The Environmental manager is required to:- Contact all the Emergency Response Agencies and the EPA to communicate the incident

details- Be available to take calls regarding the emergency

Keep appraised of the on-going situation in order to determine the appropriate level ofresponse from staffProvide the support and the technical response to the emergencyEnsure that suitable safety precautions are in place regarding anyon-site responseAdvice on notification to the public and other Agencies. IAdvice on remedial action necessary including preventative action i.e. potable watersuppliesEnsuring compliance with incident notification conditions of the licence.

NB: DURING THE OFF LOADING OF ALL BULK LIQUIDS AN OPERA TOR FROM THE AREA Mdn BE PRESENT TO-- -- --SUPERVISE THE OPERATION. IF THERE IS ANY DOUBT O.F ANY PROCEDURAL POINTS THEN CONTACT YOURSUPERVISOR. I

AREA IS A HAZARDOUS AREA FOR FLAMMABLE LIQUIDS UNDER ATEX DURING OI<'FLOADING - IGNITION SOURCESMUST BE ELIMINATED, WHICH INCLUDES THE I<'OLLOWING:

I. NO movement of any vehicles including forklift trucks2. NO hot surfaces3. NO flames and hot gases (including hot particles)4. NO mechanically generated sparks5. No Unsuitable or malfunctioning electrical apparatus6. No static discharges by good earthing and no plastics

ON ARRIVAL OF IPA TANKER A SMALL 100ML SAMPLE IS TAKEN TO LAB (RETAINS AREIHELD IN THEFLAMMABLES CABINET) AND UNIT 13 / LEADHAND / SIIIPPING DEPT. CONTACTED TOAqVISE ARRIVAL ON SITE.ON APPROVAL BY LAB (NORMALLY TAKES 1.5 HRS) LEAD HAND SHOULD CONTACT UNIT 13 / LEADHAND TOARRANGE A SUITABLE OFF LOADING TIME. I

ON APPROVAL OF TIlE BULK LIQUID & PRIOR TO OPENING THE LOCK THE LEADHAND (IN CONJUNCTION WInlTHE LAB TECHNICIAN) MUST CHECK THE FOLLOWING: I

* WARNING SIGNPOSTS MUST BE DISPLAYED, AT EACH END OF THE TRUCK, STATING THAT FLAMMABLE LIQUIDS AREBEING OFF LOADED. I

* VEHICLE ENGINE IS TURNED OFF.* THE EARTIlIS CONNECTED TO THE ISO BULK TANK FRAME ( NOT TIlE TRAILER)* TIlAT ANY APPROPRIATE VALVES (STORM DRAIN/EFFLUENT) ARE CORRECTLY POSITION* TIlAT TIlERE IS DELIVERY PAPERWORK & IT IS CORRECT FOR TIlE PRODUCT TESTED.* VERIFY THAT REAR LIGHTS OF TRUCK ARE IN GOOD ORDER (INTACT & UNBROKEN)* VERIFY THAT THERE IS SUFFICIENT CAPACITY IN THE CORRECT TANK.

* FIRE HOSES TO BE TAKEN TO TIlE FAR SIDE OF COMPOUND.* VERIFY WHICH TANK TO OFFLOAD TO & ENSURE THE CORRECT INLET PIPE IS UNLOCKED BY THI~ LEADHAND (ENSURING

THAT ALL OTHERS ARE CLOSED & LOCKED)* THE ISOLATION PUMP MUST BE UNLOCKED AND SWITCHED TO "ON" .* ENSURE DEDlCA TED II'A HOSE USED.* THAT THE TWO DUST EXTRACTORS ARE TURNED OFF AND LOCKED.

TO COMMENCE PUMPING THE GREEN BUTTON ON TIlE PUMP NEEDS TO BE PRESSED.

(1) It is the responsibility of the driver of the bulk liquid tanker to ensure that there are no ispillages during theoff loading from the tankers and from the connection or disconnection of the pipework.

To assist this requirement we request that: I

- All pipe work is capped after off loading of liquid.- That a drip tray is available on each truck and that it is utilised.- In the event of a spillage the driver must inform the Lead hand in the production area.

The spill will be dealt with as per IL-EMS-2-900 (Procedure in event df a liquidspill). An incident report will be issued in an effort to improve procedkes andprevent further spillages - a copy will be issued to the supplier. I

(2) It is the responsibility of the DRIVER to provide aU necessary samples required to ensure the quality ofthe goods before offloading is permitted. I

To assist this requirement we request that:

- All pipe work is capped after off loading of liquid.- That a drip tray is available on each truck and that it is utilised.

In the event of a spillage the driver must inform the Lead hand in the production area:.The spill will be dealt with as per IL-EMS-2-900 (Procedure in event of a liquidspill). An incident report will be issued in an effort to improve procedures andprevent further spillages - a copy will be issued to the supplier.

(6) It is the responsibility ofthe DRIVER to provide all necessary samples required to ensure the quality ofthe goods before offloading is permitted. I

Pipe work that is minimum of 25ft long with a connection to fit our 3" female camlock

Please note that Info-Lab will not be holding any pipe work (Hose)

(3) The tanker must be fitted with a Compressor to facilitate ojjloading.

(6) It is the responsibility of the driver of the bulk liquid tanker to ensure that there are no spillages during theoff loading from the tankers and from the connection or disconnection of the pipework.

