This year, AADOM and the Henry Schein Cares Foundation presented the 2016 Green Leader Award to Claudia Paye, FAADOM from Woodpoint Dental, Brooklyn, NY. Mrs. Paye was recognized for her outstanding leadership in inspiring her team to reduce waste, pollution; conserve energy, water, and money; and incorporate a sensible road for fellow teammates to do the same.

Dental practices, like all businesses, produce waste. According to the Eco-Dentistry Association, every year dental practices dispose of:

• 4.8 million lead foils• 28 million liters of toxic X-ray fixers

1. Go digital – Going paperless can help you green up your office, as well as help save money and time. Using office management software such as Dentrix and inventory control programs like Aruba can help you cut paper use and stock room problems while also making your practice more efficient.

2. Cut down on chemicals – Switching over to digital X-rays eliminates the need for development chemicals and lead foils, and they reduce radiation exposure for technicians between 75% to 90%. Another way to cut down on chemicals is to use steam sterilization for instruments.

Capturing a final impression is an indispensable step in the fabrication of indirect restorations. However, recent trends towards the use of retraction pastes which may not result in adequate sulcus width, faster setting materials to cut down on setting time, and lower cost materials with potentially inferior physical properties have created a confusing milieu for the clinician. Recently introduced Aquasil Ultra+ Smart Wetting Impression Material is an upgrade in the well-established Aquasil line of polyvinyl siloxane impression materials and has been specially designed to help clinicians in three key areas.

First, impression materials used intraorally need to work in the presence of moisture(i.e., hydrophilic) not just when cured but more importantly when they are uncured. To provide hydrophilicity where it’s needed most - in the mouth - Aquasil Ultra+ material was designed with a market leading low contact angle as tested on uncured samples at intraoral relative humidity. For the clinician, this translates to improved accuracy of impressions, potentially leading to better fitting restorations that require less chairside adjustments.

Second, impression materials must be strong at the time of mouth removal to prevent the tearing of thin sections in the impression. Impression material syringed into a sulcus is already in thin cross sections. When adequate gingival retraction is not created around equi- and sub-gingival preparations (in some cases where retraction paste is used instead of retraction cord), impression material that is expressed into thin sulci is even more vulnerable. To prevent tearing of thin extensions of material and the loss of precious detail, Aquasil Ultra+ material was designed to feature market leading tear strength at both mouth removal and after 24 hours of setting. This benefit should provide confidence to the clinician that the material placed into the mouth will be completely retrieved, and also allow laboratory techniciansthe opportunity to create extremely accurate working models.

Third, no single impression material will satisfy the infinite number of clinical situations. For single-unit cases a fast set material may be ideal, but for larger prosthetic cases or removable denture impressions options for setting times and viscosities may be needed. Aquasil Ultra+ impression material offers five different viscosities (xtra-light, light, medium, heavy, and rigid), two setting times (fast and regular), and several delivery options (digit® Targeted Delivery System, 50mL cartridges, and DECA™ 380mL cartridges). With all the variations of Aquasil Ultra+, clinicians can match the most appropriate viscosity, timing, and delivery for each specific clinical situation.

Best practices of infection prevention and control provide safety in the clinicalenvironment and treatment protocols for patients and dental professionals.Exposure to poor water quality can pose a health risk for people and conflictswith universally accepted infection prevention protocols.

Recently, news broke reporting at least 50 children being hospitalized due to chronic infections after receiving treatment at an Anaheim, CA pediatric dental clinic. Following an investigation by the California state department of health, confirmation of the cause has been related to contaminated dental unit water with Mycobacterium. Dr. Eric Handler, the CA county’s health officer, said in a statement “I’m requiring a complete replacement of the on-site water system so that we will remove the potential threat of infection.” These outbreaks were caused by contaminated water used during pulpotomies, which introduced Mycobacterium into the chamber of the tooth during irrigation and drilling. Mycobacterium can cause severe infection among immunocompetent children, and because Mycobacterium is ubiquitous in the environment, it poses a contamination risk.

