ThesePackerLegendsandmanymorehavepracticedontheGreenBayPackersRayNitschkePracticeField,formerlylocatedinGreenBay,thatnowfindsitshomeinMilwaukeeCountyMitchellParkbehindtheDomesatSouth22ndandPierceStreets.ThefieldhasahistorictraditionofexcellenceasdemonstratedbythesacrificeandcommitmentoftheGreenBayPackers’playersandcoacheswhohavesteppedonthisfieldinthepast.TheJourneyHousePackersFootballStadiumhousestheonlyfieldintheworldwithanNFLteam'slogooutsideofanNFLstadium!TheJourneyHousePackersStadiumofficiallyopenedonJune6,2013.Thesyntheticturffootballfield---–donatedtoJourneyHousebytheGreenBayPackers,whichusedthefieldaspartoftheirpracticefacilityduringthelate1990sandearly2000s–servesasthehomefieldfortheJourneyHousePackersUrbanYouthFootballProgram,whichservesover120youthages6-14,representingacross-sectionofethnically-diverse,low-incomeyouthfromMilwaukee'sinnercity.JourneyHouseseesthefootballfieldasanextensionoftheclassroomandbelievesvaluablelessonslearnedthroughthecompetitivenatureofsports.Youandyourorganization,family,orfriendscantakeyourplacebesidethesePackerLegendsbyhostinganeventonthisfield.WehopeyouhaveanenjoyableeventandhonortheTerms&RulesofUsethatkeepthefieldinworld-classcondition.

TheJourneyHousePackersFootballStadiumisavailableforrentaltocommunitygroups,sportsteamsandleagues,andindividuals

conductingfamilyappropriateactivitiesasdeterminedbyJourneyHouse.

ToReservetheStadiumandAmenities:

1. ReviewtheFootballStadiumRentalTermsandRulesofUseAgreementtodetermine

eligibilityofyouruseorevent.2. Ifeligible,completeanInitialInquiryForm.Theinquiryformwillhelpusdetermineif

theStadiumrentalisfeasibleandavailablebasedondates/times,typeofevent,etc.YoumaycompletetheInitialInquiryForm(p.6):

Insurancetoguaranteerentalreservationtostadiumrental@journeyhouse.orgorfax(414)647-0266.AcurrentCertificateofLiabilityInsurancemayberequestedfromagroupwhentheactivityisdeemeddangerousoroutsideofthescopeofasportingevent,asdeterminedbyJourneyHouse.ThecertificatemustshowcoverageforCommercialGeneralLiabilityInsuranceataminimumof$1,000,000peroccurrence,$2,000,000forgeneralaggregate.JourneyHousemustbenamedatthe“certificateholder”aswellas“additionalinsured”onthecertificate.

b. Submitanonrefundabledepositequalto50%oftherental.Thebalanceofthepaymentmustbepaidinfull30dayspriortotheevent.Nonrefundabledepositsareprocessedonafirstcomefirstservebasis.Pleasenotethatifyoureventiswithin30daysoftheapplication,thefullpaymentmustbepaidtoguaranteeyourreservation.

c. Yourreservationisnotguaranteeduntilwehavereceivedallrelevantdocuments.

5. CancellationPolicy:Fullrefundsaregrantedupto30dayspriortothescheduledrentaldate.Refundswillnotbegrantedforcancellationsmadelessthan30dayspriortotheschedulerentaldate.

6. InclementWeatherRelatedCancellationswillbedeterminedbyJourneyHouseandpostedonJourneyHousesocialmedia,includingourwebsiteandFacebookpage.

a. InclementWeatherRelatedCancellationsmustberescheduledthroughJourneyHouse.ItistheresponsibilityoftherentertonotifyJourneyHouseofthedesiredmake-update.Reasonableaccommodationswillbemadebasedonfieldavailability.Norefundswillbeissuedtoagroupaftertheseasoniscompletedforeventsthatwerenotrescheduled.

