Nuss Team WinTim Melville was 11 years old when he noticed he was having a hard time catching his breath, especially while competing in sports. His sternum was noticeably sunken and pushing against his heart and lungs, making it difficult to breathe at times. When Tim’s mother, Valerie, read an article in Kidstuff magazine about pectus excavatum, a condition also called sunken chest, she reached out to Dr. Donald Nuss at CHKD asking if he would evaluate her son for the condition. Sure enough, Tim was diagnosed with pectus excavatum and soon thereafter scheduled for the minimally invasive surgery to correct his concave chest wall. Dr. Nuss, creator of the world-renowned Nuss Procedure, performed the operation in 2001 with Dr. Robert Kelly, CHKD’s current vice president for surgical affairs and surgeon-in-chief. A single curved bar was inserted and attached beneath Tim’s ribcage to return his chest to a normal shape. After a little more than a year, the bar was removed. After the procedure, Tim noticed it was easier to breath, and he improved his performance on the baseball field. The Melvilles moved from Chesapeake to the St. Louis suburbs when Tim started high school, and he was drafted to play the sport professionally after graduation. This spring, the 6-foot 4-inch, 225-pound former Nuss patient made his major league debut pitching for the Cincinnati Reds. He is a shining example of how CHKD can help children reach their full potential and achieve their dreams. “I doubt that I’d be where I am today, doing what I do, if I hadn’t had the surgery. It changed my life for the better,” Tim says. “I’m so thankful I had the procedure done at such a young age. It allowed me to excel in the things I wanted to do in life.”

Former patient Tim Melville, who made his major league debut this spring for the Cincinnati Reds, says CHKD had a role in his success.

ernando Merino-Lopez is an 11-year-old from Virginia Beach whose charismatic smile reveals a deep dimple. He’s anxious to show off his new haircut – a cropped, closely shaved style that he just recently had the courage to get, thanks to surgeons at CHKD.

For the first 10 years of his life, Fernando kept his hair long to hide the fact that he did not have an ear on the right side. Fernando was born with microtia and atresia, a condition in which the outer ear, ear canal and eardrum are underdeveloped or absent. It occurs to some degree in approximately one in 6,000 children. Of the three levels of the condition, Fernando had grade 3 microtia – the most severe form. His outer ear consisted of a small bud of cartilage and the beginnings of an earlobe. He was completely missing his right ear canal and could not hear on his right side. In addition to the challenges of limited hearing, Fernando was teased about his missing ear. “Fernando grew his hair longer when he started to realize his ear was deformed,” Maria Merino-Lopez says. “I would tell him, ‘Fernando, don’t be bashful. Just tell them that’s how you were born.’ It was difficult.”  Five years ago, Fernando’s family lived in Washington state, and doctors there suggested a standard surgical correction to the boy’s condition. They could remove a piece of Fernando’s rib cartilage and use it as a framework to build an outer ear, but his family was reluctant to put their son through the invasive procedure. Soon after, the family moved to Virginia Beach, and Fernando’s pediatrician, Dr. Kate Crandley of Tidewater Children’s Associates, a CHKD Medical Group practice, recommended Fernando visit CHKD’s craniofacial center for a consultation. Dr. George Hoerr, a CHKD pediatric plastic surgeon, evaluated Fernando in the spring of 2014. “When I first met Fernando, I knew we could give him a good-looking ear,” says Dr. Hoerr, who proposed an advanced reconstructive ear surgery that would be much less invasive than the standard procedure. Instead of rib cartilage, Dr. Hoerr would use a synthetic implant to provide a shape for the ear – a specialized technique he has used for ear reconstruction on the East Coast for more than two decades.