3.1 PROCEDURE FOR DRAINING GEL TANK FARM BUNDING:

1. Open valve to drain bund & take a 1 litre sample of water discharged to surface water from theseparator for lab analysis (see form No. 0003). See also IL-EMS-2-500-4. I

2. When draining complete view level of oil in oil water separator ( There is a high level alarm insitu that will alert if necessary) I

3. If necessary pump oil into drums. Label clearly with material description & NCD NO. Fordisposal by an approved waste contractor.

2. WHEN FIRST FILLED, THE LIQUID LEVEL SHOULD BE MAINTAINEDBY THE ADDITION OF FURTHER WATER FOR A STABILISING HERIOD OFSEVEN DAYS (Actual levels to be recorded).

3. AFTER THE STABLLISING PERIOD THE LEVEL OF THE LIQUID SURFACESHOULD BE RECORDED AT 24 hr INTERVALS FOR A TEST PERIOD OFSEVEN DAYS (Actual levels to be recorded). I

4. THE TOTAL PERMISSIBLE DROP IN LEVEL, AFTER ALLOWING FOREVAPORATION AND RAINFALL, SHOULD NOT EXCEED 1I500th OF THE

AVERAGE WATER DEPTH OF THE FULL TANK OR 10mm.

5. ANY EVIDENCE OF SEEPAGE TO THE OUTSIDE FACES OF THEBUNDING SHOULD BE ASSESSED AND REPAIRED.

6. TEST RESULTS ARE RECORDED ..

7. SHOULD THE STRUCTURE NOT SATISFY THE SEVEN DAY TEST, THENI

AFTER THE COMPLETION OF THE REMEDIAL WORK IT SHOULD BEI

UNIT 16 GEL - CHECK THE SUB. PUMP IS SWITCHED TO "OFF"

2. EMPLOYEES-EVACUATE BUILDING TO THEIR DESIGNATED ASSEMBLY POINTS. (see table below)

3. FIRE WARDENS - ISWEEP THEIR ASSIGNED AREAS (CLOSING DOORS BEHIND THEM) & REPORTEVACUATION STATUS TO THE ASSEMBLY POINT CONTROLLER. (A.P.c.)

4. ASSEMBLY POINT CONTROLLER A.P.e.-REPORTS EVACUATION STATUS TO THE INCIDENT CONTROLLER BY RAInO.

5. EMERGENCY RESPONSE TEAM E.R.T. - IRESPOND TO THE FIRE PANEL & IDENTIFIED LOCATION WITH RADIOS &1EXTINGUISHERS & CONFIRM TO THE INCIDENT CONTROLLER IF THERE IS AFIRE,IF ABLE TO DEAL WITH & IF THE FIRE BRIGADE IS REQUIRED.

N.B: ALL SPILLS MUST BE REPORTED TO THE ENVIRONMENTAL CONTROLLER-- I

NOTE: As soon as practicable the EPA & CIR must be notified by phone & fax if there is a large spillage;the sanitary authority informed in event of a discharge to sewer & Shannon regional fisheries board in event of discharge to waters.

SMOKING IS PROHIBITED DURING HANDLING OPERATIONS IN THEVICINITY OF VEHICLES AND INSIDE VEHICLES.

ENGINES MUST BE SHUT OFF DURING LOADING/UNLOADING OPERATIONS.

VEHICLES MUST ALWAYS BE PARKED WITH HANDBRAKE ON.

THE FOLLOWING PROCEDURE MUST BE ADHERED TO DURING THE LOADINGOF DANGEROUS GOODS

1. NO DANGEROUS GOODS SHOULD BE LOADED UNTIL THE FOLLOWINGHAS BEEN CHECKED BY THE DGSA OR OTHER PERSON SUITABLY TRAINED:i

& Drivers credentials& That none of the packages to be loaded are leaking or damaged.o All labels & placards are affixed correctly on the goods and on the truck/container.II DGN section 15 is signed by the loader & section 17 by DGSA.•• That the driver has read the "Trem Card".o That the driver has the correct equipment listed on the check list

& Trem card.

2. AND CHECKED BY THE LOADER:o That the inside of the container is inspected

(see form 0001 IL-QA-2-l00-5)

3. DANGEROUS GOODS SHALL BE PROPERLY STOWED ON THE VEHICLE & SE<?UREDBY MEANS TO PREVENT THEM FROM BEING SIGNIFICANTLY DISPLACED INIRELA TIONTO EACH OTHER AND THE WALLS OF THE VEHICLE.

4. WHEN DANGEROUS GOODS ARE ON BOARD THE VEHICLE MUST BE SUPER\,ISED BY THEDRIVER.

UNLOADING OF GOODS:ALL GOODS RECEIVED ON SITE ARE CHECKED FOR DAMAGE. IF GOODS ARE S~EN TO BEDAMAGED ON THE TRUCK/CONTAINER PRIOR TO OFFLOADING OR IF GOODS A~m DAMAGEDDURING OFFLOADING IMMEDIATE STEPS MUST BE TAKEN TO SECURE GOODS 1--NDTO INSTIGATETHE SPILL PROCEDURE (IL-EMS-2-900-2) AND THE ENVIRONMENTAL OFFICIER CALLEDIMMEDIATELY.
