Los Angeles is doing just that on a daily basis, with our growing leadership in solid organ

transplantation.

heart and lung transplantation — including some of the country’s highest pediatric patient

survival rates. You also will read about the hospital’s successful programs in cornea transplan-

tation and hematopoietic stem cell transplantation.

Transplantation is a lifelong commitment to the patient on the part of an institution.

It requires the dedication of tremendous resources, including an expert medical team with

surgeons, physicians, coordinators, nurses, social workers, nutritionists, pharmacists and

rehabilitation specialists.

For more than four decades, Childrens Hospital Los Angeles has been a force in pediatric transplantation. In 1967, surgeons performed the hospital’s first kidney transplant — and that organ recipient is still thriv-ing. In 2007, Childrens Hospital established the Pediatric Transplant Center to serve as a coordinating umbrella for all of its solid organ transplant programs. The goal: to better enable its expert transplant teams to share knowledge and resources on behalf of children who desperately need care.2008

Growing up, Wesley Medel played

sports and several musical instru-

ments, was active in his church and

excelled at school. Few people around

him realized what his parents, Al

Herrera and Maribel Medel, had

known since Wesley was a baby:

that he would one day need a liver

transplant.

“He was doing all the things that

normal kids do,” says his father. “But

we were always worrying about the

future.”

Wesley was born with a genetic

disorder that caused a chemical to

build up in his liver, leading to grad-

ual and irreversible scarring. Doctors

helped Wesley avoid a transplant as

long as possible through medication

and careful monitoring.

But by the time he reached 14, he

needed a new liver. In August 2007,

he underwent a successful living

donor liver transplant at Childrens

Wesley’s story has become increas-

ingly common for the Liver and

Intestinal Transplant Program at

Childrens Hospital Los Angeles.

One of the fastest-growing programs

of its kind in the western United

States, more than 125 children have

received a liver transplant since the

program began 10 years ago, and

patients come from throughout the

United States and around the world.

The program has some of the high-

est pediatric patient survival rates in

the country, with 100 percent of liver

transplant patients in the past four-

and-a-half years reaching the critical

one-year survival milestone.

plants, a highly complex procedure

that offers the only hope for children

with irreversible intestinal failure.

(See page 9.)

“We have talented surgeons and a

dedicated, experienced team — that’s

what makes our program so suc-

cessful,” explains Yuri Genyk, MD,

surgical director of the Liver and

Intestinal Transplant Program at

Childrens Hospital and assistant pro-

fessor of surgery at the Keck School

of Medicine of the University of

Southern California (USC). “Surgery

is just one aspect. Pre- and post-

transplant care and psychosocial sup-

port for families also are important.”

Along with Dr. Genyk, the pro-

gram is headed by Medical Director

Dan W. Thomas, MD, head of the

Division of Gastroenterology and

Nutrition at Childrens Hospital and

associate professor of pediatrics at

the Keck School of Medicine. The

team includes transplant coordina-

tors, surgical nurses and a clinical

social worker, as well as a transplant

hepatologist, Sylvie Lebel, MD, who is

an assistant professor of clinical pedi-

atrics at the Keck School of Medicine.

In addition, nursing staff in the

Pediatric Intensive Care Unit and

Surgical Services/Post-Anesthesia

Care play a major role.

“Life after transplant is so differ-

ent,” explains Zoraya Fonseca,

RN, BSN, a liver transplant coordina-

tor. “We work very hard to get all

our families comfortable with the

challenges ahead.”

A team approach is especially

important because transplants some-

times need to be coordinated in as

little as 24 to 48 hours. Children with

sudden onset, or acute, liver failure

often deteriorate rapidly, and the

team must move quickly to screen a

living donor or procure a liver from

a deceased donor, using the United

Network for Organ Sharing national

matching service.

In the case of a deceased donor,

Dr. Genyk must go to the hospital

where the organ is located — some-

times flying out of state in the middle

of the night — to split the liver and

make sure it’s suitable. Meanwhile,

take place two to eight hours after

harvesting the liver.

In Wesley’s case, his family was

adamant that the transplant be per-

formed without a blood transfusion,

beliefs as Jehovah’s Witnesses.

Childrens Hospital pioneered

“bloodless” liver transplants, which

take place without the transfusion of

blood or blood products, reducing

the risk of infection and immunolog-

ical complications. “It was important

to have a living donor, so we could do

the transplant before Wesley devel-

oped a complication that required a

transfusion,” explains Ms. Johnson.

By law, however, doctors must per-

form a transfusion on children under

18 if a life-threatening complication

develops during surgery.

In transfusion-free surgeries,

patients are given a drug to boost

their reserve of red blood cells three

to four weeks prior to surgery. In

addition, surgeons utilize meticulous

techniques to minimize bleeding

during surgery and dilute a patient’s

blood to minimize the impact of any

blood loss. As a result of the hospi-

tal’s experience with transfusion-free

surgeries, less blood is used in all the

program’s liver transplants.