Page 10: Kidstuff Spring 2016

8 Children’s Hospital of The King’s Daughters

“This approach has several advantages for young patients,” says Dr. Hoerr. “Children can undergo surgery as early as 3 or 4 years of age. Without the need for rib cartilage, recovery is much easier and can be completed with fewer visits to the surgeon. The synthetic implant helps makes it easier to shape an ear that is more natural in appearance. But Dr. Hoerr was eager to give Fernando even more than a cosmetic solution. “I had great hope we could not only build Fernando a natural-looking outer ear but also give him the ability to hear in his right ear,” Dr. Hoerr says. He called on Dr. Stephanie Moody Antonio, a pediatric otologist, to evaluate Fernando for surgery to create the ear canal and eardrum necessary for hearing. Hearing tests and a CT scan were promising. There were a few important anatomical parts needed in order for Fernando to be a good candidate for ear canal reconstruction – including a cochlea and hearing nerve. “On a 10-point scale used to judge the potential for successful reconstruction, Fernando had a score of 8,” says Dr. Moody. Rebuilding Fernando’s ear would be a two-step process. First, Dr. Moody would construct an ear canal opening

and an eardrum and adjust the middle ear bones to allow Fernando to hear. Next, Dr. Hoerr would follow with reconstruction of the outer ear. “Traditionally, the outer ear would be reconstructed first and the ear canal second. In Fernando’s case, however, I had the advantage of putting the ear canal exactly where it belonged based on the underlying anatomy instead of having to adjust placement of the ear canal to the outer ear reconstruction,” says Dr. Moody. “This approach  also gave  Dr. Hoerr a blueprint of exactly where to construct the outer ear in relationship to the ear canal, maximizing both appearance and function.” That summer, Fernando had his first surgery to open the ear canal and build his eardrum. Dr. Moody drilled through the bone just behind the inner ear to create an ear canal and designed an eardrum from a small piece of tissue from Fernando’s scalp. Then, she used a skin graft to line the ear canal. The procedure was a success. “He was able to hear out of his right ear for the first time,” says Dr. Moody. Because he still has some hearing loss, future procedures are planned to reconstruct the hearing bones and continue to improve Fernando’s hearing ability.

A synthetic implant pictured here was used to shape Fernando’s new ear.

Fernando had the beginnings of an outer ear, but was born without an ear canal and was deaf in his right ear.

After the inner ear had healed for a year, Fernando underwent surgery to reconstruct his outer ear this past fall. Dr. Hoerr carved the synthetic implant to closely match Fernando’s left ear, crafting it approximately 10 percent larger than Fernando’s left ear to account for growth. An incision was made above the ear to take tissue lying just beneath Fernando’s scalp, which Dr. Hoerr stretched over the synthetic ear framework, covering it with living tissue. Once in place, Dr. Hoerr took a skin graft from Fernando’s inner arm – skin that closely matches the ear in color and texture – to cover the entire new ear. “The procedure went very smoothly,” says Dr. Hoerr. Fernando returned to surgery a month later for some minor touch ups to his skin graft, and just weeks after surgery, swelling was gone and the color of the ear began to look natural. Six months after the procedures, it was hard to tell the ear wasn’t there all along. When Dr. Hoerr, Dr. Moody and Fernando met this spring for a follow-up visit, it was a time for celebration. Not only was Fernando sporting his newly healed ear, his new haircut spoke volumes of the young man’s newfound confidence. His school friends noticed this new look too, he told his doctors. “I could hear my classmates whispering in homeroom. And I could hear them out of my new ear, which was even better,” Fernando says, a huge grin forming between his dimpled cheeks. “I looked so different that day that my teacher almost marked me absent until I said, ‘I’m here! I’m here!’ ”

Six months after surgery, Dr. George Hoerr was delighted to see Fernando – who was sporting a new outfit and haircut for the occasion.

