Who do you need to collect immunization
records for?• Every child in your program*
*who attends 5 or more days per year (applies to drop-in child care sites)
*who isn’t counted by another site (for example, afterschool care kids are counted by the school)
When do you need the immunization records?
• Before a child starts attending your program*
*Almost always
New enterers must have one of the following BEFORE ATTENDANCE:• A record showing at least one dose of
each vaccine required for the child’s age• Or an exemption
Entering childcare or preschool Starting kindergarten
Entering any grade from outside the U.S.
Entering any grade from home school

What records are required?
All shot dates must be on one of the following:
• CIS—Certificate of Immunization Status• ALERT—Immunization Information System

Immunization dates

Computer tracking systems must be approved by the State for immunization assessment and reporting in Oregon.
Nonmedical Exemptions
• Two documents needed for new nonmedical exemptions:1. Vaccine Education Certificate2. CIS form

Medical Exemptions
• Must be signed by a physician• Must list a specific medical reason• Send a copy to the Local Health
Department for review
Immunity Documentation• Used when a child:
• Had the disease• Has blood test results showing immunity
• Should be counted as “complete”

Important DatesSchools/facilities submit Primary Review Summary (PRS) pages 1-2 to the county health department
January 16, 2019
Exclusion orders mailed by the county health department February 6, 2019
Exclusion Day February 20, 2019
Schools/facilities submit updated PRS pages 2, 3 and 4 to the county health department
March 4, 2019

• Receive reporting packet from County Health Department. Read the materials and call the County with any questions.
• Review immunization records for all children.
Timeline—Fall• Notify parents if immunization
records are incomplete.• Let parents know they will be getting
an exclusion letter from the County and their child will be excluded if records aren’t updated.

• All site with 10 or more children are required to share their site’s and their county’s immunization rates twice a year:• Within 30 days of the start of the school
year• Within 30 days after Exclusion Day
Sharing Immunization Rates
• You must share rates in 3 ways:• Post on your website• Make available in your main office• Send to parents electronically or on paper
(email, newsletter, etc.)
• You can calculate your immunization rates at www.healthoregon.org/immdata
Timeline—Spring-Fall
• This is a good time to organize records• Remove records of children no longer in
attendance• Review records of children currently
enrolled to notify parents and get ahead for next year
• Validation surveys and trainings with state or county immunization staff
