Call the Illinois Department of Public Health at 217-785-5133 or email [email protected] and ask if you are eligible to recertify. If yes, request a recertification packet. 2. Complete a fingerprint-based criminal history records check. Fingerprint background check cost varies based upon the IDPH-vendor the applicant chooses. Do not proceed to the next step until the criminal history check is successfully completed. 3. Schedule a date and time to complete the 21 manual skills test with an IDPH-approved evaluator. The cost for the 21 manual skills test is $199. To schedule a time, contact KCC’s Office of Continuing Education and Career Services at 815-802-8207. Upon successful completion of the manual skills test, IDPH will mail you a second packet to register for the written exam. 4. Register to take the written exam. The written exam fee is $65. To receive a list of dates for the written test, contact KCC’s Health Careers Division at 815-802-8800. CPR Healthcare Provider Includes the information, training, demonstration and practice nec- essary to pass the CPR Healthcare Provider written examination and perform adult, child and infant cardiopulmonary resuscitation acceptable to the American Heart Association (AHA). After suc- cessful completion, participants will receive an AHA certification card. Instructor: Sheree Wertz. $105, includes the AHA required textbook.
