Each track will focus on the state-of-the-art in each discipline and will include presentations and discussions by the current experts in their respective fields. There will also be industry sponsored breakfast and lunch symposia, as well as ample opportunities to meet and greet your colleagues and friends. We are very thankful that the timing of our annual meeting did not coincide with the recent floods and that life in Prague is now returning to normal. A week ago, the city was experiencing record rainfall, the waters of the Vltava River were cresting and a state of emergency was declared in the Czech Republic. We are grateful that the waters have receded, the anti-flood barriers through- out the city have protected the city, and the historical centre is intact for our annual meeting. I hope that you will take the opportunity to visit this wonderful city and experience its unique and ancient traditions. From the Prague castle, originally the residence of princes and kings of Bohemia, to Stromovka Park, once a royal hunting park, now a beautiful place to walk, run, or relax amidst the blooms, - please take time to enjoy the culture and charm of this European city. In addition to the sights and glories of this historical city, I know you will enjoy the outstanding scientific program that has been developed by the Program Committee, under the direction of Chairpersons Niv Ad, MD, Bob Kiaii, MD Franca M.A. Melfi, MD., Daniel L. Miller, MD. The ISMICS Annual Meeting has always been the forum for presenting the latest innovations in our field and this year’s meeting will present and review new technologies, techniques, and ideas that are changing our specialty. Coronary artery bypass grafting (CABG) has been regarded as the treatment choice forunprotected left main (LM) coronary artery disease. However, recent clinical guidelines haveupgraded PCI to a viable alternative in selected LM patients.1,2 PCI offers tangible short-termadvantages over CABG as it is less invasive, reduces hospitalization duration and avoids thedisability of surgical recovery.

Hybrid coronary revascularization (HCR) may represent a valuable alternative to bothtraditional CABG and unprotected LM stenting. Combining the durability and survival advan-tage of the LIMA-LAD graft with less-invasive PCI using drug-eluting stents (DES) to treatnon-LAD vessels may reduce some of the morbidity associated with CABG and reduce theincidence of repeat revascularization associated with an all-percutaneous strategy.

HCR was introduced at Emory University in 2003. Since that time, it has come to com-

prise 20% revascularization procedures for patients with multivessel disease. Since 2009,LIMA harvesting has been performed with robotic assistance. The anastomosis is performedmanually via a 3-4cm non-rib-spreading anterolateral thoracotomy incision under direct vi-sion without cardiopulmonary bypass.

All HCR patients had a minimally invasive LIMA-LAD bypass performed before or con-comitant with PCI. The majority of lesions involved the distal LM (15/27). The majority of PCIprocedures were performed with DES (92.6%). With the exception of 1 patient, the LM wasstented into the circumflex coronary artery. The average SYNTAX score of HCR patients was23.1.

In-hospital outcomes, were similar between treatment groups, with the exception of theneed for blood transfusion, which was reduced with HCR (p = 0.01). Two patients in the HCRgroup required repeat PCI (1 in-stent restenosis, 1 progression of native disease), and 1OPCAB patient who required repeat PCI (saphenous vein graft) at a median follow up of 3.2years. No patients required reoperation.

Since this publication, HCR has been performed in an additional 20 patients with excel-lent results. The most suitable anatomy for left main hybrid approaches are patients witheither distal left main lesions extending into the bifurcation or any left main lesion plus proxi-mal LAD disease in addition to other lesions. This eliminates concerns for competitive flowbetween a percutaneously treated left main lesion and the LIMA-LAD graft.

In summary, HCR represents a safe and effective strategy with equivalent in-hospitaloutcomes for carefully selected patients with LM coronary disease. Further long-term studieswill help identify the optimal patient population in which this novel approach can be applied.References:1. identified Prague as a venue immediatelyafter our Berlin Meeting in 2010, andwhile it hardly seems that three yearshave passed, here we are gatheredtogether for ISMICS 16th Annual ScientificMeeting.

As I write this message, I am de-lighted to inform you that our pre-registration as of June 6th is 497, with atotal of 447 Professional Pre-Registrants.This is our second highest professionalmedical pre-registration in ISMICShistory, only behind 2005 in New York.Over the past 5 years we have seenISMICS attendance continue to increase,with last year’s Los Angeles meetingsetting our all-time West Coast recordwith 534 professional medical attendees.With a few hundred industry partners alsoparticipating this year, we anticipate totalattendance of over 800 at this year’smeeting.

2012 into 2013 has seen ISMICSmove ahead with implementation of itsStrategic Plan, which called for expansionof scientific programming to include allforms of innovation in surgery, tech-niques, and technological advances.Abstract review criteria was tightened toask reviewers to consider if the contentsubmitted was “ISMICS” in nature – inother words, minimally invasive, orinnovative, or forward-thinking. There istremendous commitment on the part ofyour ISMICS leadership to keep thesociety true to its origins and not have itlose its non-traditional and cutting edgepersonality. This year’s Program Chairson the Cardiac Side, Drs. Niv Ad and BobKiaii, split the Program Committee intotwo distinct onsite review groups so thatmore in-depth analysis of papers could bemade. And our Thoracic Chairs, Drs.Daniel Miller and Franca Melfi, reviewedthe thoracic papers to ensure that theytoo remained at the forefront of what isrelevant in innovative and minimallyinvasive thoracic surgery. I am sure youwill find the content in both the cardiacand thoracic sessions to be both highquality and very informative.

