Session Overview and Speaker Biographies KEYNOTE ADDRESS BY OSHA Ken Atha, OSHA V Regional Administrator – overseeing all OSHA programs within Illinois, Indiana, Michigan, Minnesota, Ohio, and Wisconsin – will share insights on the Agency’s current activities.

Ken Nishiyama Atha Ken Nishiyama Atha has been a member of Occupational Safety and Health Administration’s (OSHA’s) Senior Executive Service for over ten years and currently serves as Regional Administrator in Chicago. He leads a diverse team of professionals assuring safe and healthy workplaces and work-related benefits and rights. He has over 32 years of Federal service that includes various positions throughout OSHA, the United States Air Force, and the Air National Guard and is a retired veteran. Mr. Atha holds a Bachelor's of Science Degree from Mount Senario University, Wisconsin, an Associates in Applied Science Degree in Safety, and is a graduate of the Federal Executive Institute.

Governor’s Awards for Contributions in Health and Safety Presented by Joe Beyer, Acting Director, Illinois Department of Labor Joe Beyer In April 2017, Governor Bruce Rauner designated Joe Beyer as Acting Director of the Illinois Department of Labor (IDOL). Joe has served in numerous roles in both the public and private sectors over the past 10 years. From 2015 to 2017, Joe was a member of the Governor’s policy and operations teams. During this time, he worked closely with a number of State agencies, including Central Management Services, Veterans Affairs, Historic Preservation, and the Abraham Lincoln Presidential Library

and Museum. He also played an extensive role in large-scale projects such as comprehensive procurement reform and the disposition of the James R. Thompson Center. From 2007 to 2010, Joe spent three years at Aon Hewitt (formerly Hewitt Associates) as an actuarial associate, providing pension consulting services to clients. During graduate school, he completed internships at Deutsche Bank, William Blair, and the Federal Reserve Bank of Chicago. A native of Chatham, Illinois, Joe received a B.S. in Actuarial Science from the University of Illinois at Urbana-Champaign, a J.D. from Harvard Law School, and an MBA from Harvard Business School. He currently lives with his family in Chicago.

A WORKPLACE TRAGEDY In August of 2015 there was an accident resulting in the fatality of a 20 year old Logger in Woodlawn, Illinois. Steven Lachenmayr, Owner of Sawyer Tree Service, will talk about this incident, the ensuing life changing ramifications, and impart a first-hand perspective for the overall benefit of the event’s attendees. Steven Lachenmayr Steven Lachenmayr first went to work for Marathon Oil Company in 1975 as a Roustabout working his way through the trades as a Field Technician and eventually a Driller on workover and production oil rigs. Mr. Lachenmayr holds additional experience in both Marathon Pipeline and Refining Divisions as a Delivery Gauger, in Coker and Waste Water unit operations, and plant fire brigade/emergency response capacities. Mr. Lachenmayr operated a tree service business and currently runs a horizontal drilling operation which provides fiber optic cable needs in Illinois and Indiana.

SILICA OVERVIEW This session will focus on the functional aspects of OSHA’s new silica standard for construction including: health effects of silica containing dusts, a synopsis of the regulation, dust and exposure control methods, an overview of available reference materials, and a practical insight into engineering controls and work methods which can translate to the job site.

Eric Olson Mr. Olson has been with Western Construction Group since 2010 and currently serves as the company’s Safety Director. Prior to joining Western, he served as Safety Director for an electrical contractor, a safety professional at a major oil refinery, and an assistant director for a heavy highway contractor. Over his career, he has worked exclusively in the safety, environmental, and risk management fields with special emphasis on safety and risk mitigation and loss prevention methods. Mr. Olson holds a Master’s of Science in Industrial Hygiene and a Bachelor’s of Arts Degree in Biology with a Minor in Safety from Central Missouri State University. Mr. Olson is a Certified Safety Professional and a Professional Member of the American Society of Safety Engineers and the American Industrial Hygiene Association. He is actively involved in the Associated General Contractors of St. Louis.

