On any given day, 1,200 people walk through our doors to access one of the many services we offer to the community. We strive to work with our many healthcare partners to promote health and deliver excellence in care to more than 500,000 people in our rural and urban communities. The expertise, energy and commitment of 4,000 staff and physiciansenableustorealizethisvision.Lakeridge Health has about 600 beds that encompass nine (9) clinical programswithinanoperatingbudgetof$275million.MostofLakeridgeHealthfundingcomesfromtheOntarioMinistryofHealthandLong-TermCare.

If you have questions or want to discuss any part of your care or treatment options please speak with any member of your health care team.LakeridgeHealthCorporationviewssafetyfromanintegratedapproachcomprising of patient, staff and environmental safety. Safety is viewed as everyone’s responsibility. During your stay you will encounter many things designed to enhance your safety such as ceiling lifts and infection control guidelines.Youareencouragedtofamiliarizeyourselfwithfivepatientsafety tips so that you can be engaged and involved in your healthcare. These tipsarepartofthe“YourHealthCare-BeInvolved”initiativedevelopedby the Ontario Hospital Association’s Patient Safety Support Service with fundingfromtheMinistryofHealthandLong-TermCare.Thetipsare:

1) Be involved in your health care. Speak up if you have questions or concerns about your care.

2) Tell a member of your health care team about your past illnesses and your current health condition.

3) Bring all of your medicines with you when you go to the hospital or to a medical appointment.

4) Tell a member of your health care team if you have ever had an allergic or bad reaction to any medicine or food.

5)Makesureyouknowwhattodowhenyougohomefromthehospitalorfrom your medical appointment.

Medication InformationWhen you come to the hospital please bring an up to date medication list with you. If possible, bring all of your medications with you as well.The hospital has limited access to your prescription information because it is kept at your community pharmacy and we have no access to information about any other medications you may be taking.Medication refers to:

• Prescribed medication from a doctor o e.g. Blood pressure pills, inhalers for asthma, eye drops for

glaucoma and creams, ointments. • Medicationnot prescribed by a doctor

o e.g. ‘Over-the-counter’ medications such as Tylenol, Advil, cough syrup, and laxatives

• Vitaminso e.g.Multi-vitamins,VitaminD,irontablets

• Herbal/naturopath products o e.g. St. John’s Wort, glucosamine, garlic, weight loss pills

MedsCheck Program Managingmultiplemedicationsisnoeasytask.Withallthedifferentinstructions and potential side effects it’s hard to know if the medications you takeonadailybasisareworkingaswellastheyshould.MedsCheckisafreeprovincial government program that allows you to schedule an appointment withyourcommunitypharmacist.AtyourMedsCheckappointmentyourpharmacist will provide you with an accurate medication list that you are encouraged to keep with you and show during each visit to your pharmacy, family doctor, specialist or if you are admitted to the hospital. To be eligible for a yearly MedsCheck you must be:

• AnOntarioresidentand,• Takingatleastthreeprescriptionmedicationsperday• Youmayalsobeeligibleforanadditional“Follow-up”MedsCheckif

you:• Arescheduledforahospitaladmission(e.g.surgery),• Youhaverecentlybeendischargedfromhospitalor,• Therearechangesinyourtherapy

Discharge InformationWaiting for a Room in Long Term CareIf your care at the hospital is completed and you are waiting for a room inaLongTermCare(NursingHome)oraChronicCarefacility,youwillbe charged a co-payment fee while you wait in the hospital. Please ask to speak with a Discharge Planner or Social Worker, who will provide more information to you about these charges.Patients Wanting to Stay at Lakeridge Health Who Are Ready for DischargePatientswhonolongerrequirehospitalizationbutdecidetostayinhospitalor delay the process of discharge will meet with a Discharge Planner, or SocialWorker.IfapatientchoosestoremaininhospitaltheMinistryofHealthandLongTermCareadvisesthathospitalshavetherighttochargepatient’s the full hospital daily rate. The current daily rates for residents of Ontario are:

Lakeridge Health is “Listening”To improve care and access for hard of hearing, deafened and deaf patients, LH in consultationwiththeCanadianHearingSocietyprovidesthefollowingservices:TTYThehospitalnetworkhaspurchasedTTYs(texttelephones)forpatientswhocommunicate with others. These portable devices are available to deaf in-patients who rent telephone service at any one of our hospital sites. TTYdevicesrequiretimeforspecialinstallationaswellassometraining.Installation and training could take up to 2 business days (depending on technicianavailability).TorequesttheinstallationofaTTYdevice(ortoreturn the device), please call extension 2077. Also, LH Oshawa has a public TTYdeviceattheLHOlobbytelephonebankundertheescalators.Volume Control TelephonesVolume control telephones for the hard of hearing are available in all patient rooms. Pocket TalkersPocketTalkersareportablesoundamplifierstoassistwithverbalcommunication. They are available, free of charge, based on availability for patients who have hearing loss.To request a Pocket Talker, please speak with any member of your healthcare team.NotePatientswillberequiredtotakefinancialresponsibilityforthedevicesandsignaloanagreement/form.

Trillium Gift of LifeInJanuary2006,anewlawcameintoeffectcalledRoutineNotificationandRequest. This law requires hospitals in Ontario to report all deaths that occurintheIntensiveCareUnitorEmergencyDepartmenttoTrilliumGiftofLifeNetwork.Whatthismeanstoyouisthathospitalstaffwillaskyouabout organ and tissue donation if your loved one dies. It is everyone’s right to donate organs and tissue if he or she chooses to do so. These organs and tissues are used to save or enhance the lives of others. From the North:Follow Durham Regional Rd. 2 south to Port Perry, watch for and follow the blue H signs, then after entering Port Perry, turn right on Paxton Street.OR Follow Hwy. 12 south, turn left on Durham Rd. 2 (Reach Street), turn right on Old Simcoe Road, then left on Paxton Street.

Patient InformationCell Phones, Walkie-talkies and Other Electronic EquipmentCellphones,walkie-talkiesandotherelectronicequipmentarenotpermittedin the hospital for your safety and the safety of other patients because they may interfere with some hospital equipment.

Commitment to Energy ConservationLakeridge Health has an ongoing commitment towards reducing its energy use. By doing the small things such as turning off lights and televisions, we are able to achieve big savings which can go back into patient care.

Discharge Information - Leaving the HospitalDischarge time is 11:00 a.m. When it comes time to leave the hospital, your doctor will begin the process with a written order for discharge. It is very important that someone comes to the hospital to take you home. Before leaving your care area, be sure to check with your doctor or nurse about any prescriptions or follow-up appointments you may need. When you leave the hospital, please make sure that you have all your belongings with you including any discharge instructions you may have been given.If you are not sure about something such as how to take your medications, or what to watch for when you get home please talk to your nurse or doctor before you leave the hospital. Waiting for a Room in Long Term CareIf your care at the hospital is completed and you are waiting for a room inaLongTermCare(NursingHome)oraChronicCarefacility,youwillbe charged a co-payment fee while you wait in the hospital. Please ask to speak with a Discharge Planner or Social Worker, who will provide more information to you about these charges.Patient AccountsThePatientAccountsofficeisopenMondaytoFridayfrom8:00a.m.–4:00p.m. and is located next to Patient Information Phone in the main lobby. Prior to leaving the hospital please settle any outstanding charges. Billing and Additional ChargesDuring your hospital stay you may incur additional costs that are not covered byOHIPorotherinsuranceyoumayhave.Whenthishappensourfinancedepartment keeps track of these charges and you will receive a bill when you are discharged home. Some things not covered by OHIP include:

Patients going home after hours or on weekends will receive their bill in the mail if any charges are outstanding. Payment can be made with cash, Visa, MasterCard,debit,chequeormoneyorder.Ifyouhaveanyquestionspleasecall Accounts Receivable at 905-985-7321 extension 3203.

FoodOur food services team provides meal selections that take into consideration your special dietary needs ordered by your physician or clinical dietician. Mealsaredeliveredtoyourbedside;pleaseensurethatyourover-bedtableisclear at mealtimes. Patient LoungeA patient lounge is located on your hospital ward, equipped with a television andmagazines.