Wash This WayPatients, physicians, nurses – and even Healthy Bear – gathered in CHKD’s lobby this spring to help create a fun, educational music video reinforcing the importance of hand washing. The video kicked off a “Wash This Way” campaign created to highlight CHKD’s hand hygiene policies throughout the organization. Purposely designed to be fun and lighthearted, the video featured members from every branch of the CHKD family. Pediatricians, specialists, surgeons, nurses, administrators, staff members and patients donned sunglasses and silly hats while lip-syncing to lyrics about hand hygiene. The two-minute 80s-style hip-hop video also featured clips from a variety of CHKD locations – from the operating rooms and the rehabilitation unit to Newport News Pediatrics. Released during Patient Safety Awareness Month in March, the video is just one part of a larger initiative to continue delivering safe, high-quality healthcare to children throughout the health system. CHKD is one of more than 90 pediatric health systems that make up Children’s Hospitals’ Solutions for Patient Safety – a nationwide collaborative dedicated to sharing ideas and improvements on the safety and quality of pediatric care. Dr. Sandip Godambe, CHKD’s vice president of clinical integration and quality, says hand hygiene may seem like a simple concept, but it’s an important one that can affect the care and safety of patients and families. “Hand washing is something everyone can do to help each other and contribute to good health,” he says. “It was great to see how enthusiastic the whole team was about working together to get this valuable message out.”

Suddenly, one of the wheels caught an edge of concrete, and the board shot out from beneath him. Joel flew forward, stretching his arms out to brace himself for the fall. As he hit the ground, he remembers pain exploding in his arm as he clenched his eyelids shut. “Momma, my arm, my arm,” he yelled, cradling his left forearm to his chest. His mother, Monica, ran to his side, instinctively aware that Joel was seriously injured; he never called her “Momma.” One look at his arm confirmed her instincts. Joel’s left forearm drooped unnaturally between his wrist and elbow. Moving quickly, Monica packed Joel into the car and headed for the nearest place she knew to be open on Christmas Day – CHKD Urgent Care on Volvo Parkway in Chesapeake. “I knew they would be able to help Joel and get him any special care he needed,” Monica says. CHKD Urgent Care is no stranger to broken bones. “We see a couple of fractures in our urgent care centers every day,” says Dr. Theresa Guins, medical director of CHKD Urgent Care. “Most of the time, they are minor forearm fractures that result from falls. Usually we can send a child home with a splint and a referral to an orthopedic specialist.” Joel’s break, however, was much more serious than a common fracture. By the time he arrived at urgent care, shock was setting in – Joel was pale, sweating and in obvious pain.

Urgent Care is a special gift on Christmas Day

The unseasonably warm weather on Christmas Day offered a perfect opportunity for Joel White to try out his sister’s new gift – a specialty skateboard with a single wheel in the

front and back. The active 12-year-old quickly got the hang of it, gliding along his quiet neighborhood street in Chesapeake.

Page 18: Kidstuff Spring 2016

16 Children’s Hospital of The King’s Daughters

The urgent care team sprang into action. Dr. Vicky Leff, a pediatrician at CHKD Urgent Care, ordered X-rays and an intranasal spray of quick-acting pain medicine to begin managing Joel’s discomfort. “This was a bad break,” says Dr. Leff. “We knew he would need to see a surgeon immediately.” Urgent care staff called the CHKD transport team to pick up Joel and take him to the main hospital in Norfolk. At the urgent care, a pediatric radiology tech took X-rays of Joel’s arm, sending them electronically to CHKD’s emergency department. Nurses then placed an IV to give pain medication and stabilized Joel’s arm in a splint. Joel’s father, Joel Sr., rode in the front of the transport to the hospital with his son. “I was really worried when I saw how bad his arm looked,” Joel Sr. says. He watched from the passenger’s seat as the transport team kept Joel calm during the trip to the hospital. In Norfolk, CHKD’s emergency department received Joel’s X-rays before the transport team arrived, identifying severe breaks to the boy’s radius and ulna, both bones in his lower arm. CHKD pediatric orthopedic surgeon

