    On behalf of the staff of the Florida Association of Community Health Centers, Inc. and the Clinician’s Network Advisory Council welcome to the Annual Clinician’s Network Educational Conference. We trust your experience here in Tampa will be educational, relaxing and full of time well spent as you network with clinicians and clinical quality care teams across the state.

    Rest assure that we have worked tirelessly to ensure that this program was designed with our FQHC clinicians in mind as you are at the heart of the work of Florida’s Federally Qualified Health Centers. We have done our best to ensure a quality program that will provide you with the valuable education, resources, and networking necessary to serve Florida’s most vulnerable populations. The sessions offered have been planned to promote the sharing of “best practices” and the improvement of communication between clinical professionals across the state of Florida as you continue to strive toward providing quality primary care and eliminating health disparities.

    Improving Clinical Quality Outcomes Salons D & E Dr. Anne Gaglioti, MD, Southeast Regional Clinicians Network (SERCN)For a decade, investigators at the National Center for Primary Care at the Morehouse School of Medicine (MSM) have conducted primary care practice-based research in partnership with community health center clinicians. The Southeast Regional Clinicians’ Network (SERCN)/Morehouse School of Medicine practice-based research Network (PBRN) represents health care professionals who work in 171 federally-qualified health center (FQHC) organizations across eight southeastern states, and provides over 3 million patient visits per year to a low-income, high-disparity patient population. From this disenfranchised population, 58 percent are African American and 13 percent are Hispanic /Latino. The majority of patients are medically underserved and living in poverty; 63.2 percent are at or below poverty, and approximately 45 percent of patients served by health centers in the Southeast Region are uninsured.

    Mobile Units - Medical and Dental Salons A & BBradley P. Herremans, MBA, FACHC, Chief Executive Officer, Suncoast Community Health Centers, Inc. Suncoast’s Mobile Coaches pioneered dental and medical outreach programs throughout Hillsborough and Polk counties in Florida. The Mobile Dental program treated over 5,600 children last year for the school based sealant program for the second grades in 37 Title 1 schools and other sites. Portable Dental Program provides services at other locations such as RCMA, Head Start, small SCHC sites where no dental is available. Children screened through these programs are referred to our pediatric dentists or to our Pediatric Dental Residency Program for restorative care. The Mobile Medical program provides much needed services in the vastly underserved areas of Eastern/Southern Hillsborough County. Clients stem from Woman’s Shelters, Food Kitchens, Boy’s and Girl’s Clubs, WIC programs and various missions and churches around the county.

  • Meet the SpeakersDebra S. Andree, MD, BSN, FAAPVice-President and Chief Medical Officer Community Health CentersDr. Andree serves as Vice President/Chief Medical Officer at Community Health Centers, Inc., (CHC) located in Central Florida. Dr. Andree served as Associate Medical Director for CHC from 2014 to 2015, and continues to serve as a pediatrician and as CHC’s Chief of Pediatrics, positions she has held since 2002 and 2013, respectively. She has completed numerous research projects, including a CDC pilot study of 2D barcodes and vaccination data collection in 2014, and currently serves as a Clinical Researcher in the FSU COM and has done research with Adolescent Health Risk Assessment with the Health IMPACTS project. Dr. Andree completed her MD at the University of Miami School of Medicine in 1999. She is a current member of the American Academy of Pediatrics, and has earned numerous awards including 2002 Resident of the Year at Orlando Regional Healthcare, the 2008 FSU College of Medicine Faculty Mission Award, and the 2015-2016 FSU COM Orlando Campus Clerkship Director of the Year.

    Belma Andrić, MD, MPHChief Medical OfficerHealth Care District of Palm Beach CountyBelma Andrić, MD, MPH is the Chief Medical Officer of the Health Care District of Palm Beach County. Dr. Andric is responsible for quality and patient safety across the Health Care District and directly supervises the Trauma, Aeromedical and Quality and Patient Safety departments, with indirect oversight over the District’s other physician leaders.

    Previously, Dr. Andrić has served as the Medical Director of the Health Care District Primary Care Clinics that serve over 30.000 unduplicated patients annually. She was in charge of quality and clinical performances at eight clinics locations, accounting for twenty five medical providers.

    Dr. Andrić joined the Health Care District in 2013 after serving as the Medical Director of the Lantana Health Center for the Palm Beach County Health Department since 2009. She holds a Doctor of Medicine degree from College of Medicine, University of Novi Sad in Serbia. Dr. Andrić earned a Master of Public Health from Florida International University and received her residency training in Preventive Medicine/Public Health from the Palm Beach County Health Department as well as residency training in Surgery from Mount Sinai Medical Center in Miami Beach.

