We carried out an announced comprehensive inspectionat Heaton Norris Health Centre 2 on 4 August 2016. Thepractice was rated as requires improvement for four keyquestions (Safe, Effective, Responsive and Well Led). Thisresulted in an overall rating of requires improvement. Thefull comprehensive report on the 4 August 2016inspection can be found by selecting the ‘all reports’ linkfor Heaton Norris Health Centre 2 on our website atwww.cqc.org.uk.

This inspection was undertaken following the receipt ofan action plan that confirmed the practice would meetthe regulatory requirements previously identified by 30November 2016.

At the beginning of December 2016 the practice providedadditional information in order to demonstrate theimprovements they were making.

This inspection was an announced comprehensiveinspection on 22 February 2017.

Overall the practice is now rated as inadequate.

Our key findings across all the areas we inspected were asfollows:

• There was a system in place for reporting andrecording significant events. At this inspectionrecorded evidence in the form of team meetingminutes demonstrated that staff were kept informedof the outcome of significant event investigations.

• We noted since the last inspection that recruitmentchecks had improved so that appropriate recruitmentrecords, including Disclosure and Barring Servicechecks (DBS) for staff employed at the practice were inplace. However we observed that one employee’srecruitment file was missing references.

• Some risks to patients were assessed, however thepractice could not demonstrate that they had done allthat was reasonably practicable to ensure patientswith chronic health conditions were reviewed andassessed.

• Quality and Outcomes Framework (QOF) data for 2015/16 showed performance indicators for some patientoutcomes were below the local and national average.Unverified data for the partial year from April 2016 to22 February 2017 did not assure us that the practice

performance had improved in reviewing patients withlong term conditions. A recorded action plan tomonitor and review the practice performance was notavailable.

• At the last inspection we found records of mandatorytraining were available for some staff but trainingrecords were not consistently maintained for thepractice nurse and health care assistants employed atthe practice. At this inspection training records for staffincluding clinical staff were available.

• The practice had good facilities and was equipped totreat patients and meet their needs. Patients said theywere treated with compassion, dignity and respect andthey were involved in their care and decisions abouttheir treatment.

• Some patient feedback informed us that getting aroutine appointment usually required at least a twoweek wait and it was on occasion difficult to get anurgent appointment.

• Information about services and how to complain wasavailable and easy to understand but where similarconcerns had been expressed by patients, no actionhad been taken to minimise reoccurrence.

• Governance arrangements to monitor and review theservice provided were not supported by clearobjectives and actions plans. This had resulted in gapsin service delivery and performance.

• The provider was aware of and complied with therequirements of the duty of candour.

The areas where the provider must make improvementsare:

• Implement action to mitigate any risks to patients andto ensure care and treatment is provided in a safe way.

• Implement comprehensive systems of governance tomonitor and review the practice performance andimplement strategies to improve, including:▪ Analysing significant events and patients verbal

complaints to identify themes and take action tomitigate risk of reoccurrence.

▪ Implementing a system to track and monitor thereceipt and use of prescription paper.

▪ Undertaking regular infection control audits.▪ Providing planned and recorded support to the

practice manager with regular meetings andappraisal.

In addition the provider should:

• Improve monitoring of receipt of all the necessarypre-employment checks for all staff includingobtaining professional and character references.

• Improve communication networks with externalhealth care professionals.

• Review the availability of of non-urgent appointments.• Continue efforts to identify patients who have caring

responsibilities.• Continue to try to recruit patients to establish a Patient

Participation Group (PPG).

I am placing this service in special measures. Servicesplaced in special measures will be inspected again withinsix months. If insufficient improvements have been madesuch that there remains a rating of inadequate for anypopulation group, key question or overall, we will takeaction in line with our enforcement procedures to beginthe process of preventing the provider from operating theservice. This will lead to cancelling their registration or tovarying the terms of their registration within six months ifthey do not improve.

The service will be kept under review and if needed couldbe escalated to urgent enforcement action. Wherenecessary, another inspection will be conducted within afurther six months, and if there is not enoughimprovement we will move to close the service byadopting our proposal to remove this location or cancelthe provider’s registration.

Special measures will give people who use the service thereassurance that the care they get should improve.

The five questions we ask and what we foundWe always ask the following five questions of services.

Are services safe?The practice is rated as requires improvement for providing safeservices.

At our previous inspection on 4 August 2016, we rated the practiceas requires improvement for providing safe services as thearrangements in respect of staff recruitment were not good enough.

These arrangements had partially improved when we undertook afollow up comprehensive inspection on 22 February 2017. However,other gaps in service provision were identified. The practice remainsas requires improvement for providing safe services.

• Significant events and incidents were investigated and areas forimprovement identified and implemented. Team meetingminutes provided evidence that outcomes from significantevents were discussed. Lessons were shared to make sureaction was taken to improve safety in the practice. However ananalysis of significant events to identify themes and to respondappropriately to mitigate recurrence was not in place.

• The practice had systems, processes and practices in place tokeep patients safe and safeguarded from abuse.

• Recruitment procedures had improved althoughcomprehensive records for one employee were not available.

• Some risks to patients were assessed and managed, howeversystems to monitor infection prevention and control and thereceipt, stock and monitoring of prescription paper were notimplemented.

Are services effective?The practice is rated as inadequate for providing effective services.

At our previous inspection on 4 August 2016, we rated the practice asrequires improvement for providing effective services as datashowed patient outcomes were low compared to the local andnational average. Sufficient numbers of suitably qualified staff werenot deployed to meet patients’ needs.

Evidence reviewed at the follow up comprehensive inspection on 22February 2017 did not provide us with assurance that the practicehad improved patient outcomes. The practice is rated inadequatefor providing effective services.

• Data from the Quality and Outcomes Framework (QOF) in2014-2015 showed performance indicators for some patientoutcomes were below the national average and the practice’s

own performance in previous years. QOF data for 2015-2016showed that the practice’s performance in some areas haddeteriorated further, and unverified data supplied by thepractice for the year commencing April 2016 until 22 February2017, did not indicate the practice had improved theirperformance.

• Clinical audits demonstrated quality improvement.• Members of the primary care engagement team attended the

practice to provide patients with their flu injections in theautumn of 2016. The primary care team initiated this supportbecause the practice had had a low uptake of the flu vaccineand Public Health England were concerned for patient’s’health. The practice advised us after the inspection that theyhave a health care assistant employed who was trained toadminister flu vaccinations.

• The children’s surveillance team confirmed that data for 2015/16 children’s immunisations and vaccinations was low. Theyidentified that the lack of a suitably qualified staff member hadresulted in parents not being able to make appointments fortheir children.

• We heard that the two GP partners maintained regular contactto discuss clinical issues. However, a written record to supportthese clinical discussions was not available.

• The practice manager had implemented a plan of regularsupport meetings with the practice nurse and health careassistants. However there was no documented clinicalmeetings between the GPs and the clinical nursing and healthcare team.

• Staff received mandatory and role specific training.• Staff had had annual appraisals although the practice manager

stated it was over two years since she had an appraisal.

