The International Congress of Oral Implantologists (ICOI) was founded in 1972 with the mission to promote education and research in implantology for the benefit of all patients. Within the framework of its curricula, the ICOI has provided instruction and training in modern implantology for over 1,500 dentists worldwide over the last 20 years. The ICOI implantology curriculum was the first to integrate live surgeries into the program.

ICOI Europe/Germany will begin the Implantology Curriculum 2017 with support from the International Academy of Implantology (IAI) in Germany and gradually expand it to all ICOI regions.

Curriculum Implantology Participation Agreement 1. Registration:

You register for the Curriculum Implantology in writing by filling out the pre-printed, binding registration form. The number of participants is limited. Registrations are processed in the order in which they are received by Ady Palti Consulting IAI (called IAI hereafter) until the maximum number of participants (24 persons) is reached. You will receive confirmation of your participation within 14 days after receipt of your registration by the IAI. 2. Payment of Course Fee:

The course fee is payable in two installments upon receipt of the respective invoice. Funds should be transferred to the account of the IAI 󠅠under 󠅠the 󠅠participant’s 󠅠name 󠅠and 󠅠the 󠅠invoice 󠅠number. 󠅠The 󠅠first 󠅠installment (2,950.00 Euro) must be paid no later than four weeks prior to the start of the first module (receipt of payment). The remaining installment in the amount of 3,000.00 Euro must be paid no later than two weeks prior to the start of Module 5 (receipt of payment). A one-year membership in the ICOI is included in the course fee. 3. Cancellation:

Participants may cancel up to six weeks prior to the start of the course series without incurring any charges. If a participant cancels up to four weeks prior to the start of the course series, a cancellation fee in the amount of 10 percent of the total course fee applies; if a participant cancels up to two weeks prior to the start of the course series, a cancellation fee of 20 percent of the course fee applies. Thereafter, a cancellation fee in the amount of 50 percent of the total course fee applies. Cancellations must be explained in text format and submitted to the IAI office in Kraichtal. If the participant assigns a replacement participant up to two weeks prior to the start of the course in text format and the replacing participant concludes a course contract with the IAI no later than 14 days prior to the start of the course, no cancellation fee will be charged. 4. Program and Schedule Changes:

Ady Palti Consulting IAI strives to execute the course series in accordance with the program overview. The IAI reserves the right to change the program sequence, the event locations, and the program content. There is no claim to instruction by a certain lecturer / instructor. For important reasons, the IAI reserves the right to postpone a course module to a later date or to change the event location. In these cases, participants will be immediately notified at the address, e-mail address, telephone or fax number they indicated. Any type of claim against the IAI, especially for replacement of damages or expenses due to a postponement, is excluded, except in cases of deliberate misconduct or gross negligence. 5. Attestation / Certificate:

After completion of each course weekend, participants must have the course instructor sign their attestation page. The fully-completed attestation page confirms the 󠅠participant’s 󠅠attendance 󠅠at 󠅠the 󠅠individual 󠅠course 󠅠modules. Please send your completed attestation page with all signatures for the individual modules to the IAI at the Kraichtal office no later than 14 days prior to the last module. If all signatures are completed, you will receive your Certificate for the Implantology Curriculum of the IAI/ICOI Europe on the final weekend. 6. Inability to Attend a Course Module:

There are a few rare exceptions, a participant who cannot attend a course module for substantiated reasons may switch to another course series. A claim to such does not exist. In such an instance, immediate contact with the IAI office in Kraichtal is required. An absence should also be reported retroactively. The IAI has the right to request evidence. 7. Cancellation / Postponement of the Course Series:

In the event of an insufficient number of participants (less than 14), the IAI reserves the right to cancel the entire course series or to postpone it to a later date. In such an event, you will be notified immediately. If the course series is cancelled, all course fees already paid will be immediately reimbursed to you. If the course series is postponed, you can cancel your registration in text format within one week of receipt of the postponement or alternative date. In this case, course fees already paid will be immediately reimbursed to you. Further claims against the IAI are excluded, except in cases of deliberate misconduct or gross negligence. 8. Final Provisions:

If a provision of this contract is no longer in effect or should become ineffective, the other conditions remain untouched and stay in effect. The parties shall replace the ineffective provision with a provision that is closest to the contractual intent in an admissible manner.
