10/20/2007 06:OO Well was handed to SLB, safety meeting and rig up started.

10/20/2007 07:30 Operating time started: RIH with logging tools.

10/20/2007 09:30 Tagged bottom with logging tools (12760 FT) and started to log up. Experienced several tight pulls and had to close the caliper about 3-4 times.

10/20/2007 11 :45 Winch Failure at 6480 FT: Changes in the logging speed were noticed by the winchman, after stopping the drum and re-starting it the speed changed again, it slowed down and speed up really fast when winchman tried to adjust. After a couple of minutes winch quit working, operator stopped the joystick and try to re- start the winch without success. SLB crew started trouble shooting the winch checking hydraulic fluid, brakes, chain, winch control panel, etc. Crew contacted REW shop to inform about the failure and request support. Winch started working after 3 to 5 minutes, by that time engineer had already closed the caliper and asked operator to pull out of hole as fast as possible, but logs were not stopped yet. The winch continued working for about 3-4 minutes and then quit again. The joystick was stopped and the whole tiuck was restarted, PTO’s were kicked back in and crew continued trying to move winch from control panel. After about 5 more minutes winch started moving again but this time slower even less than 100 FT/HR. But crew continued moving tool trying to get it into casing. Winch quit again a couple of times and movement was restarted but really slow; the shop mechanics were contacted and winch continued to be trouble shooted with the mechanics on the phone. SLB crew managed to bring the tool up to 6100 FT. After about 10 minutes winch quit again for 17 minutes straight, according with drillers logs. Winchman managed to make drum move but by the time the wiiicli was re-started the cable was keyseated, the tension in the head remained normal but the surface tension was increasing. Engineer pulled up to maximum safe pull =6700 LB without any success and requested authorization from FSM to pull above MSP

FSM authorized to pull up to 7200 LB after, verifying with engineer tlie proper information. Key seat was estimated to be right below casing at 2500 FT according to the MAXIS key seat calculation tool. SLB crew worked the keyseat for about 1 hr. before the client decided to start contacting a fishing company.

10/20/2007 14:OO Client decided to start a fishing operation. SLB crew kept working with to get the keyseat to break without success. Engineer provided the client with a toolsketch that included lengths and OD’s of downhole tools and torpedo inside the head. Toolstring is as follows:

10/21/2007 0O:OO Fishing operation started. Fishing company was SMITH. Schlumberger provided fishing sling and 2-way radios for communication. All other equipment was provided by smith iiicludiiig a 3.375 (as stamped on the tool) grapple. Safety meeting was started using the appropriate HARC. Fishing rig-up was performed as per the fisherman suggestions. The rig up in this rig was different as usual since there was no mouse hole; the lower sheave was hanged on the left hand side of the wellhead and the logger was re-spotted aside the catwalk.

10/21/2003 03:OO Fishing rig-up finished and fishing operation started running pipe in the hole. 25 Stands were nm in the hole before the rig crew changed.

10/21/2007 06:OO Safety meeting was held with the day time crew, both crew worked together for a couple of stands to smooth the crew change. Fishing operation continued, when casing shoe was reached the SLB surface tension showed a change about 40 FT below the shoe, but the tool was not free, so the fishing operation continued until 6100 FT.

10/21/2007 13:20 Fish was encountered at around 6045 FT, engagement was confirmed 3 times, changes in tension were observed in the wireline as well as in the drill pipe weight. While moving the drillpipe down when the fish was being approached, the winchman advised about an increase in the wireline tension. 3 joints of pipe were pulled out and the winchman noticed a decrease in the tension up to 500 LB confirming the latch of the fish. The engagement was confirmed 3 times applying tension with the wireline up to 4500 LB and then keeping the wireline static and pulling the drill pipe joints. After the client, the

fisherman, and SLB crew all agreed that the fish was engaged, the rig crew proceed to break the weak point. Weakpoint was broken using the top drive. Weak point broke at around 6,000+ LB, according with the driller’s gauge.

