Values: Teamwork, Leadership, Collaboration, Integrity, Commitment, andCultural Sensitivity About the Harford County Health DepartmentThe Harford County Health Department (HCHD) is the operational arm ofboth the Maryland Department of Health (MDH) and local government forthe delivery of public health services to the residents of Harford County.Programs offered by HCHD are tailored to the community’s needs andinclude behavioral health, care coordination, clinical health services,disease surveillance and control, environmental health, health educationand health promotion, public health emergency preparedness, WIC(Women, Infants, and Children) nutritional services, and many others. In March 2015, HCHD was awarded national public health accreditation bythe Public Health Accreditation Board (PHAB), one of only 67 public healthdepartments of more than 3,000 eligible health departments in the countryto have received the honor at that time. Employing over 150 employees, HCHD provides public health servicesthroughout the county and has community health centers in Bel Air,Edgewood, and Havre de Grace.

Administrative Services

Birth CertificatesThe health department’s Birth Certificate program provides certified copiesof birth certificates to eligible individuals who were born after 1939 inMaryland and are in the State of Maryland Online System. Applications canbe found online on the health department website. The cost of the birthcertificate is $25.00, cash or credit card only. Death CertificatesThe health department’s Death Certificate program provides certifiedcopies of death certificates to a surviving relative of the deceased,authorized representative of a surviving relative of the deceased, a funeraldirector who was in charge of the final disposition of the body of thedeceased, an individual who has proven legal need, or a court of law. Thefee for the first copy is $16.00. Additional copies are $20.00 each.

Environmental Health Services

Building Permit ReviewThe Building Permit Review program evaluates both residential andcommercial building permit applications for properties serviced byindividual wells and/or septic systems. These permits involve proposalssuch as commercial uses, new single family dwellings, garages, additionsto dwellings, sheds, pools, decks, etc. Staff also provides comments onpre-permit plan review applications that request an assessment of theexisting property prior to formally submitting a building permit application.Finally, this program also reviews demolition permit applications regardlessof the utilities servicing the property. Food Inspection and Food Plan ReviewThe Food Control program protects the community through plan review offood service facilities and inspections of all public food facilities, includinghospitals, nursing homes, and assisted living facilities serving 17 or moreresidents. Staff inspects food vendors at public events, mobile food units,and snowball stands. Staff also investigates foodborne outbreaks, citizencomplaints, and illegal food service operations. Soil TestingThe Soil Testing program conducts soil percolation tests to determine if thesoil/site conditions are suitable for an on-site disposal system (septic),establishes the limits of the waste disposal area, specifies the type andsize of the disposal systems from test data, and evaluates lots/parcels forInnovative and Alternative (I and A) on-site systems for lots with limitingconditions. In addition, soil tests are conducted to specify criteria to designrepair failing on-site disposal systems (septic). Finally, this programadministers the Bay Restoration Fund grant that provides financialassistance to repair a failing septic system by the installation of areplacement Best Available Technology (BAT) septic tank or by connectingthe property to public sewer if available. 3

Rabies, Zoonotic Diseases, Vector Control The Rabies and Zoonotic Disease program, housed under the CommunityHygiene Division, provides low-cost rabies vaccination clinics for dogs,cats, and ferrets. Staff also conducts annual inspections of pet stores andexotic bird breeders, and issues psittacine bird permits. The Vector Controlprogram, also housed under the Community Hygiene Division, assistscommunities with prevention and control of rodent and insect infestations.Staff provides mosquito larvicidal treatments when warranted. TheCommunicable Disease staff at the health department investigatespossible human rabies exposure cases while the Environmental Healthstaff investigates animal cases and assist with the surveillance of West NileVirus and other arboviruses. Other Community Hygiene Services The Community Hygiene Division of Environmental Health also conductsinspections and permitting of camps and mobile home parks, inspections ofprivate homes for placement of foster and adopted children, andinvestigations of nuisance complaints regarding, but not limitedto, garbage, solid and hazardous wastes, and abandoned/unsafestructures. The staff in that division inspects non-public schools at therequest of the Maryland State Department of Education, and responds topublic concerns regarding tattooing and body piercing operations. AirQuality reviews open burning/fire training permit requests and investigatesopen burning complaints. In addition, staff also provides information andtechnical support on air pollution, radon, asbestos, mold, and other issuesaffecting air quality.

