My months since becoming President have been very busy with our executive search for a new Executive Director and keeping abreast of the ever changing face of health care. I find that we have lots of work to do. The pandemic is upmost in people’s minds and we at SALPN have to be prepared for the role that LPN’s will play. We all need to know and understand our scope of practice and be able to speak confidently to the same. This is the responsibility of every LPN.

When talking with LPN’s I have found that we do not understand the difference between what the regulatory body (SALPN) and our unions do for us. This causes great confusion and sometimes anger.

It is the responsibility of each of us to know the difference. Please take the time to find out what these differences are. Another way to understand more about your association is to become involved at the committee and council level. We need people to fill these roles so that we can continue to do the best work we can.

So like the farmers, let’s harvest the positives and take pride in the SALPN as we move forward with new and exciting beginnings. We have a new Executive Director that brings with him a new perspective and lots of experience in health care that we can use as seeds to work with over the winter and sow in the months ahead. As spring approaches it will bring with it renewed optimism for the growth of our profession.

I HAVE A DREAM!

As I sit here in my kitchen and we prepare for harvest, I wonder where the months have gone. The Annual General Meeting in April seems so long ago.

I would like to thank Cynthia Gutek, both personally and on behalf of the association for all her work as Interim Executive Director. She has helped lead the association with her leadership and dedication through this time of change. We greatly appreciate it.

COLiN hEiN, EXECUTiVEDiRECTOR

On August 31st, SALPN welcomed Colin Hein as our new Executive Director. Shortly after his arrival, the Newsletter staff sat down with Colin to talk to him about the changing role of LPNs, and his hopes for the future of the profession. We’re pleased to present a transcript of that interview:

Interviewer Tell me a bit about your own history, job history, where you came from previous to this and the kind of experience you have.

Executive Director I’ve been a registered nurse since 1996, when I graduated from the SIAST campus here in Regina. I worked at the Plains Health Centre for a period of a few years until they closed. I think it was 1998 when I moved over to the General. I worked at the General until ‘99, and from that point in time, my wife and I, both nurses, moved to the States. I worked as a clinical nurse coordinator in the emergency room in Fresno, California for five years. I came back in 2004, which is when I resumed my positions in Canada. I worked for quality improvement in the Regina Qu’Appelle Health Region. Then I took over the manager position at the Regina General Hospital emergency and worked as manager for a period of just over three years before I came to SALPN.

Interviewer As an RN, what kind of interaction did you have particularly in your last position as manager with LPNs in the workplace?

Executive Director When I was manager of the Regina General Hospital Emergency, I worked with a number of LPNs, and we went through a change in scope where they went essentially up into full scope. We were directly responsible for initiating that change.

Interviewer Now when you did that, how did you see the role of LPNs versus RNs? Did you see them as opposing roles?

Executive Director No, they should be complementary. That was some of the experience I brought back from California: you need to use a multi-disciplinary approach to look after patients. LPNs and RNs worked collaboratively together with EMTs or nurse’s aides, assisting and providing care, along with the doctors, the nurse-practitioners, the physician’s assistants. Everyone has a role to play, so the care is very RN-centric, especially in emergency. We only had LPNs previously employed in the observation area assisting RNs, so we wanted to bring LPNs up to their full scope, doing more.

Interviewer Was there resistance to that?

Executive Director I think initially, there was resistance from nurses feeling uncomfortable. We did it as a trial project, but it didn’t last because funding wasn’t there. We brought LPNs into the core on an interim basis for a year, and then we had them in our streaming project. There was some resistance, but after a few months, the RNs preferred to work with the LPNs. If the LPNs were sick or not there, the RNs wanted a replacement LPN.

Interviewer Why do you think that was? When they got comfortable with each other in those roles, what was the difference?

Executive Director RNs learned what skills and what tools LPNs brought to the table: they do

excellent assessments and provide excellent care. It was just not being familiar with them in that expanded role.

Interviewer Now what lessons do you take from that and what you did there to the greater workplace, the greater health care system, province-wide? What would you say are the lessons you’ve learned there that you can use provincially?