OrganizationalLeagueFeeOrganizationscoordinatingleagueplayinwhichtheirparticipantsarebeingchargedafeemustpayanOrganizationalLeagueFeeinadditiontotheHourlyUsageRates.ThisfeeisassessedtotheorganizationontheTOTALnumberofteamsparticipatingineachleagueintheseason.TheOrganizationalLeagueFeeisdueone(1)weekpriortothestartoftheleague(s).Anyadditionaldays,suchasAll-StarGamesorPlay-offsthatwerenotrequested,mustbepaidforbeforetheycanbeplayed.CleaningFeeInadditiontothecostofyourrental,aREQUIREDCleaningFeewillbechargedthatisduewithyourpayment.Ifyouchoosetoself-cleanaccordingtotheStadiumCleaningCheck-List(attached),attheconclusionofyourevent,theStadiumManagerwillwalk-throughandinspecttheStadiumalongwiththeprimarycontact/vendor.IfthecleaningiscompletedinaccordancetoJourneyHouseQualityControlStandards,theStadiumManagerwillapproveviahis/hersignature;yourcleaningfeewillbereturnedwithin10businessdays.Cleaningmustbecompletedduringrentaltimeperiod.Ifnotcompletedduringthistimeperiod,youwillbeinvoicedforeachadditionalhourandforfeittheoptionforacleaningfeereimbursement.ConcessionStandFeeIfyouwouldliketooperateyourownconcessionsandretain100%ofyoursales,theconcessionspaceareaisavailableforaflatfeeof$100perdailyrentalperiod.Youwillhaveaccesstotheapproximately675sq.ft.concessionroom,whichincludestheconcessionswindow,interiorcounterspace,electricaloutlets,andalockeddoor.Accesstotherefrigerationsystemsandsmallkitchenappliancesarenotincludedinthefee.Renterprovidestheirownconcessions,includingcoolers,paperproducts,andothersmallkitchenappliancesneeded(suchascoffeepotsorNescos).PleasenotethatthereisnorunningwaterintheStadium,includingtheconcessionarea.Ifyouwantordesirewater,youmayconsiderbriningwaterinvia5-gallonwatercoolers.YourconcessionproductsmayentertheStadiumattheapproveddesignatedtimeofarrivalandmustbetakenuponapproveddeparturetime.Atnotimewillstoragebepermittedforanyreason.PleasenotethatnovehiclesareallowedinMitchellPark,whichincludesthepathwaytotheStadium.Youmaywanttobringanon-motorizedcart,suchasawagonoraflatbed/dolly,totransportyourconcessionsuppliestotheStadiumConcessionStand.Concessionsareonlyauthorizedtobesoldthroughtheconcessionarea.SaleofAlcoholorTobaccoproductsareprohibited.Ifyouselectnottopaytheconcessionstandfee,(a)youmaynotsellconcessionsofanykindinotherareasoftheStadiumand(b)JourneyHousereservestherighttosellconcessionsduringyourevent.LongTermRentalsJourneyHouseiswillingtodiscusslong-termrentalagreementswithinterestedparties.Toinquire,[email protected]

TermsandRulesofUseAgreement

InanefforttoprovidesafeandcleanfacilitiesforallgroupstousetheJourneyHousePackersFootballStadium(“Stadium”),JourneyHousehasestablishedthefollowingtermsandrulesforuseoftheStadium,whichisdefinedasFootballField,Bleachers,Lights,Scoreboard,and,ifapplicable,ConcessionStand.Everyeffortismadetohonorrentalrequests.However,pleasenotethepriorityforStadiumusageisasfollows:

