Frecuencia: < 1% de todas las lesiones traumáticas.
Lesiones asociadas: 60-90% de los casos.
Hasta 40-60% de rupturas diafragmáticas pasan inadvertidas en el episodio agudo.
Diagnóstico tardío se asocia con un mayor riesgo de hernia y estrangulación, potencialmente mortales.
Cuando se identifica, se establece la indicación quirúrgica (abierta o mínimamente invasiva).

COMPLICACIONES DE LAS LESIONES DIAFRAGMÁTICAS.
Los pacientes que sufren un traumatismo cerrado tienen mayores tasas de complicaciones (60%) en comparación con aquellos que tienen trauma penetrante (40%)
1. Hernias.
2. Parálisis diafragmática.
3. Complicaciones pulmonares (contusión, hemotórax, derrame
pleural, empiemas.
4. Fístulas pleurales.

CIRUGÍA
1. El abordaje abdominal, es considerado unánimemente, como la vía de elección en el caso de la rotura diafragmática en su fase aguda.
2. El abordaje torácico, en la fase aguda, sólo se realiza cuando es necesario realizar una toracotomía de emergencia, generalmente por hemorragia masiva y en el curso de ella se diagnostica la lesión diafragmática. Se considera de elección, en la fase crónica sin complicaciones.

Dx de lesiones traumáticas: pacientes con trauma toraco-abdominal izquierdo, estables y sin indicación clara de laparotomía o toracotomía de urgencia por lesiones asociadas. VENTAJAS
Mayor incidencia de lesiones asociadas intraabdominales. Posibilidad de reparación de la lesión diafragmática y otras lesiones. Evita el drenaje torácico si no hay lesiones intratorácicas asociadas. Eventual exploración torácica transdiafragmática.
DESVENTAJAS:
Difícil visualización de las heridas posteriores (15% lesiones inadvertidas) Embolia gaseosa. Neumotórax a tensión.

VENTAJAS: No requiere insuflación. Sensibilidad y especificidad cercanas al 100%. Lesiones penetrantes derechas posteriores y <3cm. DESVENTAJAS: Sólo evalúa un hemidiafragma. No se pueden evaluar lesiones intraabdominales.
Toracoscopia

El abordaje laparoscópico tiene el potencial de reducir la tasa de complicaciones quirúrgicas con un retorno más rápido al nivel de actividad normal Procedimiento es técnicamente posible y seguro en centros con amplia experiencia en cirugía laparoscópica. Excelente visión del campo quirúrgico y el acceso a las lesiones.
La laparoscopia tiene el potencial para convertirse en el estándar de oro para el
tratamiento de las hernias diafragmáticas adultos en un futuro próximo.

La lesión traumática de diafragma es una entidad poco frecuente.
Su diagnóstico es difícil y requiere de un alto índice de sospecha clínica.
En la fase aguda la mortalidad esta determinada por la severidad de las lesiones asociadas.
Los métodos mínimamente invasivos, han tenido un impacto fundamental en el diagnóstico de estas lesiones.

Existe suficiente consenso en que el abordaje de este tipo de lesiones durante la fase aguda, es por vía abdominal, dado que permite el diagnóstico y tratamiento de lesiones asociadas concomitantes.
En la hernia diafragmática postraumática, en su fase crónica se prefiere el abordaje torácico.
Independientemente de la vía de abordaje utilizada, sea por vía convencional o por procedimientos mínimamente invasivos, así como el tipo de material de sutura utilizado y la técnica de reparación, el objetivo fundamental debe ser el diagnóstico precoz y el cierre definitivo de la brecha diafragmática.
