Dear Staff,

This year we will experience several changes and challenges. We are temporarily located on the Jefferson Campus. However, we will be moving into a newly renovated space in the 2014-2015 school year. We have lost several significant members of our school community. They will be greatly missed. On another note we are welcoming several new members to our staff and look forward to establishing collegial relationships with them.

We continue to focus on increasing student achievement. This will be K-6 second year using the New York State Common Core. Members of our 7th and 8th grade team will begin to incorporate the Common Core in ELA and Math. We no longer have short Wednesdays and will need to begin to seek alternatives ways to grow professionally.

With so many challenges facing us it is important to take care of ourselves and to be kind to one another. You are a phenomenal staff and we feel you have the capacity to help are students achieve academic success.

DRESS FOR RESPECT

Let your professionalism speak for you!

Clothing may not make a person, but it can be a contributing factor in unmaking a person.

Whether we want to admit it, our appearance affects how we are perceived and received in definite ways. Clothing has nothing to do with students liking a teacher. But clothing definitely has an affect on students’ respect for a teacher, and respect is what a teacher must have if learning is to take place.

I Could Not Believe What I Saw

After Christmas Vacation, one of my students left some pictures of the class holiday party on my desk. I took a look at myself and I could not believe how I looked. I looked like I didn’t care about myself.

The next day I came to school more appropriately dressed and they all noticed and commented on how nice I looked. I was so happy and they made me feel so good.

I now spend more time caring about who I am. The students care about me. I am proud of who I am. And they are also so much better behaved now as a result of who I represent.

BULLETIN BOARD DISPLAYS

Hallway bulletin boards are displays that serve as an excellent means of communication for our students, parents and visitors who enter our school by highlighting things that are happening in our classrooms. The displays and/or boards are an outgrowth of lessons being taught in classrooms.

ALL DISPLAYS AND BULLETIN BOARDS ARE TO BE NEAT, ORGANIZED, AESTHETICALLY PLEASING TO LOOK UPON, LABELED BY CLASS, AND CONTAIN NOTATION IF GRAMMAR, PUNCTUATION AND GRAMMAR ARE INCORRECT.

For Example: Work in Progress, Transitional Speller, On the Right Road.

Each grade level will be assigned specific bulletin boards in hallways to display student work, based on standards. Please record the common core standards on the bulletin boards.

CHILD ABUSE REPORTING PROCEDURES

We are all required by law to report students who were suspect may be being abused to Social Services Protective Unit.

At #5 the procedure we would like you to follow is:

1. If you see or hear a child say something that makes you think that they may have been injured or abused by someone talk to the child to see if they tell you how they got injured. Once you have talked to the child and if the events don’t appear to match with the injury you take the child to the School Nurse.

2. The Nurse then will take a look at the injury and determine whether it looks like something that needs to be investigated.

3. If it needs to be reported, you may contact CPS yourself, but please inform administration, the social worker, or nurse immediately following the call. If you do not wish to make the phone call, please ask the people listed below.

COMMUNICATION TO PARENTS / STAFF

The principal produces a Weekly Bulletin for staff, parent handbook and seasonal newsletters for parents. In addition, other communications will be distributed when the need arises in the form of memos, letters, etc.

Also, if anyone is interested in coordinating a No. 5 School yearbook and/or student newspaper, please see the principal.

Public Address System Business announcements will be made at 9:00 a.m. and at 3:25 p.m. by the

administrative and/or office staff.

The Pledge of Allegiance, School Creed, Vision statement, and student announcements will be done over the P.A. system promptly at 9:20 a.m. All staff/faculty, students and visitors are to participate, unless due to religious convictions. However, all activities and movement comes to a halt during this time.

All other P.A. announcements are given to the Principal, in writing, for approval.

Emergency announcements will be made at the discretion of the Principal, or case of absence, the substitute administrator and/or the Asst. Principal.

