1. INTRODUCTION 1.1 Aim This Building Contractor Induction Handbook aims to assist contractors and subcontractors operating in this building by providing information on relevant requirements, procedures, rules and regulations which they will need to know in the course of carrying out their work. The term ‘Contractor’ used in this handbook refers to all contractors and subcontractors carrying out works or operations in the building, whether engaged by the Building Management, Tenant or any other party. 2. GENERAL REQUIREMENTS 3.1 Contractor’s Insurance All Contractors must ensure that they are adequately insured for the purposes of their work in the building. The Building Management may require that certificates of currency of insurance policies be provided prior to any work commencement, which may include:

• Workers Compensation Insurance • Public Risk Insurance (minimum of A$10M) • Contractors All-Risk Insurance • Motor Vehicle Liability Insurance (TPC)

3.2 Compliance with Base Building Design & Materials The Building Management reserves the right to require that any or all works and installations carried out by Contractors comply with the design, materials or colours specified by the Building Management. In this regard, the Building Management may require that scaled plans, schematics, material samples or other requirements be submitted for written approval prior to commencement of any works. 3.3 Compliance with Authorities Requirements All work and installations carried out by Contractors must comply fully with all codes, standards and requirements of all authorities having jurisdiction. The Building Management reserves the right to require that Contractors furnish evidence of any or all approvals & compliances as it deems necessary.

4. BUILDING INFORMATION 4.1 General Building Description 4.2 Base Building Consultants and Contractors The list of approved building consultants & contractors for 257 Gregory Terrace is shown in Annex 10. 5.0 HEALTH & SAFETY 5.1 Health & Safety Requirements Contractors must ensure the highest standards are observed for occupational health and safety. To facilitate this, Contractors must observe all Cromwell’s requirements as detailed in the attached document, ‘Contractor’s Health & Safety Requirement’. To assist us in assessing contractors’ capabilities to control occupational health and safety risks, all contractors must complete and return the attached ‘Health, Safety & Environmental Questionnaire’. In addition to the details given in the abovementioned documents, contractors must abide by the general guidelines stated in the following sections. 5.2 Safe Work Method Statements Safe Work Method Statements (SWMS) must be prepared by Contractors and provided to their employees prior to any work being undertaken within the Building. The SWMS must include the following information: (a) Description and step-by-step sequence of the work (b) Potential hazards associated with the work and safety controls that will be in place (c) All precautions to be taken to protect personnel health & safety, including Personal

Protective Equipment to be worn (d) All applicable health & safety legislation, codes & standards and where they will be kept (e) Names & qualifications of the persons who: - Supervise the work - Inspect & approve work areas, methods, protective measures, plant, equipment & tools (f) Training provided, employees trained & persons responsible for training them (g) Plant & equipment to be used and details of inspections & checks on these plant &

equipment 5.3 Material Safety Data Sheets Material Safety Data Sheets (MSDS) for all hazardous substances must be submitted to the Building Management prior to materials being brought on site. All hazardous materials kept in the Building must have their MSDS displayed next to them. 5.4 Personal Protective Equipment Contractors must ensure that all their employees wear the Personal Protective Equipment outlined in their Safe Work Method Statements. 5.5 Safety Signs Contractors and all their employees are to observe safety signs posted on site.

5.6 Drugs and Alcohol Persons under the influence of alcohol are not permitted to work in the building and will be asked to leave the site immediately. 5.7 Public Safety Under no circumstances should members of the public be placed at risk by any work activities undertaken by Contractors or their employees. 5.8 Prohibited Work Practices The following work practices are NOT to be carried out by Contractors or their employees:

• Storing or installing combustible materials on site • Blocking access to building entrances or escape routes • Cutting any holes in existing building components without prior approval • Creating any structural load, permanent or temporary, without prior approval • Mixing of cement in the building’s toilets • Discharging any mortar water via any floor trap, sink or water discharge outlet • Interfering with the progress of business of existing tenants • Working without insurance or DA/CC consent • Commencing work before receiving approved plans from relevant authorities • Carrying out work contrary to approved plans • Working outside DA/CC approved hours • Carrying out cutting & welding works without a Hot Work Permit • Commencing work without submitting the required Safe Work Method Statement

6.0 HOUSE RULES & PROCEDURES 6.1 General All works are to be carried out in a workmanlike manner. All tenancy works are to be carried out within the tenanted premises and under no circumstances are works permitted to be carried out or materials permitted to be stored or placed along common corridors, lift lobbies or staircases. 6.2 Working Hours The Normal Business Hours of the building are from 7:00 am to 6:00 pm Monday to Friday, except Public Holidays. Should Contractors need to carry out works outside of these hours, prior arrangement must be made with the Building Management. Any works which cause noise or disturbance to other Tenants (including hacking, demolition and drilling) are to be carried out outside of the Normal Business Hours. The Building Management reserves the right to stop any works anytime if the works disturb other Tenants in the building or in adjoining buildings. 6.3 Parking All Contractors are to park their vehicles in the carpark or other parking space allocated by the Building Management. Any illegally parked vehicles will be clamped and charges will apply. No vehicle shall be parked along the service road or any other common areas. Carpark charges are applicable unless otherwise approved by the Building Management. 6.4 Access Routes