Roughly 50 percent of the hospi-

tal’s liver transplants are performed

using living donors — more than any

other pediatric transplant center in

the nation. (See page 16.) Survival

rates are slightly higher, most likely

because the quality of organs is

higher and patients don’t have to

reach end-stage liver disease before

receiving a transplant.

The hospital’s experience gives

it a unique opportunity to explore

ways to create a state of “transplant

tolerance” in patients. Dr. Genyk

and other physician-scientists at

Childrens Hospital currently are

working to develop innovative clini-

cal protocols to achieve this goal.

“The success of this research could

eliminate the need for lifetime

dependence on immunosuppression

drugs, further improving the long-

term survival and quality of life of

our children,” he says.

One reason living donor trans-

plants are possible is the liver’s

remarkable capacity to regenerate. It

is the only solid organ that can grow

back to its original size after being

resected. Of the liver’s many vital

functions, the main one is carrying

toxins, the byproducts of digestion,

from the body. It produces bile that

helps with the digestion of food and

provides important immunological

functions.

In pediatric living donor trans-

plants, surgeons typically remove

15 to 25 percent of a donor liver,

which then grows back in six to

12 weeks. Donors are screened at

USC University Hospital, where the

donor’s surgery is performed. Once

the liver is harvested, it’s immediately

brought to Childrens Hospital for

transplant.

Wesley’s donor, Mrs. Nunez,

admits that she initially felt nervous

when she agreed to donate part of her

liver. But prayer brought her a sense

of calm. “I wanted to help Wesley,”

she says. “I’m so happy, because now

he’s healthy, and he has the chance

to do whatever he wants to do.”

While Wesley’s original liver

gradually deteriorated over time,

Monserrat Ayala needed a transplant

when she was just six weeks old.

Suffering from acute liver failure,

Monserrat weighed just over six

pounds at the time of her transplant

in 2001 — one of the world’s small-

est-ever liver transplants. Today,

she’s a healthy six-year-old who

loves Dora the Explorer and play-

ing with her older sister, Cassandra.

“Immediately after the surgery, she

looked completely different,” says

her mother, Claudia Ayala. “She was

just beautiful.”

The stress of a child’s liver failure

and transplant can take a heavy toll

on families, which is why one of the

first people they meet is Clinical

Social Worker Martha Markovitz,

MSW, ACSW. Her job: provide

emotional and psychosocial support

and link families to support net-

works, financial assistance and other

resources, such as a place to stay for

out-of-state families.

“Our goal is not just to give the

child a new organ,” she says. “It’s to

help families through the transplant

journey, step by step.”

Wesley’s newest journey is just

beginning. The Long Beach teen is

playing his music and planning his

return to school. And he shares a spe-

cial bond with Mrs. Nunez, he says

— “a great friendship.” •

Nearly all patient rooms in the 317-bed New Hospital Building now under construction will be private rooms — a reflection of the hospital’s commitment to patient safety and family-centered care. The added privacy will make family visits more enjoyable and aid in all-important infection control. Private rooms also decrease the need to transfer patients to accommodate special needs, giving children a better chance to recover from serious illness. The New Hospital Building is scheduled to open in 2010.2010

Like the liver, the small

intestine or small bowel is

essential for life. Despite its

name, it is four to five times

longer than the large intestine

and has one of the biggest

jobs in the digestive system:

absorbing and digesting nutri-

ents. So when a child’s small

bowel goes into irreversible

failure, often the only hope

is an organ transplant.

Considered almost impos-

sible until recently, small

bowel transplants are highly

complex. The Liver and

Intestinal Transplant Program

at Childrens Hospital Los

Angeles performed its first

small bowel transplant in

2004. The program’s primary

goal, however, is to rehabilitate

a child’s intestine so a trans-

plant isn’t necessary.

“It’s always better to live

with your own organs. That’s

especially true with the small

intestine,” explains Dan W.

Thomas, MD, medical direc-

tor of the Liver and Intestinal

Transplant Program. “There’s

a much greater risk of rejec-

syndrome” — one of the main

conditions leading to intes-

tinal failure — their diges-

For children like Ansen

transplant. Born in 2003 with

a small intestine unable to

absorb or digest any nutrients,

Ansen had irreversible intes-

tinal failure. He survived on

intravenous nutrition while

waiting for a transplant.

plants, and that rejection can

be severe and devastating.”

The small bowel con-

tains billions of bacteria and

the most lymphoid tissue

in the body. “This makes it

extremely difficult to prevent

rejection while still avoid-

ing infections and other

complications,” says Dr.

Finding a small bowel that

fits a baby or small child is an

enormous challenge. While a

liver will regenerate, the small

bowel won’t. Although an

adult liver can be split to fit a

small child, “splitting” a small

bowel from a deceased donor

is not technically feasible in

most cases.

To determine the best treat-

ment plan, the program calls

on an interdisciplinary team,

Finally, in May 2005, a

deceased donor was allocated

for Ansen. In one 10-hour

surgery, he received a new

liver, pancreas and small

bowel. Ansen then spent 19

days in the Pediatric Intensive

Care Unit.