Visitors All visitors to the building must register at the Main Office and ask the

receptionist for a visitor’s tag. Visitors are encouraged to have advanced scheduled appointments, unless it

is an emergency.

CONFERENCE DAYS

Through an agreement between the Rochester Teachers Association and the Board of Education, schools are allocated some RTA conference days and money for use each year by full-time classroom teachers. The administration of this allocation is handled jointly by the Building Committee.

A special conference day request form is used and available in the school office. Care should be taken in completing the request form and travel voucher if reimbursement is to occur.

Copying ServicesCopying Services The copy machine is for use of all staff members for copying related to

education. It is located in the faculty lounge. City School District owned copiers should not be used to copy personal

documents. The copy machine is in great demand on a daily basis. Every individual is responsible for making their own copies.

Procedures may change, based upon operational needs.

Please be mindful that there is only one large copier for the building. This will require extreme courtesy and efficiency on everyone’s part. Limit your copying to 5 minutes or less each time you copy.

Please follow carefully the instructions for copier use.Repair charges are astronomical and can be prevented if you read directions!

STAFF WILL NOTIFY THE MAIN OFFICE WHEN NEW TONER IS NEEDED!

The purchase of copy paper is the single largest expenditure in the school’s yearly supply budget.

A CRISIS INCIDENT IS DEFINED AS:

A SITUATION THAT CAN NOT BE HANDLED SAFELY BY THE TEACHER IN THE CLASSROOM

WHEN A CHILD IS IN DANGER OF HURTING HIMSELF OR OTHERS IN THE CLASSROOM!

Please contact administration immediately when a crisis occurs.

Adult behaviors that can escalate a crisis are: Sarcasm Negative tone of voice Inconsistency Favoritism Negative body language Put-downs Outbursts Public reprimands Unfairness Apathy Inflexibility Lack of humor

Sunshine Cafe Dining Procedures

1. Enter one behind the other.2. Use indoor voices when speaking.3. Use proper dining etiquette.4. Clean your space when done.5. Keep your hands, feet and objects to yourself.6. Use your social time to study, say kind words, be thoughtful and enjoy each

It is the responsibility of every adult in the school to improve student behavior. If students are wild in the cafeteria, then it has been allowed.

Principal and Support Staff: high visibility in the dining hall, enforcing all dining hall procedures.

Dining Hall Staff: enforcing dining hall procedures in a firm and consistent manner.

Teachers: modeling and showing students how to enter and dine prior to bringing students to dining hall. Practice and reinforcement is critical to success!

DINING HALL SERVICES

MEALS

The student-dining hall serves breakfast and lunch each full-day session of school. Students who received free or reduced lunch last year will again be eligible during the first two weeks of September. Once new guidelines and applications are available, students must reapply.

Faculty and staff, we need your help in making sure that every application is returned. It is very important because we (School 5) receive funding based on those applications.

STAFF ABSENCE

It is imperative to contact the appropriate personnel when you are going to be absent from school.

Make every effort to notify Ms. Brown or Mr.Gizzi by 9:00 p.m. the evening before or the following morning by 7:00 a.m.

Occasionally, emergencies do come up. However, early notification enhances our ability to receive a suitable

substitute teacher. Berdell Chisholm will handle advanced notice substitute requests.

*All staff members must notify the office as to their return and extended absence by 2:30 p.m. – each day – so that a substitute can be obtained.

Absence During the School Day

Any member of the staff who must leave the building for any reason; illness, family emergency, duties requiring any coverage during the school day must follow procedures listed below.

For Lunch: (Sign-out book in main office)

For Planning Time: Request and receive approval; sign-out.

It is imperative that the Administrative Staff knows when you are leaving the building and there is the potential that you may not return.

It is extremely important that the Administrative Staff be aware when staff takes students out of the building, even for impromptu neighborhood excursions.

On-Going Professional Development

Special Subject and Special Related Services Teachers will meet with administration to outline initiatives, discuss solutions and receive feedback from teachers especially with the new schedule.

