Getting Started Welcome

Welcome to the TrackRecord™ Data Management Software. This program allows home users, Doctors and Healthcare Professionals to manage blood glucose results. This program also assists Doctors and Healthcare Professionals in the review, analysis, and evaluation of blood glucose test results to aid in diabetes management.

The TrackRecord™ Data Management Software can:

• Manage glucose results,

• Download test results from compatible Home Diagnostics, Inc. meters,

• Help quickly identify glucose readings that are too high or too low,

• Help identify meal related concerns,

• Help identify results within, above, or below target blood glucose levels.

The TrackRecord™ Data Management Software Kit contains:

• TrackRecord™ Data Management Software CD,

• Instruction Manual for TrackRecord™ Data Management Software,

• SmartData™ Cable,

• SmartData™ Cable Driver Installation CD with instructions.

If any of the items listed above are missing from the Kit, contact Technical Services at 1-800-803-6025 for a replacement.

Items needed that are not included in the Kit:

• Computer with:

• Pentium III or higher,

• Windows operating system XP or Vista

• USB port,

• CD-ROM drive,

• Home Diagnostics, Inc. meter with upload capability (see meter Owner’s Booklet for information on data upload capability) or logbook with results.

Install Install SmartData™ Cable

1. Remove SmartData™ Cable Driver Installation CD from jewel case. 2. Insert the CD, label side up, into the CD-ROM in the computer. Close the

CD-ROM drive. 3. Right click on “Start” at the bottom left of the computer screen and click on

“Explorer”. Go to the CD-ROM drive and double click on the file “preinstaller.exe” to begin the install. More information on installing the SmartCable™ driver is found in the SmartCable™ Instructions for Use.

4. Press “Finish”. 5. Remove CD from CD-ROM drive and replace in jewel case. 6. Locate the USB port on the computer and plug in the SmartData™ Cable.

7. Found New Hardware Wizard screen appears. 8. Click “No” and “Next”. 9. Click “Automatically find software” and “Next”. 10. Click “Finish”

Install TrackRecord™ Data Management Software 1. Remove the TrackRecord™ Data Management Software CD from the jewel

case. 2. Insert the CD, label side up, into the CD-ROM in the computer. Close CD-

ROM drive. 3. The install program automatically starts.

Note: If the install program does not immediately start, right click on “Start” at the bottom left of the computer screen and click on “Explorer”. Go to the CD-ROM drive and double click on the file “Setup.exe” to begin the install.

Note: To cancel the setup at any time, click “Cancel” on the screen.

4. The Choose Setup Language box appears. Click “OK” for English”. For Spanish, click on the arrow to access the drop down menu and highlight “Spanish”. Click “OK”.

After a few moments, the TrackRecord Data Management Software – InstallShield Wizard screen appears. Click “Next” to begin setup.

6. The License Agreement screen appears. Use the scroll bar on the right side to read the entire agreement. Click “Print” if a paper copy of the agreement is needed. Click “Yes”.

Note: If “No” is clicked, setup closes and program is not installed. If “Back” is clicked, the previous screen is shown

7. The Choose Destination Location screen appears. The install saves the program in the C drive under Program Files/Home Diagnostics/TrackRecord DMS. Click “Next”.

Note: If the program is to be stored in another file location, click “Browse” and choose where the program is to be saved.

8. The Setup Status screen appears and indicates the progress of the file install.

9. When the program is finished with the install, the InstallShield Wizard Complete screen appears.

In the middle of the screen is a checked box beside “Launch TrackRecord DMS when Finished”. If working with the TrackRecord™ DMS program immediately after installation, leave box with the check mark. TrackRecord DMS program opens immediately after clicking “Finish”.

If not working with the TrackRecord™ DMS program immediately after installation, remove the check mark by clicking once on the box. TrackRecord™ DMS program will not open immediately after clicking “Finish”.

Note: To work with TrackRecord™ DMS program later, click on TrackRecord icon on desktop or click “Start” and then “Programs”. Click on “Home Diagnostics, Inc” and then “TrackRecord DMS” to launch program.

Setup TrackRecord™ DMS Software When opening TrackRecord™ DMS program for the first time, the Setup screen appears. Read and follow Steps 1, 2, and 3.

Step 1 Use the drop down menu to highlight the correct User Mode. “Multiple – Office, Clinic, etc” is used when there will be more than one patient entered into TrackRecord™ DMS. “Single – Home User” is used when there will be one patient entered into TrackRecord™ DMS.

