4. Oil Leaks

A leak can be a sign of future engine

system failure. This failure willcause a loss of control of the lift and

its weight. This can provedevastating for the employees in itscourse. Oil on the floor is a hazard

for pedestrian and equipment traffic

Lift trucks pick up items such as

dust, dirt, paper and shrink wrap. If

these elements become soaked with

oil, a source of ignition can spark aflame. A lift truck fire produces

extensive damage and puts

employees and product at risk.5. Tires & Outriggers

The wear on the tires will not be

even and will cause the vehicle to

 become unstable.

When the tires are low this means

that you run the risk of hitting a

dock plate or running into debrison the floor.

The damaged tires will contributeto the greater vibration of theforklift, which results in increased

driver fatigue and also wears out

the forklift faster.

The worn out tires will also be

more likely to shake and jolt onharder surfaces, which can make

the driver lose control or cause the

load to become unstable.

6. Seat Belt & It’s MountsThe loss of the seat’s cushion andsupport creates an uncomfortableride with exposed wires,

increased heat and excessivevibration.

A damaged forklift seat may also

 pose a larger problem if the seat belt doesn’t work. 

As the shift goes on, operators

 become sore and irritated whichthen decreases his or her

 productivity.

Improvement

Based on the maintenance checklist, there is some damage that have on forklift. Amongthe first damage is the brakes do not grip. Before every work shift, forklift operator are required

to perform a pre-shift inspection. One item of focus is the brakes. Malfunction of the brakes can

result in serious damage, injury or death, making a regular inspection a must. Improvements to be made to the above problem is should changes with the new brakes pads and it is important the brake pedal does not fall to the floor and has enough resistance to create a safe stopping distance.

Moreover, a broken mirror can cause accidents while reversing forklift to workers orstuff. Therefore, the employer must perform inspection on every mirror wisely to avoid any

accident occur when reversing forklift. Besides that, damage of beacon lamp caused workers

around unaware about operation of forklift. Warning safety devices are considered a demand of

the environment in which the equipment operates. If an environment is loud, compliance officersmay suggest flashing beacon lamp. So, the beacon lamp should change to the new one or repair.

In addition, reverse alarm is important to employees around. Reverse alarm not workingcan cause accident because workers unaware while reversing forklift. So, for this case, the

reverse alarm should change to the new one. Other that’s, oil leaks occur on the forklift can

cause workers slipping and fall, excessive heat, fire hazard and system failure. Must have an

inspection toward machines especially the O-ring. It is because O-ring easy to damaged.Depending on the source of a lift truck oil leak, the repair can be expensive. Instead of a hose or

O-ring replacement, the issue may stem from the engine, transmission, pump, mast or control

valve. Consider lift truck oil leaks as warning signals. If not heeded, they can lead to progressionof the problem, expensive repairs and employee injury.

Besides that, worn out forklift tires can caused a serious damage to the employees and

stuff. Some forklift use only a specific tire, but if restrictions are not an issue, it is important toknow the best forklift tire options for your application. Cushion tires is most suitable for this

forklift because cushion tire have a tire that is made by bonding rubber to a metal base band.

Furthermore, cushion tire are designed for indoor use but can be taken outside for light workwhere surface are smooth.

Lastly, seat belt doesn’t work and ripped seat also a larger problem that can bring harm tothe driver. The loss of the seat’s cushion and support creates an uncomfortable ride with exposedwires, increased heat and excessive vibration. As the shift goes on, operators become sore and

irritated which then decreases his or her productivity. During an OSHA audit, a non-working seat

 belt may result in a fine under the General Duty clause of 1910.178. More importantly, arestraint system can be a life-saver in an event of tip-over or the forklift being driven off a dock.

In conclusion, provisions of the Occupational Safety and Health Act 1994 requires every

employer to provide a communication channel to allow any employee to make a complaint aboutany matter prejudicial to the safety and health at work and reported the above incident to the

Department of Occupational Safety and Health Malaysia. In addition, employers should also

conduct investigations on any accident, near miss accident, dangerous occurrence, occupational poisoning or disease and the above mentioned complaints in their workplaces.
