Mercy Hospital Mount Lawley, Perth is utilising the latest innovation in infection prevention to dramatically reduce the risk of cross infection.

The 244 bed hospital, a division of Mercy Care, has adopted the Vernacare Tuffie 5 cleaning and disinfecting wipes to control a recent outbreak of Norovirus on a rehabilitation ward.

Tuffie 5 is an all-in-one cleaning and disinfecting wet wipe that eradicates the five major groups of pathogens, including Clostridium Difficile, Norovirus and MRSA amongst many others. It is also certified to be effective against the H1N1 (swine flu) virus and the newly identified NDM-1 superbug.

In September 2010, prior to introducing Tuffie 5 at Mercy Hospital, an outbreak of Norovirus affected 19 of 24 patients (70%) and 30% of staff on the ward. When a second outbreak occurred during October the traditional chlorine solution based cleaning regime was replaced with Tuffie 5 wipes, which were used to clean all hard surfaces and equipment. In comparison to the previous outbreak the impact on the spread of infection was dramatic, achieving a 65% reduction in transmission rates among patients and an 84% reduction in the spread of infection among staff.

In addition, a subsequent single case of C. Diff was swiftly contained by adopting a Tuffie 5 cleaning protocol, and there was no transmission of infection beyond the rehabilitation ward.

“ We were very impressed by the performance of the Tuffie 5 wipes in controlling and reducing the transmission of infection”,

said Lucille Ridley, Manager Infection Control for Mercy Hospital.

“ This is helping us to achieve the best possible standards of hygiene to promote patient and staff safety. In addition, the wipes are proving popular as they save time spent by the cleaning staff in preparing chlorine disinfecting solutions.”

Tuffie 5 begins to work instantly on contact, with industry tests showing that its disinfecting properties are long-lasting - continuing to work for 12 hours after the initial cleaning has taken place.

“Tuffie 5 has been heralded as a major step forward in infection control”,

Vernacare has recently reduced the cost of its Tuffie 5 wipes by introducing new ‘greener’ space-saving flexible canisters and refill packs for wall mount dispensers – reducing packaging waste by 70% versus traditional tubs.

A macerator human waste disposal system offers many advantages over the thermal pan flusher.

The macerator system can eliminate and / or reduce many of the risks associated with the thermal method of waste disposal and sanitisation of equipment while providing whole of organisation financial performance improvement.

Macerators were installed in 6 dirty utility rooms and 5 isolation rooms in the Royal Melbourne Hospital ED redevelopment (~100 patient treatment paces) eliminationg the traditional thermal pan flusher. The opportunity to provide human waste disposal at point of care for infectious waste was particularly appealing.

The main issues in implementation were:

• Staff education relating to what is not appropriate to be put into the macerator!

• Change management related to correct use, and troubleshooting procedure

• Sourcing a disposable patient wipe that was cost neutral and compatible with disposal in the macerator

• Storage of consumables, particularly the urinal cartons as they are particularly bulky

The benefits were:

• Risk management and harm minimisation relating to the handling of human waste

• No transport of infectious waste from patient area through corridors to dirty utility rooms

• Elimination of soiled pans and bottles queued on floor of dirty utility rooms

• Elimination of unpleasant smells coming from dirty utility room

• Elimination of need to hand scrub vessels when waste not removed by flusher

• Elimination of burn risk to staff• Reduction in risk of cross infection• Reduced risk of potential for splash injury to staff• Reduced noise and smell polution for staff and

patients in close proximity to dirty utility room• Significant reduction in power and water

costs (costed as part of business case)• Positive labour saving impact for clinical

staff in that the macerator does not have to be emptied of vessels

During February 2011, 39 Vernacare Vortex Macerators were installed at the New Royal Childrens Hospital (RCH) in Parkville Melbourne. The state of the art facility, due for completion in November 2011, already has the units in readiness for patients when they start arriving at the end of the year.

