How to Complete the DS-11 Passport Application To begin, visit https://pptform.state.gov

On the first page, click on the ‘Complete Form Online’ in the lower left corner.

The first page is where you will enter all of your vital information.

Please take your time for each field and make sure you are spelling everything correctly.

Any mistakes will cause your application to be delayed.

Enter your contact information on the next page. Then when you are done, click next.

This page is not required, but we highly recommend entering your travel plans.

Proof of Travel Plans are required by the U.S. Department of State for expedite applications.

This page is not required, but you should complete each section to help with the processing of your

passport application.

The passport wizard is a ‘smart form’ that creates the passport application that you need,

based on the information you submit.

Since this is a DS-11 application explanation page, you are likely entering ‘None’ for this page.

The page that requires your parent information (and spouse information if you are or were married)

must be filled out as thoroughly as possible.

This page is a REVIEW PAGE. This is NOT the passport application.

Review the information here and make sure every section is correct and accurate.

Scroll down to the bottom of the page and click ‘NEXT’. DO NOT CLICK PREVIEW.

You must go to the NEXT PAGE.

Make sure you have scrolled to the bottom of the form and clicked “NEXT” and NOT ‘preview form’.

If you click preview form, you will not be done with the application.

You will not be able to print it from there.

This is a payment page, but you are not actually paying here.

You will pay the government fees in the form of a check when you send us the documents.

Select any option and then click “Next”.

Click anything to move past this page. You’re printing that form and sending it to us.

This is the final page of the application. Scroll towards the bottom and click the checkbox that says

“I have read and acknowledge the steps and information contained above.”

Then, click “Create Form” in the bottom left.

This will create a .pdf file that will automatically save to your computer.

Save this file and then print out this PDF. This is your DS-11 Passport Application.

Do not sign this form until you are at the Acceptance Agent.