Grade Level Meetings will be held once a week. The purpose of these meetings is to meet as a grade level w/administration, coaches and each other to discuss scaffold instruction, best practices, and looking at student work.

FIRE ALARM PROCEDURES

1. Each classroom should have directions for evacuation posted. Advise the main office

if such directions are not displayed in each classroom in which you teach.

2. Teachers should periodically inform and remind students in each class of the fire drill

directions posted for the particular room.

3. Once the fire alarm is sounded, evacuation of the building must be carried through

to completion.

4. Students and staff should begin evacuation at the first sound of the fire alarm bell.

Directions posted in the room are to be followed.

5. Books and other materials should be left in the classroom. Purses should be carried

out of the building. Teachers must carry their red fire drill folder with class list and

student sheet inside.

6. The evacuation will be in an orderly fashion with the teacher leading his/her class

and proceeding to their designated location outside the building. Everyone must

remain in his or her designated area.

7. Each teacher should stay with his/her class when outside and take attendance to

determine that all of his/her students have left the building, and are safe.

8. When a prescribed exit is blocked, the teacher must select an alternate route for

9. Teachers who are not on duty or who are expecting a class should go to the area

outside the building that they normally would if they were in class. Such teachers

10. If the fire alarm sounds during lunch, teachers in the faculty lounge should go to the

student-dining hall and assist in the evacuation of and supervision of students on the

outside. Teachers not on duty and not in the dining hall proceed to the nearest exit.

EVERYONE ELSE MUST EXIT THE BUILDING.

A. The school secretary telephones the fire dispatcher when a fire alarm initially

sounds and maintains an open line to the dispatcher until the emergency has

ended. The open line provides for the immediate transfer of information and

directions between the school and fire department.

B. When the building is declared safe, only yellow flags will signal the all clear to re-

enter the building.

C. We must walk to the sidewalks surrounding the building as quickly as possible.

Be sure to provide a clear path for the Rochester Fire Department to arrive, park

and enter the building. Safety for all is paramount.

Fund Raising / Student Activity Account

Fund-Raising

Staff members must have the approval of the Principal before sponsoring a sale or fund-raiser and before signing a contract or agreement for the school or school organization. New contracts or agreements will be sent by the Principal, to the legal department for recommendation of action, before they are signed. Taxes must be paid on any items sold; sale and fund-raisers are not tax exempt activities. You are personally liable for any uncollected taxes and penalties.

A chairperson must be designated for each fundraiser event and work under the guidance of the School Secretary.

All funds collected through fees, donations, sales or fund-raisers for projects, trips, etc., must be deposited with the School Secretary in the Main Office by the end of each day. Under no circumstances should staff leave cash or valuables in their room overnight. This requirement protects you. Loss of cash or valuables from classrooms is in no way covered by the City School District. You will be held personally liable for such losses.

All bills and receipts for supplies from vendors must be submitted to the School Secretary upon receipt.

What is induction? Induction is the process of systematically training and supporting new personnel, hopefully, beginning before the first day of school and continuing throughout the first year and beyond. Its’ purpose includes, but is not limited to:

Easing the transition into new work situation Improving job effectiveness through training of performance

expectations Promoting the district’s and school’s culture – its philosophies, missions,

policies, procedures, and goals Increasing the retention of quality employees at School No. 5

What can a new employee expect? An informational meeting and tour of the building by the Principal and/or

Assistant Principal if requested.

A formal induction to the entire staff during a full staff meeting.

Special designated time with the Administration to hear and address concerns.

Assignment of a buddy that will focus on helping you adjust to your new work situation.

The induction sends a message to employees that they are valued ---- that we want you to succeed, grow, and thrive. It is an invitational message of care and concern.

