Dear ASU Faculty and Staff,   Courier drop boxes are available on the Tempe and Downtown Phoenix campuses.  Exam answer sheets that need to be scanned and processed by University Testing and Scanning Services should be placed in the centrally located courier drop boxes for delivery by ASU Mail Service couriers.  The couriers will make two trips daily, morning and afternoon, from each drop box to University Testing and Scanning Services.  Drop box locations are:      TEMPE:   Hayden Library, immediately in front of the circulation desk     DOWNTOWN:  University College, UCENT, Room 300  Faculty and staff have the choice to bring exams to University Testing and Scanning Services at 1130 East University Drive, Suite 204, or the exams may be left in one of the secure drop boxes for delivery by the ASU Mail Services courier.  UTSS will upload reports to MyASU within one business day of receipt of the exams.  Instructions for submitting exams to UTSS for processing can be found at https://uoeee.asu.edu/classroom‐exam‐scoring‐procedure.  Additional information about downloading exam results and reports from MyASU can be found at http://help.asu.edu/node/1274.   Answer sheets for each version of an exam should be placed into a separate campus mail envelope.  Each envelope should include an Examination Scoring Request form (the purple sheet), the answer key, and the students’ answer forms (bubble forms).  The Examination Scoring Request form, which includes the 5‐digit class number, is necessary for UTSS to process the exams and provide the appropriate reports.  A single envelope or group of envelopes from the same instructor should be placed inside a large, clear plastic bag to keep the envelopes together and protected from the elements.  Plastic bags are available for purchase from ASU Stores.  One courier request form (instructions attached) can then be inserted into each plastic bag containing the campus mail envelope(s).  Just be sure that the courier label can be viewed through the bag.  Each bag will require a courier label.  Neither boxes of exams nor course evaluations can be accommodated by the courier drop boxes.  These items will need to be delivered directly to UTSS.  Exams that were dropped off will be returned by ASU Mail Services to the department of the instructor listed on the Examination Scoring Request form.  If you wish to pick up your exams from UTSS, please note that on the Exam Scoring Request form.  You may contact University Testing and Scanning Services at 480.965.7146 for information about exam processing or ASU Mail Services at 480.965.1263 for information about the courier service.  Sincerely,  Eva Jones Testing Manager, University Testing and Scanning Services Email: [email protected] Phone 480.965.7146 Fax: 480.965.6859

More Information: Preparing Classroom Exams for Processing

1. Exams must be completed on Scantron (formerly NCS) examSYSTEMII form # 229633.

2. A #2 pencil must be used to make heavy black marks. Answer sheets completed with pens or markers will result in improper scoring and cannot be processed. Be sure answer sheets have no stray marks or writing anywhere but the place indicated (on either front or back sides.) Any mark placed near the black horizontal bars or the black squares renders scoring impossible. Forms should not be bent, folded, or otherwise mutilated. Changed responses should be erased completely with no visible smudges.

3. A left justified identification (ID) number is REQUIRED on ALL answer sheets. It must be from 2 to 10 digits long. Do not use duplicate ID numbers. The 10 digit ASU ID, the 7 digit Posting ID, or a pre-set private number are some examples of what could be used. Students' name fields must also be left justified.

4. Complete an answer key on an examSYSTEM II form with a #2 pencil. Label this sheet, "KEY" in the NAME grid. Leave the ID grid BLANK on the key sheet.

5.� Currently, multiple answers for a single question cannot be processed (only one answer for each question can be correct). Universal credit may be given for an item by marking more than one or all responses.

6.� Different versions of an exam cannot be merged for statistical .PDF reports. We can merge multiple versions into one Excel file. This file will contain ID #, Name & Raw score. (Please check “Merged Files” on the Exam Scoring Request form for this option.)

7. Arrange answer sheets so that they are all facing in the same direction. Improperly sorted forms will interrupt the optical scanning machine and delay processing considerably. Place answer key on top.

8. Submit only completed sheets for processing. Make sure that no other papers or blank answer sheets are included as they interrupt the scanning machine and delay processing.

9. Place answer sheets in a campus envelope (one envelope per answer key). Do not send tests through campus mail! Do not use staples, paper clips, or rubber bands to group forms as these can mutilate forms and result in delayed processing and/or improper scoring.

10. Bring the forms to the University Testing and Scanning Services, 1130 E. University, Suite 204. The office will be open from 7:30 – 5:00 for drop-offs/pick-ups.

11. Complete one purple EXAMINATION SCORING REQUEST form per envelope (one envelope per version of exam). Specify class number, instructor's name, Subject & Catalog number, Test Name, Number & Version, number of questions to be processed, phone number or email in case of questions (and to notify you for pickup), and types of reports desired. When you drop off exams, please time stamp the exam request form by inserting the upper right corner of the purple form into the time-stamp machine. Then, attach the purple form to the outside of the envelope with a single piece of tape.