Are services caring?The practice is rated as good for providing caring services. At ourprevious inspection on 4 August 2016, we rated the practice as goodfor providing caring services as data showed patient were generallysatisfied with the service they received.

Evidence reviewed at the follow up comprehensive inspection on 22February 2017 confirmed no change in rating for this key questions.

• Data from the national GP patient survey from July 2016showed patients rated the practice at a comparable level toother practices in the locality.

• Patients said they were treated with compassion, dignity andrespect and they were involved in decisions about their careand treatment.

• Information for patients about the services available was easyto understand and accessible.

• The practice had started to build a list of patients who werealso carers.

Are services responsive to people’s needs?The practice is rated as requires improvement for providing effectiveservices.

At our previous inspection on 4 August 2016, we rated the practice asrequires improvement for providing responsive services as thearrangements in respect of responding to patients’ needs andpatient access to timely appointments was not good enough.

These arrangements had not improved when we undertook a followup comprehensive inspection on 22 February 2017. The practice isremains as requires improvement for providing responsive services.

• Data available indicated patients’ health care needs were notbeing met in a timely manner.

• Information received from external health care professionalsindicated that communication from the practice about issuesexperienced at the practice, were not always communicatedeffectively.

• Feedback from patients reported that there was frequently await up to two weeks for a routine appointment, althoughurgent appointments were usually available the same day.

• The practice logged nine verbal complaints in relation toappointments for 2016/17. Evidence of action to respond to therepeated patient concern was not available.

• The practice had good facilities and was well equipped to treatpatients and meet their needs.

• Information about how to complain was available and easy tounderstand. The practice had responded appropriately to onewritten complaint since the last inspection. Logs of verbalcomplaints were maintained but action taken in response tothis was not proactive.

At our previous inspection on 4 August 2016, we rated the practice asrequires improvement for providing well led services as thegovernance arrangements to monitor service delivery were notadequate.

These arrangements had not improved when we undertook a followup comprehensive inspection on 22 February 2017. The practice isnow rated as inadequate for providing well led services.

• The GP partners were the practice owners and clinical careproviders but evidence that the partners were proactive indeveloping and improving the service and leading the smallstaff team was not available.

• The practice had a statement of purpose which detailed thepractice’s aims and objectives and there was a businessstrategy in place. However this had not been reviewed andthere were no specific goals or actions identified to improveservice delivery and patient outcomes.

• The practice told us about how they were addressing theshortfalls to improve their QOF performance but no specificrecorded action plan to address and monitor progress for eachidentified area had been developed.

• Actions taken by the practice were reactive and not proactive.• The practice team was small and there was a clear staffing

structure and staff were aware of their own roles andresponsibilities.

• Staff told us there was an open culture within the practice andthey had the opportunity to raise any issues on a day to daybasis and felt confident and supported in doing so.

• The provider was aware of and complied with the requirementsof the duty of candour. The practice had systems in place fornotifiable safety incidents and ensured this information wasshared with staff to ensure appropriate action was taken.

• A patient participation group (PPG) was not yet established,however two patients we spoke with confirmed they had beenapproached to join a PPG.

Older peopleThe provider was rated as inadequate for two key questionseffective, and well-led and requires improvement for providing safeand responsive services. The concerns identified overall affected allpatients including this population group.

• Quality and Outcomes Framework (QOF) date for 2015/16 andunverified data for 2016/17 showed a deterioration inperformance when compared to the local and nationalaverages.

For example:

• 2015/16: 96% of patients with COPD (chronic obstructivepulmonary disease) had a review undertaken including anassessment of breathlessness using the medical researchcouncil dyspnoea scale in the preceding 12 months, which wasbetter that the Clinical Commissioning Group (CCG) average of91% and the England average of 90%. However unverified QOFdata for this indicator for the partial year from 1 April 2016 to 22February 2017 showed the practice’s achievement to havedeteriorated at 78%.

• Members of the primary care engagement team also attendedthe practice to support them with their flu campaign in theautumn of 2016. The primary care team initiated this supportbecause the practice had had a low uptake of the flu vaccineand Public Health England were concerned for patients’health. The practice advised us after the inspection that theyhave a health care assistant employed who is trained toadminister flu vaccinations.

• Gold Standard Framework (GSF) or palliative care meetingswere held approximately every two to three months, andcommunity health care professionals attended these.

• Quality and Outcomes Framework (QOF) date for 2015/16showed that the registered provider performed poorly whencompared to the local and national averages. Unverified datafor 2016/17, the partial year (April 2016 to 22 February 2017) didnot show significant improvements.

For example:

• The record of diabetic patients with a blood pressure reading140/80mmHG or less recorded within the preceding 12 monthswas 52%, compared to the Clinical Commissioning Group (CCG)average of 81% and the England average of 78%. UnverifiedQOF data for this indicator for the partial year from 1 April 2016to 22 February 2017 showed the practice’s achievement as 48%.

• 28% of patients with diabetes registered at the practicereceived a diabetic foot check compared with the CCG averageand the England average of 88%. Unverified QOF data for thisindicator for the partial year from 1 April 2016 to 22 February2017 showed the practice’s achievement as 38%.

• Longer appointments and home visits were available whenneeded.

Families, children and young peopleThe provider was rated as inadequate for two key questionseffective, and well-led and requires improvement for providing safeand responsive services. The concerns identified overall affected allpatients including this population group.

• Quality and Outcomes Framework (QOF) date for 2015/16 andunverified data for 2016/17 showed a deterioration inperformance when compared to the local and nationalaverages.

For example:

• 73% of patients with asthma, on the register had an asthmareview in the preceding 12 months, which compared to theClinical Commissioning Group (CCG) and England average of75%. However unverified QOF data for this indicator for thepartial year from 1 April 2016 to 22 February 2017 showed thepractice’s achievement to have deteriorated at 34%.

• Childhood immunisation rates for the vaccinations given werealso below the CCG averages. Data available for 2015/16showed deterioration in achievement when compared to datain 2014/15.

• The children’s surveillance team were supporting the practice.They confirmed that data for 2015/16 children’s immunisationsand vaccinations was low. They identified that the lack ofsuitably qualified staff resulted in parents not being able tomake appointments for their children

• Data for 2015/16 showed that the practice performed similarlyto the CCG and England average for the percentage of womenaged 25-64 who had received a cervical screening test in thepreceding five years with 81% compared to 82% for therespective benchmarks.

Working age people (including those recently retired andstudents)The provider was rated as inadequate for two key questionseffective, and well-led and requires improvement for providing safeand responsive services. The concerns identified overall affected allpatients including this population group.

However:

• The needs of the working age population, those recently retiredand students had been identified and the practice had adjustedthe services it offered to ensure these were accessible, flexibleand offered continuity of care.

• The practice was proactive in offering online services as well asa full range of health promotion and screening that reflects theneeds for this age group.

• Extended hours appointments were available for those patientsunable to attend appointments during normal working hours.

People whose circumstances may make them vulnerableThe provider was rated as inadequate for two key questionseffective, and well-led and requires improvement for providing safeand responsive services. The concerns identified overall affected allpatients including this population group.