Max Currie, Service Manager for EBOS Healthcare, stated “The hardest part of the installation was locating all the rooms where the Vortex Macerators were to be installed. This took the installation team a day and a half just to place the units in each pan room.”

The size of the New RCH is enormous. The Lower Ground level occupies the same surface area as the MCG!

The old RCH will be demolished and regenerated into parkland in a process taking 1 year following the completion of the new hospital.

The New RCH is the third major Hospital in the Parkville health precinct to adopt macerators as the preferred method of human waste disposal. Both the Royal Womens and Royal Melbourne Hospitals have also taken advantage of the Infection Prevention benefits along with the Power and Water savings to be made for years to come from the Vernacare Vortex Macerator system.

Fractional Exhaled Nitric Oxide (FeNO): Airway inflammation is a central process in asthma and other lung diseases4.

Being able to measure this inflammation and monitor the patients reaction to medication is regarded as a gold standard in the management of respiratory diseases. It is increasingly recognised that the measurement of Feno in particular constitutes a novel way to monitor separate aspects of diseases. These include asthma, COPD and interstitial lung diseases, that are not assessed by other means, such as lung function2.

To summarize the feedback from our trials, these are the main drivers of hospital’s positive reception of the Vernacare pulp kidney dish:

1. Single use over reusable. Most hospitals use both the disposable and reusable kidney dish, but they see the benefit of replacing them with the disposable kidney dish only (price of pulp kidney dish is comparable with the

price of the single use plastic one)

2. Biodegradable. Another example of hospitals going green. The Vernacare pulp kidney dish is biodegradable so it fits in the future ‘green’ plans of most hospitals

3. Reducing the risk of cross infections. No surprise here, as preventing infections is top of mind for most flagship hospitals.

Infection prevention products business Vernacare has won the prestigious Queen’s Award for Enterprise (Innovation Category) in the UK– for its unique detergent proof disposable wash bowl .

The washbowl was developed to offer an alternative to the common practice of re-using traditional plastic bowls there by reducing the risk of cross infections.

This is the world’s first single-use pulp wash bowl, capable of holding both warm water and detergent, and is used across hospitals globally. The four litre bowl is used for hand washing, general surface cleaning, wound care, shaving, continence and general patient care

The Queen’s Awards for Enterprise are made annually in the UK by Her Majesty the Queen on the advice of the Prime Minister, and are granted for five years.

Dean Harding from EBOS Healthcare, Vernacare’s distribution partner in Australia said: “This is a honour for Vernacare, and for the hospital clinicians who helped develop the product. The wash bowl is at the forefront of the global drive to reduce hospital acquired infections and improve environmental performance, and this achievement has been recognised by the Queen and the UK Prime Minister.

The patented wash bowl was developed in response to demand from infection prevention nurses who identified that re-using plastic wash bowls without properly cleaning and drying them presented a significant infection risk.

The washbowl is part of a bundle of measures that hospitals take to reduce levels of hospital acquired infections, and detailed evaluation of the Vernacare wash bowl across four hospitals showed that 98.5% of nurses agreed that the bowl reduced the likelihood of infection.

Vernacare’s business was founded on

innovation almost 50 years ago when the company pioneered the ‘Vernacare System’ which provides single use pulp products for patients who are not fully mobile and a macerator to dispose of the products in healthcare settings. This is a best practice method used in more than 40 countries, and by 90% of UK hospitals.

The wash bowl is made from 100% over issued recycled newsprint (without using bleach or dye). It provides a time saving, hygienic and environmentally friendly alternative to the time consuming process of chemically disinfecting, washing and drying plastic washbowls at high temperatures, and resolves the problem of disposing of plastic which is almost impossible to biodegrade. It also provides hospitals with cost savings on energy consumption and clinical/landfill waste disposal.

I joined EBOS Healthcare in 2007 in the role of Eschmann Product Manager, when we became the exclusive distributor in Australia of the market leading range of Eschmann Operating Tables, Lights and Accessories. It was a very exciting yet challenging time filled with learning, training and developing all things Eschmann with the Sales Team and in hospitals.
