50à80% MAIS neutralise NAC per os ?– <1h post-ingestion si DSI à risque,ou h?
et/ou toxiques carbo-adsorbables
Traitement antidote NAC
• prend la place du glutathion comme donneur de radicaux SH
• initié dans les 10h post-ingestion (Shriner 1997)
• trois schémas 1P.O:EU 2IV:EUR• ingestion<10h: efficacité équivalente• ingestion>10h : P.O ou IV Smilkstein
>Prescott• ingestion >15h : efficacité moindre
(Makin 1995)
Fluimucil, mucomyst
Traitement antidote NAC
• 1) IV (Prescott) : dose initiale de charge 150 mg/kg (sur 15 min.), puis 50 mg/kg sur 4h, puis 100 mg/kg sur 16h. Dose totale 300 mg//kg; durée totale 20h. (consc, NV, charb)
• 2) IV (Smilkstein) : dose initiale de charge 140 mg/kg, puis 70 mg/kg toutes les 4h, à répéter 12 fois. Dose totale 980 mg/kg; durée totale 48h.
• 3) P.O (Rumack) : dose initiale de charge 140 mg/kg, puis 70 mg/kg toutes les 4 h, à répéter 17fois. Dose totale 1330 mg/kg; durée totale 68 h.

CONCLUSION
• paracétamol = toxique lésionnel cytolyse hépatique j3 (sévère à mortelle)
• signes cliniques initiaux : ni prédictifs, ni spécifiques
• meilleur indice de gravité : paracétamolémie (entre h4-h16)
• antidote : NAC• France : max 8g / unité de conditionnement• ⇒ risques limités• transplantation hépatique : pour les formes