Costs

RentersareresponsibleforallcostsincurredinconnectionwiththeuseoftheStadium.Thecurrentcostsandpaymentscheduleisoutlinedinthisdocument.AdditionalcostsmaybeincurredifadditionalcleanupisneededordamagetoStadiumorrelatedStadiumequipmentoccurs.Yourorganizationwillbeinvoicedforadditionalcosts.Cleaning–RequiredforAllRentalsInadditiontothecostofyourrental,amandatoryCleaningFeewillbechargedthatisduewithyourpayment.Ifyouchoosetoself-cleanaccordingtotheStadiumCleaningCheck-List(attached),attheconclusionofyourevent,theStadiumManagerwillwalk-throughandinspecttheStadiumalongwiththeprimarycontact/vendor.IfthecleaningiscompletedinaccordancetoJourneyHouseQualityControlStandards,theStadiumManagerwillapproveviahis/hersignature;yourcleaningfeewillbereturnedwithin10businessdays.Cleaningmustbecompletedduringrentaltimeperiod.Ifnotcompletedduringthistimeperiod,youwillbeinvoicedforeachadditionalhourandforfeittheoptionforacleaningfeereimbursement.PaymentandRentalReservationGuaranteeToguaranteereservation,submitanonrefundabledepositequalto50%oftherental.Thebalanceofthepaymentmustbepaidinfull30dayspriortotheevent.Nonrefundabledepositsareprocessedonafirstcomefirstservebasis.Pleasenotethatifyoureventiswithin30daysoftheapplication,thefullpaymentmustbepaidtoguaranteeyourreservation.Yourreservationisnotguaranteeduntilwehavereceivedallrelevantdocuments,includingpayment,signedTermsandRulesofUseAgreement,andcurrentCertificateofLiabilityInsurance(forleagues,organizations,andgroupswhoseactivitiesaredeemeddangerousasdeterminedbyJourneyHouse).ProofofCurrentCertificateofLiabilityInsuranceLeaguesand/orOrganizationsmustprovideacurrentCertificateofLiabilityInsurancetoguaranteerentalreservation.AcurrentCertificateofLiabilityInsurancemayberequestedfromagroupwhentheactivityisdeemeddangerousoroutsideofthescopeofasportingevent,asdeterminedbyJourneyHouse.ThecertificatemustshowcoverageforCommercialGeneralLiabilityInsuranceataminimumof$1,000,000peroccurrence,$2,000,000forgeneralaggregate.JourneyHousemustbenamedatthe“certificateholder”aswellas“additionalinsured”onthecertificate.

TheFanCodeofConductJourneyHouseiscommittedtoprovidinganenjoyableandsafegameexperience.Pleasekeepinmindcoachesandtheirrespectiveschoolsorfootballorganizationsareresponsiblefortheirplayers,coaches,coachingstaff,administrators,andspectators.YouareresponsibleforyourguestsandmakingthemawareoftheTermsandRulesofUseGuidelines.FailuretofollowJourneyHouseestablishedrulescouldimpactthesafetyandlongevityofthefieldturfforfutureusersandmayresultinimmediateterminationoftheevent.IrresponsibleconductwillnotbetoleratedatJourneyHousePackersStadiumandinsomecasesmayresultinejectionand/orarrestfromthepractice/training/gameand/orevent.Accordingtoourwinninggameplan,thefollowingwillNOTbetolerated:

• Fighting,taunting,oranyactionthatmayharmorendangerothersinthestadium.• Smoking.JourneyHousePackersStadiumisasmoke-freefacility.• Intoxicationorothersignsofalcoholanddrugimpairmentthatresultsinirresponsible

behavior.• Abusiveorfoullanguageandobsceneactions.• Failingtofollowinstructionsofstadiumpersonnel.• Anyotherconductdeemedinappropriate• Spectatorsonthefield.

