Unihealth Source Program

Designed to provide elderly, disabled and low-income persons a wide range of long-term health services, either in their home or the community.

Program requirements are:• 65 years old or older and/or disabled• Receiving Medicaid and SSI• Medical conditions such as diabetes, high blood

pressure, lung disease, arthritis, Alzheimer’s disease, dementia, heart disease, or stroke that require monitoring and regular doctor’s care.

Independent Care Waiver Program

This program is designed to provide services to individuals with physical and/or traumatic brain injuries. This program helps individuals stay in their community, but provides them with same level of care as being in a hospital.

Community Care Services Program

This program is designed by Medicaid to provide Personal Support Services to individuals needing assistance to carry out their Activities of Daily Living due to an illness, injury or old age.
