFORMATION• Customer care officers are available round the clock for all
your queries during and after your stay in the hospital• For the well being of other patients, kindly limit the number
of visitors coming to meet you when you are hospitalized• Ambulance services are available in Zulekha Hospital and
can be utilized on prior approval from the administration
Payments for the same have to be made prior engaging the
ambulance.
