These values should then be used in correspondence with this table to help you identify what actions should be taken to resolve a hazard. You should use

tactics from a more risky hazard rating if it is practicable to do so (for example, eliminating a hazard that rates at a 5 may be more practical than changing

work practices instead). Eliminating the hazard is always preferred, but choose the option that mitigates the risk the most while still being a practical

solution.

Hazard Rating Timeframes and Tactics to use when resolving a hazard

1

The hazard should be dealt with immediately. Eliminate the hazard where practicable. If eliminating the hazard is not possible, substitute the hazard with a safer alternative or isolate away from the hazard while elimination procedures are enacted.

2

The hazard should be dealt with ASAP. This can mean a slightly slower response time if there are other concerns such as injuries to be dealt with. Substitute the hazard with a safer alternative to prevent reinjury where possible.

3

The hazard should be dealt with within 24 hours. In this case, isolating the hazard away from people is a good way to mitigate the risk associated with it.

4

The hazard should be dealt with within 48 hours. In this case, adapting tools or equipment that is currently in use to reduce the risk or applying administrative procedures such as changing work practices can be ideal here.

5 The hazard should be dealt with within one week. In this case, changing work practices should be enough to mitigate this type of hazard, as it is low risk and a low frequency of occurrence.

6 The hazard should be dealt with within two weeks. As the least effective step to mitigate hazards, using Personal Protective Equipment to make everyone safe is ideal.

HAZARD PERCEPTION CHECKLIST

Version 1.0 Hazard Perception Checklist 29/06/2020

This checklist covers a number of common areas of Occupational Safety & Health that you may encounter on a day to day basis in the workplace. Please tick

“yes” if the statement is correct, or “no” if it is not. If the answer is “no” please provide some comments as to what the situation actually is like and rate the

potential risk of not following proper Occupational Safety & Health protocol. At the end of the document you will be able to record suggested

improvements to mitigate the risks of these hazards. Some of these items may not be as relevant to your branch, so if that is the case, please just tick “yes”

to that question.