However:

• The practice held a register of patients living in vulnerablecircumstances including those with a learning disability.

• The practice offered longer appointments for patients whowere vulnerable or with a learning disability.

• The practice worked with other health care professionals in thecase management of vulnerable patients.

• The practice informed vulnerable patients about how to accessvarious support groups and voluntary organisations.

• Staff knew how to recognise signs of abuse in vulnerable adultsand children. Staff were aware of their responsibilities regardinginformation sharing, documentation of safeguarding concernsand how to contact relevant agencies in normal working hoursand out of hours.

People experiencing poor mental health (including peoplewith dementia)The provider was rated as inadequate for two key questionseffective, and well-led and requires improvement for providing safeand responsive services. The concerns identified overall affected allpatients including this population group.

However:

• Data from 2015/16 showed that 100% of patients diagnosedwith dementia had had their care reviewed in a face to facemeeting in the last 12 months, which was better than the CCGaverage of 85% and the England average of 84%.Unverified QOFdata for this indicator for the partial year from 1 April 2016 to 22February 2017 showed the practice’s achievement as 100%.

• 100% of patients with schizophrenia, bipolar affective disorderand other psychoses had a comprehensive, agreed care planrecorded in the preceding 12 months, which was better thanthe local and the England average.

• The practice worked with multi-disciplinary teams in the casemanagement of people experiencing poor mental health,including those with dementia.

• Patients with a diagnosis of dementia had annual reviews andcare plans were recorded.

• The practice had told patients experiencing poor mental healthabout how to access various support groups and voluntaryorganisations.

• The practice had a system in place to follow up patients whohad attended accident and emergency where they may havebeen experiencing poor mental health.

What people who use the service sayThe national GP Patient Survey results were published on7 July 2016. The results showed the practice wasperforming below that of the Clinical CommissioningGroup (CCG) and England averages. A total of 312 surveyforms were distributed, and 96 were returned. This was aresponse rate of 31% and represented approximately 6%of the practice’s patient list.

• 73% of patients found it easy to get through to thispractice by phone compared to the CCG average of79% and national average of 73%.

• 79% of patients were able to get an appointment tosee or speak to someone the last time they triedcompared to the CCG 89% and the national average of85%.

• 81% of patients described the overall experience ofthis GP practice as good compared to the CCG averageof 89% and the national average of 85%.

• 67% of patients said they would recommend this GPpractice to someone who has just moved to the localarea compared to the CCG average of 83% and thenational average of 78%.

As part of our inspection, we also asked for CQC commentcards to be completed by patients prior to our inspection.We received 23 comment cards, all of which were positiveabout the standard of care received. The comment cardsreferred to GPs and other staff by name and gaveexamples of where the practice had supported them withtheir health care needs. Patients said they had enoughtime to discuss their concerns that they felt listened toand involved in decisions about their treatment.However, feedback from patients referred to concernsabout long waits for routine appointments and twopeople commented that they had experienced waits toget an urgent appointment. Similar comments weremade at the previous inspection in August 2016.

We spoke with two patients after the inspection bytelephone they were complimentary about the staff andthey care they received from the GPs.

Background to Heaton NorrisHealth Centre 2This inspection was an announced follow upcomprehensive inspection to check whether the providerhad taken action to improve the areas we identified,including two breaches of regulation at the previouscomprehensive inspection undertaken on 4 August 2016.The practice was rated overall as requires improvement.The full comprehensive report on the 4 August 2016inspection can be found by selecting the ‘all reports’ link forHeaton Norris Health Centre 2 on our website atwww.cqc.org.uk.

Heaton Norris Health Centre 2 is located within a HeatonNorris Health Centre, on Cheviot Close, in Heaton Norris,Stockport. Another GP practice is also located within thehealth centre and other health care services are available inthe building including podiatry, district nurses, healthvisitors and physiotherapy.

Heaton Norris Health Centre 2 is part of the NHS StockportClinical Commissioning Group (CCG). Services are provided

under a general medical service (GMS) contract with NHSEngland. The practice has approximately 1520 patients ontheir register. The practice is a partnership between twoGPs.

The practice provides two GP consultation rooms and apractice nurse treatment room. The health centre buildingis managed by NHS Property Services and provides patientservices on the ground floor with facilities to assist andsupport people with disabilities. There is an independentpharmacy within the health centre. A small car park isavailable to the rear of the building with additionalcommunity parking available nearby.

Information published by Public Health England rates thelevel of deprivation within the practice population group asthree on a scale of one to ten. Level one represents thehighest levels of deprivation and level ten the lowest. Bothmale and female life expectancy at 77 years and 81 yearsrespectively is below both the CCG and England average of79 years (male) and 83 years (female).

The practice patient profile for 2016 indicates that thepercentage of patients under 18 years of age is slightlylower at 16% than the local and national averages of 21%,although the percentage of young patients up to the age offour years is 5%, which reflects the local and nationalaverages of 6%. The practice has a higher percentage (24%)of patients over the age of 65 years when compared to thelocal average of 19% and national average of 17%.

Both GP partners (one male and one female) work parttime to provide the equivalent of one full time GP. Thepractice employs a practice manager and two reception/secretarial staff. The practice had tried to recruit a practicenurse, and had recently employed an agency practicenurse for one full day per week, although this had been

increased the week before our inspection to two full daysper week. The practice also employs a health care assistanton a locum basis on Monday morning for four hours. Thepractice also used regularly (until recently) a locumassistant practitioner (advanced health care assistant) forfour hours on a Friday morning.

The practice reception is open from 8am until 6.30pmMonday to Friday. GP consultation sessions are availableMonday, Tuesday and Friday mornings and afternoons. GPconsultations are also offered on Wednesday and Thursdaymornings, however on the afternoons of both these daysthe practice telephone lines divert patients to the local outof hour’s provider, Mastercall. Evening appointments areprovided once a week until 7pm on alternate Tuesday andFridays. The practice is also open one Saturday morningeach month.

When the practice is closed patients are asked to contactNHS 111 for Out of Hours GP care.

The practice provides online access that allows patients tobook appointments and order prescriptions.

Why we carried out thisinspectionWe carried out an announced comprehensive inspection atat Heaton Norris Health Centre 2 on 4 August 2016. Thepractice was rated as requires improvement for four keyquestions (Safe, Effective, Responsive and Well Led). Thisresulted in an overall rating as requires improvement. We undertook an announced follow up comprehensiveinspection of Heaton Norris Health Centre 2 on 22 February2017. This inspection was carried out to review the actionstaken by the provider and to check whether the provider ismeeting the legal requirements and regulations associatedwith the Health and Social Care Act 2008, to look at theoverall quality of the service, and to provide a rating for theservice under the Care Act 2014.

How we carried out thisinspectionBefore visiting, we reviewed a range of information we holdabout the practice and asked other organisations to sharewhat they knew. We carried out an announced visit on 22February 2017.

During our visit we:

• Spoke with a range of staff including one GP partner, thepractice manager and the agency practice nurse. Wespoke with the health care assistant by telephone afterthe inspection visit to the practice.