Access to the works area is restricted to the approved route as directed by the Building Management. Any access routes or other passageways must not be obstructed. 6.5 Loading, Deliveries & Goods Lift Usage The loading or unloading of renovation materials, equipment, debris &/or other bulky items may be carried out only in the loading bay or such locations as directed by the Building Management. No materials shall be left in public areas longer than required to deliver them to the required floors. Deliveries and removals are restricted to the following hours:

Monday to Friday 9.30 am to 12.00 pm (noon)

2.00 pm to 4.00 pm

Saturday and Sunday All Day The moving of equipment, material, debris &/or other bulky items is restricted to the Goods Lift. Such moves and all moves involving dedicated Goods Lift usage require the formal approval of the Management Office at least 24 hours in advance. Such usage will be allocated in 60-minute blocks subject to availability. Deliveries outside of Normal Business Hours (Mon to Fri 7:00 am to 6:00 pm) also requires the formal approval of the Management Office at least 24 hours in advance. See Annex 1 for the form to be used. A cleaning fee of A$200/- per occasion is payable if any Contractor’s workers are found using lifts other than the Goods Lift (or other lift approved by the Building Management). In addition, the Contractor is responsible to make good any damage found in the lift(s) used. 6.6 Protection to Lift Car, Carpets & Common Areas All finishes and installations in common areas & lobbies such as doors, walls, ceiling, architraves, flooring and lifts are to be well protected by the Contractors for all deliveries/moves and throughout the fitout period. Such protection may include heavy plastic sheeting &/or carpet on plywood for floors (including access routes) as well as padded canvas sheet for vertical surfaces, to the satisfaction of the Building Management. All temporary protective linings shall be removed after practical completion. Any damage must be made good by the Contractor to the satisfaction of the Building Management.

6.7 Protection to HVAC services To prevent the entry of dust into supply air ducts, any such ducts must be sealed for the whole duration of the fitout works. 6.8 Hoarding Where required, hoarding shall be erected at locations as directed by the Building Management. The hoarding should cover the span from the existing floor to the ceiling, and all joints must be plastered. All hoarding materials used must be of new sheets of gypsum board and finished in white or off-white emulsion paint, or in other materials and finishes as approved by the Building Management. For retail units, the Building Management may require that the hoarding be finished according to a design or theme as deemed appropriate by the Building Management. 6.9 Cleaning & Rubbish Removal The Contractor is to minimize any dirt, dust or stains affecting any common areas of the building arising from works carried out. The Contractor is to regularly vacuum the dust mat & carpeted areas surrounding the renovation boundaries and keep clean all staircases, corridors and walls at access routes. Any stains, scratches or markings shall be made good to the Building Management’s satisfaction, at the Contractor’s cost. Contractors are to clear rubbish and debris from the site daily and in an orderly manner. There is to be no dumping within the tenanted premises. All rubbish and debris must be disposed of in approved bulk bins &/or dumping sites as directed by the Building Management. A clearance fee of A$500 is payable by the Contractor on each occasion of illegal dumping, as reimbursement for clearance by the Building Management. 6.10 Smoke Free Policy Smoking is strictly prohibited at all times at any part of the Building, including outside the front entrance or on any car park level. 6.11 Fire Indicator Panel (FIP) Isolation & Sprinkler Drainage Policy All base building FIP isolations (point, zone & main FIP) require the Building Management’s approval. All level 5 Central Computer Facility (CCF) FIP isolations (point, zone & main FIP) require the Bureau of Meteorology and the Building Management’s approval. Tenants &/or contractors who require the main FIP to be isolated from the Fire Brigade connection (eg. for any major works which may affect the fire alarm system) for more than 10 minutes are required to submit this request in the form shown in Annex 2. For any main FIP isolation under 10 minutes, fire services contractors must have a technician present at the FIP whilst works are being carried out. Any such isolation of the system should be minimal and confined to office hours. For any isolation exceeding 10 minutes, the Building Management will notify all tenants’ Authorized Representatives in advance. During all isolation works, the Emergency Warning & Intercom System (EWIS) public address system must be in Full Automatic Mode and must NOT be isolated. Upon completion of the works, the fire services contractor must: • Reinstate all isolated points on the FIP • Reinstate the FIP to full automatic mode • Advise the Building Management

Draining of Sprinkler System If draining of the sprinkler system is required, the following procedures must be followed, in addition to those detailed above: 1. For tenancy works, an attendance charge of A$300 is payable by the tenant per drainage

during office hours, or $500 if done after office hours. 2. The time & details must be recorded in the Building Incident Register at the building reception desk. 3. One person is to isolate the ASE & EWIS system and remain in the Fire Control Room, and monitors the FIP for alarms. 4. The 2nd person is to isolate & drain the system on the affected floor. 5. The person in the Fire Control Room isolates the valve monitors & any other items

registering an ‘alarm’, then de-isolates the panel & the EWIS system and returns the system back on line.