Today, the four-year-old

still relies on a feeding tube,

but he’s also able to eat on

his own. He loves playing

soccer with his dad, Elias

Santos. While Ansen’s exten-

sive hospitalizations delayed

his speech development, he’s

catching up. “He’s healthy

now, and we’re so happy,” says

his mother, Mayra Santos. “It’s

a miracle.” •

This is a story about love and

teamwork, and how together, they

saved a life.

In October 2007, Ashley Hopkins,

then 19, gave her younger sister,

Nicole, 18, the best possible gift:

one of her kidneys. While the teens

were in recovery from their dual

surgeries at Childrens Hospital Los

Angeles, their parents, Judy and Dale

Hopkins, took turns sleeping beside

each daughter’s hospital bed. The

girls’ eldest sister, Britney, helped

keep their spirits up by continually

showing them cell-phone photos of

each other in recovery.

The whole Hopkins family pulled

together, working with the hospi-

tal’s dedicated kidney transplant

team. Nicole, who was first treated at

Childrens Hospital for kidney prob-

lems as an 18-month-old toddler,

is no longer living in pain and fear.

She is now attending college.

She has the lifelong commit-

ment of the caregivers at the Kidney

Transplant Program, part of the

hospital’s Pediatric Transplant

Center. Founded in 1967, the Kidney

Transplant Program maintains one

of the largest pediatric kidney trans-

plant volumes in the nation, and has

performed nearly 900 transplants

since its inception.

“Our surgeons, transplant teams

and other caregivers are making a

long-term difference in very sick chil-

dren’s lives,” notes Carl M. Grushkin,

MD, head of the hospital’s Division

of Nephrology and professor of

clinical pediatrics at the Keck School

of Medicine of the University of

Southern California.

The program has reported an

excellent one-year and three-year

patient survival rate of 98 percent.

The United Network for Organ

Sharing (UNOS) puts Childrens

Hospital in the top three pediatric

kidney transplant programs nation-

wide in the most important category

— patient survival.

Dr. Grushkin has been with the

program since its first transplant 40

years ago. “Our first recipient is alive

and well today with the same kid-

ney,” he notes proudly.

His division oversees one of the

two largest dialysis programs any-

where for children with chronic kid-

ney failure. The hospital was the first

in the world to provide this service

to children.

The Kidney Transplant Program’s

multifaceted medical team includes

veteran transplant coordinators,

nurses, dietitians and a social worker.

Children with chronic kidney disease

receive pre-transplant evaluation and

management, all transplant options

and expert post-transplant care.

Physicians from nephrology and

urology routinely collaborate to

determine whether surgical treat-

ment is suitable for children like

Nicole.

Hospital and associate professor of

clinical urology at the Keck School

of Medicine, performed the surgery

on Nicole. Robert M. Naraghi, MD, a

member of the Division of Urology,

led the surgery on Ashley.

Adjacent operating rooms were

set up in the Burtie Green Bettingen

Surgery Center. The two surgeries

were synchronized to enable the sur-

geons to first remove one of Ashley’s

kidneys, examine it, then transplant

the organ into her sister — with min-

imal waiting time for either girl.

A father himself, Dr. Hardy under-

stands what the Hopkins family faced

as the girl’s parents and grandparents

waited. “They’re a great example of

a family getting on board to support

our effort. We have a great, caring,

interdisciplinary team — backed by

cutting-edge science. But we can’t do

this alone.”

Research incontrovertibly dem-

onstrates that a living-related donor

gives kidney transplant patients a

greater chance at long-term survival.

(See page 16.) “Being a living donor

is a wonderful thing for family mem-

bers to do for one another,” says Dr.

Hardy. “Is there a more loving thing

that you can do for somebody than

give them an organ?”

ery one doesn’t necessarily put the donor

in jeopardy.

At their parents’ home in San

Gabriel, Nicole and Ashley sit arm-

in-arm, giggling. Ashley says of her

sister, “She was in so much pain.

Now her whole expression has totally

uplifted. It shows that love overcomes

any obstacle.”

Nicole, now committed to tak-

ing an immunosuppressant drug

regimen for life, calls her sister “an

inspiration,” and the two break up

crying and hugging. Mr. Hopkins

enthuses, “Laughter has returned to

our family.”

There wasn’t much laughter when

Nicole went into renal failure last

summer. Sixteen years earlier, Nicole

had been on dialysis for a few weeks,

but her kidney problems cleared up.

In the intervening years, she lived

an active life, taking no medications.

She was in high school, playing bas-

ketball and softball, when a routine

medical exam revealed elevated

creatinine levels (a marker for kidney

function).

Hard decisions had to be made

quickly. This time, the Hopkins

family opted to bypass dialysis for

transplant. “The team prepared us

for everything,” says Mrs. Hopkins.

“Their level of compassion relieved

tremendous stress.” She credits the

entire team, including Gwen Green-

Brown, RN, MSN, CNS, CPNP, neph-

rology nurse practitioner; Karen Van

Wert, RN, MSN, NP, senior trans-

plant coordinator; and Hallie Stone,

MSW, social worker. “They took our

two girls under their wing. They all

have families themselves, so they love

children and treat you like you’re

part of the Childrens Hospital

family,” adds Mrs. Hopkins.