• We spoke with two patients and reviewed 23 commentcards

• We observed how reception staff communicated withpatients.

• Reviewed a range of records including staff records andenvironmental records.

• Reviewed an anonymised sample of the personal careor treatment records of patients.

• After the inspection we spoke with associated healthcare professionals from the childhood surveillance teamand the primary care team, (Public Health England).

To get to the heart of patients’ experiences of care andtreatment, we always ask the following five questions:

• Is it safe?• Is it effective?• Is it caring?• Is it responsive to people’s needs?• Is it well-led?

We also looked at how well services were provided forspecific groups of people and what good care looked likefor them. The population groups are:

• Older people• People with long-term conditions• Families, children and young people• Working age people (including those recently retired

and students)• People whose circumstances may make them

vulnerable• People experiencing poor mental health (including

people with dementia).

Our findingsAt our previous inspection on 4 August 2016, we rated thepractice as requires improvement for providing safeservices. We found that the arrangements in respect of staffrecruitment were not sufficient in that Disclosure andBarring Service (DBS) checks and information such as proofof identification, references, qualifications and registrationswith the appropriate professional body were notconsistently obtained. In addition staff who undertook therole of chaperone did not have a DBS check nor was a riskassessment available.

These arrangements had improved when we undertook afollow up comprehensive inspection on 22 February 2017.However, other gaps in service provision were identified.

Safe track record and learning

There was a system in place for reporting and recordingsignificant events.

• Staff told us they would inform the practice manager ofany incidents and there was a recording form available.The agency practice nurse provided two examples thatshe had recently raised and believed these would bereviewed as significant events. Staff confirmed there wasan open, safe environment to raise issues. A policy wasin place to support the recording of notifiable incidentsunder the duty of candour. (The duty of candour is a setof specific legal requirements that providers of servicesmust follow when things go wrong with care andtreatment).

• The practice carried out investigations of the significantevents identified, and staff confirmed they wereinformed of any improvements to practice or systems. Atthe previous inspection, evidence that learning fromsignificant event investigations was shared was notconsistently available. However, at this inspectionrecorded evidence in the form of team meeting minutesdemonstrated that staff were kept informed of theoutcome of significant event investigations.

• We noted that five out of the seven significant eventssupplied at the previous inspection and this inspectionand dated from September 2015 were for medicine orprescription related incidents. The practice managerconfirmed that an analysis of significant events toidentify themes was not undertaken.

Overview of safety systems and processes

The practice had systems, processes and practices in placeto keep patients safe and safeguarded from abuse, whichincluded:

• Arrangements were in place to safeguard children andvulnerable adults from abuse. These arrangementsreflected relevant legislation and local requirements.Policies were accessible to all staff. The policies clearlyoutlined who to contact for further guidance if staff hadconcerns about a patient’s welfare. Both GP partnersshared the lead for safeguarding. The GP confirmed theyattended safeguarding meetings when possible andprovided reports where necessary for other agencies.Staff demonstrated they understood theirresponsibilities and all had received training onsafeguarding children and vulnerable adults relevant totheir role. GPs were trained to child protection or childsafeguarding level 3.

• A notice in the waiting room advised patients thatchaperones were available if required. All staff whoacted as chaperones were trained for the role, howeverthe practice manager confirmed staff were scheduledfor update training this year. Since the last inspectionthe practice had ensured that all staff undertaking thisrole now had a current Disclosure and Barring Service(DBS) check in place. (DBS checks identify whether aperson has a criminal record or is on an official list ofpeople barred from working in roles where they mayhave contact with children or adults who may bevulnerable).

• The practice was maintained and cleaned by the NHSProperty Services. We observed the premises to beclean and tidy. The practice manager was the infectioncontrol lead. However the last recorded infection controlaudit undertaken at the practice was in October 2015.The practice manager confirmed that no further audit orrecorded checks had been undertaken and that theaction to review the practice after six months had notbeen undertaken. An infection control audit would haveidentified that the disposal privacy curtains used at theGP practice had last been changed in July 2016. Bestpractice guidance indicates these should be changedevery six months.

• The arrangements for managing medicines, includingemergency medicines and vaccines, in the practiceminimised risks to patient safety (including obtaining,prescribing, recording, handling, storing, security anddisposal). There were processes for handling repeat

prescriptions which included the review of high riskmedicines. Repeat prescriptions were signed beforebeing dispensed to patients and there was a reliableprocess to ensure this occurred. The practice employeda pharmacist for approximately two hours per week tosupport the GPs with patient medicine reviews anddischarge medicines. The practice carried out regularmedicines audits, with the support of the local clinicalcommissioning group pharmacy teams, to ensureprescribing was in line with best practice guidelines forsafe prescribing. Blank prescription forms and padswere securely stored. However, the practice managerconfirmed the records monitoring the use andtraceability of prescription paper in the practice werenot available as these were ‘missing’. All consultationrooms were locked when not in use and windows werebarred but prescription paper was left in printersovernight. Increased security of these would reduce thepotential risk of theft.

• Patient Group Directions had been adopted by thepractice to allow nurses to administer medicines in linewith legislation. These had been signed by a GP partnerthe day before this inspection to allow the agency nurseto administer medicines. The agency nurse had notsigned these at the time our visit. Health care assistantswere trained to administer vaccines and medicines andpatient specific prescriptions or directions from aprescriber were produced appropriately.

• At the previous inspection, we found that a recruitmentpolicy was not available and there were gaps in the fourstaff recruitment files we reviewed. At this inspection, arecruitment policy was available. We reviewed sixpersonnel files including one for an agency worker. Wenoted improvements including proof of identification,references, qualifications, and registrations with theappropriate professional body. One staff file did nothave a DBS check in place, however, this person did nothave face to face contact with patients and the practicehad a risk assessment in place for staff without a DBScheck. This same employee, did not have professionalor character references available. This was identified atthe previous inspection.

Monitoring risks to patients

Risks to patients were assessed and managed.

• There were procedures in place for monitoring andmanaging risks to patient and staff safety. There was a

health and safety policy available and electrical andclinical equipment had been checked in November 2016to ensure the equipment was safe to use and wasworking properly. At the previous inspection on 4 August2016 and at another visit to the building in November2016 (this was the inspection of the neighbouring GPpractice) we reviewed the building fire risk assessmentand the records of weekly fire alarm checks. Thepractice had copies of other risk assessments in placefor the premises such as Legionella. (Legionella is a termfor a particular bacterium, which can contaminate watersystems in buildings).

• The practice was a small practice and the normalday-to-day staffing including one GP, one practicemanager and a receptionist. Arrangements were inplace for planning and monitoring the number of staffand mix of staff needed which was based on the numberof patients registered at the practice. The practice hadbeen trying to recruit and retain a practice nurse for avery long period of time. At this inspection the practicehad used an agency practice nurse for about six weeksfor one day per week. The practice confirmed that theagency nurse hours had now increased to two days perweek, the week prior to our visit.