Also in an effort to keep things fresh,the format of our Masters Day waschanged – and this year both the morning

and afternoon sessions start off in asingle room with one portion focused onprogramming of interest to all three topic-driven sessions, presented through Stateof the Art Lectures and then “GreatDebates.” Then the sessions split foranother two hours of topic-specificprogramming. And on Wednesdayafternoon ISMICS also welcomes apartner in educational programming in thefirst-ever offering of a joint ISMICS/EACTS course on Thoracic RoboticSurgery. Friday morning at ISMICS willfeature a “Live on Tape” session withthree Live in a Box videos being pre-sented on Transapical Valves, Mini-MitralValves, and Ablation. And finally,Saturday morning ushers in two morenew program concepts, shortenedpresentations and mini-cardiac debates,as well as a Thoracic “Movie Day.” Wehope that you can attend these newsessions, and look forward to yourfeedback post-meeting.

The Estech COBRA Fusion is intended to ablate cardiac tissue during cardiac surgery using radiofrequency (RF) energy when connected directly to the Estech Electrosurgical Unit (ESU). The Estech COBRA Fusion may be used for temporary cardiac pacing, sensing, recording, and stimulation during the evaluation of cardiac arrhythmias during surgery when connected to a temporary external cardiac pacemaker or recording device. Puskas of Emory University, focused on“OPCAB/Off-Pump Surgery.” Dr. DavyCheng of London Health Sciences Centrein Ontario, Canada served as primaryresearcher and biostatician. The Consen-sus Conference was held last month atThe Shelbourne Dublin Hotel in Dublin,Ireland on 25-27 May 2013.

ISMICS has assembled this interna-tional panel of experts to share theirknowledge and expertise and to reviewthe current literature as related to OffPump Coronary Artery Bypass Surgery.

The main objective of the conference wasto determine whether off-pump coronaryartery bypass surgery (OPCAB) issuperior to conventional on-pumpcoronary artery bypass surgery (CCAB)with respect to clinical outcomes andresource-related outcomes, and todetermine whether effects differ acrosspre-specified risk groups and settings.

The primary outcomes of interest aredeath, stroke, AMI, RBC tx and need forreintervention, both in the short term andlong-term follow-up. Secondary outcomesinvolve all other clinically relevantoutcomes listed above, and resource-related outcomes.Systematic review ofrandomized trials were conductedaccording to recent guidelines set out byCochrane Collaboration, and will bereported according to the PRISMAguidelines. Comprehensive searches ofthe literature were done in Medline,Embase, and Cochrane library.

POSTER COMPETITION GOES ELECTRONICThe ISMICS Poster Competition remains one of the highlights of the Annual Meeting and provides attendees a

unique opportunity to review their colleagues’ research. The Poster Competition is a peer-reviewed session conductedin two rounds. The First Round will be delivered on Thursday from 17.00 - 18.00 in the Congress Hall, with topic groupspresenting concurrently. Over 100 posters will be presented. For the first time, all ISMICS competition and displayposters will be persented in an all-electronic format.

Topics include: Revascularization Robotic, Heart Failure Revascularization, Imaging And Facilitating, Technology ForCardiac Surgery, Valve Aortic, Valve New Designs, Valve Mitral Mini, Thoracic, Aortic And Endovascular Therapies,Arrhythmias, and Perfusion. During these rapid-fire presentations, authors are allowed a maximum of three minutes,with two minutes for discussion. Participants score each presentation on quality of the presentation and significance ofthe research. Semi-finalists are selected for the final round and will be announced on Friday morning.

Final Rounds will be delivered from thepodium in the main scientific session onFriday from 16.30 - 17.30. Presenters areagain allowed a maximum of three minutes,followed by two minutes for audiencequestions and discussion. Judges willreview and the winner will be announced onFriday evening.

As many of you know, last year ourown ISMICS journal INNOVATIONSreceived recognition by the NationalLibrary of Medicine and be included inPubMed/MEDLINE. As the journal is nowindexed, all presenters at the ISMICSAnnual Meeting are expected to submittheir work to the journal, and we areoffering both a pre-Meeting Best Manu-script as well as a post-Meeting BestManuscript cash award. The journal iscontinuing to grow under the efforts of ourEditor in Chief, Dr. Ralph Damiano, and ourManaging Editor, Carol Blasberg. While ISMICS has always had a veryactive Board of Directors, ScientificProgram Committee and NominatingCommittee, through the years othercommittee activity was sporadic. Thisyear we have had several very engagedcommittees, focused on Membership,Electronic Content and Technology, andInvestments. Each of these committeeshad participated in conference calls,taken action, and presented reports andrecommendations to the Board ofDirectors. Sincere thanks to the Chairsand committee members for yourcommitment to moving ISMICS forwardand offering your time and insight.

ISMICS also remains committed tobeing an inclusive and welcoming society.We hope that many of our non-memberattendees here in Prague will makeapplication to join the Society while youare here. Each year as a write this message, Ireflect on what an honor it is for me to

serve as your Executive Director. I thinkof the many Presidents I have had theprivilege to work with over the years, andhow grateful I am that you remainengaged and involved with ISMICS. I amalways impressed by the commitment ofthe Board of Directors, and your willing-ness to give so freely of your time to ourSociety. I sincerely thank Dr. AlanMenkis, this year’s President, for yourstewardship of ISMICS over the pastyear, as well as our many talks andmeetings in so many places around theworld over the past 12 months. It hasbeen an absolute pleasure.

While I am your Executive Director, Icould not do what I do for ISMICS withoutmy amazing team.

From weathering (literally) floods toplanning numerous events across andocean and continent, and then handlingjust about everything in between, theyhave simply been outstanding.
