1. Complete and sign all required registration documentation and return to Heavenly

Cradle as soon as possible

2. The following documentation has been included for your convenience to complete at

home and make copies of when and if required and to be handed over to Heavenly

Cradle when bringing your child to the daycare

a. Permission to administer Non-Prescription Medication

b. Permission to administer Prescription Medication

3. The following additional documentation need to be given to Heavenly Cradle with the

registration documentation.

a. Copies of ID documentation for Mother, Father, Guardian and persons who

would have your permission to pick up or drop off your child

b. Copy of child’s Birth Certificate

c. Copy of child’s Immunization Chart

d. Photo of child (as resent as possible)

AGREEMENT

THIS IS AN AGREEMENT ENTERED INTO BY AND BETWEEN:

(Hereinafter called “the Baby Nursery”)

REMUNERATION

1. REMUNERATION FOR CARE The remuneration for the care of the Child is: Full day R1050.00 per month, Yes Half day R750.00 per month and Yes Part time R120 per hour Yes (Please tick “YES” on the service you require) Subject to increase from time to time agreed upon. Fees are payable in advance in cash or EFT on the first day of every month. No cheques are accepted. If the Parent does not comply timorously with the conditions of this clause, the Baby Nursery is entitled to charge a penalty of 5% on each and every payment which is late and may also refuse to take the Child into her care until such monies and penalties have been paid. No refunds will be allowed for such periods during which the Child is not placed in the care of the Baby Nursery, during the existence of this agreement. 2. TERMINATION

This agreement can be terminated by either party giving one calendar months’ notice, which must be given on the last working day of the preceding month. 3. REGISTRATION FEE The Parent shall pay a non refundable registration fee of R500 for Full Day and registration fee of R250.00 for Half Day. Registration fee could be paid over the 1st 2 months that the Child is placed in the care of the Baby Nursery. 4. LEAVE 4.1 The Baby Nursery Owner/Manager is entitled to twenty two (22) working days paid leave per annum. A minimum of ten continuous days must be taken in one session, unless otherwise agreed to by the Parent. The Parent will be notified three (3) months in advance prior to the Day mother taking leave. The Parent hereby agrees to pay the Day mother for twelve (12) months of the year. 4.2 The Baby Nursery Owner/Manager may decide herself when such leave shall be taken during the year but this will usually be in accordance with school holidays. 4.3 Should this agreement be terminated for any reason whatsoever, the Baby Nursery Owner/Manager shall be compensated for any leave due to her, subject to a maximum of twenty two (22) days. The compensation for such twenty two working days leave shall be calculated in proportion to the remuneration per month as set out in clause 1 above. 4.4 If an emergency arises relating to the family of the Baby Nursery Owner/Manager or the Day mother herself, the Baby Nursery Owner/Manager shall be entitled to take the necessary compassionate leave. Compassionate leave will be paid for by the Parent. 4.5 If an extreme emergency or serious illness or death arises, the Nursery will remain open and the parent do not have to make alternative arrangements for the care of their child. 4.6 Heavenly Cradle Baby nursery will only be closed between 16 December and first working day of January the next year. 5. SICK LEAVE The Baby Nursery Owner/Manager is entitled to fifteen (15) days paid sick leave per annum, subject to the condition that such sickness is a serious contagious disease certified by a medical practitioner or in the case of hospitalization and recuperation. During this period, the Parent do not have to arrange for alternative care for the Child / Children as supervision would be in place. 6. TIME OF CARE From Monday to Friday, with the exclusion of public holidays when no service shall be rendered, the Child will remain in the care of the Baby Nursery Owner/Manager from 06:30 to 17:30. 7. DAILY NECESSITIES All clothes and possessions of the Child must be clearly marked. Sufficient alternative clothing and all daily necessities not included in the care option and necessities as determined by the Baby Nursery Owner/Manager must be supplied.