The process of preparing the sis-

ters for surgery started last August.

“We do everything we can to smooth

the way for each patient and family,”

says Ms. Van Wert.

When the girls went into sur-

gery, Ms. Green-Brown, who, like

Dr. Grushkin, had known Nicole

as a toddler, gave her a stuffed bear.

Nicole explains, “She said it would

be my guardian angel. It was. I love

Childrens Hospital!”

The Hopkins’ story demon-

strates the positive outcomes that

consistently happen in the Kidney

Transplant Program. There are other

exciting possibilities on the horizon.

Roger De Filippo, MD, a surgeon in

the Division of Urology and assis-

tant professor of urology at the Keck

School of Medicine, is exploring the

idea of rescuing failing kidneys with

stem cell therapy and tissue engineer-

ing in his laboratory at The Saban

Research Institute of Childrens

Hospital Los Angeles.

As Dr. Hardy says, “Our expertise

is dealing with renal failure, and we

do an outstanding job. But I’d much

rather prevent a child’s kidneys from

failing. So Childrens Hospital has a

two-pronged attack. Prevent what

you can. Cure what you can’t. And

continue loving the children.” •

Spacious patient rooms in the seven-level, 460,000-square-foot New Hospital Building will feature three distinct zones: family, with space for overnight stays; patient, with colorful ceiling murals and other kid-friendly features; and clinical, designed to facilitate the work of caregivers. Like one in every 100 babies,

Matthew Nelson was born with a

heart defect. In his case, the left

heart — meant to pump oxygen-rich

blood out to his body — was too

small and malformed to function

properly. When he was just days old,

Orange County doctors tried to help

by dilating one of his heart valves.

Almost immediately they realized

that Matthew needed much more

dramatic intervention, and he was

airlifted to the Heart Institute at

Childrens Hospital Los Angeles.

He landed in the right place.

Dr. Starnes and his team can per-

form exceptionally complex heart

surgeries — frequently making alter-

native therapy possible for youngsters

who, elsewhere, would be waitlisted

for transplant. Under his leadership,

the Heart Institute has been des-

ignated a Center of Excellence and

helps set national and international

standards for treating children with

congenital and acquired heart disease.

Quickly, the Heart Institute’s med-

ical team confirmed that Matthew

needed a new heart. Without one,

he would not survive. Yet, even to

be registered for transplant, he also

needed a family. The couple planning

to adopt him had bowed out, uncer-

tain they could provide the postoper-

ative care he would require. So Linda

and Robert Nelson stepped forward.

In 1980, after raising three daugh-

ters of their own, the Nelsons began

adopting special needs children who

face enormous challenges, such as

neurological impairment and cere-

bral palsy. Although Matthew would

be number eight, the Laguna Niguel

couple never hesitated adding him to

their busy household. “We fell in love

the second we saw him at Childrens

Hospital,” says Mr. Nelson. “He had

such intense strength in his eyes.”

Even with committed parents

on board, Matthew faced the nearly

insurmountable problem of being

matched with a donor heart. “At

times, it felt like that needle in the

haystack,” says Mr. Nelson. Then,

shortly after their new son turned

four months old, they got the call

they’d been waiting for. A compat-

ible heart had become available. The

following afternoon, Matthew’s sur-

gery was led by Brian L. Reemtsen,

MD, director of the Heart and Lung

Transplant Program and the Ryan

Winston Family Chair in Transplant

Cardiology at Childrens Hospital

and assistant professor of cardiotho-

racic surgery at the Keck School of

Medicine.

“As they wheeled him in, I knew

he’d be fine,” says Mrs. Nelson. “I

kept thinking about the incredible

gift of life that the donating parents

had given us.”

the Heart Institute’s Pulmonary

Hypertension Program and assis-

tant professor of pediatrics at the

Keck School of Medicine. “Before

surgery, Matthew required multiple

intravenous medications just to keep

his heart pumping,” she explains.

“Immediately after transplant, he

was like a whole new baby and has

thrived ever since.”

Still, “heart transplant is a lifelong

condition,” says Senior Transplant

CCRN, CCTC. Now a year old,

monitor for organ rejection and

infection. Close surveillance will be

a permanent part of Matthew’s life.

That helps explain why the Heart

Institute utilizes every medical

treatment possible to avoid or delay

transplant. “Heart transplantation is

a miraculous achievement,” explains

Dr. Szmuszkovicz, “but it is a miracle

best reserved for when absolutely

necessary.”

Dr. Reemtsen agrees. “The longer

any child can live safely with his own

heart, the better. That’s why we work

to stay ahead of the curve on every

front.” •

“I’m glad Childrens Hospital was

there,” says 10-year-old Lydia Hand.

“Or I wouldn’t be here.”