    Dr. Jeffrey Beal, MD, AAHIVSMedical Director, HIV/AIDS SectionBureau of Communicable DiseasesDivision of Disease Control & Health ProtectionFlorida Department of HealthDr. Jeffrey Beal has over 30 years’ experience in the care and treatment of HIV/AIDS patients. He currently serves as the Medical Director for the HIV/AIDS Section at the Florida Department of Health. The HIV/AIDS Medical Section provides guidance, clinical consultation and technical assistance for Florida’s practitioners and county health department staffs. The focus of the section at present is the implementation of an aggressive plan to eliminate HIV transmission and reduce HIV-related deaths. The HIV/AIDS Section has programs and resources to support the HIV Care Continuum starting with linking patients into care on through achieving viral suppression. Dr. Beal’s clinical practice is currently working with HIV patients at the Hendy and Lee County Health Departments.

L. Brumback Primary Care Clinics. Through a grant awarded to the Health Care District in December 2012 by the United States Health Resources and Services Administration (HRSA), the District is managing and operating the C. L. Brumback Primary Care Clinics as Federally Qualified Health Centers.

    Anne Gaglioti, MDAssistant Professor of Family MedicineDirector, Southeast Regional Clinicians NetworkNational Center for Primary CareMorehouse School of MedicineAnne Gaglioti, MD is originally from Cleveland, Ohio. She did her medical school and residency training in Family Medicine at Case Western Reserve University in Cleveland. She then completed fellowship training in Primary Care Health Policy and Research at Georgetown University and the Robert Graham Center for Policy Studies in Washington, DC.

    Dr. Gaglioti’s clinical practice has focused on care of underserved and marginalized populations and she has served as a provider in correctional settings, community health centers, and academic health centers. Her academic career as a teacher and researcher has been focused on these topics as well and she has served in the past as a faculty member at Georgetown University, University of California San Francisco, and the University of Iowa. Currently, she is an Assistant Professor of Family Medicine in the National Center for Primary Care at Morehouse School of Medicine in Atlanta, Georgia.

    As director of the Southeast Regional Clinicians Network, a practice-based research network made up of Federally Qualified Health Centers across eight southeastern states (Georgia, Florida, Mississippi, Alabama, North Carolina, South Carolina, Kentucky, and Tennessee) she conducts practice-based research focused on high disparity, high impact conditions. She is also a health services researcher and her work at the National Center for Primary Care with secondary data analyses focuses on the intersection of primary care and health equity in at-risk populations.

    Michael F. Gervasi, D.O.President and Chief Executive OfficerAs a practicing physician for over 20 years, Dr. Gervasi brings an abundance of knowledge to FCHC. He is board certified in Family Medicine and has been a member of the executive committee of the Florida Society of the American College of Osteopathic Family Physicians. He received his certification in Health Care Quality Management form the American Board of Quality Assurance and Utilization Review Physicians, and he frequently lectures on risk management and quality improvement. Dr. Gervasi has received the Samuel L. Salman, D.O. Award for General Practice through Nova Southeastern University where he also serves as Clinical Associate Professor. He has also chaired and served on many local, state and national committees, including the eight-state Southeast Healthcare Consortium and the Clinician’s Network of the Florida Association of Community Health Centers.

Herremans, MBA, FACHCChief Executive OfficerSuncoast Community Health Centers, Inc, Riverview, FLBradley Herremans received his Bachelor Degree in Business Administration, (Management/Economics) from Ferris State University in Big Rapids, MI and his Master’s in Business Administration, (Finance) from Western Michigan University in Kalamazoo, MI. Mr. Herremans is a Fellow in the American College of Health Care Executives.

    During the last 8 years with Suncoast Community Health Centers, Inc. he has worked hard to improve the quality of the facilities and infrastructure; investing over 12 million dollars to renovate all of the clinics, improve equipment and implement Suncoast’s electronic health records. In addition, he added several clinics including mobile dental and mobile medical coaches. By the end of 2016, patient visits increased by 49% to over 204,000 and the number of employees increased by approximately 250 to over 420 dedicated team members. He was instrumental in stabilizing Suncoast’s financial situation allowing the organization to be well-positioned for future activities.

    In 2014 he served as Co-Chair for the Mobile Health Clinic Association annual meeting in Savana, Georgia where he was awarded the Mobile Healthcare Leadership Award. Mr. Herremans is a retired Colonel in the United States Air Force. Mr. Herremans serves the community as a member of the Board of Directors for the Florida Association of Community Health Centers, Inc., Hillsborough County Health Care Plan, and AllCare Options, LLC.

    Dr. Wilhelmina N. Lewis, M.D.Vice President of Clinical Operations and Chief Medical OfficerFlorida Community Health CentersDr. Lewis was promoted to Chief Medical Officer after serving as Clinical Coordinator at FCHC›s Fort Pierce Health Center. She is a Board Certified Family Practice Physician with a background in Geriatrics and a graduate of the University of Cincinnati College of Medicine in Cincinnati, Ohio. She completed her residency at the University of Cincinnati College of Medicine and University Hospital. Dr. Lewis is Chair of the St. Lucie County Diabetes.

