Introduction Approach and principles Integration of the health, well-being and social care system on the Isle of Wight requires collaboration across boundaries. The approach used in developing this programme structure is to focus on person-centred outcomes rather than individual organisations’ activities. In doing so, it recognises that a key strength of My Life A Full Life is its collaborative and inclusive working across the Island. In developing the programme structure and workstreams that will deliver the programme, we have categorised them into two types working with each other in a matrix: a) Person centred work streams: focused on directly delivering care and well-being outcomes b) Enabler work streams: supporting and cutting across all person centred work streams In order to structure work streams set out on the following page, we have used the following principles and met with key stakeholders to agree this. Workstreams should be cross cutting i.e. across organisational boundaries. Workstreams are centred on delivering key outputs and outcomes and not based on specific organisations. Workstreams should be based on the future care model rather than current projects within organisations. Focus is on strong and broad based representation from organisations across the system: primary care, secondary care, voluntary sector, mental health, local council, CCG, and people representatives (see slide 5). Workstreams should be aligned to development of value propositions focusing on delivering specific care model outcomes and outputs.
