SCHOOL HISTORY IntegrityCollegeofHealth,furtherreferredas“ICH”,wasfoundedin2007bytheRegisteredNursesBeaMartinandVellaMasip(previouslyVellaDennis).Thevisionandpassion the foundershad,combinedwithexpertiseandloveofteaching,helpedtheschoolgrowsuccessfully.Fromthedayitwasfounded,ICHhaseducatedhundredsofstudentsthatarenowgainfullyemployedintheirfields.ThemanagementandstaffofICHarestronglycommittedtomaintaininghighstandardsofqualityand integrity in all aspects of operating an educational institution. It provides training equipment including,medicalmannequins, stethoscopes, blood pressure cuffs, and blood withdrawal paraphernalia.Additionally,itincludesskillslabwithsink450sq.ft.Theoryinstructionandskillstrainingareperformedinthisclassroom.Astudentlibraryof109sq.ft.alsoisinsuite110.ClassroomB,locatedinsuite103,has1700sq.feetanddedicatedforlectures.Itisequippedwithaprojectorandscreenforpresentations.Officespaceislocatedinsuite102withthefollowingdedicatedareas:

Allclinical/externshipinstruction(patientcareeducation)isprovidedinassignedfacilities.VNstudentswillhaveopportunityinmorethanonelistedclinicalfacility;MAstudentswillbeassignedtooneexternshiplocation.Nameandlocationsofallpartneringclinicalfacilitiesarelocatedunder“ClinicalFacilityList,”pg.43.The school provides large parking area. ADMISSION REQUIREMENTS AhighschooldiplomaorGEDisarequirementfortheVocationalNursingprogramandMedicalAssistantprogram.TheprogramsofferedbyICHareopentobothmenandwomenwhopossessahighschooldiploma/GEDoritsequivalent(evidencedbyanevaluationoftranscriptsbyareputableevaluator), and pass a Wonderlic entrance exam. We are unable to accommodate “ability-to-benefit”studentsandwearenotabletosponsoranystudentsfromoutsidetheUnitedStatesofAmerica.Wedonotprovidevisaservicesorvouchforstudentstatus.Thecollegedoesnotofferhighschooldiplomaprograms.Toqualifyforenrollment:

1. Theapplicantmustbeatleast18yearsold(forVNstudents;requirementbyBVNPT).2. TheapplicantmustbeahighschoolgraduateorGEDequivalent.3. PHYSICAL REQUIREMENTS Collegedoesnotdiscriminatebasedonphysicalhandicaps.However,studentsmustbementallyandphysicallycapableofsafelyparticipatinginclassroom,clinical/externinstructiondailytosuccessfullycompletetheprogram.IntegrityCollegeofHealthencouragesallstudentswhomayhaveaconcernregardingaphysicalissuetovisittheschoolandallowtheschooltoevaluateindividualcircumstances.Theschoolmayallowastudenttoattendaportionofthetrainingonatrialbasis(uptofivedays)withoutbeingobligatedfortuitioncharges.(Foradditionalinformation,referto“StudentswithDisabilities”).

ADMISSIONS PROCEDURES Applicantsmustapplyforadmissionsatanytimepriortoenrollment.ApplicantsmustsubmitacompletedAdmissionsApplicationtotheSchooltoapplyforenrollment.Proofofidentificationintheformofadriver’slicense,socialsecuritycard,birthcertificateorotherformofidentificationmustberequiredpriortoadmission.TheWonderlictestmustbepassedatqualifyingpassrateforthedesiredprogram(passratevariesbyprogram).FinancialAidorpaymentarrangementsmustbearranged,andstudentmustbeabletomeetfinancialobligations.ProgramDirectorordesigneemust complete admission interview and provide approval before enrollment. Student to payapplicableregistrationfee.BACKGROUND CHECKS & DRUG SCREEN Acriminalbackgroundmaypreventsomeonefromobtaininglicensure,clinicaltraining,externshipplacement or employment in their chosen field. When a criminal background must be fullyinvestigated, itmay takeup tooneyearor longer inorder toobtainadecision froma licensingboard.Applicantsorstudentswithacriminalbackgroundshouldcheckwiththerelevantlicensingbodyfortheirstancetowardsvariouscriminalbackgroundsituations.Studentswill not be allowed to continue their enrollment in the following programswithout acurrentcriminalbackgroundcheckdeemed“negative”or“clear”:VocationalNursingDiploma,andMedicalAssisting.Astudent’sbackgroundcheckwillbeinitiatedimmediatelyuponenrollment&nolaterthanthefirstdayofclass.Abackgrounddeemedtobe“clear”isrequiredtoremainactiveinanyoftheaboveprograms.Thecostofacriminalbackgroundcheckisincludedinthetotaltuitionoftheseprograms,but students in the Vocational Nursing program must pay for the criminal background checkrequiredbytheCaliforniaBoardofVocationNursing&PsychiatricTechnicianswhenapplyingfortheVocationalLicense.Studentsmaybesubjecttoadditionalbackgroundchecks,possiblydrugscreening,atanytimeifrequestedbyaclinicalfacility,employer,orexternshipsite.FailuretopassanybackgroundcheckordrugscreenrequestedbyanExternship,EmployerorClinicalSitemaypreventastudentfromgoing to that Externship or Clinical Site & may put their program completion at risk. Clearbackground checks are required for registration in clinical nursing courses. A student may bedeniedaccesstoclinicalfacilitiesbasedonoffensesappearingonthecriminalrecordeventhoughsuchoffensesmayhaveoccurredmorethansevenyearsago.