Environmental Water Quality The Environmental Water Quality program reviews and issues well permitapplications in accordance with State regulations, conducts groutinspections, evaluates well completion reports, and ensures that unsafewells are properly abandoned and sealed. Additionally, the staff collectswater samples from private water supplies for analysis of bacteria, nitrates,turbidity, and sand. They analyze results, issue certificates of potability,respond to concerns about the safety of private drinking water supplies,collect miscellaneous water samples including volatile organic compounds(VOCs). Finally, this program inspects a type of public water systemknown as a Transient Non-Community Water System (TNCWS). Septic InspectionThe Sanitary Construction Inspection and Enforcement program inspectsnew, replacement, and upgraded septic systems, investigates complaintsfrom citizens concerning on-site waste disposal systems, and takesappropriate enforcement action according to State and Local regulations. Public Swimming Pools and SpasThe Public Swimming Pool and Spa Inspections program, housed underthe Community Hygiene Division, licenses all public/semi-public swimmingpools. Staff conducts routine inspections, issues pool operator certificatesto persons successfully completing an approved pool operator’s course,and maintains a database on all public and semi-public swimming poolsand certified pool operators in Harford County.

Family Health Services

Dental The Dental Program provides comprehensive oral health care services tochildren from birth through age 20 and pregnant women who are enrolledin Medical Assistance. The clinic services include X-rays, cleanings,sealants, fluoride treatments, oral health education, nutrition education,restorations (fillings), extractions, root canals for adults, crowns, spacemaintainers for missing primary teeth, and referrals to specialty care whennecessary. Also, the clinic provides dental care on two Saturdays a monthto Harford County residents living with Human Immunodeficiency Virus(HIV). The health department's Dental Program has several outreach programsincluding: a school-based oral health screening program for pre-k classesand kindergarten classes in eight Title I elementary schools in HarfordCounty, a dental and sealant program in 2nd and 3rd graders in the Title Ischools, an oral health screening program for children, and a toothbrushing program for pre-kindergartners at Halls Cross Roads andMagnolia Elementary Schools. Care Coordination PlusCare Coordination Plus assists clients in accessing community resourcesand the medical care they need. All Harford County residents are eligibleregardless of age or the type of medical insurance they have. The CareCoordinators work closely with the University of Maryland UpperChesapeake Health Care System hospitals and other local agencies toplan and coordinate medical care and the day to day needs of HarfordCounty residents.

School-Based Health Centers In partnership with Harford County Public Schools, the School HealthReadiness program provides health care, mental health care, andpreventive services to students who may lack access to health care. Theseservices are provided at four Title I elementary schools in Harford Countyand the Alternative Education Program. Medical Assistance TransportationThe Medical Assistance transportation program assures access to care bycoordinating transportation services for Harford County Medicaid recipientswho need transportation to and from medical services but have no othermeans of transportation available to them. Modes of transportationprovided include: ambulatory (cab and passenger van), wheelchair van,and ambulance. WICThe Special Supplemental Nutrition Program for Women, Infants, andChildren (WIC) is a public health nutrition program under the USDAproviding nutrition education, nutritious foods, breastfeeding support, andhealthcare referrals for income-eligible women who are pregnant orpostpartum, infants, and children up to age 5. Clients who are eligible mustlive in Maryland, meet income guidelines, and have a nutritional need. Lead Poisoning Follow-UpThe Lead Poisoning Follow-Up program provides tracking and follow-up forchildren, up to age 6, with elevated blood lead tests and provides theparent with health education regarding lead risk reduction. The programalso coordinates follow-up care with health care providers. If the patientwas exposed in a rental property, the property owners are notified ofelevated lead levels.

