“2011-2015 Health Plan” in Catalunya

“The Health Plan” is the main instrument and reference framework for all public actions in health.

The “Health Plan” sets ICT as the instrument for the change to achieve a new heathcare model.

The “Health Plan” sets a new healthcare model “citizen- centered”, aiming at the

active involvement and co-responsability in own care

A decentralised System

Current scenario

• The Catalan healthcare model is a multi-provider one integrated in a unique public network.

• It enhances the autonomous management of each provider.

• Providers are free to select their information systems; however 85% of the primary care centres have the same system (eCAP)

• Interoperability among systems must be guaranteed.

Clinical practice is connected with ALL the agents demanding a change in behaviour

The Project Historia Clínica Compartida de Catalunya follows a decentralised model, interfacing

through interoperability and common standards. It allows access, in an organised manner to all

relevant information of the EHRs distributed over the healthcare centres of the country

In 2012 there was a steady and gradual deployment both, of the

CPS ,and their functionalities for all citizens in Catalunya