Note: If “Skip, Setup Later” is clicked, TrackRecord™ DMS starts at the Main Menu screen. The User Mode defaults to “Multiple – Office, Clinic, etc.” and the meter data port is not configured for uploading of results. The User Mode may be changed by clicking the System Settings icon on the Main Menu screen. See System Settings for more information.

Step 2 Click “Meter Connection”. The Meter Quick Setup screen appears.

Note: SmartData™ Cable driver must be installed before Step 2 can be completed.

Follow the three steps on the Meter Quick Setup screen. After clicking “Setup”, the Meter Quick Setup screen shows “Detecting Meter”.

Note: For connecting the SmartData™ Cable to the computer and the meter, see Install, Install SmartData™ Cable.

If the cable and meter are connected correctly, the Setup screen shows “Setup Complete”.

Meter Quick Setup Screen “Meter Not Detected”

Unplug the cable from the meter and the computer and plug back in. Click “Setup”. If the Meter Quick Setup screen appears again with Meter Not Detected, contact Technical Services at 1-800-803-6025.

Step 3 After the User Mode is chosen and the Meter Setup is complete, click “Finish”. The TrackRecord™ DMS program goes to the Main Menu screen.

If “Skip, Setup Later” is clicked, TrackRecord™ DMS starts at the Main Menu screen. The User Mode defaults to “Multiple – Office, Clinic, etc.” and the meter data port is not configured for downloading of results. The User Mode may be changed by clicking the System Settings icon on the Main Menu screen. See System Settings for more information. The meter data port may be configured later. See Downloading Results section for more information.

Note: Consult a Doctor or Diabetes Healthcare Professional about using this software to help manage patient’s diabetes. Always ask a Doctor or Diabetes Healthcare Professional when there is a question about the patient’s diabetes care plan.

Home Diagnostics, Inc. does not recommend accessing or altering the database of the TrackRecord™ Data Management System Program. Home Diagnostics, Inc. will not be liable for data manipulated outside of this program.

Uninstall If the database is to be saved, be sure to move the database file (TrackRecord.mdb) found in the TrackRecord DMS program folder to a safe storage area on the PC. See File/Options/Help, Options for more information on saving and restoring data.

1. Click on “Start” at the lower left of the computer screen. 2. Click on “Settings” and “Control Panel”. 3. Click on “Add/Remove Programs”. 4. Find and highlight the TrackRecord™ Data Management Software Program

and click “Remove”. 5. Follow the Uninstall Instruction Wizard.

Note: If the TrackRecord™ Data Management System program is to be re-installed into the computer, reboot the computer before re-installation.

Using the TrackRecord™ DMS Beginning the TrackRecord™ DMS

If a TrackRecord™ Data Management Software icon is on the Desktop, double click on the icon.

Note: If there is no TrackRecord icon on the desktop, click on “Start” at the lower left of the computer screen. Go to “Programs”, find “Home Diagnostics” and click on “TrackRecord™ Data Management Software”.

TrackRecord™ DMS opens to the Home Page and then automatically goes to the Main Menu screen.

Note: The Home Page shows if the system has been idle for more than 10 minutes of inactivity. Click on the screen and the last screen that was showing appears.

The Main Menu screen contains five icons: Download Results, Patient Management, System Settings, Reports, and an internet link to the Home Diagnostics, Inc. website.

- Transfer results from a Home Diagnostics meter with upload capabilities to the TrackRecord™ DMS.

- Add new patients, patient information, view patient information, edit patient information, delete patient. Add, delete, edit results manually, set schedule for mealtimes and bedtimes.

- Customize profiles for time/date formats, meter com port, set default schedule for mealtimes and bedtimes, set default favorite report, set units of measure, set result type.

- Print reports such as a summary report, log book report, conformance report, glucose trend report, and extended log book report.

Patient Management Managing Patients

Configuring TrackRecord™ DMS User Mode Note: If the User Mode was chosen in the Setup screen, skip to Adding a New

Patient in this section. Note: Meter does not have to be plugged into the SmartData™ Cable when

managing patients.

If the User Mode was not chosen in the Setup screen and “Skip, Setup Later” was clicked, the User Mode must be setup before entering patient information or downloading results.

Configure for Single Patient

If there will only be one patient, it is recommended to switch to the “Single” mode. This reduces the number of clicks to perform many functions within the program. To switch the user mode to “Single”:

1. From the Main Menu screen, click on the “System Settings” icon.

2. Use the drop down menu for the User Mode to highlight “Single”.

4. The Main Menu screen appears. TrackRecord™ DMS is now configured for only one patient.

Configure for Multiple Patient If the system will be used for more than one patient, it is recommended to set the User Mode to “Multiple – Office, Clinic, etc.”. This allows adding, viewing, deleting, and finding more than one patient. Follow the steps for Configure for Single Patient but choose “Multiple – Office, Clinic, etc.” for the User Mode instead of “Single”. The TrackRecord™ DMS will be configured for multiple patients.