Quiet RoomA quiet room is available for you and your family when privacy is needed.

Patient Transfers Occasionally, it will become necessary to transfer a patient from one bed to another or from one room to another or from one site to another. There are various reasons why the movement of a patient may be necessary such as a patient may need to be isolated for medical reasons and require accommodation in a private room. There are two types of internal transfers, urgent and non-urgent.Regardless of the reason for a transfer, the need to move one patient may resultinoneormoreotherpatientshavingtoberelocated.Yourhealthcareteamrecognizesthatmovingcanbedisruptiveandmaybeupsettingforyou.Everyeffortwillbemadetominimizetransfers.A. Non-urgent Patient Transfers

A patient may be suitable for transfer from one area to another within a Program.Thehealthcareteamwillnotifythepatient/SubstituteDecisionMakerand/orsignificantotherintheeventthatsuchatransferwillhappen. The patient will go on a waiting list for transfer and information about the receiving area will be provided. When a bed becomes available, thepatient/SubstituteDecisionMakerand/orsignificantotherwillbenotifiedoftheupcomingmove.

B. Urgent Patient TransfersOccasionally, due to unforeseen circumstances, a patient will require immediate transfer from one room to another or from one area to another. Every effort will be made by the healthcare team to accommodate the patient in an appropriate bed and room, in a timely fashion with as little disruption to as few patients as possible.

Reasons for urgent transfers include:• Apatientmaybediagnosedwithaninfectionrequiringisolation.The

patient now requires a private room to avoid placing other patients at risk.

• Apatientinanon-secureareamaysuddenlyrequireasecureenvironment for their safety and protection.

• Apatientisatriskforinjury(i.e.falls,orharmtoself)andneedstobecloser to the team station for monitoring.

• Aprivateroommayberequiredforapatientreceivingend-of-lifecare.*Changeinthetypeofcaremayrequireyoutobemovedtoanothersitewhere the appropriate care can be provided.

Personal Small Appliance ItemsAll personal items with a plug, such as radios, must be checked by Facilities Staff for safety reasons. Please make arrangements with your nurse as required. Personal computers requiring email access are not permitted in the hospital.

Spiritual and Religious CareAtLakeridgeHealthPortPerryweofferSpiritualandReligiousCaretoprovide comfort, reassurance, encouragement, caring and resources for our patients and their families. Spiritual&ReligiousCareServicesinclude:friendlyvisits,spiritualorreligious support, worship, faith resources, prayer and crisis support. OurstaffChaplainandateamofcommunityon-callpastorsprovide24hour– 7 days per week spiritual care and can be contacted at any time by hospital staff in an emergency. If you would like a visit from a community On CallChaplain,pleaseletoneofournursingstaffknowandavisitwillbearranged.

SmokingLakeridge Health Port Perry is a smoke free building. Smoking is only permitted in the designated smoking area, outside to the north of the northwest entrance.

Rentals can be arrangedthroughtheNurses’Station.TheHospitalityNetworkCustomerService Representative will collect payment during their regular coverage hours.Pleaseseechannel81forHospitalityNetworkcoveragedaysandhours.

Visiting HoursVisiting hours are 11:30 a.m. to 7:30 p.m. Special visiting arrangements can be made for patients upon request. Thereisamaximumoftwovisitorsatatimeperpatient,withauthorizedexceptions for palliative care patients, or visitors travelling from a distance.

Your RoomYouwillbeassignedalockerorcupboardinyourroomwhereyoucanstoreitemssuchasclothingandshoes.Locksareprohibitedfromuse.Mostroomsareequippedwithelectricalbedsthatraiseandlowerautomatically.Yourbed will also have side rails for your safety as well as to assist you in moving. Everybedisequippedwithacallbuttonforassistance.Youwillalsobeprovided with a bedside table and an over-bed table.

Your WashroomIf you are sharing a room you will also be sharing a washroom. We ask that visitors use the public rest rooms rather than patient washrooms.