Lydia — an energetic, bright and

healthy girl who loves to read, plays

the flute and is writing a mystery

novel — is aware of her own per-

sonal history. But she may not fully

appreciate how she and her Lancaster

family helped make medical history

in September 1998 when she became

the first pediatric living donor liver

transplant performed at Childrens

Hospital Los Angeles, receiving a

portion of a liver from one of her

grandmothers, Sherry Marquez.

In the decade since that historic

surgery, Childrens Hospital has

emerged as a national leader in pedi-

atric transplantation, most notably

living donor transplants, successfully

providing this option in liver, kidney

and lung transplants for children

otherwise out of alternatives.

donor transplant program has saved

the lives of many children who oth-

erwise would have died on the organ

donor waiting list,” says Henri R.

Ford, MD, vice president and chief of

surgery at Childrens Hospital, as well

as professor and vice chair for clinical

“We are committed to growing

Inspiring this commitment is the

shortage of organs for children from

deceased or cadaveric donors. Nearly

98,000 people — 2,000 of them under

the age of 18 — are waiting for organ

Sharing (UNOS).

At Childrens Hospital, the Liver

and Intestinal Transplant Program

now performs the highest volume of

living donor liver transplants nation-

wide among pediatric centers; about

half of its procedures involve living

donors.

Survival rates are slightly higher

than transplants from cadavers.

“This is most likely because patients

are not as ill and the quality of donor

organs is better when the transplant

is done,” notes Yuri Genyk, MD,

surgical director of the Liver and

Intestinal Transplant Program at

Childrens Hospital and assistant pro-

fessor of surgery at the Keck School

of Medicine.

In kidney transplants, “having a

living donor — particularly a related

donor — is proving to be the best-

case scenario for the patient,” says

Brian E. Hardy, MD, head of the

Division of Urology at Childrens

Hospital and associate professor of

clinical urology at the Keck School of

Medicine.

Dr. Hardy has been collaborating

with James C. Cicciarelli, PhD, clini-

cal professor of urology and pathol-

ogy at the Keck School of Medicine,

on clinical research analyzing

nationwide UNOS data. The numbers

point to “irrefutable evidence” of the

benefits of living donor transplanta-

tion, with higher survival rates at one

year, three years and five years post-

transplant. Next, the investigators are

exploring the reasons why.

Childrens Hospital also holds the

distinction of being the site of the

world’s first pediatric double-lung

lobe transplant, from living parents

to their son, in 1993. (See page 19.)

The hospital is one of the few pro-

grams nationwide to offer living

lobar lung transplantation. Of 165

heart, lung and heart/lung trans-

plants performed here since 1993,

55 have involved living donors, often

parents anxious to provide a life-

saving solution for their child.

Nicole and Scott Hand understand

that feeling. They didn’t know much

about transplant medicine when

their daughter was born with severe

jaundice. They learned fast. Lydia

was diagnosed with biliary atresia, a

rare disease in which the bile ducts

can’t properly eliminate bile from the

transplants — Dr. Genyk — would

join the hospital within a month.

“That was an answer to our prayers,”

says Mrs. Hand.

Multiple friends and family mem-

bers volunteered to be Lydia’s donor.

Her father, a match, did not other-

wise qualify. Both grandmothers,

Linda Hand in Georgia and Sherry

Marquez in Lancaster — matched.

Doctors zeroed in on Mrs. Marquez,

who remembers undergoing “gobs

and gobs of tests.” With her grand-

daughter’s liver failing, she was ready

for the operation to begin.

On Sept. 30, when Lydia was just

six months old, the team of surgeons

led by Dr. Genyk and Rick Selby, MD,

division chief of abdominal organ

transplantation at the Keck School of

Medicine, performed the first living

donor liver transplant at Childrens

Hospital.

As Lydia’s surgery progressed, her

parents received updates from Senior

Transplant Coordinator Stephanie

Johnson, RN, BSN, MBA, CCTC. “At

one point, Stephanie said, ‘The liver

is making bile.’ It sounds gross,” says

Mrs. Hand, “but those were wonder-

ful words to us.”

That night, the Hands visited their

daughter in the Pediatric Intensive

Care Unit. What they saw amazed

them: Lydia had rosy cheeks. “We’d

never seen her skin pink before,”

recalls her mother. Equally captivat-

ing: “she smiled,” says Lydia’s dad.

“She hadn’t smiled much before that.”

There was a lot to smile about.

Five days later, Mrs. Marquez went

home from USC University Hospital;

within a few months, her liver would

regenerate itself. Best of all, Lydia’s

immune system accepted her grand-

mother’s gift.

Initially, Lydia took 11 medica-

tions daily to fight infection and

organ rejection. Today she takes only

two and sees Dr. Thomas just once

a year. A big sister to Mark, age six,

and Anna Grace, age one-and-a-half,

she has few restrictions. “That’s what

we want,” says Dr. Thomas, associate

professor of pediatrics at the Keck

School of Medicine. “We want all our

kids to live a full life.” Lydia and her

family agree. •

When Lydia was two months old,

she underwent a Kasai Procedure at

Childrens Hospital to create a new

duct from a piece of her intestine. It

failed to correct the problem — or

avoid the need for transplant. The

UNOS list also yielded no results.