STUDENT INJURIES

If a student suffers a minor injury, send or bring the student to the Nurse’s Office (if closed, to Main Office). If there is evidence of seizure or if any injury appears to be serious (involving head, back, serious bleeding), THE STUDENT SHOULD NOT BE MOVED; but made comfortable where he/she is. Hallway or area where the injury occurred should be cleared of unnecessary persons. CONTACT THE MAIN OFFICE IMMEDIATELY. A staff member involved with the incident is to notify the Nurse’s Office of the injury along with a brief verbal description of the incident.

If an injury is such that speedy intervention is needed, the Nurse will confer with the Principal and she will make the necessary phone call to summon help. If the Principal is not in the building, the Administrator-in-Charge will act on her behalf.

Any staff member witnessing a student injury must provide the Nurse information for the Student Accident Report. The Nurse should forward the white Pupil Injury Data form to the office for typing of the Student Accident Report. Copies of this report are forwarded to the parent, Blue Cross/Blue Shield, and the district’s Student Data Department.

If inquiry is made concerning an accident by anyone not connected with the Board of Education or the Corporation Counsel’s Office, the person should be referred immediately to the Principal or Administrator-in-Charge.

EMPLOYEE INJURIES

If an employee of the City School District sustains injuries, immediate treatment should follow the guidelines for students noted above.

A Worker’s Compensation Worksheet should be filled out immediately by the employee. The worksheet is then returned to the Main Office for the Principal’s signature and processing.

All staff keys are TO BE left in the key case in the office at the END OF EACH DAY. NEVER GIVE KEYS TO STUDENTS. The School secretary is the coordinator of all keys. If you find it difficult to remember where your keys are please make a copy.

A lost and found department is maintained in the main office. Please send all found articles, to the main office. Articles will be kept for a 2-week period before being disposed of. Please inform students of this policy and encourage them to check promptly for lost article(s). ALL BOOKS SHOULD BE SENT TO THE LIBRARY.

Please check your mailbox every day before school, after school and, if possible, during lunch period. Also, important information will be posted on the bulletin board in the main office and faculty lounge.

Nurse’s Office Hours

Office Hours:9:30 - 11:00 and 12:30 - 3:30.We have to ensure that the school health office completes the following services mandated by the law:1. Vision, Hearing, Scoliosis Screening2. Exclusion of students from school for infectious diseases (and return to school)3. Maintenance of a health record as part of cumulative school record.4. Review of records to ensure that immunizations meet state guidelines.5. Care of students with special health needs including health assessments for

CSE and developing Individualized Health Plan.6. If medications are given in school, a licensed professional must administer

them (unless the student is self-directed).7. Skilled nursing procedures (ex: blood glucose, self-catherization)

The following services are non-mandated by law. Since these are often the services that our customers consider to be the most important, we try to make time to meet them to the best of our ability.

1. Assessment and care for acute and minor illness and injury care.2. Administration of medication3. Referrals for health and mental health issues when appropriate.4. Tracking, counseling and follow-up for vision, hearing, scoliosis failures, immunizations, and pregnant students.

For the 2013-2014 school year, the health office will be responsible for these activities at School #5. We may also occasionally be required to cover other schools to administer medications or do skilled nursing procedures when substitutes are not available. In order to safely administer medication to a highly distractible population and to perform other necessary skilled nursing procedures,

we appreciate your cooperation in not sending children to the health office during this time except in an extreme emergency.

Self-Directed Medication Policy

A licensed nurse will give medication to all children, unless the child is determined by the licensed professional nurse to be self-directed. The nurse will be working with all children who receive medication to be self-directed. A child is considered to be self-directed by the licensed nurse if, considering the child’s emotional and/or reasoning abilities rather than age or grade level, the child is consistently able to do all of the following: Identify the correct medication (example: such as color, shape). Identify the purpose of the medication (example: why take this medication). Determine the correct dosage is being given (example: number of pills). Know the time the medication is needed during the day (example: before/after

lunch). Able to describe what will happen if the medication is not taken (example:

unable to complete schoolwork, trouble breathing). Refuses to take medication, if child thinks something listed above is not correct.