ALTERNATE STUDENT POLICY WhenanewVNCourseisscheduledtostart,andhasthemaximumnumberofstudentsenrolled,astudentmaybecomeanalternatestudentincaseofacancellationordropinthecourse.Eachnewcohortmaytemporarilyaccept2additionalstudentstemporarilyadmittedtoIntegrityCollegeofHealthfornomorethansix(6)weeks,whilewaitingforapositiontoopen.Thestudentwillattendeachscheduledclass,andparticipateinlectures,includingtesting.AsanAlternate,thestudentwillnotberequiredtopayanymonthlyfeesordeposit. ENGLISH PROFICIENCY CollegedoesnotprovideEnglish-as-a-Second-Language(ESL)instruction.Alltesting,lecturesandclinicalcommunicationareinEnglish,andmusthavedemonstratedproficiencyofatleast10thgradelevel. This will be evaluated by theWonderlic entrance exam. Students are required to passWonderlic entrance exam at 10th grade level. Students are required to speak Englishwhen aninstructionalsettingnecessitatestheuseofEnglishforeducationalorcommunicationpurposes.AllcoursesaretaughtinEnglish.

TRANSFER OR EXPERIENCE CREDIT Theawardingofcreditsforcourseworkcompletedatanyotherinstitutionisatthesolediscretionof College. ACCIDENTS/ILLNESSES/INSURANCE/SAFETY On campus all accidents which occur during or after classes involving personal injury and/ordamagetoequipmentmustbereportedimmediatelytotheinstructororProgramDirector.

A. Accidents involving personal injury must be reported to the Advisement Centerimmediately.

B. StudentsmustbecoveredbyStudentLiabilityandMedicalinsurance.

OffCampusIllnessorAccidentswhenstudentisatafacility.Whenastudentbecomesillorisinjuredatanaffiliatinghealthcareagency,thefollowingprocedureistobefollowed:

A. Iftheinjuryisofanabsoluteemergencynatureorisareportablehospitalincident,thestudentreceivesinitialemergencycareattheinstitutioninwhichtheywereinjured.

B. If the student belongs to an HMO, a pre-paid medical pan, or has a private healthinsuranceplantheyobtainallexceptessentialemergencycarefromtheirownagency,ordesignatedphysician.

C. If the student has no health insurance, they contact Integrity College of HealthAdvisementCenterOfficeandreceivedirectionfromthatoffice.

D. Allinjuriesincurredatanaffiliatingclinical/externagencymustbereportedtotheICHAdvisementCenterOfficeregardlessofwheretheyareobtainingcarewithin24hoursoftheoccurrenceoftheincidenttocompletenecessaryforms.

E. All injuries incurred at an affiliating clinical/extern agency must be reported to theinstructor in charge of the clinical course within 24 hours of the occurrence of theincident.

1. COMMUNICABLE DISEASE Studentsknowntobeinfectedorsuspectedofbeinginfectedwithacommunicabledisease(asdeterminedby theRegulationsof theCalifornia StateDepartmentofHealth Services for theControlofCommunicableDiseases)willbelimitedorrestrictedfrompatientcontactuntiltheyarefreeofsuchdiseaseorincapableoftransmittinginfection.Theprocedure,shouldthisoccur,is:

A. Students will report infections immediately to their Clinical Instructor, and theVocationalNursingorMedicalAssistingProgramDirector.

B. TheClinicalInstructorandtheProgramDirectormutuallyagreeuponareassignment,whichwill limitorrestrictstudent frompatientcontact.Consultationwiththe facilityinfectioncontrolnurseshouldbeobtainedifindicated.

C. If reassignment cannot bemade, studentswill be sent home. (SeeAttendancePolicy,whichremainsinforce.)

D. Ifastudenthasbeensenthome,medicalclearancefromthestudentphysicianmustbeobtainedbeforereturningtotheClinicalfacilityorclassroomsetting.