Executive Director Well, I think provincially, they realize that collaborative practice is truly the way to go. You need to have that multi-disciplinary team provide the care. We have to have a balanced approach, and it’s fiscally responsible as well that it complements the patients’ care. We don’t need RNs to do everything just like we don’t need LPNs to do everything.

Interviewer In your early days as new executive director, have you had people question how an RN, somebody who comes from an RN background, can work for LPNs and have their best interests at heart? Have you had that question asked of you and how do you respond to it?

Executive Director I have had that question initially. My answer would be that I have a good understanding. I’ve worked with LPNs for a long, long time, and I understand the LPN’s role and what they can potentially do. Given that understanding, I can be a good advocate for them.

I’ve also spoken a fair amount about collaboration, especially collaboration with other professional associations. Being a registered nurse in good standing, I have a good relationship with the SRNA. I think that the LPN Association and the Registered Nurse Association need to work closer together on many issues.

We’re both doing the same work on different tables, and if we could bring that work together

and work more collaboratively between the two associations, I think that we would probably be able to get a little bit more mileage out of our respective associations.

Interviewer As you come into this position, what do you see as the two or three key priorities that you have for yourself as a new executive director looking forward to the next year or two for LPNs in Saskatchewan?

Executive Director I think that the public and the health care organization as we know it needs to be aware of what the LPNs’ true, full scope is, and I think we need to continue to promote the capacity in what the LPNs are able to do. I think that’s important. We continue to advocate. There’s many departments and units that don’t utilize LPNs to their full potential. In acute care, long-term care, there are a lot of changes that we could make to support and assist the LPNs in advancing in those roles.

Interviewer Now the role of a registrar’s body like SALPN is to protect the public. That’s the primary role. What’s the fine line between protecting the public and advocating for the full utilization of a group of professionals?

Executive Director Well, yes, we wear two hats. We have the advocacy role obviously, we advocate and promote the profession, but yes, there’s the fine line where we have to regulate and oversee LPNs and do what’s best for the profession and the public, but there’s a lot of grey area.

Interviewer okay, do you see the role of LPNs within the health care system expanding even beyond the current scope of practice, or do you see your job and SALPN’s job as getting all LPNs to that full scope of practice?

Executive Director I think the first thing we need to do is get all LPNs up to the point where they’re working at full scope. When LPNs are utilized at full scope they improve patient outcomes and the populations health.

IMPoRTANT REMINDERAt the 2008 Annual General Meeting, in Saskatoon, the voting delegates approved an amendment to the SALPN’s Regulatory Bylaw 11 to include the following clauses:

(e) complete courses or challenge programs, approved by the Council, in health assessment and administration of medications (f) as of December 1 2012, complete or challenge the health assessment program or basic program equivalent

(g) as of December 1 2010, complete the medications administration program or basic program equivalent.

According to the bylaw, LPNs will be required to complete:

- PhAR 264 Administration of Medications offered by SIAST. All LPNs graduating after 1993 are deemed to have the equivalent education.

- NURS 227 health Assessment for LPNs offered by SIAST or they can challenge the course. LPNs can challenge the course by contacting SIAST. LPNs graduating in 2001 and later are deemed to have the equivalent education

This bylaw amendment was approved october 2008 and is now a requirement of licensure by the dates specified above.

NEW SIAST COURSES!!!!! Canadian Practical Nurse Registration Examination Preparation Workshop. This two day workshop will assist nurse graduates who are preparing to write the Canadian Practical Nurse Registration Exam (CPNRE). The workshop includes theory review, exam writing practice and multiple choice exam strategies. Both internationally educated and Canadian graduate nurses will find the workshop helpful as they prepare to write the CPNRE. Foot Care: Principles and Practices (NURS 1656)

You will learn the anatomy, structure and function of the nail, foot and skin. You will learn how to identify common nail and skin pathology, as well as structural pathology of the foot and related nursing interventions. You will learn about common instruments used in foot care and how to care for them, utilizing Health Canada’s guidelines. You will learn how to complete a foot assessment and develop an individualized plan of care.