If the child is determined to be self-directed, the school health aide or principal (or his/her designee) may give the child medication. A licensed nurse will instruct the school health aide and principal (or his/her designee) about the child’s medication, the proper dose, and when it is to be taken and possible effects.

Administration of Medication on Field Trips

Please adhere to the following procedure when planning for any student receiving medications to participate in a school sponsored activity off-school grounds:

1. Self-directed students: The nurse will instruct teacher or appropriate school staff accompanying

students regarding the administration schedule, action, and possible side effects of the medication.

Place medication in a single dose medication envelope properly labeled with:- Student’s name- Name of medication and dosage- Date and time to be given

Record information appropriately on student’s medication and health record.2. Not Self-directed students: The parent or guardian may attend the activity and administer the medication.

The parent may personally request another adult who is employed by the school to voluntarily administer the medication on the field trip. In this case, a written request needs to be submitted from the parent to the CSD.

The student’s health care provider can be consulted and may order the medication time to be adjusted or the dose eliminated. If no other alternative can be found, a licensed professional (i.e. RN, LPN, or MD) must administer the medication. A child may not be prevented from participating in an educational activity, such as a field trip, solely on the basis of a special health need.

For the most efficient use of the Health Office Staff, please continue to handle minor problems. The health office does want to be involved in situations requiring nursing intervention (new medications, suspected serious injuries, new medical problems, seizures, and medical transportation).

If you have any questions about what is an appropriate referral, please talk to the health office.

HEALTH SERVICES TO SCHOOL STAFF:

With the limited resources the health office is unable to provide health services for school staff.

School staff should consult with their personal physician regarding any health concerns.

If a staff member is injured at school, they should complete an incident report. These forms are available in the Main Office. Please consult with your physician regarding medical follow-up if needed.

In the case of a medical emergency, health office staff will assist with emergency care until an ambulance arrives.

The following procedures should be followed if a staff member is exposed to blood or body fluids.

CONTACT WITH BODY FLUIDS

Urine, feces, and vomit should be cleaned up safely by the custodian to prevent the spread of disease. Gloves should be worn when having contact with urine, feces, or vomit.

THOROUGH HANDWASHING WITH LIQUID SOAP (found in designated areas) AND WARM WATER AFTER HANDLING BODY FLUIDS IS ESSENTIAL.

DO NOT USE BAR SOAP, IT BREEDS GERMS.

MAINTENANCE OF BATHROOMS IS DONE BY THE CUSTODIAL STAFF.

The custodial staff cleans spills of any type.

Clothing soaked in blood should be handled as little as possible. Refer student with blood on their clothing to the Health Office. Their clothing will be placed in a sealable, leak proof plastic bag, labeled “biohazard precautions” and given to them to take home for laundering.

CONTACT WITH BLOOD EXPOSURE Definition: contact with blood or other body fluids in an open area of your skin

or in a mucous membrane.

Immediate follow-up:1. Wash the affected area immediately with soap and water after direct contact

has ended. Notify the Principal/Assistant-Principal immediately.2. The Principal/Assistant-Principal will complete the “Blood and Body Fluids

Incident Form” with you.3. Contact CoMed (Wilson Medical Center) for follow-up. They will evaluate

your exposure with you and provide treatment and follow-up. They are aware of the most recent recommendations from the US Public Health Service. It is very important that you report any blood exposure immediately in order to receive immediate and thorough counseling and treatment.

ADMINISTRATIVE POLICY REGARDING USE OF “911” EMERGENCY SYSTEM

The following administrative policy shall be observed in school buildings in the use of the “911” system to summon police, fire or medical assistance in emergency situations.

1. A staff member may use internal school telephones to summon police, fire or medical assistance by dialing “911”, ONLY when the staff member observes circumstances which present clear, immediate and substantial danger to the life, safety or health of one or more persons in the school building or on school grounds, which requires the assistance of such emergency personnel in order to be contained or treated.

