Welcome to our 12th Annual Cornell Scott Golf Classic!
Our annual golf tournament is named for Cornell Scott, a founding member and longtime board member of the Community Health Network of Connecticut (CHNCT) Foundation. When Hill Health Center - the first community health center in Connecticut - opened in New Haven in 1968, Mr. Scott joined as Director of Training and Education. In 1972, he was appointed Executive Director, a position he held until his passing in 2008, at age 73, after a long illness.
During Mr. Scott’s 40-year tenure, Hill Health Center developed a broad array of services, sites and programs for the New Haven area’s low-income population. Mr. Scott also earned a reputation nationwide for his work with the National Association of Community Health Centers, Inc. He was president of the Association in the 1980s when the U.S. health center program faced extinction in Congress. His work is credited with helping to preserve national funding that led to the growth of community health centers across the nation. To honor Mr. Scott’s legacy, the Hill Health Center was renamed the Cornell Scott-Hill Health Center in 2009.

Over her distinguished 20-year broadcast career, Emmy Award nominated News 8 Medical reporter Jocelyn Maminta has anchored and reported for televisions stations in North Carolina, Milwaukee, Texas and at News 8 in Connecticut.
A political science graduate of the University of Missouri in Columbia, Jocelyn works tirelessly for many nonprofits across Connecticut. She has been honored countless times for her efforts.
She serves on the Board of Friends of Yale-New Haven Children’s Hospital, the Arts Council of Greater New Haven, and is a member of the Junior League of Greater New Haven.
Jocelyn is co-founder of Caroline’s Room, a safe haven for families coping with the challenges and uncertainties surrounding the birth of a premature baby. There are Caroline’s Rooms in Connecticut, Florida, Indiana, Missouri, Ohio, Texas and Virginia. The first Caroline’s Room is at Yale-New-Haven Children’s Hospital and a second is located at Connecticut Children’s Medical Center.
She is also the CEO of Frangi Pangi, fine hosiery for that bare legged look for women of all color. Made in the USA with profits supporting Caroline’s Room.

Ryan Kristafer is the host of ‘CT STYLE’ on WTNH, which airs weekdays at 12:30PM after News 8 at Noon. He also produces a daily "Cruisin' Connecticut" feature segment at the station. Ryan is also an
Executive Producer for Full Send Productions which is currently behind the new ‘Restaurant Road Trip’ show that airs within ‘CT STYLE’ every Friday.

2016 Grant-Making ProgramProceeds from the Cornell Scott Memorial Golf Classic support our Grant Making and Community Donation Program. Every year, CHNCT Foundation assesses the impact of our funds and programs on the overall efforts to improve health services at the Federally Qualified Health Centers (FQHCs). In November, Four FQHCs were awarded $4,000 in grant funding for behavioral health or nutrition education programming. Three additional FQHCs were awarded in-kind Cooking Matters programming valued at $4,000 each. These grants were used to implement programs in 2017.

KHAIR (pronounced “care”) serves at-risk youth by offering one-on-one mentoring relationships and a range of workshops designed to teach important life skills and bolster self-confidence. Participants, who are referred by behavioral health professionals, are asked to commit to six life skill workshops designed to nurture confidence and build healthy relationships.