12. Work requests are processed within one business day. When the work is completed it will be available on MyASU, and you will be contacted at the number specified on the EXAMINATION SCORING REQUEST form. For convenience, you may choose to pick up your exams when submitting your next request. Exams MUST be picked up within 30 days, as UTSS cannot store or shred these documents.

Glossary of Terms

Absolute Frequency Distribution Report-Bar graph report that shows the number of students that received all possible raw scores for the exam. The raw score is listed and the number of times the score wasachieved on the exam.

Answer Key-The correct answers according to this pattern:

(*) an asterisk-response is correct ( ) a space-responses ignored

(1-5) the correct answer (1=A,2=B, etc.)Asterisk-(*)

Represents each respondent on the Absolute Frequency Report or the correct answer on the Individual Item Response Report.

Average(Mean) ScoreSum of scores divided by the amount of students taking the exam.

Bar Graph-Graphic representation of the number of students that received each score.

Difficulty Factor-A value showing how difficult the question was to answer. The higher the difficulty factor-the easier the question is. Values between 0.3 and 0.7 are most effective. It is calculated as the frequency of the correct response to an item divided by the total number of respondents.

Discrimination Index-Value that shows the ability of an item to discriminate between the students who received a high score on the exam and those that received a low score. The factor is calculated by subtracting the number of students in the lower quarter that chose the response from the number of students in the upper quarter that chose the response and dividing that value by the number of students in the upper quarter. A negative value means that students receiving a low score select the option more than high scoring students. The ideal response should be a positive value. Difficulty can affect the discrimination index. Items that are too easy or too hard will not discriminate well.

Formula Scoring-A scoring method that will penalize a student for guessing. For example, if there 5 responses and the formula factor is 4, ¼ of a point will be deducted for guessing the wrong answer.

Frequency-The number of students that received a particular score.

Highest Score-The number representing the most correct questions.

Identification Number-A unique number that was entered on the answer sheet by the student in the Identification # field.

Individual Item Response Report-Multiple Students Per Page-A report showing how all the students responded to each question.

Individual Item Response Report-One Student Per Page-A report showing how one of the students responded to each question.

Individual Tests Results Report-A report listing the students that have taken the exam and their scores and the percent of the questions that were answered correctly.

Item Analysis Report-A report listing each item on the exam that allows you to analyze test difficulty using the Difficulty Factor and Discrimination Index values.

Kuder Richardson 20-A coefficient of reliability. This reliability measure of test items to used to discriminate between high and low scoring students. It measures inter-item consistency. A higher value indicates a strong relationship between items on the exam. The Kuder Richardson 20 will always be greater or equal to the Kuder Richardson 21. Values range from 0 to 1.

Kuder Richardson 21-A coefficient of reliability. This reliability measure of test items is used to discriminate between high and low scoring students. It approximates inter-item consistency. A high value indicates a strong relationship between items and a lower value a weaker relationship. Values range from 0 to 1.

Lowest Score-The number representing the least correct questions.

Lower Quartile-The number of students in the 25th percentile or lower that selected each test item.

Mastery-The number of correct answers required for mastery of the content.

Median Score-The score corresponding to the 50th percentile. Exactly half the scores are lower and higher.

Number of Items-The number of questions on the exam.

Number of Respondents-The number of students included in the report.

%-Percent of correct responses

%ile-The ranking of the individual according to percentile within the total group.

Question-The question number and the possible answers to the question on the Item Analysis Report. If a row is marked Oth (other) it is the number of students who marked no answer or multiple answers. An asterisk to the right indicates the correct answer.

Respondent Symbols-Symbols such as *,-, etc. used on the Individual Item Response Report showing how the student answered.

RS (Raw Score)-The possible number of correct answers.

Stanine-A nine point standard score scale corresponding to percentiles.

Std. (Standard) Deviation-The range above and below the average score where the majority of the scores lie. The more the scores are spread out, the higher will be the standard deviation.

Subtest-A subset of an exam. The exam can be divided into subtests so that individual scores for the different parts of the exam can be calculated.

Test Score Distribution Report-A report that compares raw scores to various types of standard scores and distributions such as Z-scores, T-scores, and stanines.

Total-Represents the complete count for the items, mastery, raw scores and the percentages for all subtests.

Total Count-The number of students who responded to each question option.

Total in the Upper/Lower Quartile-Number of students in the upper quarter and the number of students in the lower quarter.

Total %-The percentage of students who responded to each question option.

T-Score-The standard score based on raw scores with a mean of 50 and a standard deviation that is 10.

Upper Quartile-The number of students in the 75th percentile or higher that selected each test item.

Z-Score-The standard score based on raw scores with a mean of 0 and a standard deviation that is 1.