3. HAZARDOUS MATERIALS

IntegrityCollegeofHealthunderstandsthathazardousmaterialisasubstance,beitsolid,liquidorgas,iscapableofharminghumans,propertyandtheenvironmentifmishandled,stored/disposedincorrectly.ICHmaintainssafehandlinganduseofthesematerialsthroughtrainingwithproperinstructions,preciselabelling,storage,disposal,and,provisionofmaterialsafetydatasheets(MSDS).Contaminatedmaterialssuchasneedles,syringes,etc.,mustbedisposedofinthesharpcollectorscontainers.Theuseofthesecontainersismandatory,and,studentsarenotallowedtodisposethesharpcollectorunderanycircumstances.

4. EXPOSURE TO RADIATION TheICHcampusdoesnothaveanyradiationequipmentincampus,butdependingontheclinicalfacilitiesandontheassignedarea,havethepotentialriskforstudentswhichmayincludetheexposuretoradiation.ThepolicyofICHforallstudentswhoareexposedwhileparticipatingintheeducationalprogram,istoobtainimmediatemedicalattentionand/orlaboratoryfollowupsasnecessary.

5. FIREARMS AND WEAPONS IntegrityCollegeofHealthhaszerotolerancetoanytypeofviolencecommittedbyoragainstanyofICHfaculty,staff,and/orstudentsincludingphysicaland/orverbalthreats.Threateningstudents/employeesinengaginginviolentactsareprohibited.Firearmand/orweaponpossessionisabsolutelybannedincampusandwithinthebuilding’sproperty.Violationofthepolicyconstitutegravemisconductthatmayleadtostudentterminationandnotificationtothelocalpolice/lawenforcement.IntegrityCollegeofHealthhavesurveillancecamerastomonitorthecampus.

6. HEALTH SERVICES INJURY PROTOCOL:

FINANCIAL AID EffectiveOct13,2013,IntegrityCollegeofHealthoffersFinancialAidforqualifyingstudents.WedoparticipateinFederaland/orStateFinancialAidprograms.Afterpaymentofregistrationfeeandsuccessfulcompletionofentranceexam, theFinancialAidOfficer isavailablebyappointmenttodiscussfinances.

HOUSING IntegrityCollegeofHealthdoesnotofferonoroffcampusdormitoryaccommodationsandhasnodormitoryfacilitiesunderitscontrol.StudentsmayfindapartmentsnearCollege.Rentchargesvaryper location, sizeofapartment,and facilities in thecomplex.Cost rangesofapartments inclosevicinityrangefrom$1,095(studio)to$3,000(3bedroom).Thecollegehasnoresponsibilitytofindorassistastudentinfindinghousing.

Thewrittennoticeofcancellationneednottakeanyformandiseffectiveifthewrittennoticeshowsthat you no longer wish to be bound by this Agreement. You will be given two [2] Notice ofCancellationformstouseatthefirstdayofclass,butyoucanuseanywrittennoticethatyouwish.Iftheschoolhasgivenyouanyequipment,includingbooksorothermaterials,youshallreturnthemtotheschoolwithinthirtydaysfollowingthedateofyourNoticeofCancellation.Ifyoufailtoreturnthisequipment,includingbooksandothermaterials,ingoodconditionwithinthethirty[30]dayperiod,theSchoolmaydeductitsdocumentedcostfortheequipmentfromanyrefundthatmaybeduetoyou.Onceyoupayfortheequipment,itisyourstokeepwithoutfurtherobligation.IfyoucancelthisAgreement,theSchoolwillprorateanymoneythatyoupaid,lessanydeductionfor equipment not timely returned in good condition, within thirty days after your Notice ofCancellationisreceived.Note:

Ifastudentobtainsaloantopayforaneducationprogram,thestudentmustrepaythefullamountoftheloanplusinterest,lesstheamountofanyrefund.Ifthestudenthasreceivedfederalstudentfinancialaidfunds,thestudentisentitledtoarefundofmoneys not paid from federal student financial aid programs. Withdrawing from a Course after the Seventh (7th) Day after enrollment Youhavetherighttowithdrawfromacourseofinstructionatanytime.YouhavetherighttocancelthisAgreementandobtainarefundofchargespaidthroughattendanceatthefirstclasssession,orthe seventh day after enrollment, whichever is later. The School will remit a refund less aregistrationfee,ifapplicable,nottoexceed$75.00withinthirtydaysfollowingyourwithdrawal.Youareobligatedtopayonlyforeducationalservicesrenderedandforunreturnedequipment.Therefundshallbetheamountyoupaidforinstructionmultipliedbyfraction,thenumeratorofwhichisthenumberofhoursofinstructionwhichyouhavenotreceivedbutforwhichyouhavepaid,andthedenominatorofwhichisthetotalnumberofhoursofinstructionforwhichyouhavepaid.Ifyouobtain equipment, as specified in the Agreement as a separate charge, and return it in goodconditionwithin thirty days following the date of yourwithdrawal, the School shall refund thechargefortheequipmentpaidbyyou.Ifyoufailtoreturntheequipmentingoodcondition,allowingforreasonablewearandtear,withinthisthirty-dayperiod,theSchoolmayoffsetagainsttherefundthedocumentedcosttotheschoolforthatequipment.Youshallbeliablefortheamount,ifany,bywhichthedocumentedcostforequipmentexceedstheproratedrefundamount.Thedocumentedcostoftheequipmentmaybelessthantheamountcharged,andtheamounttheSchoolhaschargedinthecontract.Inanyevent,youwillneverbechargedformorethantheequipmentchargesstatedinthecontract.Foralistofthesecharges,seethelistonthefirstpageofthisagreement.IFTHEAMOUNTYOUHAVEPAIDISMORETHANTHEAMOUNTYOUOWEFORTHETIMEYOUATTENDED,THENAREFUNDWILLBEGIVENWITHINTHIRTYDAYSOFWITHDRAWAL.IF

THEAMOUNTYOUOWEISMORETHANTHEAMOUNTYOUHAVEALREADYPAID,THENYOUWILLHAVETOMAKEARRANGEMENTSTOPAYTHEBALANCE.Studentswhoseentiretuitionandfeesarepaidbyathird-partyorganizationarenoteligibleforarefund.

HYPOTHETICAL REFUND EXAMPLE: Assume that a student, upon enrollment in a 400-hour course, pays $2,000 for tuition, $75 forregistrationand$150,documentedcost to school, forequipmentas specified in theEnrollmentAgreementandwithdrawsaftercompleting100hourswithout returning theequipmenthe/sheobtained.Theproratarefundtothestudentwouldbe$1,462baseduponthecalculationsstatedbelow.Ifthestudentreturnstheequipmentingoodconditionwithinthirty[30]daysfollowinghis/herwithdrawal,theSchoolshallrefundthechargefortheequipmentpaidbythestudent.RefundCalculation

Forthepurposeofdeterminingtheamountyouoweforthetimeyouattended,youshallbedeemedtohavewithdrawnfromthecoursewhenanyofthefollowingoccurs:

Ø YounotifytheSchoolofyourwithdrawalortheactualdateofwithdrawal.Ø TheSchoolterminatesyourenrollment.Ø Youfailtoattendclassesforathree[3]weekperiod.Inthiscase,thedateofwithdrawalshall

bedeemedtobethelastdateofrecordedattendance.Ifanyportionofyourtuitionwaspaidfromtheproceedsofaloan,thentherefundwillbesenttothelenderortotheagencythatguaranteedtheloan,ifany.Anyremainingamountofrefundwillfirst be used to repay any student financial aid programs fromwhich you received benefits, inproportiontotheamountof thebenefitsreceived.Anyremainingamountwillbepaidtoyou. STUDENT TUITION RECOVERY FUND (STRF) EffectiveJanuary1,2015STRFwillbereported,butnofeesarerequiredtobecollected.TheStudentRecoveryFund (STRF)wasestablishedby theLegislature toprotectanyCaliforniaresidentwhoattendsaprivatepostsecondaryinstitutionfromlosingmoneyiftheyprepaidtuitionand suffereda financial lossasa resultof the school closing, failing to liveup to its enrollmentagreementorrefusingtopayacourtjudgment.TobeeligibleforSTRF,youmustbeaCaliforniaresidentandresideinCaliforniaatthetimetheenrollmentagreementissignedorwhenyoureceivelessonsataCaliforniamailingaddressfroman approved institution offering correspondence instruction. Students who are temporarilyresiding inCalifornia for thesolepurposeofpursuinganeducation, specifically thosewhoholdstudent visas, are not considered a California resident. Orientation Orientationsaregenerallyheldwithinthemonthpriortothestartdateofprogram.Ifthestudentsdidnotattendorientationororientationwasnotheldbeforestartofcourse,orientationwillbeprovidedonthefirstdayorclass.Emphasizedduringthesemeetingsareissuesrelatedtobehavior,commitmenttoobtainingemployment,educationalobjectives,studentrightsandresponsibilities.

TRAINING DESCRIPTION Training and instruction takes place at the School’s facility and or designated training facility.Classroom and field training may take place in separate locations, as deemed necessary toaccommodatelocalresourcesandconditions.Forexample,classroomand/orlecturetrainingmaytake place in a classroom/lecture environment and/or a field/lab environment. The number ofstudentsperclassrangefrom15studentsto30lecturestudentsandupto20laboratorystudents.Generally,classeswillreceiveorientationtrainingasasinglegroup;however,theSchoolmaydeemappropriatetodividetheclasssizeintosmallerunits.