Arrangements to deal with emergencies and majorincidents

The practice had arrangements in place to respond toemergencies and major incidents.

• There was an instant messaging system on thecomputers in all the consultation and treatment rooms,which alerted staff to any emergency.

• All staff received basic life support training and clinicalstaff received this annually.

• The practice had a defibrillator available on thepremises and oxygen with adult and children’s masks. Afirst aid kit and accident book were available.

• Emergency medicines were easily accessible to staff insecure areas of the practice and all staff knew of theirlocation. All the medicines we checked were in date andstored securely.

• The practice had a comprehensive business continuityplan in place for major incidents such as power failureor building damage. The plan included emergencycontact numbers for staff.

Our findingsAt our previous inspection on 4 August 2016, we rated thepractice as requires improvement for providing effectiveservices. We found that data from the Quality andOutcomes Framework (QOF) in 2014-2015 showedperformance indicators for some patient outcomes werebelow the national average and the practice’s ownperformance in previous years. Sufficient numbers ofsuitably qualified staff were not deployed to meet patients’needs and formal systems to monitor and support thepractice nurse and health care assistants were notimplemented.

Evidence reviewed at the follow up comprehensiveinspection on 22 February 2017 did not provide us withassurance that the practice had improved patientoutcomes.

Effective needs assessment

The practice assessed needs and delivered care in line withrelevant and current evidence based guidance andstandards, including National Institute for Health and CareExcellence (NICE) best practice guidelines.

• Both GP partners worked opposite days to each otherbut the GP we spoke with confirmed they maintainedregular contact by telephone to discuss practice issues.

• The practice manager confirmed that the practicepharmacist kept the GPs up to date with best practice inrelation to prescribing.

Management, monitoring and improving outcomes forpeople

The practice used the information collected for the Qualityand Outcomes Framework (QOF) and performance againstnational screening programmes to monitor outcomes forpatients. (QOF is a system intended to improve the qualityof general practice and reward good practice). QOFachievement for 2014/15 was 89% compared to theaverage for the Clinical Commissioning Group (CCG) for thesame period which was 96% and for England 95%. Themost recent published results from 2015/16 showed thepractice’s achievement to have deteriorated to 77% of thetotal number of points available. The CCG average for thesame period was 97% and the England average was 95%.

The practice provided unverified QOF data for the partialyear from April 2016 to 22 February 2017. This showed the

practice had achieved approximately 73.5% of the totalpoints available. The GP partner and practice manager saidthey were confident they would improve their QOFachievement further in the remaining five weeks to the endof March and the full year QOF timescale. However, therewas no specific recorded action plan to address andmonitor the practice’s progress, in achieving theperformance indicators for patients with a long termcondition.

The rate of clinical exception reporting for QOF year 2015/16 was 4% for the practice and the CCG. (Exceptionreporting is the removal of patients from QOF calculationswhere, for example, the patients are unable to attend areview meeting or certain medicines cannot be prescribedbecause of side effects).

The practice QOF data for 2015/16 showed the practice wasan outlier for the diabetic indicators and the hypertensionindicators. These indicators were also outliers in 2014/15.The QOF data recorded below is from 2015/16 and forcomparison, data is also included from this current QOFyear, 2016/17.

• The percentage of patients with diabetes on the registerin whom the last blood test (HBbA1c) was 64 mmol/molor less in the preceding 12 months was 63%, comparedto the CCG average of 80% and the England average of78%. Unverified QOF data for this indicator for thepartial year from 1 April 2016 to 22 February 2017showed the practice’s achievement as 66%, which was aslight improvement on last year’s figures.

• The record of diabetic patients with a blood pressurereading 140/80mmHG or less recorded within thepreceding 12 months was 52%, compared to the CCGaverage of 81% and the England average of78%. Unverified QOF data for this indicator for thepartial year from 1 April 2016 to 22 February 2017showed the practice’s achievement as 48%.

• The record of diabetic patients whose last measuredtotal cholesterol was 5mmol/l or less within thepreceding 12 months was 70%, compared to the CCGaverage of 85%, and the England average of80%. Unverified QOF data for this indicator for thepartial year from 1 April 2016 to 22 February 2017showed the practice’s achievement as 58%.

• 28% of patients with diabetes registered at the practicereceived a diabetic foot check compared with the CCG

average and the England average of 88%. UnverifiedQOF data for this indicator for the partial year from 1April 2016 to 22 February 2017 showed the practice’sachievement as 38%.

• 66% of patients with hypertension had their bloodpressure measured and was 150/90 mmHg or less in thepreceding 12 months compared to 84% for the CCG and83% for England. Unverified QOF data for this indicatorfor the partial year from 1 April 2016 to 22 February 2017showed the practice’s achievement as 61%

2015/16 QOF data for other indicators included:

• 96% of patients with COPD (chronic obstructivepulmonary disease) had a review undertaken includingan assessment of breathlessness using the medicalresearch council dyspnoea scale in the preceding 12months, which was better that the CCG average of 91%and the England average of 90%. However unverifiedQOF data for this indicator for the partial year from 1April 2016 to 22 February 2017 showed the practice’sachievement to have deteriorated at 78%.

• 73% of patients with asthma, on the register had anasthma review in the preceding 12 months, whichcompared to the CCG and England average of75%. However unverified QOF data for this indicator forthe partial year from 1 April 2016 to 22 February 2017showed the practice’s achievement to have deterioratedat 34%.

The practice maintained their achievement for patientsdiagnosed with dementia.

• 100% of patients diagnosed with dementia had hadtheir care reviewed in a face to face meeting in the last12 months, which better than the CCG average of 85%and the England average of 84%(2015/16). UnverifiedQOF data for this indicator for the partial year from 1April 2016 to 22 February 2017 showed the practice’sachievement as 100%

There was some evidence of quality improvementincluding clinical audit.

• We reviewed two clinical audits, one for dualantiplatelet therapy whereby the re-audit In January2017 identified the action implemented following theinitial audit in January 2016 had been effective inmonitoring and optimising care for the patientsreceiving this treatment. Dual antiplatelet therapy is atreatment strategy that combines aspirin and another

medicine such as clopidogrel to reduce the risk of aheart attack (myocardial infarction). The second clinicalaudit was a review of the practice’s use of the medicinemethotrexate used for rheumatoid arthritis and otherauto immune conditions. The re-audit identified anadditional two patients on this treatment than wereidentified in the original audit. The explanation for thiswas attributed to a change in the patient electronicrecording system and an improved search facility. There-audit identified some improvement in the frequencyof blood test monitoring for patients and that furthermonitoring and improvement was required.

Effective staffing

Staff had the skills, knowledge and experience to delivereffective care and treatment.

• Staff spoken with said they received an induction whenthey commenced employment. The practice managerconfirmed that staff undertook a range of on line e-learning provided by Stockport CCG and since the lastinspection a training matrix had been completed toshow staff progress.