FALLING OBJECT HAZARD PREVENTION, CONTROL, AND MITIGATION In addition to increasing awareness, this course will highlight best practices and provide attendees with tools for preventing, controlling, and mitigating against falling objects. This course will address regulatory requirements regarding falling objects, with a focus on programs proven in industry to protect our workers. We will discuss the importance of integrating falling object prevention into daily work practices beginning with worker training, job planning, work execution, and demobilization. The course will also address the reporting of falling objects and the value of risk based investigations to prevent recurrence. Gary Hartman Gary Hartman, CSP, OSHT, is the Health and Safety Team Lead for the WRB Refining Wood River Refinery in Roxana, IL, where he has worked for over six years and has over 10 years of refining Health and Safety experience. Gary has a bachelor’s degree in Safety Sciences and holds two professional safety certifications. He is also a United States Marine Corp Infantry veteran with industry experience as a machinist apprentice, injection mold line operator, and five years as a tree trimmer.

EMERGENCY MEDICAL RESPONSE TEAM – DEVELOPMENT & MAINTENANCE Whether it is a one person remote work team or a worksite with a thousand people first-aid is a necessary component to quickly address an accident when all of our best safety and health management systems fail. This breakout session will walk you through the creation, development, certification and sustainability of a first class, state certified EMR team and the value they bring to their fellow employees and their organization. Mark Maue Mark Maue is an Electrical Instruments and Controls Technician for Prairie State Generating Company, LLC at its Power Generation station just outside Marissa, IL, and also is a member of its volunteer Emergency Medical First Responder (EMR) team. Mr. Maue has been an Illinois State Certified Emergency Medical Services (EMS) First Responder (EMR) for eleven years and an Illinois State Certified EMS Emergency Medical Technician-Basic (EMT-B) for five years. Mr. Maue currently holds the rank of Assistant Chief with the Santa Fe Fire Protection District (Village of Bartelso) and has earned Basic and Advanced Firefighter Fire Service Certifications from the State of Illinois Office of the State Fire Marshall (OSFM). He will complete his requirements for his OSFM Fire Officer I certification in the near future.

OSHA PERSPECTIVE ON MACHINE GUARDING Continuous modifications and integration to workplace equipment, machinery, and processes increase the potential for employee injury; but these also provide opportunities for innovative approaches to employee protection. This presentation will cover OSHA’s frequently violated machinery safety standards, relevant OSHA interpretations and directives, and applicable consensus standards. Discussion will include: innovative machine guarding approaches; control reliability – what it is, what’s changing and why you should care; and risk assessment/hazard reduction vs OSHA compliance – how they are two sides of the same coin.

Aaron Priddy Aaron Priddy is the Area Director of the OSHA Fairview Heights Area Office. Prior to his tenure as an Area Director, Mr. Priddy was the Assistant Area Director of the Cleveland Area Office, OSHA Region V Machine Guarding Coordinator, and Compliance Officer with the Cleveland Area Office. Mr. Priddy has been with OSHA for 17 years. Prior to his tenure with OSHA, Mr. Priddy managed safety and health programs for a contract maintenance aviation firm, and provided consultation services through the North Tennessee Private Industry Council.

FALL PROTECTION AND WHY IT IS IMPORTANT Alex Green has lead safety at several multi-billion dollar construction project sites. Mr. Green’s talk will walk you through his personal experience with two specific tragedies he was involved in related to fall protection and relate these back to the importance of task-by-task fall protection basics. Alex Green Alex Green is a Power Plant Safety Specialist for Prairie State Generating Company, LLC located in Marissa, IL. Alex holds several professional certifications in the ES&H field and is currently working to complete his Safety Management Specialist (SMS) certification through the Board of Certified Safety Professionals. Mr. Green is also the power plant’s Emergency Response Team (ERT) Coordinator, a certified General Industry outreach trainer, and holds memberships with the NFPA and the St. Louis Chapter of ASSE. Prior to joining Prairie State, Alex worked 14 years for Bechtel Corporation as a Construction Coordinator and ES&H Project Manager where he gained a wide range of experience as well as earning various Competent Person designations. Alex is a U.S. Navy veteran with 30 years of experience as a welder, fabricator, and mechanic.

