I want to get information regarding Selection of Epidemiologist (in districts) and Epidemiologist (for SSU), for which interview was conducted on

15th

July,2011 under IDSP. Please provide the following information point wise either as per serial numbers:

1. Please provide me the copy of advertisement, which is advertised for the post of epidemiologist in IDSP, Haryana & interviewed on 15 th

July.

2. Please provide me the following details of all the candidates on the given format below, who attended the interview on 15th July for

the post of Epidemiologist in IDSP, Haryana :

d) All the factors considered for selecting the epidemiologist (Please specify in detail with weightage)…………………………..?

e) Please provide the copy of original documents indicating the criteria decided by the technical committee or any other(please specify

name and chairman of member?) for selecting the epidemiologist?

f) please provide the original copy of document indicating the marks given for interview by each interview member?

4) Whether any graduate of home science is a Medical Graduate or not? Please reply in Yes or Not(If Yes then provide the document)?

5) Provide the list of the interview panel members indicating their Name, Designation, who conducted the interview for the post of

Epidemiologist on interviewed on 15 th July?

6) Provide the original attested copy of all the Original office letters and office orders issued/e mail sent to the members of interview panel or

interviewer for conducting/taking and attending the interview for the post of epidemiologist in IDSP, Haryana?

7) Provide the duration and time of interview conducted for the post of Epidemiologist From………. To ……… on dated………….?

8) Provide the name of the candidate selected for the post of Epidemiologist at SSU in Panchkula (Haryana) on the basis of the interview

conducted on15 th July?

9)Provide the Education Qualification of the candidate selected for the post of Epidemiologist at SSU/IDSP on the basis of interview conducted

on 15 th July?

10) Provide the reason for selecting the Epidemiologist at SSU/IDSP, even though she/he is not fulfilling the eligibility criteria mentioned in the

advertisement published in newspaper by IDSP, Haryana?

11) Please provide the attested copy of all the rules/regulations/guidelines, under which She/He is selected for the post of Epidemiologist at the

state level or SSU/IDSP/

12 Whether the result of the candidate selected was published in any news paper /local news pape5r or any other media? Yes or not, if no then

provide the reason and provide the guideline stating so?

13) Whether MD(NRHM) and DGHS is asked for permission in written about selecting the candidate(who is not fulfilling the eligibility criteria as

you mentioned in the advertisement) for the post of Epidemiologist at SSU/IDSP ? Please answer in Yes or Not and also provide the Copy of

original document in which permission is asked by the interview panel and the copy of original document in which permission is given by the

MD(NRHM) and DGHS for selecting the none eligible candidate for the post of Epidemiologist at SSU/IDSP?

14) Whether MD(NRHM) and DGHS is Informed about the following points/Issues of the Selected candidate For the position of Epidemiologist at

panchkula or at SSU/IDSP:

a) She/He is not fulfilling the eligibility criteria, advertised in the news paper by SSU/IDSP, Haryana? Please give reply in Yes or Not and

also provide the document stating so?

b) Please provide the Attested Copy of original documents/letter/Application sent to MD (NRHM) and DGHS, Haryana (as asked in

question number 14(a)?

c) She/He is selected for the post of Epidemiologist at SSU, Panchkula even though she is not fulfilling the eligibility criteria as

mentioned the advertisement. Please provide the copy of original letter stating this issue to MD (NRHM) or DGHS Haryana?

15) Provide the copy of attendance list signed by the candidate?

16) provide the copyof attendance list signed by the interviewer?

are concerned with IDSP, Panchkula and NRHM so please collect the information from concerned authorities /departments and send it to me.

If information regarding to some points is not fall within the IDSP/NRHM jurisdiction then give the details of the department from where

information can be obtained. Please check the Rule, regulation and concerned Judgments of Central Information Commission before giving

any excuse regarding Third party information because all the points are made after considering the RTI Act and Judgment of Central