Family Health Services- Continued

Healthy Families AmericaHealthy Families, a program under the Family Health division, provideshome visiting services to all prenatal and postpartum families in HarfordCounty. All pregnant women and families with children under the age of 3-months are eligible to enroll for the Healthy Families program. Enrolledfamilies are able to participate until their children reach three years of age.Families are assigned Family Support Specialists who share the latestinformation about prenatal and early childhood development, promotepositive parenting skills through parent-child interactions, and connectfamilies with health care and local community resources. Healthy FamiliesHarford County helps parents to take action in reducing life stressors andmaximizing opportunities to bond with their babies, during and afterpregnancy. Helping Families RecoverThe health department is committed to the recovery of pregnant andpostpartum women with substance use disorders and their families. TheHealth Department is pleased to partner with local OB/GYNs, AddictionTreatment Centers, and other local agencies to coordinate the right carethroughout pregnancy and beyond. These care coordination efforts willallow Harford County residents to access a wide range of healthdepartment services including: addictions/behavioral health: medicationmanagement, outpatient/ intensive outpatient treatment, and peer services,birth certificates, care coordination, dental care, family planning andreproductive health, HIV/ STI treatment, immunizations, infants andtoddlers, Maryland health insurance, tobacco cessation, transportationservices, and Women, Infants, and Children (WIC).

Maryland Children’s Health ProgramMaryland Children’s Health Program works through the Maryland HealthConnection to assist Maryland residents with applying for health insurance,assisting with the Managed Care Organization (MCO) and primary caredoctor selection process. Eligibility for health insurance products offeredthrough the Maryland Health Connection is based on household size andincome. Maternal and Infant Home Visiting The goal of the Maternal and Infant Home Visiting program is to reduceinfant mortality by linking families with certain risk factors to communityservices, health care providers, assessing the safety, and promoting safesleeping environments for the infant. The program also assesses mothersfor postpartum depression and assists mothers to visit their obstetricians fora postpartum exam and to discuss family planning. Infants are followed forthe first three months of life. MEGAN's PlaceMEGAN's Place is a safe, non-judgmental place for at-risk, pregnant andpostpartum women and their families who need assistance withinformation/guidance, resources/services, and care coordination/support. Asupportive and restorative place for building family resilience, MEGAN'sPlace will help connect families to the services they need.

Administrative Care CoordinationThe Administrative Care Coordination Unit (ACCU) is a local resource forinformation for Medical Assistance (Medicaid) recipients. The programincreases the client's overall awareness of the Medicaid program andprovides HealthChoice and Medicaid education. The program assists therecipients with choosing a managed care organization and health careprovider while teaching the importance of preventive health care. TheACCU Ombudsman acts as an advocate for recipients with complexmedical needs and complaint resolution. Nurse MonitoringNurse Monitoring provides assistance with activities of daily living,medication management, shopping, and light housekeeping to medicalassistance recipients. Also, clients may be eligible for an environmentalassessment, adaptive equipment and more. Adult Evaluation Review ServicesThe Adult Evaluation Review Services conducts comprehensiveevaluations to identify services available to help individuals remain in thecommunity, or in the least restrictive environment while functioning at thehighest possible level of independence and personal well-being.

Behavioral Health Services

Clinical Behavioral Health - Adult ServicesThe health department provides an array of outpatient treatment andsupport services for adolescents and adults suffering from a mental illness,a substance use disorder, or a behavioral health condition. Servicesinclude evaluations, treatment and referral, mental health evaluations,substance use treatment, group counseling, individual counseling,intensive outpatient treatment, continuing care, family education andsupport, and medication management. Services are individualized andbased upon the needs of the client. Services offered at the healthdepartment vary from low-intensity to high-intensity outpatient treatment. Atadmission, clients are assessed to determine the best course of treatment. Peer Recovery and Support ServicesThe Peer Recovery and Support Services program provides continuingcare services, support services, and education to clients in an effort tosupport retention in treatment and long-term recovery. Services areprovided by peer recovery specialists. Local Addictions AuthorityThe health department’s Local Addictions Authority provides a localconnection between the Maryland Behavioral Health Administration andcounty-based public behavioral health treatment. The Local AddictionsAuthority supports the strengthening and expansion of substance use andaddiction counseling services and assists in bringing statewide behavioralhealth initiatives and program awareness into the county.