ZeroToleranceUseand/orpossessionofalcohol,tobacco,controlledsubstances,pepperspray,andweaponsarestrictlyprohibitedatJourneyHousePackersFootballStadium.ViolationwillresultinimmediateterminationatthediscretionofJourneyHouse.Ifterminationoccurs,renterisresponsibleforallfeesandliabilitiesdueimmediatelywithnooptionofreimbursement.InflatableStructuresInflatablestructuresofanykind,includingbouncehouse,arestrictlyprohibited.VehiclesNovehiclesarepermittedtodriveonMilwaukeeCountyParkspathwaytotheStadium,includingtheStadiuminterioratanytime.Youmaybringanon-motorizedcart,suchasawagonoraflatbed/dolly,totransportequipment,concessionsupplies,and/orotheritemstotheStadium.

ParkingFreeparkingisavailableonthestreetorthenearbyMitchellParkparkinglot,locatedatthecornerofSouth27thandWestPierceStreet.WaterPleasenotethatthereisnorunningwaterintheStadium,includingtheconcessionarea.Pop-upTents,Canopies,andPortableFoldingChairsPop-uptents,canopies,andportablefoldingchairsareprohibitedinStadium.ConcessionStandOperationsIfyouwouldliketooperateyourownconcessionsandretain100%ofyoursales,theconcessionspaceareaisavailableforaflatfeeof$100perdailyrentalperiod.Youwillhaveaccesstotheapproximately675sq.ft.concessionroom,whichincludestheconcessionswindow,interiorcounterspace,electricaloutlets,andalockeddoor.Accesstotherefrigerationsystemsandsmallkitchenappliancesarenotincludedinthefee.Renterprovidestheirownconcessions,includingcoolers,paperproducts,andothersmallkitchenappliancesneeded(suchascoffeepotsorNescos).PleasenotethatthereisnorunningwaterintheStadium,includingtheconcessionarea.Ifyouwantordesirewater,youmayconsiderbriningwaterinvia5-gallonwatercoolers/jugstoassistwithsalesofcoffee,hotchocolate,tea,orforuseincleaningorotherpreparation.YourconcessionproductsmayentertheStadiumattheapproveddesignatedtimeofarrivalandmustbetakenuponapproveddeparturetime.Atnotimewillstoragebepermittedforanyreason.PleasenotethatnovehiclesareallowedinMitchellPark,whichincludesthepathwaytotheStadium.Youmaywanttobringanon-motorizedcart,suchasawagonoraflatbed/dolly,totransportyourconcessionsuppliestotheStadiumConcessionStand.Concessionsareonlyauthorizedtobesoldthroughtheconcessionarea.AlcoholorTobaccoproductsareprohibited.Ifyouselectnottopaytheconcessionstandfee,(a)youmaynotsellconcessionsofanykindinotherareasoftheStadiumand(b)JourneyHousereservestherighttosellconcessionsduringyourevent.

RightofRefusalJourneyHousemayrefuserentaloftheStadiumtoperson(s)and/ororganizationswhoplantoconductactivitiesdeemeddetrimentaltophilosophiesandgoalsofJourneyHouse,Inc.GroupswishingtorenttheStadiummustreadandsignthisFootballStadiumRentalTermsandRulesofUseAgreementpriortobeinggrantedauthoritytousetheStadium.ApplicationDenialArentalapplicationmaybedeniedforthefollowingreasons:

1. Stadiumisnotavailableatrequestedtimes.2. Applicationisnotreceivedtwo(2)weekspriortotheevent.3. TheapplicanthasoutstandingfeesowedtoJourneyHousefromprevioustransactions.4. TheproposedeventisdeemeddetrimentaltophilosophiesandgoalsofJourneyHouse,

Inc.