• The practice was using a nurse from an agency toensure they had suitably qualified staff working at thepractice. The agency practice nurse and one health careassistant we spoke with confirmed that they wereundertaking health reviews for patients with long termhealth conditions. Staff confirmed they were up to datewith the training relevant to their role and responsibility.Certificates of training were also available. Since the lastinspection, the practice manager had introduced aplanned schedule of one to one meetings with thepractice nurse and health care assistant and brief noteswere available of these meetings.

• Clinical team meetings were not held, however thepractice nurse confirmed the GPs were accessible todiscuss any issues or concerns or to seek advice andsupport.

• Staff confirmed and records supported that staffemployed over a year received an annual appraisal.However, the practice manager had not had anappraisal for over two years. Staff confirmed that thesmall staff team enabled easy on the job access tosupport and discussion about different issues andconcerns.

• Staff files for those who had been employed for a longtime included training certificates for safeguarding,basic life support and information governance.

Coordinating patient care and information sharing

The information needed to plan and deliver care andtreatment was available to relevant staff in a timely andaccessible way through the practice’s patient recordsystem.

• This included care and risk assessments, care plans,medical records and investigation and test results.

• The practice shared relevant information with otherservices in a timely way, for example when referringpatients to other services.

• Systems to monitor and track the status of patient careplans, referrals and hospital discharges weremaintained and responded to when issues wereidentified.

Meetings took place with other health care professionals ona regular basis. Care plans were reviewed for patients whorequired palliative care and those who had complex healthcare needs.

Consent to care and treatment

GPs sought patients’ consent to care and treatment in linewith legislation and guidance.

• The GP and the practice nurse understood the relevantconsent and decision-making requirements oflegislation and guidance, including the Mental CapacityAct 2005.

• When providing care and treatment for children andyoung people, staff carried out assessments of capacityto consent in line with relevant guidance.

• Where a patient’s mental capacity to consent to care ortreatment was unclear the GP assessed the patient’scapacity and recorded the outcome of the assessment.

Supporting patients to live healthier lives

The practice identified patients who may be in need ofextra support. For example:

• QOF data for 2015/16 showed the practice achievementin recording the smoking status of patients in thepreceding 12 months was 83% which was below both

the CCG and national average of 95%. The practicemanager said patients attending for appointment wereasked to complete a smoking status slip to try andincrease the data the practice held.

• The practice’s uptake for the cervical screeningprogramme was 81%, which was comparable to the CCGand England average of 82%. However, the practiceexception reporting was higher at 11% compared withthe CCG exception reporting of 4% and the Englandaverage of 7%. The practice manager confirmed thepractice implemented a policy to send reminder lettersto patients who did not attend for their cervicalscreening test. There were systems in place to ensureresults were received for all samples sent for the cervicalscreening and the practice followed up women whowere referred as a result of abnormal results.

• The practice said they encouraged its patients to attendnational screening programmes for bowel and breastcancer screening. Data supplied from the NationalCancer Intelligence Network (NCIN) for 2015/16indicated that the practice’s screening rates for breastand bowel cancer were below both CCG and Englandaverages. For example, females aged between 50 and 70screened for breast cancer in the last 36 months was57% compared to the CCG average of 70% and Englandaverage of 73%. Data for bowel screening showed thatpatients between 60 and 69 years of age screened forbowel cancer in the last 30 months was 39% comparedto the CCG and England average of 57%. Public HealthEngland had been attending the health centre to raiseawareness of cancer screening to encourage patients atboth GP practices located in the building, to makeappointments for screening or undertake the nationallyavailable screening tests.

• Childhood immunisation rates for the vaccinationsgiven were also below the CCG averages. Data availablefor 2015/16 showed deterioration in achievement whencompared to data in 2014/15.

• For example, childhood immunisation rates for themeasles, mumps and rubella (MMR) vaccine for fiveyears old were 66.7% for MMR1 and MMR2 vaccinationscompared to the CCG average of 92% for MMR1 and 89%for MMR2 in 2014/15.

• Data for 2015/16 showed the practice achieved 54.5%for MMR1 and MMR2 for five year old children comparedto the CCG achievement of 90% for MMR1 and 86% forMMR2.

• 2015/16 immunisation data for children aged up to twoyears of age showed the practice achieved significantlybelow the national average target score of 90% for allfour sub-indicators. For example, 61% of children aged 1received the full course of recommended vaccines and71% of children aged 2 received MMR, pneumococcalbooster and meningitis C booster vaccine.

We spoke with a member of the children’s surveillanceteam to discuss the practice’s achievements for childhoodimmunisation. They confirmed that previously the practicehad not communicated with them to advise that there wasno one available to undertake childhood immunisations.They said they therefore continued to send out letters toparents asking them to to attend their GP surgery for theirchild’s immunisations. However, parents were unable toget their children vaccinated because there was no practicenurse available.

The children immunisation team had recently visited thepractice before the inspection because they wereconcerned about the low numbers of children recorded ashaving received their immunisations. However a samplecheck on the patient electronic records identified that thiswas was due to a recording issue in the patient electronicrecord.

Members of the primary care engagement team alsoattended the practice to support them with their flucampaign in the autumn of 2016. The primary care teaminitiated this support because the practice had had a lowuptake of the flu vaccine and Public Health England wereconcerned for patients’ health. The primary care teamcontacted patients to advise them of the flu clinics andprovided trained staff to give flu vaccinations to thepractice patients. The primary care team confirmed thatthey contacted patients to advise them of the fluvaccination availability but many patients refused it. Thepractice advised us after the inspection that they have ahealth care assistant employed who was trained toadminister flu vaccinations.

Patients had access to appropriate health assessments andchecks. These included health checks for new patients andNHS health checks for patients aged 35–74. Appropriatefollow-ups for the outcomes of health assessments andchecks were made, where abnormalities or risk factorswere identified.

Our findingsAt our previous inspection on 4 August 2016, we rated thepractice as good for providing caring services as datashowed patients were generally satisfied with the servicethey received.

Evidence reviewed at the follow up comprehensiveinspection on 22 February 2017 confirmed no change inrating for this key questions.

Kindness, dignity, respect and compassion

We observed members of staff were courteous and veryhelpful to patients and treated them with dignity andrespect.

• Curtains were provided in consulting rooms to maintainpatients’ privacy and dignity during examinations,investigations and treatments.

• We noted that consultation and treatment room doorswere closed during consultations; conversations takingplace in these rooms could not be overheard.

• Reception staff knew when patients wanted to discusssensitive issues or appeared distressed they could offerthem some privacy to discuss their needs.

As part of our inspection, we also asked for CQC commentcards to be completed by patients prior to our inspection.We received 23 comment cards, all of which were positiveabout the standard of care received. The comment cardsreferred to GPs and other staff by name and gave examplesof where the practice had supported them with their healthcare needs. GPs were identified by name and weredescribed as being responsive to individual circumstances.Comment cards highlighted that staff respondedcompassionately when they needed help and providedsupport when required. Patients said they had enough timeto discuss their concerns that they felt listened to andinvolved in decisions about their treatment. However somepatients also referred to concerns about long waits forroutine appointments and two people commented thatthey had experienced waits to get an urgent appointment.Similar comments were made at the previous inspection inAugust 2016.