Sheldon St. Clair was on call that day. He reviewed Joel’s X-rays and recommended immediate surgery to realign the bones before his arm became too swollen. Within 30 minutes of his arrival at CHKD, Joel was being prepped for surgery. In the operating room, Dr. St. Clair realigned Joel’s radius and ulna, inserting titanium rods into each broken bone to hold them in place as they healed. The surgery took nearly an hour, and Joel would spend two nights in the hospital recovering. His arm was then cast from his hand to his shoulder to keep his injury as stable as possible. CHKD’s child life specialists helped brighten the family’s unexpected holiday stay. Though a trip to the hospital wasn’t part of the family’s Christmas plans, Monica and Joel Sr. were impressed with the smooth and quick care Joel received during the entire experience and grateful that CHKD Urgent Care was close by and able to coordinate care from the beginning. “We know how valuable urgent care is to busy families when their child has an unexpected urgent medical need and their pediatrician’s office is closed,” says Dr. Guins.

X-rays taken at CHKD Urgent Care on Christmas Day show Joel had severely broken both bones in his lower left arm.

Titanium flexible nails were surgically inserted into Joel’s broken bones to realign them, appearing as white lines in the center of his bones.

Page 19: Kidstuff Spring 2016

SPRING 2016 17

In an effort to provide convenience and integrated care to even more families in Hampton Roads, CHKD opened another urgent care center at Loehmann’s Plaza in Virginia Beach this spring, and plans are underway to open additional centers at Landstown Commons in Virginia Beach, at the Tech Center in Newport News, and in Williamsburg. “We really operate like a well-oiled machine. CHKD has set up a pretty seamless system that is extremely beneficial to patients, saving time and stress,” Dr. Guins says. Monica agrees. “Everything went so smoothly, and the urgent care staff was amazing,” she says. “I’m so grateful we lived close by - it made a tough situation so much easier.” As he healed, Joel worked hard to keep up with school and activities. By February, he was fitted for a shorter cast that ended at his elbow, and by March, he was able to wear a soft black cast. He performed physical therapy at home with his mom, rebuilding strength and flexibility in his arm and was pleased when Dr. St. Clair told him his bones were healing faster than expected.

Today, Joel has turned his attention from skateboarding to soccer. The rods in his arm will be removed this summer. The only reminders of his broken arm are three tiny scars on his wrist, forearm and elbow. And, of course, he’ll always have an interesting story to tell about his holiday break.

Luke Orlando gives SaraLee a grateful embrace during his physical therapy session at CHKD.

Spotlight

Puppy LoveLuke Orlando arrived at CHKD using a wheelchair. The 9-year-old was diagnosed with a rare condition that caused unbearable pain in his feet – even the slightest touch to his skin was excruciating. He was admitted to CHKD’s inpatient rehabilitation unit for treatment and found the perfect partner to help him get moving again. Enter SaraLee – a 2-year-old golden retriever now part of CHKD’s child life department. As CHKD’s new facility dog, SaraLee is specially trained to provide mental, physical and emotional support to young patients and their families in the hospital. “Having a dog around can provide a lot of comfort and normalize a child’s experience while they are here,” says Shannon Hood, manager of CHKD’s child life program and SaraLee’s handler. Shannon was matched with the young dog this spring and spent two weeks with Canine Companions for Independence, the organization that trained the dog. SaraLee lives with Shannon and comes to work with her as part of the child life team at CHKD every day. Interactions with a therapy animal are proven to help lower stress and anxiety levels, regulate blood pressure, increase patient mobility and distract from pain. “Simply stroking SaraLee’s fur can give children a sense of calm,” says Shannon. A facility dog can also provide motivation – a young patient might be inspired to get out of bed if he can take SaraLee for a walk down the hall. A simple game of fetch can double as a therapy exercise to improve balance and coordination in a child who needs physical therapy after a serious illness or injury. This was certainly true for Luke. SaraLee’s friendly personality provided the perfect distraction from Luke’s pain, and her eagerness to play motivated him to move during therapy sessions. By the end of his three-week stay at CHKD, Luke could do more than take SaraLee on walks; he could race her down the hallway without pain. And SaraLee was able to fetch something Luke had been missing weeks before ... his smile.