HOMEWORK Students should considerhomework as an integral part of their trainingprogram. Students areexpectedtobepreparedforupcomingcoursematerialandreviewpreviouslylearnedinformationonaregularbasis. Homeworkmaybeassignedbyindividualinstructorsonanas-neededbasis.Studentsshouldbepreparedforhomeworkassignmentsandallowtimeeachdayforhomework.

STUDENT CONDUCT

CLINICAL/EXTERNSHIP EVALUATION 1.Theinstructorevaluatestheachievementlevelofthestudentanddeterminesifthestudenthassatisfactorily completed course objectives. This evaluation is based on behavioral objectivesdescribed in vocational nursing and medical assisting courses with a clinical rotation. Thebehavioral objectives for each course and evaluation rating scale define the standards for a CR(credit) grade for the clinical portion of the Vocational Nursing Courses. Medical AssistantpreceptorsandMAInstructorwillcompletetheexternshipevaluation.Ratingscaleandevaluationcriteriaaredistributedtostudentsinwritingatthebeginningofthesemester.2.EarnafinalgradeofCR(credit)inclinicallaboratoryportionsofnursingcoursesandexternshipoftheMedicalAssistingprogram.IfassignedaNoCredit(NCR)asaclinicalperformancegrade,thestudentisnotmeetingSAPwillfailthesemester.Studentmustrepeatsemesterorisdismissedfrom program. If the student is assigned aminimal pass (MP), the studentwill be referred toremediation.

A. Students who are assigned minimal pass must remediate to successfully meet SAP.

B. Students,whodonotearnafinalgradeof“CR”(credit)inlaboratoryorclinicalportionsof program, after remediation, will be dropped from the program. To continue inprogram,studentmustrepeatthesemester,includingtheoryandclinical(appropriatetuitionchargesapply).

QuantitativeRequirement-PaceofCompletionVNstudentsmustsuccessfullycomplete100%ofallattemptedcumulativeclockhourstograduatefromaprogram.MAstudentsmustsuccessfullycomplete80%ofallclockhours.TheMaximumTimeframe tocompleteallhoursarewithin150%oroneandahalf times thescheduledprogramlength.Forexample,ifaprogramis12monthsinlength,thenthestudentwillhaveamaximumof18monthsintotaltosuccessfullycompleteallrequirementsfortheprogram.SAPEvaluationPeriods:Therearespecificandroutineevaluationperiodsduringtheprogramofstudytodetermineifthestudentismeetingsatisfactoryacademicprogress.TheICHSAPEvaluationPeriodisperformedforallstudentsatthesametimesduringtheprogram.TheSAPEvaluationPeriodisthesameastheFinancialAidpaymentperiodforthosestudentsthatareeligibleforFinancialAid.Presently,the

MA Program is a total of 800 hours, the SAPEvaluationPeriodisonlydoneatthe400-hourperiod.SAPEVALUATION1. SAPAppealProcessWhenastudentlosesFSAeligibilityduetofailuretomakesatisfactoryprogress,theyarepermitted a Financial Aid Appeal. The student has 10 business days followingnotification. Appeal formcanbeobtained from theFinancialAidOfficer. When theAppealFormiscompleted,itissubmittedtotheProgramDirectorforanAppealPanelreview. TheAppeal Panel consists of theProgramDirector, Lead Instructor, and theFinancialAidOfficer.Studentmayappealtheresultbasedonthefollowing:Hisorherinjuryorillness;thedeathofarelative;orotherspecialcircumstances. Thestudents’appeal must clearly explain the identified reasons for failure to make satisfactoryprogress.Theappealmustalsoidentifychangesthatwillallowandsustainsatisfactoryprogressatthenextevaluation.TheAppealPanelwilldetermine,basedonthesubmittedappeal, if the student should be able to meet the SAP standards by the end of thesubsequentpaymentperiod.SAP-Probation