Then Dan W. Thomas, MD, head

of the Division of Gastroenterology

and Nutrition at Childrens Hospital,

told the Hands that a surgeon with

specific expertise in living donor

Lungs came late to the world of pedi-atric organ transplantation. It wasn’t until the early 1990s that specialists perfected the delicate surgical tech-niques and developed the right medications to keep patients healthy after surgery.

Still a fairly rare procedure, with fewer than 100 done worldwide annu-ally, lung transplants in children are now remarkably successful, particularly when performed at the Heart Institute of Childrens Hospital Los Angeles.

“We now have kids 12 years out of surgery, doing great,” says eithne f. MacLaughlin, MD, a pediatric pul-monologist at Childrens Hospital and associate professor of pediatrics at the Keck School of Medicine of the Univer-sity of Southern California.

Many, like Tahnee Smith, 18, have cystic fibrosis, a genetic disorder caus-ing inflammation and congestion that eventually destroy the lungs. By the time she was 11, Tahnee was hospital-ized at Childrens Hospital nearly every month for infection. At one point, Dr. MacLaughlin mentioned lung trans-plantation as a possible future option. “from that moment, I wanted it,” Tahnee says.

After four more years of medical management, her name went on the

national transplant waiting list. Thir-teen months later — in a wheelchair, on round-the-clock oxygen — Tahnee learned that donor lungs from a deceased donor had been found. In March 2006, her transplant was per-formed by Brian L. Reemtsen, MD, director of the Heart and Lung Trans-plant Program and the Ryan Winston family Chair in Transplant Cardiology at Childrens Hospital, and assistant professor of cardiothoracic surgery at the Keck School of Medicine.

The Heart Institute has gained international recognition for surgical innovation since its director, Vaughn A. Starnes, MD, performed its first lung transplant in 1993. Dr. Starnes, the Hastings Distinguished Professor and chair of the Department of Cardio-thoracic Surgery at the Keck School of Medicine, pioneered the living donor lobar transplant, in which youngsters receive two lower lung lobes, one from each parent or other compatible adult.

“We can transplant enough tissue to save the child and still protect the health of the donors,” says Dr. MacLaughlin. In patients who receive these transplants, lung tissue actually expands to handle the increasing need for air volume.

Cardiologist jacqueline R. Szmusz-kovicz, MD, heads the Pulmonary Hypertension (PH) Program. PH con-stricts vessels that should transport blood from the heart to the lungs. “The best solution is sometimes lung transplant, because if the lungs are replaced, the heart recovers,” says Dr. Szmuszkovicz, assistant profes-sor of pediatrics at the Keck School of Medicine.

for other patients, powerful medi-cations that dilate the lung’s blood vessels are the answer. “These thera-pies are complicated,” explains Dr. MacLaughlin. “But with the expertise available at the Heart Institute, they’re possible here.” not all youngsters respond well enough to such treat-ments. Doctors are conducting clinical research to identify predictive markers which may lead to new therapies.

Tahnee Smith cannot imagine better therapy than the double-lung transplant she received two years ago. Currently a freshman at Antelope Valley College in Lancaster, she has an active social life and a passion for writing and photography. “I have a lot I want to see and accomplish,” she says. Happily, she now has time to do it all. •

For Precious Ramirez, the stage is like a second home. The exuberant seven-year-old dazzles audiences with her tapping heels, contagious smile and color-ful costumes. “I love to dance,” she says.

With her folkloric dance team of 14 other girls, she performs traditional Mexican dances at local parades, school assemblies and dance competitions. When not in front of a large audience, she enjoys soccer and putting on a show for her family with her impersonation of singer Miley Cyrus.

Her poise is evident, especially when someone asks her why her eyes look different from one another. She simply replies that she has “a special eye.” Precious was born with Peter’s Anomaly, a rare congenital disorder in which the cornea of the eye may be scarred, and cataracts and glaucoma can develop. When she was one week old, she received a corneal transplant on her right eye, thanks to Jonathan Song, MD, director of the Cornea Institute in The Vision Center of Childrens Hospital Los Angeles, and assistant professor of clinical ophthalmology and pediatrics at the Keck School of Medicine of the University of Southern California.

“I told Dr. Song, ‘My little girl’s vision is in your hands,’” recalls Angie Perez, Precious’ mom. “We are so grateful.” In turn, Dr. Song — whom Precious looks forward to visiting — is pleased his young patient is thriving. “Our goal was for Precious to be able to use both of her eyes. To see how well she’s doing is just wonderful.” •

only 25 years ago, children with a

rare immune system disorder called

Severe Combined Immunodeficiency

(SCID) had only one hope for sur-

vival: to live in isolation, such as in a

protective plastic bubble. That reality

gave the disease its unfortunate nick-

name — “bubble baby disease.”

Children with ADA-deficient

SCID are born with a missing gene

that prevents them from fighting

any infections. (ADA, or adenosine

deaminase, is an enzyme found in

the tissues; its deficiency can lead to

destruction of B and T cells.)