Clinical Behavioral Health - Youth and Adolescent ServicesThe health department offers behavioral health assessment and treatmentto children and adolescents in need. Additionally, the health departmentoffers Teen Diversion; a psychiatric rehabilitation day program housedwithin the Harford County Public School system. Treatment is focused ondiverting adolescents, ages 13 through 17, from a more restrictivetherapeutic or educational placement and reintegrating them back into thecommunity. Transitioning includes close coordination with the teen’s homeschool, outpatient treatment team, and family. Adolescents typicallyparticipate in outpatient treatment for a minimum of 12 weeks, although thelength of treatment may be longer, or shorter, based on treatmentprogress.

Clinical Health Services

Emergency Contraception The health department's family planning clinic will assist women who failedto use their contraceptive during sex or who fear their contraceptive did notwork by providing emergency contraception on a walk-in basis. Emergencycontraception can be taken up to five days after unprotected sex and doesnot end a pregnancy that has already been established. Family Planning The Family Planning clinic provides gynecological exams including breastexams, Pap smear testing, sexually transmitted infection (STI) testing andtreatment, HIV counseling and testing, birth control options, education andsupplies, reproductive and preventative health education, pregnancytesting services and emergency contraception. The clinic also providescolposcopy services for women who need follow-up treatment from anabnormal Pap smear. The clinic accepts insurance and has a sliding-feescale for uninsured or underinsured. The staff of the family planning clinicconsists of physicians, nurse practitioners, registered nurses, and clericalstaff. Pregnancy Testing The health department Family Planning Clinic offers urine pregnancytesting for women who suspect they may be pregnant. During theappointment for pregnancy testing, clients are educated and providedpreconception health counseling. Payment for testing is a sliding fee basedon income or insurance. Emergency PreparednessThe Emergency Preparedness program is responsible for planning andresponding to any health-related issue that may occur as a result of adisaster. The program staff works closely with other first responders at thelocal, regional, and state levels as well as other Harford County agencies.

ImmunizationsImmunizations to prevent vaccine-preventable diseases are offered forboth children and adults. Childhood immunizations are offered byappointment to uninsured and underinsured children ages 2 through 18years of age. A limited supply of vaccines are also available for insuredchildren, as well as adults. Please call the HCHD for vaccine availabilityand to make an appointment. Immunization clinics are held on Tuesdays atthe HCHD Edgewood location. TuberculosisActive Tuberculosis (TB) cases are followed by the health departmentnurses using direct observed medication therapy. Patients are required totake their medication in the presence of a healthcare worker for the entiretreatment time. Tuberculin skin testing is available at no charge to adoptiveand foster parents, and persons exposed to someone with active TBdisease. Individuals who test positive for TB will receive a medical reviewto confirm diagnosis and possible need for treatment. PrEPPrEP stands for pre-exposure prophylaxis for HIV prevention. PrEP shouldbe considered for people who are currently HIV-negative, but may be athigh risk for getting HIV infection. PrEP involves taking a pill every day thatcan help to protect you from getting HIV. Providers perform comprehensiveneeds assessments, offer treatment, and link clients to other medical,mental health, or social services as needed. Most insurances are accepted.Sliding fee scales are available for the uninsured. Medication paymentassistance programs are available which makes PrEP low cost or even nocost.