2. The School Principal or his/her designee shall be notified BEFORE a “911” telephone is placed or, if that is not practical under the circumstances, as soon as practical immediately after the call has been placed. Notification of the placement of a “911” telephone call shall be made by the staff member who made the call or his/her designee.

3. Internal school telephones shall not be blocked or locked so as to be incapable of making telephone calls to “911” emergency services.

4. Use of internal school telephones to call “911” emergency services in violation of this policy may result in discipline subject to the provisions of the applicable bargaining unit’s collective bargaining agreement.

School CommitteesSchool Committees are teams of adults who collaborate to establish expectations and goals that align with the School Mission Statement.

These committees solicit, consider, and value the input of each team member. No individual will be allowed to dominate the discussion, nor will one individual be expected to carry the workload of the team. A facilitator who will meet with administrators to share ideas, concerns, and gain support for initiatives will lead each committee.

The committees work towards consensus. Committee members will support a decision once it is clear that there is a consensus.

STAFF IDENTIFICATION CARDS

All staff are required to wear the Rochester City School District identification badge while on work time. This procedure is mandatory and it is your “keyless” entry to the building. In addition, the ID badge helps to identify school employees to visitors in our building.

All lost or stolen cards must be reported immediately.

ID badges can be obtained at Central Office from 8:00am to noon and from 1:30pm to 3:30pm. The guard at the front desk will direct you on where to go once you arrive. They can also be obtained at The Service Center at 835 Hudson Avenue from 5:30am to 8:30am, building #2, third awning on the right. There is a security operations center sign at the door. If the badge is not working or damaged then please bring badge with you, if the badge was lost more than once then there will be a $5 charge.

Telephone UsageIn an effort to meet the most essential needs of our school, the following procedures are expected to be followed. Assistance and cooperation from all staff is requested.

1. Messages will be directed to voicemail during the instructional day. No phone calls should be sent directly to classrooms during instructional time unless the office has been previously notified.

2. EMERGENCY CALLS from family members or Dr.’s offices, etc., will be put through to the classroom. If there is no answer, every effort will be made to locate you by acquiring the assistance of an administrator.

3. Current phone books will be available in the following locations for your use: faculty lounge, main office and library.

4. Important district and school extension numbers will be provided yearly for all staff please refer to this information for making calls.

5. Cell phones usage is prohibited unless an emergency or school related in the school building.

EVERY adult is responsible for general supervision of students at any and all times and in all parts of the building.

UNSAFE AND UNDESIRABLE BEHAVIOR ON SCHOOL PROPERTYSHOULD BE STOPPED IMMEDIATELY BY ANY ADULT.

JOHN WILLIAMS SCHOOL NO. 5 DISCIPLINE POLICY

The purpose of this school policy is to establish and enforce discipline at School No. 5 in order to ensure a pleasant and safe environment. This policy will ensure consistency. Students will know that appropriate behaviors are expected in all areas of our school.

This policy is to be enforced by all adults in the building.

Parent involvement remains vital to ensuring safe schools. Parents must find no excuse for children solving problems with abusive language or physical force. As primary role models, parents must demonstrate respect for each other; use reasoned discourse, and honor legitimate, non-violent authority.

PARENTS ARE EXPECTED TO SIGN A WRITTEN STATEMENT, ACKNOWLEDGING THAT THEY HAVE READ, UNDERSTAND AND WILL ENFORCE THIS POLICY.

The Classroom Management of Discipline Problems

Behavior problems that occur in classrooms/on school grounds are handled by staff members present and in charge of students at that time. Collaborative effort among school staff will be used to ensure consistent resolution of problems. Problems will be identified and consequences will be clearly explained to students.

The following steps will be taken to address behavior problems:STEP 1 & 2 Disciplinary Referral - Minor

a) Incident/behavior is discussed.b) Teacher/staff gives verbal reprimand.c) Teacher/staff gives consequences.d) Behavior Contract is completed with student. Note: Each participant

receives a copy of the Behavior Contract. Teacher requires the student to post in a visible, but private location for the student to review. Encourage the parent to review at home, as well.