Note: If “Skip, Setup Later” is clicked in the Setup screen during installation, the User Mode defaults to “Multiple – Office, Clinic, etc.”. The User Mode may be changed at any time, but if “Single” User Mode is chosen and multiple patients have been entered in the system, only the first patient entered can be viewed. The User Mode must be on “Multiple – Office, Clinic, etc.” to add a new patient or to view, change, or delete patients previously entered in the system.

Patient Information On the Patient Information screen, begin by typing in a unique identifying number for the patient, such as a social security number or a patient ID number.

Note: Single User Mode does not have a Patient ID number.

The fields listed in BOLD (FIRST NAME is in first block, last name, street address, and city) must be entered. Middle initial, Gender, Date of Birth, State, Postal Code, Country, Phone, and email address is optional.

Doctor / Insurance Click the Doctor / Insurance tab on the Patient Information screen. Information on this screen is not required. Type in doctor information, diabetes educator information, primary insurance information and secondary insurance information. All information on this page is optional.

Healthcare Click the Health Care tab on the Patient Information screen. The Health Care screen appears. Fields entered on this tab reflect the results that are viewed in the program and how the reports are printed. Setting choices on this screen affect the information printed on the reports and the way results are viewed on the Meter Results tab.

Click on the drop down menu next to “Diabetes Type”. Click on the diabetes type (unknown, type I, type II, or gestational) for the patient.

Click on the drop down menu next to “Glucose Units”. Click on the glucose units of measure (mg/dL or mmol/L) used for results.

Enter glucose target ranges (before meal, after meal, overall target, and hypoglycemic) for the patient.

Medication Click the Medication tab on the Patient Information screen. The Medication screen appears. On this screen, any medications, including insulin, may be added for reference.

The Add Insulin screen appears. Click on the insulin type to highlight and click “OK”. If name is not listed, click on the Additional Insulin box and type in the name.

Note: More than one insulin type may be added at the same time. To add more than one insulin type from the list, press and hold the Ctrl button while clicking on the insulin name. Release the Ctrl button and click “OK” when finished.

The Add Medication screen appears. Click on the name of the medication and click “OK”. If medication name is not listed, click on the Additional Medication box and type in the name.

The first box shows the meters with their serial numbers from which results have been downloaded. The second box shows results with the date, time, timeslot (from Schedule screen) and if the result is a control result or a manually entered result. Results that are entered manually are marked with a checkmark under the Manual column. Control results are marked with a checkmark under the Control column.

Results Downloading Results From a Meter

Download results to TrackRecord™ DMS from any Home Diagnostics meter that features upload capability (see meter’s Owner’s Booklet to find out if the meter has upload capability).

Connecting to Meter Before initiating a download, the meter must be connected to the computer that is running the TrackRecord™ DMS program. Connect the meter to the computer by using a USB SmartData Cable.

Initiating a Download Once the meter and the computer are connected to the SmartData™ Cable, start the TrackRecord™ DMS program (see Beginning the TrackRecord™ DMS). The Main Menu screen appears. Begin the download by clicking on the Download Results icon from the Main Menu screen.

If the Download Results icon from Main Menu is chosen, Download Results screen appears and TrackRecord™ DMS begins to connect to the attached meter. The first picture flashes as the meter and the computer are connecting.

After the download is complete, if the meter’s serial number has already been assigned to a patient in the “Multiple – Office, Clinic, etc.” or if “Single” User Mode was chosen, the Patient Information screen appears with the assigned patient information.

All meter results contained in the meter’s memory are downloaded into the meter results tab. Multiple downloads of the same result, time, and date are not added. If more results are added to the meter’s memory, only the results that have not been previously downloaded are downloaded to the meter results tab.

Delete the meter by highlighting the meter in the first block and clicking on “Delete Meter” on the right side of the computer screen. Enter the date of the result. The date defaults to today’s date in the format mm/dd/yyyy. To change the date, click on the month and type the correct month, then click on the day and type the correct day, and click on the year and type in the correct year. Click on the hour in the time box. Enter the hour of the result. Click on the minutes in the time box. Enter the minute of the result. Click on “AM” and enter AM or PM.