    Darren MarshPharmDCommunity Health Centers, Inc. is proud to have an active pharmacy program, with a pharmacy team working hard to bring effective and affordable medications to our patients. CHC purchases pharmaceuticals under the Federal 340b program, and is an active participant in Pfizer’s Sharing the Care Program and other prescription assistance programs. These programs enable us to deliver lower cost medications to CHC’s growing patient population.

    Dr. Peralta, DMD Premier Community Health CenterJose Peralta, DDS joined Premier Community HealthCare Group, Inc. as the Dental Director in 2015 and was promoted to Associate Medical Director in 2016. Dr. Peralta has practiced dentistry for over twenty years. After earning his Doctor of Dentistry at the University of New York’s College of Dentistry, Peralta moved to Florida and completed his Residency at the University of Miami and its teaching facility, Jackson Memorial Hospital. Prior to joining Premier, Dr. Peralta practiced dentistry for a private practice in Central Florida. Concurrently, Dr. Peralta serves as a Major in the Army Reserve Dental Corps and recently returned from deployment in the Middle East. As the Dental Director at Premier, Dr. Peralta is focused on reducing oral health disparities, increasing oral health services and improving access to care for Pasco’s neediest population. He excels at collaborating with other practitioners and clinical/support staff to develop strategies that improve processes and ensure the delivery of high quality, compassionate and comprehensive oral health services to all patients. Dr. Peralta’s staff leadership responsibility includes the dental department and overseeing the patient experience initiative. The Dental team consists of 6 highly qualified oral health practitioners who care for nearly 4,000 patients and provide approximately 10,000 visits.

  • Tadd Richards, B.S., RDHDental Hygienist and Dental Outreach CoordinatorTreasure Coast Community HealthTadd Richards, B.S., RDH is the practicing Dental Hygienist and Dental Outreach Coordinator for over five years at Treasure Coast Community Health. Mr. Richards hold his BS in Health Science and is a Registered Dental Hygienist where he has over 20 years experience in Dental Hygiene.

He began his education and career at the University of Miami - Miller School of Medicine from 1992-1999, where he assisted in the implementation of the first departmentalized management information systems designed to provide point of care access to online medical research. He then served as a member of the executive team for Community Health of South Dade, Inc from 1999-2001. Community Health of South Dade, Inc. one of the largest community health centers in the country serving a large mental health and HIV/AIDS population. In this capacity, he held the title of Director of Management Information Systems and Logistics.

    From 1999-2011, Mr. Romillo served as the Sr. Vice President and Chief Information Officer for Health Choice Network, Inc. and as the contract CIO for 45 member Community Health Centers, Mental Health centers and Homeless Health Care Centers in Florida, Kansas, Missouri, Maryland, Rhode Island, Texas, North Carolina, New Mexico, Utah, and Hawaii.

    In 2011, Mr. Romillo was named Executive Vice President and Chief Operating Officer for Health Choice Network. Romillo is responsible for the day-to-day operations of Health Choice Network, Inc. and Health Choice Network of Florida.

    Currently Health Choice Network, under Mr. Romillo’s leadership has implemented cost effective cutting-edge technology solutions to reach more than 8,600 active Users, across 416 remote access locations, and in 21

    different States. In 2015, the health centers served over 3.5M Patient visits and synchronized well over 185TB of health information technology.

    Vicki Soule’ , M.S., M.B.A.,  CEOTreasure Coast Community HealthVicki Soule’ M.S., M.B.A. is the CEO for Treasure Coast Community Health. Previous to her current eight years at Treasure Coast Mrs. Soule’ spent over 20 years with Acute & Ambulatory Care Management. Mrs. Soule’ attended Wayne State University, received her MBA from Saginaw University along with a Fellow from American College of Healthcare Executives.

    Noelle Stewart, D.O.Medical DirectorCL Brumback Primary Care ClinicsNoelle Stewart, DO serves as the Medical Director of the nine C. L. Brumback Primary Care Clinics, Federally Qualified Health Centers owned and operated by the Health Care District of Palm Beach County. Dr. Stewart, who is fluent in English, Spanish and Haitian Creole, previously served as a provider at the Brumback Clinic location in Delray Beach, Florida.

    Dr. Stewart holds a Doctor of Osteopathic Medicine from Nova Southeastern University in Fort Lauderdale, Florida. She completed her residency training at Palm Beach Centre for Graduate Medical Education in West Palm Beach, Florida. She is Board Certified with the American Osteopathic Board of Internal Medicine

    The Brumback Clinics provide medical and dental care for adult and pediatric patients with or without insurance. Thanks to the efforts of Dr. Stewart and her medical team, the Brumback Clinics are one of six honorees nationwide that in 2017 received an “80% by 2018 National Achievement Award” from the National Colorectal Cancer Roundtable (NCCRT), an organization co-founded by the American Cancer Society and the Centers for Disease Control and Prevention. The 2017 award recognizes the Brumback Clinics for achieving a colorectal cancer screening rate that is twice the national average for community clinics and one of the highest in the country.
