Jobsite Safety Scorecard Evaluations

The Scorecard System

Scorecard Calculations• The Scorecard is broken out into 9 categories of specific hazards

& control measures in the roofing industry.

• The points for each category will range from 0 to 10.

• Each category starts with 8 points. Additional points can be added for going above minimal standards, and/or points can be subtracted from being below minimal standards.

• The points given in each category will be multiplied by it’s designated weight factor (30%, 10%, or 5%) to develop a category score.

• The nine category scores will be tallied to calculate the cumulative score. The cumulative score will range from 0 to 100 points. A cumulative score of 80 points, or higher, meets minimal standards.

• The jobsite scores are tallied and divided by the number of jobsites visited to determine the member’s average score.

Determining Category Points:• As previously stated: Every category starts off with 8 points –

meaning the jobsite is meeting minimal standards.

• HC&A has had extensive discussions on how to award points for jobsites exceeding minimal standards, and how to subtract points for jobsites falling below minimal standards.

• HC&A has developed a scoring criteria to be utilized as a guideline for adding and subtracting points.

• If no hazard exists for a particular category, the score is automatically increased to a 10. This is based on the golden rule of loss prevention – Eliminating the hazard is always preferred over controlling the hazard.

Awarding & Subtraction of Points

Fall ProtectionIncluding Skylight & Roof Openings

• Guardrails in lieu of Warning Lines & Safety Monitor (+1 GC / +2 Co policy)

• Scaffold Catch Platforms w/ Guardrails & Netting (+1 point CG / + 2 Co policy)

• Personal Fall Arrest outside warning lines (+ 1 Point GC / +2 Co Policy)

• Parapet Wall in lieu of Warning Lines / Warning Lines (+2 Points – Prorated)

• PFAS inspection plan in place (+ 1 point)

• Installation of Skylight cages (+ 1 point)

• Fall Protection Crew set-up job (+1 point)

• Employees continually out of view of safety monitor (up to - 4 points)

• Improper inspection of / use of PFAS (-1 Point per deficiency observed)

• Improper use of horizontal lifelines (up to – 4 points depending upon severity)

• Warning lines too close to edge (up to 2 points). Warning lines not maintained (-1 point non work area / - 2 points work area

• Roof opening not covered / marked / secured – (-1 point per opening / 3 point max)

• No guardrails around hoists, conveyors, hot pipes (- 2 point per exposure)

Complete lack of fall protection– Automatic 0 & notification to LC Committee.

Material Handling

• Team lifts (+1 point)

• Mobil equipment / 4 wheel carts (+0.5 Points)

• Motorized Carts (+ 1 point)

• Employee Stretching / Exercise Program (+ 2 points)

• Mechanical Hoists in lieu of Hand Derrick (+ 1 point)

• Individual lifts (not adjustments) of heavy / awkward materials / equipment (- 1 point)

• Materials carried up the ladder (- 2 points)

• Improper counterweight in derricks (- 1 point)

• Damaged Rigging (up to -1 point per sling / chain)

Ladders & Scaffolds

• Access to project via interior elevator / staircase (+ 2 Points)

• Scaffold Tagging Program (+1 GC / +2 Co Policy)

• Aerial Lift Training Certificates available for review (+1 Point)

• Railing on stairs (change of elevation great than 19”) (1 point for Co)

• Use of Rolling staging vs. working from ladders (+1 point)

• Ladder not used in accordance with original intent (-1 point per ladder / max 3 points)

• Ladder not tied off / secured against displacement (-1 Point per ladder / max 3 points)

• Ladder not extend three feet / hand hold at top (-1 point under 3 feet / -2 points under 2 feet)

• Scaffold not inspected daily (-1 point no deficiencies to – 5 points for noted deficiencies)

• Foreman unaware of who is controlling scaffold system (-1 point)

Personal Protective Equipment

• 100% Hardhats & Safety Glasses (+1 point each = total of 2 points – entire crew)

• 100% Hardhat & Safety Glasses (+1 for GC policy – entire crew)

• Use of gloves for SM crew / laborers (+1 point – entire crew)

• Kettle Operators Leather aprons / smocks/ boot covers (+1 point)

• Supply of PPE readily available on site (+ 1 point)

• Lack of Hardhats w/ overhead exposures (- 1 point per EE / 3 point max)

Housekeeping

• Continuous transporting of debris into dumpster (- 1 Point)

• PFAS stored in separate gang boxes from chemicals, cutting blades, etc. (+1 point)

• Dumpster Tarped w/ Chute (+ 1 point)

• Documented daily jobsite safety inspection by foreman (+ 1 point)

• Debris stock piled on roof / not removed at end of shift (-1 point)

• Clutter near ladder / staircase landings (-1 point)

• Poor access (walkways) to jobsite (-1 point to –3 points)

• Food / drink garbage on roof (-1 point)

• Poor / Sloppy Storage of Equipment ( -1 point to – 3 points)

Electrical

• Assured Grounding Program (+1 point / + 2 points w/ GFCI)

• Documented GFCI inspection program (+1 point)

• Overhead power lines de-energized (+ 2 points)

• Overhead power lines insulated by power company (+ 1 point)

• No GFCI or Assured Grounding Program (- 1 points)

• Damaged Electrical Cords (0.5 points not in use to / -1 point per cord / -5 point max)

• Work within 10 feet of unprotected warning lines (- 1 point < 300 volts / -2 points > 300 volts)

Fire Prevention / Material Storage

• Flammable Materials removed from project at end of day (+ 1 point)

• Fire Extinguishers highly visible (+ 1 point)

• No Smoking on roof (+1 point GC policy / +2 Points Co Policy)

• Failure to secure materials down at end of shift (- 1 point)

Emergency Preparedness

• Name, Address, & Phone # of Local Occupational Health Clinic onsite (+ 1 point)

• Crewmember having current Basic First Aid / CPR certification (+ 1 point per person)

Captive Goals for 2006:

1. Consider the use of a guardrail system in lieu of Safety Monitors. 2. Ensure all employees have received comprehensive training on the roles &

responsibilities of the Safety Monitor. HC&A has a downloadable training presentation available on our RCCS, LLC Loss Control Services webpage.

3. Consider purchasing a pre-engineered Horizontal Lifeline System. Again, talk with other members to see what they use & how well they work.

4. Provide proper stairs or ladders for changes in elevations greater than 19 inches. At the same time, stop using sections of cut-up ladders. This is not how the manufacturer intended them to be utilized.

5. Do not leave fall protection systems behind for other trades, unless contractually obligated. If you do leave your fall protection systems in place, make sure they meet minimal OSHA Compliance.

6. Strictly enforce the “NO SMOKING” Policy when working with flammable materials.