Similar to the issues identified in Southern California, 20 children were hospitalized after complications from receiving pulpotomies at a GA pediatric dental clinic earlier this year. The Georgia Department of Public Health stated “the source of infection has been determined to be contaminated water in dental units.” According to the CDC MMWR, all water samples from the seven dental stations had bacterial counts above the

American Dental Association recommended ≤500 colony-forming units (CFU)/mL (average = 91,333 CFU/mL); M. abscessus was isolated from all water samples. Dental unit waterline contamination was first reported in 1963.¹ Research has shown microbial counts can reach >200,000 CFU/mL within 5 days after installation of new dental unit water-lines² and contamination levels of up to 1 million CFU/mLof dental unit water have been documented.3, 4 In 2011, the fatal case of an 82-year-old otherwise healthy woman who developed Legionnaire’s disease after a dental visit⁶ was reported.

The goal of effective dental waterline treatment is to reduce the number of opportunistic microorganisms present in the water, thereby helping to break the chain of infection. Based on standards for potable drinking water by the Environmental Protection Agency, the American Public Health Association, the American Water Works Association, and the Centers for Disease Control and Prevention guide-lines (2003) state the number of bacteria in water used as a coolant/irrigant for nonsurgical dental procedures should be less than 500 CFU/mL.5

Water flowing through long and narrow dental unit waterlines (DUWL’s) can harbor dangerous bacteria. When untreated, or improperly maintained, these contaminated DUWL’s can potentially harm your patients, your staff and your practice’s reputation.

3. HEATING AND AIR-CONDITIONING IMPROVEMENTS

*In an enamel fluoride uptake study, varnish was applied to sound, polished enamel specimens. These specimens were each paired with a specimen that had an artificial incipient lesion formed, and the lesioned enamel was not treated with varnish. The paired specimens were immersed into artificial saliva for 5 minutes, after which they were placed in fresh artificial saliva; this procedure was repeated at 15 minutes, 30 minutes, 1 hour, and 4 hours, and the specimens remained in artificial saliva for a total of 24 hours. After 24 hours, remaining surface varnish was removed. One layer of enamel was removed from each specimen and fluoride content was measured. Nupro® White Varnish had significantly more fluoride uptake into both sound and lesioned enamel than VOCO Profluorid® Varnish and 3M Vanish™ Varnish.

21st Century Local Anesthesia – The Wand STA, A Computer-Controlled Local Anesthesia System

Milestone Scientific Inc., Wand Dental, Inc. manufacturers of the original Wand®, have introduced the Wand STA™ System as an alternative to the 165 year-old technology of the traditional dental syringe . The Wand STA is a computer-controlled local anesthetic delivery instrument designed to improve the injection process for both the dentist and patient. The Wand STA produces a precise, consistent flow of anesthetic below the patient’s pain threshold.

The Wand STA incorporates a plastic, light-weight, ergonomically-designed handpiece that offers the dentist unprecedented control of a bonded needle attached to the handpiece. The handpieces are available in three sizes of needles, which will allow all traditional and newly discovered innovative injections. A dentist can even shorten the plastic handpiece, making it invisible to patients who are fearful regarding the injection process.

The Wand STA allows dentists to administer Intraligamentary Injections with confidence and comfort as the it’s unique feature, Dynamic Pressure Sensing (DPS), delivers audio and visual feedback during the injection – enabling dentists to administer a more comfortable, more precise injection for their patient. DPS confirms proper needle placement and monitors the delivery of anesthesia below the patient’s pain threshold. Dentists and their patients love the Wand STA because there is no collateral numbness of the lip, face, or tongue after the procedure. The ability to anesthetize a single tooth is a tremendous enhancement to any dental practice. For example, patients who have to return to work can do so without having their lip and tongue numb. Some dentists and patients get anxious during palatal injections, which can be difficult to administer and can be painful to the patient. With the Wand STA, you can offer innovative, comfortable palatal injections, such as the AMSA and P-ASA, using a pre-puncture technique – followed by delivery of a controlled flow of anesthetic below the patient’s pain threshold. These successful palatal injections anesthetize multiple teeth and related gingival tissues without producing collateral numbness to the patient’s lip or face. These are great injections for Cosmetic Dentistry allowing immediate assessment of the patient’s smile line.

STA stands for single tooth anesthesia, but dentists shouldn’t limit their use of the Wand STA. You can use it to deliver every type of injection. If you’ve been missing your mandibular block injections, the Wand STA can be the solution to achieving more success. Since a bi-rotational insertion technique can be utilized with the Wand Handpiece, the Wand STA needle doesn’t deflect the way a traditional dental syringe needle does. Without needle deflection, the needle will arrive closer to the injection site, which produces a faster onset and a more accurate and successful block injection.