We spoke with two patients after the inspection bytelephone they were complimentary about the staff andthey care they received from the GPs.

Results from the national GP patient survey (July 2016)showed patients felt they were treated with compassion,dignity and respect. The practice’s satisfaction scores onconsultations with GPs and nurses were comparable to theclinical commissioning group (CCG) and England averages.For example:

• 92% of patients said the GP was good at listening tothem compared to the CCG average of 92% and theEngland average of 89%.

• 91% of patients said the GP gave them enough timecompared to the CCG average of 91% and the Englandaverage of 87%.

• 100% of patients said they had confidence and trust inthe last GP they saw compared to the CCG average of97% and the England average of 95%.

• 93% of patients said the last GP they spoke to was goodat treating them with care and concern compared to theCCG average of 89% and the England average of 85%.

• 94% of patients said the last nurse they spoke to wasgood at treating them with care and concern comparedto the CCG average of 93% and the England average of91%.

• 81% of patients said they found the receptionists at thepractice helpful compared to the CCG average of 89%and the England average of 87%.

Care planning and involvement in decisions aboutcare and treatment

Patients told us they felt involved in decision making aboutthe care and treatment they received. They also told usthey felt listened to and supported by staff and hadsufficient time during consultations to make an informeddecision about the choice of treatment available to them.Patient feedback from the comment cards we received wasalso positive and aligned with these views. We also sawthat care plans were available for patients with dementiaand palliative care needs.

Results from the national GP patient survey showedpatients responded positively to questions about theirinvolvement in planning and making decisions about theircare and treatment. Results were similar to local andEngland averages. For example:

• 93% of patients said the last GP they saw was good atexplaining tests and treatments compared to the CCGaverage of 88% and the England average of 86%.

• 86% of patients said the last GP they saw was good atinvolving them in decisions about their care comparedto the CCG average of 85% and England average of 82%.

• 92% of patients said the last nurse they saw was good atinvolving them in decisions about their care comparedto the CCG average 88% and the England average of85%.

The practice provided facilities to help patients be involvedin decisions about their care:

• Staff told us that translation services were available forpatients who did not have English as a first language.

• A hearing loop system was available for those peoplewith hearing impairment

Patient and carer support to cope emotionally withcare and treatment

Patient information leaflets and notices were available inthe patient waiting area. These advised patients how toaccess a number of support groups and organisations.Information about support groups was also available onthe practice website.

Since the last inspection, the practice had compiled acarers list. This had 13 patients on it, which equated to lessthan one percent of the practice patient population. Thepractice manager confirmed she was still building this list.Patients were given questionnaires to complete and askedto return to the practice so appropriate support could beprovided. Staff confirmed they supported patients directlyas required, and this included offering patients a cup of tea.

GPs told us that if families had suffered bereavement, theyoffered support to the families and sent condolence lettersas required.

Our findingsAt our previous inspection on 4 August 2016, we rated thepractice as requires improvement for providing responsiveservices. We found that the healthcare needs of somepatients were not always met in a timely manner becausethere was insufficient staff employed with the appropriateclinical skills to meet these needs. This included patientswith a chronic health care needs and children’simmunisations and vaccinations. The practice loggedpatients’ verbal complaints but did not record thepractice’s response to these.

These arrangements had not improved when weundertook a follow up comprehensive inspection on 22February 2017.

Responding to and meeting people’s needs

The practice was aware of their local patient demographic.Information received from external health careprofessionals indicated that communication from thepractice about issues at the practice were not alwayscommunicated effectively. For example, children’s publichealth services were not notified of the lack of suitablytrained staff to undertake immunisations, thereforechildren missed their vaccinations.

• The practice offered evening appointments with a GP,once a week on alternate Tuesdays and Fridays until7pm.

• There were longer appointments available for patientswith a learning disability or special health care needs.

• Home visits were available for older patients andpatients who had clinical needs that resulted indifficulty attending the practice.

• The practice offered reviews of patients diagnosed withdementia and care plans were recorded for thesepatients.

• Same day appointments were available for children.• The practice supported the needs of homeless and

disadvantaged people and had working relationshipwith ‘The Wellspring’ in Stockport. The Wellspring was acharity that provided a resource centre for homelesspeople.

• The practice was also responsive to patients who wereasylum seekers.

Access to the service

The previous inspection identified that patients did notalways received timely access to health care services. Thisinspection did not identify significant improvements. Forexample:

• Patient feedback indicated that some felt the wait oftwo weeks for a routine appointment was too long andwe heard of one example where a patient with a longterm condition had to wait two weeks to be seen.Patients also told us they sometimes had to wait to getan urgent appointment.

• Published data from 2015/16 also indicated patients’needs, especially for those with long term conditionswere not being met. Performance was below that of thelocal Clinical Commission Group (CCG) and the Englandaverage.

• Feedback from external health care professionalsindicated that they had provided additional support tothe practice because the practice was not sufficientlystaffed to deliver preventative services such as fluvaccinations.

At the previous inspection we identified that the healthcareneeds of some patients were not always met in a timelymanner because there were insufficient staff employedwith the appropriate clinical skills to meet these needs. Thepractice had employed an agency practice nurse about sixweeks prior to our visit and the week before our visit, thepractice nurse’s hours had increased from one day perweek to two days.

The practice reception was open from 8am until 6.30pmMonday to Friday. GP consultation sessions were availableMonday, Tuesday and Friday mornings and afternoons. GPconsultations were also offered on Wednesday andThursday mornings, however on the afternoons of boththese days the practice telephone lines diverted patients tothe local out of hour’s provider (OOH) Mastercall. Eveningappointments were provided once a week until 7pm onalternate Tuesdays and Fridays. The practice was open oneSaturday morning each month.

At the time of our visit, the agency practice nurse providedappointments all day Wednesday and all day Thursday.

At the previous inspection in August 2016 the practice toldus they had reviewed patient demand for appointments in2015 and as a consequence they had increased patientappointments to 15 minutes so that those patients withseveral health issues and those who required their long

term health condition monitoring could be reviewed withinthis appointment time. Both GPs felt this had increasedpatient satisfaction with the quality of the GP appointmentand it had enabled the GPs to carry out reviews whenrequired. However data from 2015/16 and the unverifieddata supplied by the practice at this inspection for the yearApril 2016 to February 2017 did not provide evidence of anysignificant impact the longer appointments had made inimproving the number of patients benefiting from a reviewof their long term condition.

We reviewed the appointments for the day before theinspection and this showed that the practice offered 13appointments in the morning. Five of these appointmentswere 15 minutes long, there were three book on the dayurgent appointment slots, two urgent appointments slotsfor children and three slots allocated for telephoneconsultations. Appointments for the afternoon includedfour 15 minute appointments, three ten minuteappointments, one appointment for children, two urgentappointments and two telephone consultations. Theelectronic appointments schedule showed there was oneroutine appointment available on 3 March 2016 and twoappointments on 6 March 2016.