Public Health EducationThe Public Health Education Unit supports Maryland’s Cigarette RestitutionFund Tobacco Program through community and school-based healtheducational initiatives, tobacco treatment services and tobacco retail salesenforcement and compliance initiatives directed at the use of all tobacco,including smokeless and vaping products. The Unit offers training, provides personalized educational presentations,and performs outreach to community organizations and schools targeted atpreventing the use of all combustible, smokeless, and electronictobacco/nicotine products. Tobacco treatment information and support forthose seeking assistance in quitting tobacco use is available and NicotineReplacement Therapy (NRT) products, including patches, lozenges, andgum, also may be accessible at no cost to qualifying individuals. Public Health Education performs educational outreach to all licensedtobacco vendors in the county in support of efforts to increase retailercompliance and to promote public awareness of youth access laws.Additionally, the program coordinates annual, random, unannouncedtobacco sales compliance checks of tobacco retail sales outlets forpurposes of enforcing existing regulations prohibiting the sale ordistribution of any tobacco product to individuals under 21 years of age. Hepatitis CThe health department offers free 20-minute finger-stick testing for Hepatitis C, by appointment only, 9 a.m. to 4 p.m., Monday through Friday.For treatment, most insurances are accepted and sliding fee scales areoffered for the uninsured. Abstinence and sobriety are not needed fortreatment. Medication payment assistance programs are available.

HIV/AIDS ServicesThe health department offers free 20-minute finger-stick testing, confidential,and anonymous HIV counseling and testing services. Education is providedfor clients on the importance of protection and the risks associated with thedisease. Clients are educated on how to reduce the chances of HIV/AIDSexposure. For eligible HIV positive individuals, the following services areavailable: a monthly medical clinic, case management services, and an HIVsupport group. PrEP, a program designed to stop the transmission of HIV, isavailable for those who are at risk for HIV but are not HIV positive. Also, thehealth department's Dental Clinic provides dental care on two Saturdays amonth to Harford County residents living with HIV. Sexually Transmitted Infections (STI) The STI Clinic provides testing for sexually transmitted infections includingChlamydia, Genital Herpes, HIV, HPV, Gonorrhea, Syphilis, Trichomoniasis,and Viral Hepatitis. Treatment and/or referrals for treatment are provided.The clinic provides education to the patient in regard to STI’s andprevention. There is a walk-in clinic on Wednesday mornings and scheduledappointments the rest of the week. Most insurances are accepted and foruninsured clients, a sliding-fee scale is available. Infectious DiseaseThe Communicable Disease Surveillance and Control (CDSC) programfocuses on preventing or limiting the spread of reportable infectiousdiseases. This is achieved through disease surveillance, case investigation,contact follow-up, and implementation of control measures when necessary.The health department relies on healthcare providers, laboratories,hospitals, and citizens to report diseases.

Naloxone TrainingThe health department provides educational training under the MarylandOverdose Response Program. This program is designed to educate andtrain community members who, through their occupation, family or socialexperience, may encounter a person who is experiencing an opioidoverdose. Harm ReductionHarm Reduction utilizes a spectrum of strategies to help people who usedrugs improve drug use health as well as sexual health and connectsindividuals to substance use treatment centers and recovery support. Staffoffers services including HIV and Hepatitis C screening, HIV care referrals,Hepatitis C treatment, PrEP, naloxone training and distribution, andeducation regarding fentanyl test strips. Harm Reduction recognizes thatthere are many pathways to recovery and engages people who use drugsin a non-stigmatizing, nonjudgmental manner.

Hearing and Vision ScreeningThe Hearing and Vision Screening program provides screening to studentsenrolled in private pre-schools and other schools that are currentlyapproved by the Maryland Department of Education. Screenings areprovided to children in April and May of each year. Breast and Cervical Cancer ScreeningIn partnership with University of Maryland, Upper Chesapeake Health, theHarford County Breast and Cervical Cancer Program provides breast andcervical cancer early detection testing to low-income, under-served, under-insured, and uninsured women in Maryland. Screening services offeredinclude breast exams, mammograms, Pap tests, and HPV testing. Womenages 21-64 may be eligible for cervical cancer screenings and womenages 40-64 may be eligible for breast cancer screening services. Colorectal Cancer Screening The Colorectal Cancer Screening Program provides no cost colonoscopiesfor uninsured or underinsured men and women. The program also providespatient navigation services for qualifying individuals who are fully insuredbut need help navigating the health care system to get screened forcolorectal cancer. To be eligible, you must be a Maryland resident, age 50-75, or under 50 with an increased risk for colorectal cancer (personal orfamily history of colorectal cancer, polyps, or colitis) and meet theprogram’s income guidelines.