Reports Tips on Getting Good Reports

Reports are generated to give an overall pattern of blood glucose results over time. This pattern is used by Doctors and Healthcare Professionals to prescribe medication and/or evaluate goals. Comparing individual results from different meter models is NOT recommended for identifying the true overall pattern because results may vary and this may not reflect the true pattern(s). Once results are entered, TrackRecord™ DMS can run 5 different reports.

Report Types Summary Report The Summary Report provides a quick snapshot of the extended log book, glucose trend, and conformance reports. The Header of the report shows the patient name, patient address, city/state, glucose target ranges, hypoglycemic range, reference method, date of report, Doctor name, and date range of report. The Body of the report contains the extended log book report, a glucose trend report, and an overall conformance report.

Log Book Report The Log Book Report shows a record of all the glucose readings in a time period chosen when printing the report. Log Book Reports may aid in insurance billing purposes. The Header of the report shows the report date, patient name, address, doctor, diabetes type, if insulin is required or not, the date range of the report, the units of measure, and the result type. The Body of the report shows the results sorted in order by date and time. Manually entered results and control test results are indicated as such. The number of blood glucose results is calculated, as is the number of controls, the number of test strips used, and average tests per day.

Conformance Report The Conformance Report is a pie chart report that helps identify meal related problems and which results are within, above, or below target ranges for each time slot. The Header of the report shows the patient identification, the report date, and the time range chosen when printing the report. The Body of the report details the conformance of the glucose results for each time slot. Each chart is divided into percentages of results within target, above target, below target, and hypoglycemic percentages. Pre- and Post- Meal charts are shown side by side for comparison to identify trends. The Pre-Meal charts are stacked as well as the Post-Meal charts for comparison of trends. If there is no result for a time slot, the place is held by the words “No Results”.

Glucose Trend Report The Glucose Trend Report displays possible trends in glucose results. This report helps quickly identify quickly glucose readings that are too high and too low. The Header of the report identifies the patient name, the doctor, and the date range specified for the report. The body of the report is set up as an X-Y graph with the glucose results with the units of measure chosen in the Health Care tab under the Patient Information screen on the Y axis and the date/time on the X axis. The top shaded area indicates the high target glucose range, the middle shaded area indicates the low target glucose range, and the lowest shaded area indicates the hypoglycemic range. The results are displayed as data points (dots) connected with a line. The dots (results) appear in chronological order from oldest to most recent. All results for the date range specified are included in the report.

Note: The line that connects the glucose results in the graph does not reflect actual glucose levels. Mealtimes, medication, or exercise can cause blood glucose levels to be higher or lower throughout the day. Actual testing before and after these events are required to get a better picture of how they affect glucose levels.

Extended Log Book Report The Extended Log Book Report is a more comprehensive report than the Log Book Report. The Header of the report shows the patient name, patient address, city/state, doctor, glucose target ranges and hypoglycemic level chosen in the Health Care tab of the Patient Information screen, reference method, the date of the report, and the date range of the report chosen when printing the report. The Body of the report details the results in a table format. The results are listed within their assigned time slots. The report then calculates the average result for the time slot, the number of results above the target range for that time slot, the number of results below the target range for that time slot, the number of hypoglycemic results for that time slot, and the percentages for above target, below target, within target, and hypoglycemic.

The Meter Results screen appears. The results used in the report are from the meter(s) with a check mark next to the meter. If a meter’s results are not to be included in the report, click on the check mark of the meter not to be included.

Running a Report After clicking “Next” on the Select Meters for Report screen, the Select a Report/Select a Date Range screen appears. Select the type of report needed by clicking on the circle next to the chosen report.

Note: For a description of the report types, see the section Reports, Report Types.

Next, select the date range. Click on the circle next to either “last 7 days”, “last 14 days”, “last 30 days”, “last 60 days”, or “last 90 days. These date ranges give a report using today’s results and the previous results for the days chosen.

If “Custom Date Range” is chosen, a date range may be entered manually or chosen using the drop down menu. The drop down menu shows the days in the months and may be changed by clicking on the arrows on either side of the month to go back and forth. When the chosen month appears, click on the day to add it to the “From” or “To” section on the screen.

The Report Preview screen appears. The preview report may be printed to the default printer by clicking on the Print button on the top left of the screen. To return to the Select a Report/Select a Date screen, click on the Back icon on the left side of the screen.

System Settings System, Patient and Schedule Settings

System Setting may be accessed either by clicking on the System Settings icon on the Main Menu screen; System Settings icon from the left side of the Patient Information screen; or by clicking on “Options” in the upper left of any TrackRecord™ DMS screen and then clicking on “System Settings”.