In School Suspension

September 2013

Dear Staff,

The purpose of this letter is to formalize in writing the procedures as they relate to our new alternative to suspension room. Listed below are the operational details of how the program will work:

If the Principal determines that a students behavior, would have in previous years, resulted in a suspension then that student will be considered for the ISS.

An administrator will contact the family and discuss the behavior as well as our alternative to suspension program as a consequence.

Once the parent has been notified, the student will be assigned to the ISS and the classroom teacher will be notified of the number of days.

When a student is assigned to the ISS it is the responsibility of the teacher to provide the ISS staff with a copy of the assignments for the day prior to the start of school.

The ISS staff will call the classroom teacher and report attendance on a daily basis. Classroom teachers remain responsible for recording attendance.

The ISS staff will communicate with each classroom teacher prior to the student re-entering the classroom.

The Principal is the only person who is able to make a referral to the ISS.

I will serve as the administrative liaison to the ISS this year. If you have questions or concerns I ask that you speak with me directly.

SECOND CHANCE INTERVENTION ROOM

The Second Chance Intervention Room is a location to provide students with a safe place to analyze their feelings with appropriate adult engagement. Students should remain in the room for 20 minutes or less.

Guidelines are as follows:

Teachers should send the student to the room with a referral/fast pass ONLY AFTER EXHAUSTING ALL OTHER APPROPRIATE STRATEGIES TO MAINTAIN THE STUDENT IN THE CLASS.

Stage 1: Handle in the classStage 2: Send to another class for support (Buddy Class)Stage 3: Second Chance Intervention Room

Each Grade Level will provide a file folder of meaningful student work quarterly to be housed in the Second Chance Intervention Room.

Students are encouraged to arrive in the room under their own power.

Teachers will call to let the Second Chance Intervention staff know that a student is on his/her way.

Calling home should not be the responsibility of the Second Chance Intervention staff. The classroom teacher will have the background on interactions with the various parents/guardians regarding the specific situation. The Second Chance Intervention staff will call if the behavior escalates while in the Second Chance Room.

A record log will be maintained on student, situation, and number of times student is sent to the room. It is not the intention of the Second Chance Intervention Room to be used as a regular form of classroom discipline.

The school wide disciplinary referral process will also be implemented.

There are designated students who will be able to use the alternative to the disciplinary referral process. The students are chosen based on teacher

requests. A student may choose to use the FAST PASS to avoid a crisis for a 15-minute break.

The FAST PASS is a pass that a student chooses as an alternative problem-solving strategy.

It is understood that situations arise in which students (and teachers) need to be apart from the group or individual to prevent a problem. This is where a student would use a FAST PASS. The Second Chance Intervention Room can be used in this way. It does not need to be a written referral. However, a teacher will need to call first to ensure that there are not already several other students in the Second Chance Room in crisis. The time for separation will need to be specific and limited (15 minutes).

The FAST PASS may be used for additional reasons such as:

DISRUPTION OF THE EDUCATIONAL PROCESS

WHEN A CHILD NEEDS A 10-15 MINUTE TIME-OUT

STUDENT NEEDS A QUIET PLACE TO DO HIS/HER WORK.

CHILD IS UPSET OR ANGRY AND NEEDS TO GET OUT OF THE ROOM.

STUDENT NEEDS TO TALK TO SOMEONE.

The FAST PASS is different from a referral because it encourages students to use their own problem-solving skills and to make good choices, even if you have to suggest using the FAST PASS. This gives the child ownership of problem.

CLASSROOM DISCIPLINE POLICYIn order to further a more extensive common language in our school, there is a recommendation that every classroom teacher post the same basic classroom rules. These rules are aligned with the #5 School Creed. Each class can define the specifically. They are as follows:

School-Wide Student SupervisionThe way our students enter and exit the building and how they are supervised while in the hallways is critical for maintaining an orderly, productive school environment.