Nine-year-old Raven Savoy spent

many days in hospitals from the time

she was only a month old, battling an

upper respiratory infection followed

by double pneumonia. Physicians in

discovered that Raven suffered from

ADA-deficient SCID.

While Raven didn’t have to live

in a bubble, she did have to undergo

painful, twice-weekly enzyme injec-

tions to combat her disease. Then,

last year, her physicians presented

Raven’s family with new hope —

Childrens Hospital Los Angeles could

provide a possible cure using innova-

tive gene therapy.

“When we first heard about this

new treatment, we weren’t sure,”

recalls Raven’s mother, Stacie

Williams. “What if something went

wrong? But we had to try.”

Mrs. Williams felt increasing

confidence when she learned that

Childrens Hospital was home to one

of the nation’s leading Hematopoietic

Stem Cell Transplant (HSCT)

programs. In fact, it is the only

hospital in the United States cur-

rently treating SCID patients with

gene therapy. Donald B. Kohn, MD,

head of the hospital’s Division of

Research Immunology/Bone Marrow

Transplantation and past president

of the American Society of Gene

Therapy, is considered one of the

leading experts on the use of gene

therapy for immunodeficiency

diseases.

The HSCT Program attracts

patients globally and performs

approximately 40 life-saving trans-

plants for a variety of conditions,

including cancer, aplastic anemia

and complex genetic diseases such

as SCID and sickle cell disease. More

than half of its patients are under the

age of five.

On Nov. 1, 2007, Raven became

the first patient at Childrens Hospital

to take part in a groundbreak-

ing clinical trial — a collabora-

tion between The Saban Research

Institute of Childrens Hospital Los

Angeles and the National Institutes

of Health in Washington, D.C.

Raven’s family had already gone

through the difficult process of

searching for a bone marrow donor,

with no matches. This time, she

wouldn’t need a donor.

Cells from the bone marrow in

her hip were drawn during an hour-

and-a-half-long procedure, led by Dr.

Kohn, co-director of the Childrens

Center for Cancer and Blood

Diseases and director of the Gene,

Immune and Stem Cell Therapy

Program at The Saban Research

Institute. Raven’s cells were purified

in the laboratory and the missing

gene added. After five days, the mod-

ified stem cells were intravenously

injected back into her.

Fifteen to 20 percent of SCID

patients have a sibling who is a

match for the HSCT transplant, the

least complicated transplant with

the highest rate of success, explains

Dr. Kohn, professor of pediatrics

and molecular microbiology and

immunology at the Keck School

of Medicine of the University of

Southern California. “Until now, the

remaining 80 percent had to rely on

bone marrow from a parent, which

has about a 50 percent success rate, or

from an unrelated donor or umbilical

cord blood.”

A major advantage in working

with the patient’s own bone marrow

is that it eliminates the life-threaten-

ing rejections patients often develop

toward a donor’s cells or the reverse

reaction when a donor’s T cells react

against the patient’s body.

To allow her immune system to

develop, Raven spent a month in a

protective room with state-of-the-art

HEPA filtering at Childrens Hospital

before going home to Texas. Infection

control is critical for such patients,

which is why the entire New Hospital

Building will be HEPA-filtered when

it opens in 2010. The HSCT Inpatient

Unit will expand from 11 to 14

HEPA-filtered isolation rooms.

Each positive-pressure room will

feature advanced-laminar airflow,

allowing air to flow out of the room

instead of in, so any airborne germs

that may infect the patient are kept

away. In addition, two of those

rooms will have negative pressure

anterooms for when a patient has an

infection that could spread to other

patients.

Raven, who no longer needs those

painful injections, exemplifies the

great strides made for children with

SCID in the last two decades. “We

are hoping that this new treatment

becomes the standard of care,” says

Dr. Kohn, “allowing these kids to not

just survive, but live long, full lives.” •

Physician-scientists in the Heart Institute at Childrens Hospital Los Angeles are exploring promising new ways to increase the availability and success of heart transplants for chil-dren who desperately need them.

fortunately, they have the support of equally farsighted philanthropists.

Bob and judy Winston have dem-onstrated their exceptional vision and leadership by investing in the Heart Institute’s mission in several impor-tant ways, including the founding of an innovative educational program for medical professionals.

The first symposium, The Winston Pediatric overview of Solid organ Transplantation, attracted more than 100 physicians, nurses and transplant coordinators from across the coun-try to The Saban Research Institute of Childrens Hospital Los Angeles in october 2006.

Its agenda represented the col-laborative vision of Mr. Winston and Vaughn A. Starnes, MD, director of the Heart Institute at Childrens Hospital, as well as Hastings Distinguished Professor and chair of the Department of Cardiothoracic Surgery at the Keck School of Medicine of the University of Southern California.

“our world needs people who dis-play leadership and integrity,” says Mr. Winston. “That’s exactly the kind of people we’ve met at Childrens Hospital. We’re pleased to help them move forward, building on the excel-lence they’ve already achieved.”

