Exploring the barriers and facilitators to maintaining a healthy weight in
pregnancy and the post-partum period.
Lou Atkinson, Jemma Edmunds, David French, Alison Baxter Applied Research Centre in Health & Lifestyle Interventions
Background
• 32% of women of childbearing age are overweight and 21% are obese
• Maternal overweight/obesity has negative consequences for foetal and maternal health: – Infants 3.5 times more likely to require neonatal intensive care. – Increased risk of still birth and infant death. – Increased risk of maternal death.
Maternal obesity also constitutes a major risk factor for childhood obesity; • Among low-income children, maternal obesity in early pregnancy more than doubles the risk of obesity at 2 to 4 years of age (Whitaker, 2004).
Research Aims
To explore: • The behavioural, environmental, social and
psychological barriers and facilitators to healthy eating (HE) and physical activity (PA) among women of childbearing age and their infants
• The types and feasibility of service which could improve HE and increase PA among pregnant women and post-partum women
This study was commissioned by Nuneaton & Bedworth Borough Council.
Method
5 focus groups: • Pregnant women (2 groups; N = 15 women), • Women in the post-partum period (2 groups; N = 11
women) • Health care practitioners working with these groups
(1 group; N = 8 practitioners).
Data was analysed using thematic analysis.
Results - Themes
Knowledge & Skills - HE & PA were very low priority – most women had poor eating habits and low PA levels pre-pregnancy, and this continued into their pregnancy - Lack of awareness of the importance of HE and PA during pregnancy - Most knew what not to eat, but lacked knowledge about what they should be eating - Lack of basic food preparation, cooking and storage skills
Results - Themes
Barriers & Facilitators Healthy Eating Expense, lack of time, lack of knowledge, lack of skills, lack of service provision, preference for unhealthy foods (including cravings), accessibility of healthy vs. unhealthy foods. Physical Activity Lack of knowledge, lack of/poor advice, pregnancy constraints (e.g., tiredness, poor mobility and low self-esteem), safety concerns, lack of/inadequate service provision &advertising of services, lack of time, expense, and childcare.
Results - Themes
Psychosocial Issues Motivation to eat healthily and take physical activity seems to come from more short-term affective outcomes (i.e. weight, appearance, feeling good, energy, etc), not longer-term concerns about health.
Low levels of knowledge, and therefore, concerns about the adverse impact of a poor diet and physical inactivity on the child.
Training Issues Women are often not weighed at ante-natal appointments and report not being advised on what constitutes a healthy weight gain.
Health professionals working with pregnant & post-partum women are not confident in advising on physical activity.
Results
Preferred Services & Support
Women were very clear about desired service provision: -Group-based activities, in local places, at convenient times, with crèche facilities. -Dietary interventions including cookery courses, community nutritionists, community kitchens, healthy cookbooks/recipes, websites and discount schemes/financial support. - Physical activity interventions including exercise classes or walking groups specific to new mums or pregnant women, swimming activities and free trials at local gyms & leisure centres. - Practical weaning courses to help them cook healthily during the weaning period. - Post-partum weight loss interventions - Services should be more widely advertised and promoted by health professionals.
Key Recommendations
• HE & PA interventions are needed. These should be community based, include practical demonstrations and materials to take away, and should offer childcare facilities. NHS, Children’s Centres and Leisure Services should work together to provide this.
• PA interventions need to be tailored specifically to pregnant women and new mums, to overcome fear and embarrassment.
• Health professionals need training to reinforce the importance of PA during pregnancy, and to be able to confidently advise pregnant women on how they can safely be active.
• Midwives should discuss the importance of maintaining a healthy weight as early as possible in pregnancy, and post-partum weight loss interventions should be offered.
• Much more support should be provided on the weaning process, and on cooking, freezing and re-heating food.
Summary
There is a fundamental deficit in the necessary knowledge, motivation and skills, to enable
women to maintain a healthy weight in pregnancy and the post-partum period.
Future Developments
Recommendations for future research include: • Exploring how to frame messages about the
importance of maintaining a healthy weight before, during and after pregnancy, to increase motivation
• Assessing the impact of weighing vs. non-weighing during antenatal appointments
• Developing brief and complex interventions to increase knowledge and skills