This is the responsibility of all employees who work at #5 school. To ensure that student safety is apparent, high visibility of all adults at the beginning and end of the day is required. Special subject staff and support staff will be assigned to support students during the beginning and end of each day.

Classroom teachers are required to stand at the door and/or hallway to greet students EVERY MORNING!

In addition, the monitoring of students during the day helps to enhance the school climate.

1. The Uniform Committee will select days to have the 100% Uniform Challenge. During morning announcement the principal will announce that it is “Uniform Challenge Day”. If a class has 100% of the students in uniform the teacher will send a notification slip to the main office indicating that all students in the class are in their school uniform.

2. All classes who win the challenge will receive an award that could include a snack, token, lunch with principal/assistant principal in the classroom or free time in the gym.

GOALS and PRINCIPLES of BOARD POLICY on ELIGIBIITY

I. The Board of Education has the authority to establish reasonable standards and eligibility

requirements as prerequisites for eligibility for co-curricular, extracurricular and

interscholastic athletic activities. These standards apply to entry qualifications as well as to

continued participation in such activities.

Eligibility requirements include academic standards, behavioral standards, and training

standards that are to be met by all students participating in extracurricular activities, including

athletics. These requirements are to be applied equally to all student participants.

The Board is committed to promoting academic achievement for all students and motivating

students to achieve to their highest potential. In recognition of the high cost and impact of

dropping out of school on the student drop-out, the student's family, and the greater

community, the Board is further committed to promoting the highest level of student retention

and graduation possible academic achievement for all students. Accordingly the Board is

committed to eligibility standards for extracurricular activities, including athletics that

promote and support academic success student retention leading to graduation. It is the Board's

expectation and directive that the RCSD in the administration of these eligibility requirements

will implement them only in a manner to advance the BOE's stated commitment to always

promoting academic achievement for all students.

The Rochester City School District has gone to great length to insure that every child has extracurricular opportunities at our schools. Schools and the athletic departments are eager to provide sports to every child within the Rochester City School District.

Please read the attached information on how to sign up for teams, get free physicals and get requaled (cleared) so that students will be able to participate in Modified Sports. Every student must be requaled (cleared) by the nurse to participate.

SHIFTS IN MATHEMATICSShift 1 Focus Teachers significantly

Teacher Improvement Plan (TIP)

The NYS Commissioner’s Regulation (30-2.10) requires that any teacher with an annual professional performance review rated as Developing or Ineffective shall receive a Teacher Improvement Plan. A TIP shall be developed in consultation with the teacher and union representation shall be afforded at the teacher’s request. A TIP is not a disciplinary action. At the end of a mutually agreed upon timeline, the teacher, administrator and mentor (if one has been assigned), and a union representative (if requested by the teacher) shall meet to assess the effectiveness of the TIP in assisting the teacher to achieve the goals set forth in the TIP. Based on the outcome of this assessment, the TIP shall be modified accordingly.

The TIP is used exclusively for those teachers whose annual teacher evaluation composite score is rated “developing” or “ineffective”. The final evaluation must be based on at least one formal observation completed by the Principal during the current school year. The final evaluation includes evidence from all four Domains and encompasses much more than the formal observation (e.g. informal observations, etc.).

A TIP is completed collegially between the teacher whose rating is “developing” or “ineffective” and supervising administrator, preferably the principal. They set professional goals to ensure growth toward improved student outcomes. Working towards this growth in an environment of professional respect is an expectation for all parties.

The TIP should be developed any time after the final evaluation has been completed, but no later than the tenth (10th) day of the new school year. The TIP should be structured around each of the four Domains. TIP goals/activities should be structured so that no more than four-five at a time are addressed. The following should be included on the TIP:

All participants in the TIP meeting should be listed on the TIP. Periodic follow-up sessions should be conducted to assess the teacher’s progress.