The student has had their federal student aid eligibility reinstated as the result of anapprovedappealandisplacedonSAP–Probationstatus.ProbationlastsuntilthenextSAPEvaluationPoint.ThestudentwillbereturnedtogoodSAPstandingifstudentismeetingallSAP requirements at the SAP Evaluation Point. IncompleteIncomplete (I)will be given if a student is unable to complete coursework. The (I)willbecomeaFailure(F)iftheworkhasnotbeencompletedwithin14daysfromtheendofthecourse(e.g.VNI-VNXII).Ifcourseworkiscompleted,theGPAwillberecalculatedwiththepassinggrade. If thegradeprogressestoa(F), theGPAwillberecalculatedfollowingthegrade change. For repeating students, the second grade replaces the first grade. REINSTATEMENT AstudentwhosesuspensionhasresultedindisqualificationfromaProgrammaybereinstatedintheProgramatthenextavailableentrypoint,ifspaceisavailable;approvaloftheProgramDirector,theproblemcausingthedismissalhasbeenappropriatelyremediated.A student who is disqualified from the Program for unsatisfactory attendance or forunsatisfactorycompletionofalectureportionofacourse,orfailuretomeettheprerequisiterequirementsforthecourse,willbereinstatedintheprogramatthenextscheduledentrypoint,upon application by the student and upon satisfactory completion of the written terms forreinstatement, if any.Reinstatement is contingentuponavailable spaceandapprovalby theProgramDirector.

A student who is disqualified from the Program for not passing may be reinstated in theprogram at a time determined by the Program Director. Students thus disqualified mustcompletethetermsforreinstatementasdelineatedinwrittencommunicationgiventhestudentat the time of the disqualification. Students are permitted only one reinstatement fordisqualificationfromtheprogramforunsatisfactoryacademicand/orclinicalperformance,andexcessiveabsence.Astudentwhohastwicebeendisqualifiedshallnotbere-admittedtotheprogram.

Studentswhointerrupttheirownprogressintheprogram,andwhohavesatisfactoryacademicand attendance records, and passing clinical may be reinstated in the program at the nextavailableentrypointprovidingspaceisavailable.

Studentswhofailtomeettheprerequisiterequirementforacoursewillbereinstatedatthenextavailable entry point providing space is available, after they complete the prerequisiterequirementandsubmittherequireddocumentation.

Studentswhowishtore-entertheProgrammustbeevaluatedbytheProgramDirectorpriortore-entry.Conditionsforre-entrywillbeoutlinedinacontractgiventothestudentandinserted

EXITEXAMS/AssessmentTesting(VocationalNursing)ALLATIASSESSMENTEXAMSMUSTBECOMPLETEDANDPASSEDTOGRADUATEFROMTHEVOCATIONALNURSINGPROGRAM.Integrity College of Health (ICH) provides assessment tools and materials for our students toprepareandassesshowpreparedtheyare to take theNCLEX-PNexamaftergraduation. Theseassessmenttoolsaretothebenefitofthestudenttoidentifyspecificsubjectareasofstrengthsandweaknesses.Therearefocusedreviewmaterialsthatcanhelpthestudentbecomeproficientintheareas of weaknesses. ICH utilizes the preparation and assessment materials from AssessmentTechnologiesInstitute(ATI).ATIisacompanythathasalong-demonstratedhistoryinprovidingtheresourcesandassessmenttoolsfornursingstudentsinpreparationfortheNCLEXexam.Thepurpose of using these assessment andpreparationmaterials, is to prepare the student for therigorsoftheofficialtest,andwillbetreatedwiththeutmostseriousness,asallexamsarerecognizedthroughouttheprogram.ATI Examinations are taken by second and third semester students. There is a total of sevenspecialtyexamsinpreparationforthefinal“ComprehensivePredictor”exam.Studentsareallowedtwoattemptsperspecialty.Ifuponcompletionofthefirstattemptofanexam,thestudentisnotsuccessfulatachievingatleasta“level1”,thestudentwillhaveremediation.ATIprovidesareviewprogramcalledthe“focusedreview.”Aftertakingtheassessmenttest,itshowsareasofweakness

Ifthesecondattemptisnotsuccessfulinmeetingatleasta“level1”,thestudentwillmeetwiththeProgramDirectorordesignee,re-evaluatetheremediationplan,completeafocusedreview,andscheduleathirdtestingdate.AnytimeastudentrequirestakingtheATIexaminationmorethantwice,thestudentmustpurchasetheexamfromATI.Studentwillnotbeabletoproceedtothenextassessmenttestuntiltheyhavepassedthepresenttest.TheATIassessmentsdonotreflectonthesemesterlettergrade,althoughallmustbecompletedbeforegraduationandstudentcannotgraduateuntilallarecompleted.TheATIComprehensivePredictorExamisthefinaloftheATIeightexams. TheComprehensivePredictormustbepassedwithascoreof77%orhigherinthe“PredictedProbability”ofPassingNCLEX-PNontheFirstAttempt.Ifthestudentdidnotobtaina“77%PredictedProbabilityofPassing NCLEX on First Attempt,” or above, the student will follow the same procedure inremediation as stated above. The student cannot graduate from the program withoutcompletingallexamsatthelevelsindicated.Dateswillbescheduledforthe“retake”exam,andmustbeperformedonatimelybasis.StudentswillbeissuedanIncomplete(I)ifallATITestshavenot been completed. Students have 30 days after graduation date to complete ATI exams. Ifassessmentexamsarenotcompletedwithin30daysandpassedallothersemesterrequirements,maintainingatleast75%gradeincoursework,thestudentwillhavetheoption:

1. StudentwillbeeligibletoenrollinIndependentstudy. Studentwillcontinuetocompletetheassessmentexamsuntilmeetinggraduationstandards.Studentwillberesponsibleforthe cost of subsequent remediation ATI assessments exams during Independent Study.Studentmustcompletewithinthe150%programlimit(6-monthsfromprogramenddate)orthestudentwillnotgraduate. Thetranscriptwill indicate,“GraduateNO”,“PassedExitExamNO.”

2. IfstudentisunabletocompleteExitexams,ICHcanassistthestudenttoapplytoBVNPTto“Challenge”theNCLEX-PNexam.StudentwillhavetheoptiontochallengetheNCLEX-PNasa“non-graduate”ofanAccreditedNursingProgram.IfsuccessfulpassingtheNCLEX-PNexam, the student will be able to obtain the license as a LVN, but there are importantlimitations.ThestudentcanonlyusethelicenseinCalifornia,anddoesnothavereciprocityto other states; there may be restrictions to applying to Registered Nursing Programs(withoutgraduationfromanaccreditednursingprogram);somehospitalsandfacilitiesmaynothirewithoutgraduationfromanaccreditednursingprogram,evenwiththeVocationalNursingLicense.Thisisnotthecolleges’proceduralrecommendationforastudenttoobtaintheirlicense.Ifthestudentclearlyunderstandsthelicenserestrictions,andwantstopursuethechallengeoption,ICHwillassistwiththeapplicationprocess.Asanote,ifthestudentisunable tosuccessfullypassATIassessment testing in thecollege, thismay indicate thestudentmaynotpasstheNCLEX-PNduringthechallenge.

Remediation/StudentSuccessProgramwillberequiredwhenstudentobtainstestscoreonexamlessthan75%.VNStudentsaregiventwoopportunitiesinthefirstsemestertoretakeregularclasstestsandonechanceforretakesinsecondandthirdsemester.Finalexamscannotberepeated.

ATTENDANCE Studentsareexpectedtoattendeachscheduledclassmeeting,laboratorysession,andclinicalorexternshipassignmentprintedintheofficialscheduleofclassesand/orassignedbytheinstructorduring the semester. The student is expected to report on time for all classes and laboratoryassignments,includingthosescheduledinoff-campusfacilities.Up to16missedhoursmaybemadeup inTheory. Theoryhoursmustbemadeup in classbyassignmentsgivenbytheinstructor.Thestudentmustschedulethemake-uptimeoncampuswithaproctor.Thismayincludepresentations,research,conceptmaps,etc.Anytheoryhoursmissedover 16 hours is considered excessive, and the student will be at risk of being dropped fromprogram.Up to18Clinical hoursmaybemadeupduring the lastweekof the semester forVNstudents.Upto16hoursmayberescheduledwithexternshipsiteforMAstudents. TARDINESS Tardiness timeand time lostby leaving lectureand laboratoryearlyarecumulativeandwillbeappliedtoabsencetime.Anytardinessinexcessof4hoursmustbemadeupeitherinclassroom,skillslaborclinical.Cumulativetardinessleadingtoexcessiveabsencesof18hoursormorewillbenotedinstudentfile,andstudentisatriskofbeingdropped.Astudentmaypetition forre-admissionbecauseofexcessabsencesonlyonceduringtheentireprogram.

ABSENCES Studentsareexpectedtoattendalltheoryandclinical/externship.Thestudentistogiveatleast30-minute notice to school or instructor of the reason he or she will be absent, as well as anydocumentation pertaining to the nature of the absence. Attendance is a vital component of thecurriculum.Studentsareallowedonlytwoexcusedclinical/externshipabsences(mustbemadeupinclinical/externship),andtwotheoryabsences(whichmustbemadeupwithassignments).Anyadditionalabsenceswillbeconsideredexcessiveandmayresultinthestudenthavingtorepeatthecourseorsemester.FailureforaVNstudenttonotifytheclinicalinstructorthirtyminutesinadvanceofanabsencefroma scheduled clinical assignment, so that patient assignmentsmaybe adjusted,will result in theissuance of a “Warning Probation.” MA students must notify the externship preceptor andInstructoratleast30minutesinadvanceofanabsencefromanexternshipassignment.AsecondoccurrenceoffailuretonotifytheVNclinicalInstructororMAInstructorinatimelymannerwillresult in the student being dropped from the course. STUDENT SUSPENSION AND MATTER OF STUDENT IMPAIRMENT FROM ALCOHOL, DRUG ABUSE AND EMOTIONAL ILLNESS Aninstructormaysuspendastudentfromclassorclinicalareaif,intheprofessionaljudgmentoftheinstructor,thestudentposesadangertothephysicalsafetyofthestudentortoothers.IntegrityCollegeofHealth’scatalogdescribesregulationspertainingtoappropriatebehaviorinclassrooms. Inthematterofstudentsimpairedbyalcoholism,drugabuseandemotionalillness,theVocationalNursingfacultyrecognizes:

