• Analyze recent studies regarding rational transfusion strategies in different patient populations

• Appraise the recent literature regarding common inpatient situations (e.g., hepatic encephalopathy, severe alcoholic hepatitis, penicillin allergy)

• Apply recent evidence to the care of patients with acute ischemic stroke

• Adding rifaximin to lactulose is more effective than lactulose alone in the

treatment of overt hepatic encephalopathy

• Adding pentoxifylline or acetylcysteine to prednisolone does not improve 6-month

survival, but adding IV NAC might improve short-term mortality

• 72 yo right-handed woman is admitted with R arm weakness and expressive aphasiashe noticed upon awakening. Head CT w/o contrast is normal. MRI confirms MCA stroke

Penicillin Allergy

• PRE-PEN®– Available in 0.25 mL vials

– Necessary but not sufficient for allergies to the major determinant

• Negative skin tests indicates <<< 5% chance of reaction to PCN (mostly dermatologic)

Pre-Pen®: Procedure

• Prick test– Volar forearm– Using needle (22-28 ga), place a drop on

skin, then prick through the drop– Read at 15 min (or before) for wheal,

erythema, pruritis• POS = wheal >5 mm

– If Negative, then intradermal

• Intradermal– Upper outer arm– Tuberculin syringe

• Create 3 mm (0.02 mL) blebs with PRE-PEN and saline control

– Read at 20 minutes• POS = pruritis and increase bled to >5 mm
