Signify Health can test for these antibodies. If VZV or other live attenuated immunizations are required, administer at least 1 month prior to initiation a Transaminase and total bilirubin levels.

Accessing In-Home TestingAn Overview of the Services Available for Your Patients

For your commercially insured eligible patients initiating therapy with ZEPOSIA

Pre-Initiation Testing (dependent on Start Form request)▶ Heart rate and blood pressure will be taken and recorded ▶ A blood sample will be collected and sent for processing▶ An ECG will be conducted▶ Current and prior medications will be evaluated

A risk assessment will be conducted to help identify any potential cardiac issues and medical conditions that would prevent the patient from initiating therapy on ZEPOSIA.

Requesting In-Home ServicesThere are 2 ways to request in-home services for your patients:

ZEPOSIAportal.com Visit the website and request the service when enrolling a patient through the portal

ZEPOSIA Start Form Check the appropriate box on the patient’s Start Form indicating a need for in-home services and ZEPOSIA 360 Support™ takes care of the rest

What Happens Next?A licensed Signify Health healthcare professional (HCP) will conduct a baseline assessment, and upon completion, the results will be sent to you to retain in the patient’s medical record. The results will also be sent to the ZEPOSIA 360 Support™ team.

Who Is Eligible?All commercially insured patients over 18 years of age who are prescribed ZEPOSIA will be eligible for in-home services, except for patients living in MA, MI, MN, and RI.

Billing ConsiderationsSignify Health will submit claims to the patient’s insurance and bill the patient directly based on their coverage for any co-pay, co-insurance, or deductible costs.

Signify Health is in-network for many commercial plans.