When asked why he supports Children’s Hospital of The King’s Daughters, Rick Burnell doesn’t hesitate. “It’s simple,” he says. “This hospital saved my

granddaughter’s life.” As former president of Armada Hoffler Real Estate Company and subsequently Atlantic Commercial Real Estate Service, Rick has always been active in the community and keenly aware of CHKD’s role as a leader in healthcare for children in Hampton Roads. He raised his three now-grown children in the area, but the 2004 birth of his granddaughter Lillian gave him an intimate and life-changing perspective on the importance of the hospital. Just days after her preterm birth, Lillian was rushed to CHKD’s neonatal intensive care unit with necrotizing enterocolitis – a deadly intestinal infection. As Lillian fought for her life, Rick was overwhelmed by the hospital’s efforts to save his granddaughter and comfort his family. “We experienced such great care and kindness during our time in the NICU with Lillian,” says Rick, describing the work of the chaplains, physicians and nurses. “The experience left a profound impression on me. I became aware of how much time, attention and expertise

everyone gave to help children with very serious health issues. It changed our whole family.” Lillian’s care inspired Rick to increase his annual gift to help other children at CHKD. “Everything they do at the hospital is about the kids. I tell others that they’d just need to spend an hour at the hospital to understand how lucky we are to have this great resource in our community,” says Rick. Despite her difficult start, today Lillian is a spunky 11-year-old who is thriving thanks to her devoted parents Rachel and David and an extended family of specialists at CHKD that includes neurologists, physical therapists and nephrologists. “We know that she would not be here without them,” Rachel says. “We will be forever thankful to CHKD.” In addition to Lillian, Rick is grandfather to Grace, Julia and Ellis, and he feels comforted knowing that CHKD will be available as his family grows. “To those whom much is given, much is expected,” he says. “Those are words I live by. I give to CHKD because I’m a firm believer that every child in our community deserves good healthcare, and it’s up to the community to support that care and make it accessible.”

Rick Burnell (center back) gathers with his family (from left back) Jay Burnell, Shel Burnell, Tammy Burnell, David Hale (from left front) Julia Burnell, Lillian Hale, Grace Burnell and Rachel Hale.

Grant Walner traveled the world in service to his country as chief petty officer in the United States Navy. When he passed away at age 92, his final act of

altruism included an unexpected and generous gift to CHKD and the community’s children. “We were very surprised and grateful for the significant gift Mr. Walner left to CHKD,” says Susan Carriker, director of planned giving at CHKD, adding that almost half of planned gifts arrive as surprises after the donor has passed away. “We appreciate his generosity and only wish we had the opportunity to thank him and show him how his gift would make a difference in the lives of our children.” Grant was born in Oklahoma in a home with no running water or electricity. He joined the U. S. Navy as a young man and learned about nuclear energy – the latest technology of the day. Grant traveled the world with the Navy, sailing around the Cape of Good Hope three times and visiting countries and Naval bases across the globe during World War II and the Korean War. After leaving the Navy, Grant put his knowledge of nuclear power to work for companies in Connecticut, Texas and Georgia. When he lost his wife, Joann, of 39 years in 1995, Grant recalled happy times of their years together stationed at Little Creek

and summers vacationing at the Virginia Beach oceanfront. He returned to the area that held such special memories to spend his final years. Relationships were an important part of Grant’s life, and he maintained pen-pal relationships with friends throughout the years. While he and Joann never had children, he loved children. Known for his hospitality and generosity, Grant welcomed and encouraged friends to bring their children and grandchildren during visits to his home. Grant passed away in 2012 and was interred on March 4, 2013, at Arlington National Cemetery. Though he did not tell anyone why he chose to give to CHKD, his benevolent gift to the children leaves a legacy of compassion and care for the community he called home. “Mr. Walner’s recognition of our mission speaks of his generous heart and lifelong dedication to serve his community. We are so grateful that he recognized the important work of CHKD,” says Susan. “Every gift we get makes a difference to the children and families we care for, and ultimately the entire community benefits from the charitable actions of humble donors like Grant Walner.”