On the System Settings screen, there are 3 tabs: System Settings, New Patient Settings, and Default Schedule.

System Settings Meter COM Port Setting If the Meter COM Port has not been set up during installation (see Setup TrackRecord™ Software, Step 2), use drop down menu to enter the COM port where the SmartCable™ is plugged into the computer (1-10). If a serial SmartData™ Cable is used, most computers have a labeled COM 1 and COM 2 port already set up in the computer.

Click “Meter Connection” at the bottom of the System Settings screen to set up the COM port.

The “Meter Quick Setup” screen appears. Follow the directions on the screen. To plug in the SmartCable™ to the meter and computer, see Downloading Results. Once the SmartCable™ is plugged into the meter and computer, click “Setup”.

Note: To cancel the meter setup, click “Cancel”. The System Settings screen reappears.

If the COM port and the meter are detected and the COM port is set up correctly, the screen sends the conformation, “Meter Setup Successfully”. Results may be downloaded from the meter.

If the COM port and/or the meter are not detected, the screen sends the notice “Meter not Detected”.

If the “Meter not Detected” message appears, unplug the meter and the USB connector and reconnect. (To plug in the SmartData™ Cable to the meter and computer correctly, see Downloading Results.) Repeat the directions on the “Meter Quick Setup” screen. If the “Meter not Detected” notice still appears, contact Technical Services at 1-800-803-6025.

Note: The SmartData™ Cable driver must be installed before COM ports can be configured. See the SmartData™ Cable Drivers and User Documentation Manual for more information on installing the SmartData™ Cable drivers.

System Idle Setting From the drop down menu, choose System Idle. “Enabled” returns to the Home Page after 10 minutes of inactivity.

If “Enabled” is chosen for System Idle and TrackRecord™ DMS is not used for 10 minutes, the Home Page screen appears. Press the space bar or click on the screen to return to the last screen before inactivity. It is recommended to enable this feature for privacy reasons. “Disabled” does not return to the Home Page after 10 minutes of inactivity.

Date Format Setting Date format – TrackRecord uses your current Windows Regional Date Settings programmed from your computer. The Date Format cannot be changed using TrackRecord DMS. To change the Date Format used by TrackRecord, exit TrackRecord DMS and change the Date Format programmed in the Windows Regional Settings in your computer. TrackRecord DMS is compatible with any of the date formats listed below.

Time Format Setting Time format (24 hour clock or 12 hour clock). – The current Windows Regional time setting. This is non-editable from TrackRecord DMS. To change the time format the user must exit TrackRecord DMS and go to Windows Regional settings and change the time format.

User Mode Setting If the User Mode (Single, Multiple) was not chosen during installation, use the drop down menu to choose if TrackRecord™ DMS is to be used for a single patient or multiple patients.

Synchronize Clock Setting Use the drop down menu to choose if meter time/date is to be synchronized with the computer clock when results are downloaded.

Note: Synchronizing the meter time/date with the computer clock will change the meter time and date to match the computer time/date.

• Working Days and Non-working Days (Mon – Sun)

File There are 5 choices under “File” in the upper left of the Main Menu screen.

• New Patient Cntrol+N – Brings up the Patient Information screen. Patient Information screen can also be accessed by pressing the Control key on the computer keyboard simultaneously with the “N” key.

• Print Reports Cntrl+P – Brings up the Reports screen. Select a Report/Select a Date Range screen can also be accessed by pressing the Control key on the computer keyboard simultaneously with the “P” key.

• Print Setup – Allows the report printer to be changed.

• Export – Brings up Export Patient screen. Allows highlighting and adding of patient’s information / results to another program database (Excel or Access).

Options There are 3 choices under “Options” in the upper left of the Main Menu screen.

Main Menu Screen

Backup Backup and restore data by going to the “Backup” and “Restore” options located in the “Options” menu on the Main Menu screen. The “File / Save” screen appears. The filename defaults to “TrackRecord” with the current date.

Note: The file name can be changed.

Restore To restore the patients and their results from a prior backup, select “Restore” from the “Options” menu on the Main screen. The “File Selection” screen appears. Highlight the backup file to restore and click “Open”. If a patient’s name does not exist in the active database but it is in the backup database, the name and information with the results are added to the active database when the backup is restored. However, if a patient’s name does exist in the active database, only the results from the backup are restored to the active database. Existing results and information is never overridden.

System Setup System Setup brings up the “System Setup” screen where new patient profiles can be customized for time/date formats, meter COM port are set, default schedules are entered, etc. See System Settings for more information.