TerminationRentalactivities/games/eventsoftheJourneyHousePackersFootballStadiumaresubjecttoallmunicipalordinancesinadditiontoallrulesandregulationsgoverningtheStadiumandcanbeterminatedimmediatelyatthediscretionofJourneyHouse.Ifterminationoccurs,renterisresponsibleforallfeesandliabilitiesdueimmediatelywithnooptionofreimbursement.CancellationPolicyFullrefundsaregrantedupto30dayspriortothescheduledrentaldate.Refundswillnotbegrantedforcancellationsmadelessthan30dayspriortotheschedulerentaldate.InclementWeatherRelatedCancellationsInclementweatherrelatedwillbedeterminedbyJourneyHouseandpostedonJourneyHousesocialmedia,includingourwebsiteandFacebookpage.InclementWeatherRelatedCancellationsmustberescheduledthroughJourneyHouse.ItistheresponsibilityoftherentertonotifyJourneyHouseofthedesiredmake-update.Reasonableaccommodationswillbemadebasedonfieldavailability.Norefundswillbeissuedtoagroupaftertheseasoniscompletedforeventsthatwerenotrescheduled.

StadiumFieldRegulations

Pleasehelpuskeepthefieldinasafeandplayableconditionbyfollowingtheseturfregulations.Pleasekeepinmindcoachesandtheirrespectiveschoolsorfootballorganizationsareresponsiblefortheirplayers,coaches,coachingstaff,administrators,andspectators.YouareresponsibleforyourguestsandmakingthemawareoftheTermsandRulesofUseGuidelines.FailuretofollowJourneyHouseestablishedrulescouldimpactthesafetyandlongevityofthefieldturfforfutureusersandmayresultinimmediateterminationoftheevent.Regulations(Team/ApparelRegulations)

a. Playersarenotpermittedtowearshoeswithspikes(pyramidorpin)longerthan½inchin

length.

b. Players,coaches,schoolemployees,athletictrainers,gameofficials,and/orspectatorsmustwearturfshoes,tennisshoes,orrubbercleatsonthefield.Metalcleatswillnotbeallowedonthestadiumfield.Allshoesmustbecleanbeforeenteringthefield;cleatsmustbewashed.

c. Anytapeusedbyplayers,coachesorathletictrainersmustberemovedfromthefield

followinganyevent.Regulations(AllUsers,IncludingTeamsandSpectators)

a. Clearwateristheonlybeveragethatispermissibleonthefieldturf.Nosportsdrinksorany

otherbeverageisallowed.

b. Atnotimewillgum,seeds,nutsoranykindoffoodbepermittedonthefieldorwithintheconcreteframeofthefieldturf,thisincludes,players,coaches,schoolorparkemployees,gameofficials,athletictrainersandoranyspectators.

c. Atnotimewillconfettioranysmallnon-degradablematerialsbeusedonornearthefield.d. Atnotimewillbleachoranycorrosivecleanersbeusedonthefieldexceptthosecleaners

permittedbyJourneyHouseand/orMilwaukeeCountyParksmaintenancestaffandundertheirstrictsupervision.

e. Onlyfreestandingfieldmarkersandsportsequipmentmaybeusedontheartificialsurface.

Nostakes,poles,ormarkersofanykindmaybedrivenintothefieldsurface.

f. AthleticequipmentisnottobedrugonthefieldandistobemovedbytheteamsonlyunderthesupervisionoftheJourneyHouseFieldTurfManagerorJourneyHouseemployees.

g. Thestorageofanyequipmentisprohibitedonthefieldareas.h. Nodogs,animalsorpetsofanysortarepermittedwithintheStadiumFieldArea(withinthe

fencedareaofthefield)atanytime.i. Nobikes,rollerblades,strollers,and/ormotorizedvehiclesareallowedonthefieldatany

time.j. Noautomobilesandortrucksallowedbeyondthegate/entranceofthefieldatanytime.k. Useand/orpossessionofalcohol,tobacco,controlledsubstances,pepperspray,and

weaponsarestrictlyprohibitedatJourneyHousePackersFootballStadium.l. TheStadiumisaSmokeFreeFacility.Nosmokingispermittedonthefieldorinthestadium

atanytime.Smokingwillbeallowedindesignatedareasonlyawayfromfieldandconcessionareas.

m. Allspectatorsmustbeseatedinthebleacherarea.StandingontheStadiumsidewalksis