The Winstons also have provided funding for a state-of-the-art, pri-vate patient room in the expanded Cardiothoracic Intensive Care Unit that will be part of the new Hospital Building at Childrens Hospital.

In addition, the couple has estab-lished the Ryan Winston family Chair in Transplant Cardiology as a tribute to their grandson, Ryan. following a suc-cessful heart transplant, Ryan passed away a year later at age 10.

The Ryan Winston family Chair in Transplant Cardiology is held by Brian L. Reemtsen, MD, director of the Heart and Lung Transplant Program at Childrens Hospital and assistant professor of cardiothoracic surgery at the Keck School of Medicine. “We have a responsibility and a commit-ment to our young patients, and to the Winstons, to further the science of transplant medicine,” he says.

The Winstons’ involvement is criti-cal to the program’s ability to advance

clinical research into pre- and post-operative transplant care, he adds. Studies under way include the impact of infection on chronic rejection and the viability of new medications in improving organ survival.

In the planning stages for 2008 are investigations designed to make more life-saving transplants pos-sible for kids, including a study of ABo-incompatible transplants. These enable young infants to receive an organ from a donor without a match-ing blood type (possible until one year of age). A second study will focus on the Berlin Heart, a device that acts as an artificial heart for young children waiting for transplant organs.

The Winstons extend their philan-thropy to other causes. Mrs. Winston, a former high school teacher, is a long-time champion of Childhelp, which is dedicated to abused and neglected children. Mr. Winston has established The Winston Center for Leadership and ethics at his alma mater, Boston College. “nothing gives us more plea-sure,” he says, “than knowing we are helping young people survive the chal-lenges they face and grow into fine adults.” •

Endowment FundsAn endowed fund is a permanent fund established by a donor or our board of trustees, where the principal funds are held intact to ensure earning distributions in perpetuity. Annual interest earnings from an endowed fund provide a predictable, stable stream of support for the hospital program specified by the donor. The minimum gift to establish an endowment is $25,000. In fiscal Year 2007 (July 1, 2006 – June 30, 2007),

Childrens Hospital Los Angeles once again demonstrated

that it is the only hospital in the greater Los Angeles area

with the physician specialists, research and treatment

programs necessary to treat the full range of serious ill-

nesses and injuries that afflict children. Among the year’s

highlights were:

new Hospital buildingIn March 2007, steel began rising on the New Hospital

Building under construction at Childrens Hospital.

By early winter, the framework was completed on the

seven-level, 460,000-square-foot in-patient building,

which is on schedule for its opening in 2010. The state-

of-the-art facility will feature the finest in patient and

family-centered care.

patient careThe American Alliance of Healthcare Providers (AAHP)

honored Childrens Hospital with a 2007 Hospital of

Choice Award, making it one of only 15 hospitals in

California to receive this accolade and one of just seven

pediatric hospitals nationwide. The AAHP honors hospi-

tals that go above and beyond the call of duty in the quest

for excellence.

The Vision Center at Childrens Hospital Los Angeles

debuted in 2007. The hospital’s newest Center of

Excellence builds upon the success of its Division of

Ophthalmology and treats more than 12,000 patients each

year, making it the largest center of its kind in the United

States. Through its six institutes, The Vision Center is the

only pediatric program worldwide with expertise in virtu-

ally every ophthalmologic subspecialty area.

ResearchThe Saban Research Institute of Childrens Hospital Los

Angeles was awarded four grants from the California

Institute for Regenerative Medicine (CIRM) — three in

2007 alone — and is now the only research institute in a

freestanding children’s hospital in California to achieve

this distinction. The first grant came in 2005, with a three-

year, $2.4 million stem cell training grant. The

hospital’s latest CIRM grants include a $2.8 million Shared

Research Laboratory grant to remodel and equip laborato-

ries dedicated to human embryonic stem cell research.

education The Saban Research Institute took the lead in the launch

of a new nationwide, four-center research training pro-

gram funded by the National Institute of Diabetes and

Digestive and Kidney Diseases of the National Institutes

of Health. Armed with a five-year, $970,000 grant, the

Undergraduate Short Term Education Program for

Underrepresented Persons (STEP-UP) exposed 65 stu-

dents to research in diabetes, endocrinology, metabolism

and obesity.

community supportOn July 31, 2007, the McCourt family and the Los Angeles

Dodgers announced they are joining Childrens Hospital

Los Angeles and City of Hope to support cancer research

by establishing ThinkCure as the official charity of the

Dodgers. Frank and Jamie McCourt, the owners of the

Dodgers, are committed to making ThinkCure a major

force for research advances that will help children and

adults.

international impactIn 2007, Childrens Hospital created the Los Angeles-

Be’er Sheva Saban Collaborative Program with The

Saban Children’s Hospital at Soroka University Medical

Center, located in Be’er Sheva, Israel. The program was

founded with a generous, three-year grant from hospital

Trustee Cheryl Saban, PhD; Haim Saban; and the Saban

Family Foundation. In 2007, one resident from Childrens

Hospital and two residents from The Saban Children’s

Hospital participated in the expanding collaborative

program. •