1. Thesearediseasesandshouldbetreatedassuch.2. Personal and health problems involving these diseases can affect academic and clinical

performanceandthattheimpairednursingstudentisadangertoselfandagravedangertotheclientsinherorhiscare.

3. Nursingstudentswhodevelopthesediseasescanbehelpedtorecover.4. Itistheresponsibilityofthenursingstudenttovoluntarilyseekdiagnosisandtreatmentfor

anysuspectedillness.5. Confidentialhandlingofthediagnosisandtreatmentofthesediseases isessential. Impaired

students pose a danger to self and a grave danger to the patients in the students care. Aninstructor may suspend an impaired student from class in accordance with Section A theinstructorwill notify the ProgramDirector. The Vocational Nursing ProgramDirectorwillnotifytheAdvisementCenterofthesuspension.TheProgramDirectorwillrecommendoneofthefollowing:

POLICY FOR TEMPORARILY IMPAIRED STUDENTS Studentswhobecometemporarilyimpairedduetoillness,injury,surgicalprocedure,orpregnancywhile in the program will provide the school with a letter from his/her doctor stating whatrestrictionspertain to thestudent’sability toperforms in theclassroomandclinical/externshipsettings.Everyeffortwillbemadeonthepartoftheschooltoenablethestudenttocontinuesafelyin the program and still receive the required learning experience. If the studentsmust take amedicalleaveofabsence,thestudentmaynotreturntoschooluntilawrittenreleaseisreceivedfromthephysician.Students completing the programwill complete required applications. Critical thinking requires these intellectual abilities to provide optimalnursing care. The goal of the Clery Act is to ensure students, prospectivestudents,parentsandemployeeshaveaccesstoaccurateinformationaboutcrimescommittedoncampusandcampussecurityprocedures.Itrequiresinstitutionsofhighereducationthatreceivefederalfinancialaidtoreportstatisticsonspecifiedcrimesonornearourcollegecampusandtoprovideothersafetyandcrimeinformationto members of the campus community. Allvictimsofcrimesorthosethathaveobservedacrimewillbedirectedandassistedtocontactlocallawenforcement. Ifthevictimfeelsunabletoreportthecrime,theCollegewillrespectthevictims'confidentialityandanonymityrequestaslongastheCollege,students,andstaffandnotplacedatanyriskordanger.IftheCollegedeterminestheremaybearisktostaffandstudents,locallawenforcementwillbecontacted.

AnyincidentinvestigationbytheCollegeandresolutionwillbemaintainedconfidentially.PrivacyoftherecordsspecifictotheinvestigationismaintainedinaccordancewithstatelawandthefederalFamilyEducationalRightsandPrivacyAct(FERPA).Anypubliclyreleasedreportsofcrimeswillnotbecontainnameornamesofvictimsorinformationthatcouldleadtotheidentificationofthevictim.InformationaboutvictimsismaintainedprivatelyinaccordancewithTitleIXandFERPA.Theaccusedandtheaccuserarebothentitledtothesameprivacyandconfidentiality.Bothpartieswillbeable tohaveequalrights inpresenting theircasewithwitnessesduringahearingat thecampus.

Once an informal or formal complaint of sexual assault, misconduct or harassment has beenreceived, an investigationwill be conducted under themost confidentialmanner. Disciplinaryactionforastudentthathasfoundtobeguiltyofoffensecanbeexpelled,suspended,orplacedonprobation. Staff accused and found guilty of sexual harassment,misconduct or assault will beterminated.

DrugandAlcoholAbusePreventioninformationInaccordancewiththeDrug-FreeWorkplaceActof1988(P.L.100-690),theDrug-FreeSchoolsandCommunitiesActof1989(P.L.101-226)and34CodeofFederalRegulationPart84,SubpartF,thisinstitutioniscommittedtomaintainingadrug-freeworkplaceandadrug-freeschool. Drugandalcoholabusecanleadtoliver,heartandotherchronicdiseases,lowbirthweight,birthdefectsandinfant mortality in expectant mothers, and death. The unlawful manufacture, distribution,dispensing,possessionoruseofdrugs,alcoholorothercontrolledsubstancesatthisinstitutionisstrictly prohibited.