The practice had carried out a patient satisfaction survey in2015. The analysis of the 52 responses received showedthat overall patients were satisfied with the service theyreceived. Areas identified for improvement includedreviewing the patient appointment system and on the dayappointment availability and increasing the length of timefor routine patient consultations. However no additionalsurveys had been undertaken to obtain the views ofpatients regarding the effectiveness of these actions.

Feedback we received from patients at this inspection wassimilar to the previous inspection in that patients weredissatisfied with the waiting time of up to two weeks to geta routine appointment. Complaints made by patients alsoreflected patient dissatisfaction regarding appointmentsfor example the log or ‘complaints ledger’ for 2016/17detailed 13 verbal complaints raised by patients. Theanalysis of these by the practice manager identified thatnine of these complaints were about appointments. Theanalysis recorded, “most patients complain because theycannot get in on the same day of phoning” and “they arealways asked to ring back to see if there are any

cancellations or if they would prefer a telephoneconsultation…”. Analysis of verbal complaints undertakenby the practice manager in 2014/15 and 2015/16 identifiedsimilar patient concerns.

The week before our visit the practice had commencedanother patients’ survey. The practice had received 11responses to this so far. We saw one of the responses andthis reflected similar feedback we had received regardingaccess to routine appointments.

Results from the national GP patient survey showed thatpatient’s satisfaction with how they could access care andtreatment was below that of local and England averages.

• 68% of patients were satisfied with the practice’sopening hours compared to the CCG average of 77%and the England average of 76%.

• 73% of patients said they could get through easily to thepractice by phone compared to the CCG average of 79%and the England average of 73%.

• 79% of patients were able to get an appointment to seeor speak to someone the last time they tried comparedto the CCG 89% and the national average of 85%.

Listening and learning from concerns and complaints

• The practice’s complaints policy and procedures were inline with recognised guidance and contractualobligations for GPs in England.

• The practice manager was the designated responsibleperson who handled all complaints in the practice.

• The practice had received one written complaint sincethe last inspection. This had been investigated andresponded to with openness and transparency.

The practice manager maintained a log of all verbal issuesand concerns raised by patients. The log or ‘complaintsledger’ for 2016/17 detailed 13 verbal complaints raised bypatients. The complaints log did not record the patients’details or when the complaint was made. We noted thatcomplaint logs from previous years identified similar issuesand concerns. Evidence that actions to respond orminimise reoccurrence was not available. For example, theoutcome recorded by the practice manager in response topatient concerns regarding waiting time for appointmentwas “Nothing at the present”.

Our findingsAt our previous inspection on 4 August 2016, we rated thepractice as requires improvement for providing well ledservices. We found the absence of a practice nurse hadincreased the demands on the practice manager byincreasing the number of areas of direct responsibility theyhad. This potentially had resulted in gaps in theeffectiveness of the governance framework including themanagement of effective recruitment and maintainingwritten records consistently.

These arrangements had not improved when weundertook a follow up comprehensive inspection on 22February 2017.

Vision and strategy

The practice had a statement of purpose which detailedthe practice’s aims and objectives and these included “Toensure that patients are seen by the most appropriatehealthcare professional as quickly as possible” and “Tofocus on prevention of disease by promoting good healthand prophylactic medicine.” Evidence that the practice hadimplemented a planned programme of improvement toachieve these objectives was not available.

At the previous inspection the practice manager wasunable to locate the practice business plan. At thisinspection a copy of the five year business plan andplanning strategy was available. The practice managerconfirmed that this was first recorded about seven yearspreviously and was not regularly reviewed or used as a toolto monitor and evaluate the practice’s progress in achievingits objectives.

The practice manager confirmed that recorded business orpartner meetings to discuss the practice’s progress werenot undertaken.

Governance arrangements

The practice had not improved its governance frameworkto support the delivery of the strategy and good qualitycare. Some actions had been undertaken since our lastinspection in August 2016. However, this inspectionidentified other gaps in monitoring the service whichcollectively indicated an inadequate monitoringframework.

For example:

• Systems to monitor and review the practiceperformance and implement strategies to improve,including analysing significant events to identify themesand responding to patients verbal complaints and totake action to mitigate risk of reoccurrence were eithernot established or ineffective.

• The practice told us about how they were addressingthe shortfalls in the QOF data but no specific recordedaction plan to address and monitor progress for eachidentified area had been developed. The practice hadincreased the number of practice nursing hours theweek before our inspection suggesting this increase wasin response to our follow up inspection visit rather thanto the significant shortfalls in QOF achievement.

• The practice had been monitored and supported byassociated health care professionals such as childsurveillance and the primary care team to ensurepatients’ outcomes were achieved.

• Patient Group Directives had been signed by a GPpartner the day before the inspection visit, even thoughthe agency nurse had been working at the practice forsix weeks.

• Staff recruitment records had improved and all but onestaff member did not have a record of interview or aprofessional or character reference available.

• The practice was not proactive in responding to patientsconcerns regarding access to appointments.

• An infection control audit had not been undertakensince October 2015.

• The record of prescription paper entering the buildingand log of prescriptions provided to each printer andclinician was reported to be missing.

• The practice manager had not had an appraisal for twoyears.

However there was

• A small staff team with a clear staffing structure and staffwere aware of their own roles and responsibilities.

• Practice specific policies were available to all staff.

Leadership and culture

The GP partners were the practice owners and clinical careproviders but evidence that the partners were proactive indeveloping and improving the service and leading thesmall staff team was not evident.

• The partners did not have recorded partner meetings orrecorded business meeting with their practice manager.Two staff meetings held in January 2017 indicated a GPpartner attended these; however evidence of clinicalmeetings was not available.

• Although there was a business strategy in place this hadnot been reviewed and there was no specific goals oractions identified to improve service delivery andpatient outcomes. It was confirmed by the practicemanager that services were provided on a day to daybasis.

• Actions taken by the practice were reactive and notproactive. For example the increase in agency practicenursing hours the week before our inspection visit andthe signing of Patient Group Directions (PGDs) to allownurses to administer medicines in line with legislation,the day before our visit.

• The practice used ‘locum’ or non permanentarrangements for health care assistants and practicenurses potentially leading to a lack of overall lack ofcommitment to the practice. One staff memberconfirmed this.

However since the last inspection:

• The practice manager had undertaken a range of one toone meetings with the practice nurse and health careassistant.

• Staff told us there was an open culture within thepractice and they had the opportunity to raise anyissues on a day to day basis and felt confident andsupported in doing so.

• Staff said they felt respected, valued and supported atthe practice.

Seeking and acting on feedback from patients, thepublic and staff

The week before our visit the practice had commencedhanding out questionnaires to patients requestingfeedback on how to improve the practice. The practice hadreceived 11 responses to this so far.

The practice manager confirmed that a specific patientparticipation group (PPG) was not yet established but saidthat approximately seven patients had confirmed theywould consider joining the group. Two patients we spokewith confirmed that they had been approached to join thePPG. One was enthusiastic about this.

Continuous improvement

Clear evidence that the practice was focused proactively onimproving and developing the service was not available.However, feedback from associated health careprofessionals indicated that the practice manager was veryreceptive to offers of support and assistance.