10/21/2007 13:50 Cable started to be spooled back to the dmm. Cable movement was slowed down around 1000 FT and stopped at 100 FT, the remaining of the cable was spooled by hand. At the end of the cable weak point was found as it was intended.

10/21/2007 14:OO Rig crew started pulling out the pipe, rig crew commented the noticed an ovei-pull at around 4500 FT. While SLB was rigging down the sheaves and tie-down chain the drill pipe was moved up and down a couple of times.

10/21/2007 19:30 SLB crew got ready to unload sources but fish did not show up on surface. Another meeting was held with the client, fisherman, rig crew and Schlumberger. The fishing equipment was disassembled and the grapple was verified; only the bottom 3 threads looked as they had engaged the fish downhole. The grapple was tested in a spare head and even though it was inarked as 3.375 it would not fit the 3.375 tools. Another grapple which was available on location and inarked as 3.5 was tested in the tools and it fit proper1 y . The engineer updated the toolsketch given to the company man with the tension and compression limits of the tools and clarified with the fisherman issues related with the location of the sources in the toolstriiig and the hazards related during the Znd attempt of the fishing operation.

A The following is a sunnnary of NRC and/or Agreement States regulations that must be followed when abandoning an irretrievable well logging source(s). The specific regulations are found in 10 CFR 39.15 and equivalent regulations in Agreement States. An irretrievable well logging soiirce means any licensed radioactive sealed source that becomes lodged in a well and cannot be retrieved after reasonable efforts have been made to recover the source(s).

If a well logging source is irretrievable, the following requirements must be implemented.

a) The source(s) must be innnobilized and sealed in place with a cement plug. The cement has to be dyed red in Texas as a condition of the Texas Railroad Commission (others occasionally).

A mechanical deflection device must be set at some point in the well above the cement plug to prevent inadvertent intrusion on the source, unless the cement plug and sources(s) are not accessible to any subsequent drilling operations. The mechanical device can be devices such as a whipstock, old drill bit, etc. For LWD, drillpipe andor collars left on top of the BHA usually are approved as a deflection device.

A pelinanent identification plaque, (supplied by your QHSE Manager) made of stainless steel (or brass, bronze and inonel), must be mounted at the surface of the well unless the mounting of tlie plaque is not practical (i.e. subsea completion).

The STC RSO (or his designee) will notify the NRC or Agreement State of the abandonment plan developed by/with the client. The NRC or Agreement State must approve the abandonment plan prior to implementation. The federal and/or state oil and gas well permitting agency noiinally will also require that they approve the abandonment plan. The contact with the well peiinitting agency is the responsibility of the well owner/client but we should advise hiidher of that fact as a courtesy.

If any changes must be made to the abandonment plan submitted to the NRC or Agreement State, tlie STC RSO must be infolined so that he/she can get approval of the modification, as appropriate. The actual abandonment must not begin until any abandonment plan or modification to that plan is approved by the appropriate agency.

The STC Radiation Safety Officer must file a written report with the NRC or Agreement State within 30 days after the abandonment. The facility management should file a written report within 10 days to the STC Radiation Safety Officer via tlie Health, Safety and Environment Manager. The facility report should cover the final abandoninent details such as:

a) Actual date of abandonment.

Any changes in the data sent with the approved abandonment plan.

A new well diagram if different from the that submitted with the approved abandonment plan

Any information pertinent to the abandonment that the STC RSO may not have for his/her final report.

If all data sent to the STC RSO is still applicable for the final report, an e-mail or fax is to be sent to the STC RSO confirming that fact so that he/she can be ensured that the data sent to the appropriate agency is totally accurate. Most facilities send a completely new report since inany these documents often are incomplete or are poor quality fax reproductions.

If there are any questions regarding these procedures, discuss them with your Operations Manager.

FACILITY MANAGEMENT MUST ENSURE THAT THE FINAL ABANDONMENT REPORT IS SUBMITTED TO THE STC RADIATION SAFETY OFFICER.