The tenant is responsible for any charges arising from accidental tripping of the fire-alarm system during fitout works, including fees charged by emergency services attending the building. In addition, a re-set fee of $500 per incident is also payable by the tenant. 6.12 Permit to Work Prior to commencing work, the Contractor must obtain a Permit to Work issued by the Building Management (See the form in Annex 3). Copies of this permit must be displayed prominently at the work site, to the satisfaction of the Building Management, to inform staff, tenants and visitors of the nature & duration of work and to serve as evidence of the Building Management’s consent. 6.13 Hot Work Permit All hot works involving cutting/welding in the tenanted premises are generally forbidden. Where it is absolutely necessary to carry out such works, the Contractor must first obtain the Building Management’s written permissions (See Annex 4). 6.14 Temporary Electricity Supply Temporary electricity supply, if necessary, may be provided to the Contractor. See Annex 5. The following administrative fees are payable by the Contractor:

• A$10 per day for a 30 amp single phase supply. • A$20 per day for a 60 amp single phase supply. • A$30 per day for a 30 amp 3 phase supply. • A$40 per day for a 60 amp 3 phase supply.

The Contractor must install a temporary distribution board with protective devices, to prevent nuisance tripping. Should the Building Management be required to reset any nuisance tripping, a fee of A$100 per incident will be payable. Electric welding equipment or any power tools exceeding 5A must NOT be connected to this temporary electricity supply. 6.15 Security Passes All Contractors & their staff must at all times wear Security Passes issued by the Building Management while working in the building.

Tenants’ contactors are required to submit a list of their workers (see Annex 6) at least 2 days prior to commencing work on site. Workers whose names are not listed may be barred from entering the building. Workers arriving on-site must sign-in at the Reception Front Desk prior to entering the premises. The Building Management reserves the right to request for any other document(s) as deemed necessary to confirm the eligibility of any person to remain and work in Australia. No illegal worker shall be allowed to work within the building. The Contractor is fully responsible and shall indemnify the Building Management for any illegal worker found on site. 6.16 Worker Management The Contractor is to provide the names & contact details of their supervisor(s) who is authorised to control the workers employed in the building. The Contractor is responsible for ensuring that their workers conduct themselves in an acceptable manner at all times while in the building. Smoking, gambling, littering, spitting or any unruly behavior are strictly prohibited. All workers are to be properly attired at all times within the building. Any workers found breaching the rules and guidelines set out by the Building Management shall be ordered to leave the building and their Security Pass withdrawn immediately. The Building Management reserves the absolute right to refuse to admit any worker whose admission would in the opinion of the Building Management be undesirable or detrimental to the building &/or its tenants. 6.17 Spot Checks The Building Management reserves the right to make spot-checks/inspections during the works carried out. The Building Management also reserves the right to intervene in a manner which it deems necessary to mitigate or rectify any discrepancies or unauthorized work/activities found during the inspection, including: • to immediately stop such works/activities. • to give any immediate instructions to the Contractor to do any rectification/repair work. • to access the tenanted premises to do urgent rectification/repair work deemed necessary. 6.18 Indemnities The Contractor shall indemnify the Building Management unconditionally and irrevocably and agrees to keep the Building Management so indemnified against all actions, suits, proceedings, claims, costs and demands it or any other person(s) now or anytime thereafter may have against the Building Management relating to any act, omission or negligence of the Contractor or its employees in carrying out the works. 7. ENVIRONMENTAL MANAGEMENT All Contractors are required to meet their legal obligations relating to waste minimization and environmental protection. This should include (but not be limited to) the following:

• Stormwater drains are for clean rainwater and are not to be used to discard paint, sand, cement or chemicals

• Paint or other chemicals should not be allowed to soak into the ground • All redundant or unused materials should be recycled in the appropriate bins • Chemical spills should be cleaned-up immediately using the spill kit available from the

Building Management • Noisy equipment should be regularly checked

• Any environmental concerns should be reported to the Building Management The maximum penalty for most pollution offences is now A$250,000 for companies and A$120,000 for individuals. 8. EMERGENCY PROCEDURES 8.1 Incidents & Injuries All incidents and injuries must be reported to the Contractor’s designated supervisor or to the Building Management for recording and/or further action. The report is to be completed as soon as possible prior to the affected persons leaving the building. See Annex 8 for the suggested template. 8.2 Emergency Procedures