FALL PROTECTION AWARENESS In this session, Process Marketing Group will also provide a primer regarding the applicability of OSHA standards, the calculating of fall distances, available fall protection products on the market, how to properly don and wear a harness, inspection of personal fall arrest system components, and the importance of rescue protocols in the event that a fall arrest should occur. Jay Petry Jay Petry is President of the Process Marketing Group which provides safety solutions to construction, industrial, and utility organizations throughout the Midwest. As a manufacturer’s representative for over 21 years along with Process Marketing Group’s recent partnership with Guardian Fall Protection, Mr. Petry is knowledgeable in evaluating fall protection programs, conducting facility surveys, and making recommendations for proper fall protection equipment in all applications.

EXCAVATION AWARENESS Trenches and excavations pose serious hazards to various trades. In this session, an OSHA representative will disseminate: OSHA’s National Emphasis Program (NEP), commonly cited 1926 Subpart P standards, an overview of excavation hazards, soil classifications including specific benching/sloping measures, available resources, and an overview of two fatality events investigated by the Fairview Heights Area Office. Dustin Miller Dustin Miller is a Compliance Assistance Specialist (CAS) with the OSHA Fairview Heights Area Office. Prior to his tenure as a CAS, he served as a Compliance Officer. Mr. Miller has been with OSHA for 15 years. Prior to his tenure with OSHA, Mr. Miller held previous positions at a centrifugal cast foundry/machine shop operation and industrial construction at a petrochemical facility. Mr. Miller holds a Bachelor’s of Science in Safety Management and a Master’s of Science in Human Resource Development from Indiana State University along with OHST and CHST certifications.

IMPLEMENTATION OF A BEHAVIOR BASED SAFETY (BBS) PROGRAM This presentation will provide an overview of the Marathon Petroleum Illinois Refining Division’s BBS program from conception through implementation. The functional administration of the BBS program and the continuous improvement process – which has led to program accreditation – will be reviewed. The presentation will also share some of the early pitfalls and how those hurdles were overcome. Representatives from Marathon Petroleum Illinois Refining Division will discuss their BBS personnel structure, data collection and reporting, management of turnarounds, and some of the other unique tactics taken on by their BBS Team. Levi Baker Levi Baker has 22 years of machine shop experience and has been a Machinist with Marathon Petroleum Illinois Refining Division for the last eight years. Mr. Baker served as a member of the refinery’s BBS Steering Committee Team for three years before recently stepping into the dedicated role as a BBS Coordinator. Mr. Baker has worked internally with work groups to create specific BBS criteria and with the refinery’s IT department to increase effectiveness of the BBS data collection and user interfaces. Upon completion of his 2-1/2 year term as the BBS Coordinator, Mr. Baker will return to his chosen profession as a Machinist. Eric Nickless Eric Nickless led Marathon Petroleum Company’s Illinois Refining Division as the Behavioral Based Safety Coordinator from 2013 through 2016. During Mr. Nickless tenure he spearheaded many process improvements and worked directly with behavioral specialists to assist them in obtaining a worker’s perspective. From his efforts in the field of BBS, he has been appointed as a Commissioner with the Cambridge Center for Behavioral Studies. This appointment includes site visits to facilities that have applied for BBS accreditation with the Cambridge Center for Behavioral Studies and to create recommendations to improve their BBS processes.

LACK OF ATTENTION TO EQUIPMENT TESTING & INSPECTION: A DEADLY CHOICE The purpose of this presentation is to detail hazardous industrial process conditions associated with inadequate equipment inspection and testing that are frequently encountered by OSHA. Organizational causes of these conditions will be discussed. Potential complications associated with state and local jurisdictional oversight that often gives employers a false sense of well-being will also be discussed. The reasons for exponential concern as aging equipment is operated past its originally intended design life will be disseminated. James Nelson James Nelson is a Mechanical Engineer with the OSHA Salt Lake Technical Center’s Health Response Team. Over the past 40 years, Mr. Nelson has pursued a career of system and component design, operation, diagnostics and modification, in the power, petroleum and chemical industries. Serving as Engineering Manager or Technical Services Director in each of these industries, Mr. Nelson has been responsible for development and administration of owner/user inspection and R stamp programs. Equipment monitoring and failure assessment have been a central part of Mr. Nelson’s career. For the past 6 years, Mr. Nelson has provided support to OSHA and state plan enforcement offices throughout the country in the technical assessment of catastrophic process failures.
