Member Dental Directory
This directory lists the names, addresses and telephone numbers of L.E.H.B. Dental Care
Network Providers. Members may select any dental care provider; however, using an
L.E.H.B. Dental Care Network provider will minimize any out of pocket expenses. If a member
selects a participating L.E.H.B. Dental Care Network Provider, the member will enjoy the
highest level of dental benefits at the lowest possible cost. Members using an L.E.H.B. Dental
Care Network participating provider will have no office visit co-pay and no out of pocket
deductible as long as you do not exceed your maximum annual dental allowance.
For your convenience, this list is grouped by zip code. All L.E.H.B. Dental Care Network
providers for active/retired members are listed. Some providers only provider services to the
active/retired LEHB covered members’ group while some also belong to the retired non LEHB
covered members’ group. Some providers are willing to continue to provide services for their
current patients going who no longer have LEHB coverage. Members who are retired and NO
LONGER have LEHB coverage are strongly urged to ask their current dental care practitioner if
he or she is also an L.E.H.B. Retiree Plan Participating Provider. If your provider does not
participate in the retiree plan, please encourage him or her to join.