• Best location for emergency service to arrive on-site: 8.3 Evacuation Procedures In the event evacuation is required, the Building Management will activate its Emergency Procedures for the building. Contractors & all their workers should be trained in advance on their role should this happen. If evacuation is required, the Emergency Evacuation & Intercom System (EWIS) will sound the Evacuation signal (a fast whoop) for 10-15 seconds. Instructions for evacuation will also be announced. On hearing this, all supervisors & Contractors on site shall: 1. Cease all work immediately, switch off all plant/machinery where possible and leave all

tools behind. 2. Leave the building through the nearest fire exit route and assemble at the Emergency

Evacuation Assembly Point (see Annex 9). 3. No lifts shall be used.

4. If you notice any persons who are not aware of the evacuation, make them aware that they are to evacuate the building.

5. When you have left the building, do not re-enter the building until the “all-clear” has been

given by the Emergency Services. 6. The Contractor’s supervisor (or other responsible staff) should check the assembled workers

for any missing persons. Any personnel not accounted for must be reported to the Building Management, its Warden &/or the Emergency Services immediately.

7. The Building Management &/or Warden will also check-off the assembled workers against the

daily Contractor attendance list. 8. All Contractors must remain at the assembly area until the Building Management, its Warden

or the Emergency Services instructs them that they may return to the building.

Terms and Conditions 1. This form is to be used for all works requiring isolation or impairment of Fire Protection services, eg.

maintenance or work carried out by contractors or draining/charging of tenants’ sprinkler systems for their fitout.

2. The isolation/impairment should be as short a duration as possible and preferably confined to office

hours. The system is to be returned to an operational state prior to the close of the day. 3. The contractor is also responsible for notifying the Fire Brigade before commencing the works. 4. Upon completion, the contractor is responsible for restoring the system to full operational function,

including Diesel &/or Electric pumps, Valves and the Fire Indicator Panel. 5. The contractor also agrees to abide by the Building Management’s House Rules and all other

Terms and Conditions 1. This permit is required for works to be carried in the building by contractors and subcontractors,

including tenancy fitout works. Copies of this permit must be displayed prominently at the work site to inform staff, tenants and visitors of the nature & duration of work and to serve as evidence of the Building Management’s consent.

2. Tenants & their contractors are given a copy of the Tenancy Fitout Handbook, and the guidelines,

procedures, terms and conditions and other contents therein are to be complied with by Tenants and their contractors.

3. For general works in the building other than tenancy fitout works, a copy of the Building Contractor

Induction Handbook are given to contractors & subcontractors. The guidelines, procedures, terms and conditions and other contents therein are to be complied with by all contractors and subcontractors.

4. For tenancy fitout works, a Fitout Deposit is payable prior to commencing work. The amount payable

is A$1 per sq ft of NLA, with a minimum of A$5,000 per tenant. This Fitout Deposit will be refunded without interest after the tenant completes the fitout works, subject to the Tenant & their contractors complying with all requirements as stated in the Tenancy Fitout Handbook.

5. All contractors/subcontractors must have adequate insurance cover and must ensure that all their

workers comply with OH&S requirements and safe work practices. 6. All contractors and subcontractors must comply with the House Rules as detailed in the Tenancy

Fitout Handbook / Building Contractor Induction Handbook, including rules on security, cleaning, moving/deliveries, rubbish and worker management.

7. Notwithstanding any of the above, the Building Management reserves the right to amend all or any of

its procedures, rules and regulations pertaining to tenancy fitouts and contractors’ works as may be required from time to time.

8. The Building Management reserves the right to withdraw this Permit to Work should the contractor or

subcontractor fail to comply with its requirements or instructions.

to carry out fitout or other works in the Building. 2. The following administrative fees are payable by the contractor:

• A$10 per day for a 30 amp single phase supply. • A$20 per day for a 60 amp single phase supply. • A$30 per day for a 30 amp 3 phase supply. • A$40 per day for a 60 amp 3 phase supply.

3. The contractor must install a temporary distribution board with protective devices, to prevent

nuisance tripping. Should the Building Management be required to reset any nuisance tripping, a fee of A$100 per incident will be payable. Electric welding equipment or any power tools exceeding 5A must NOT be connected to this temporary electricity supply.

4. The contractor also agrees to abide by the Building Management’s House Rules and all other

requirements detailed in the Tenancy Fitout Handbook (for tenancy fitout works) or the Building Contractor Induction Handbook (for all other works). The Contractor shall obtain this from the Building Management and by signing this application, is deemed to have read & understood their contents.
