Why participate in a wellness program?

The choices you make today should be viewed as an investment in your most important asset - YOU!

Did you know on average adults makes about 35,000 conscious decisions each day (a child makes about

3,000). We can unknowingly be influenced by environmental cues such as plate size, package size, people

around us, ads on the radio & TV, and other distractions. In fact, we make 226.7 decisions each day on

just food alone according to researchers at Cornell University (Wansink and Sobal, 2007). You have an

overwhelming amount of choices and each choice has a result—good and bad. No one is perfect and you

will not always make the best choice, but knowing more about yourself and understanding the choices you

face has a huge impact on your overall well-being.

Understanding your current behaviors and identifying any risk, is the first step towards your well-being

journey! Each step and each set-back on your journey will help you make more healthy choices such as,

drinking more water, obtaining a primary care doctor and receiving your preventive screenings.

Who is eligible to participate? HEALTHYyouyouyouyou is a voluntary wellness program. However, we

encourage your participation (off company time). Employees

and their spouses covered on a Perrigo medical plan are eligible

to participate and earn a wellness discount, but more importantly

you will have access to valuable resources in life-long wellness!

COMING SOON! Employees who are benefits eligible but not

enrolled in a Perrigo medical plan will be able to participate and access valuable resources!

Steps to Get Started:

1. Employees and covered spouses* each need to create

an account** by going to www.perrigo.limeade.com.

2. Click the “Get started” button.

3. Follow the registration steps.

*Spouses enroll by using the EE ID number + S + last 4 of their own SSN (e.g. 400010S3768).

After registering, you and your covered spouse can participate in the HEALTHYyou you you you wellness program

and start earning points towards a Partial or Full Wellness Discount. Participate between Jan. 3, 2017

— Nov. 30, 2017 to earn a wellness discount in 2018!

Level 1 (Partial Wellness Discount)

To achieve a Level 1 Partial Wellness Discount (500 points), you and your covered spouse must

individually complete the following activities between Jan. 3, 2017 - Nov. 30, 2017:

1. Biometric Health Screening (onsite; on company time or offsite; off company time);

2. Well-Being Assessment (online questionnaire);

3. Tobacco Free (verified via cotinine test) or reasonable alternative Breathe Easy program; and

4. Healthy Standards (must meet 2 of 4) or reasonable alternative My Health Matters program.

Note: If you accumulate 500 points before completing these requirements, you must still complete these 4

requirements to be eligible for a wellness discount.

What if I use Tobacco Products or Test Positive?

If you are a tobacco user, or your cotinine test produces a

positive result, you can complete the Breathe Easy program

as a reasonable alternative to satisfy the Tobacco Free

requirement and earn points towards your wellness discount.

Register Online: Allow yourself enough time to register and

complete the six-week coaching program prior to Nov. 30,

2017.

IMPORTANT:

• Consumer Directed Health Plan (CDHP): If you are enrolled in the CDHP plan and use your annual

physical with your doctor to complete the cotinine test, it will be subject to your medical deductible.

To avoid out-of-pocket costs, have your cotinine test completed during a Perrigo onsite biometric

screening, at a LabCorp Patient Service Center or the Perrigo Medical Center (Michigan only).

• Second Hand Smoke: Cotinine tests can produce a positive result for individuals who have high

exposure to second hand smoke, producing the same risk factors as a smoker. If you receive a

positive result, the six-week Breathe Easy program can be completed as a reasonable alternative,

satisfying the Tobacco Free requirement.

Level 2 (Full Wellness Discount) To achieve a Level 2 Full Wellness Discount, you and your covered spouse must individually earn a total

of 1,000 points (500 points from the four Level 1 requirements on page 4) and 500 more points through

participation in wellness activities by Nov. 30, 2017.

What are Wellness Activities?

HEALTHYyou you you you wellness activities are tools and resources designed to engage participation and assist you

in making and maintaining good choices on your well-being journey. Wellness activities are created

around four major categories:

What if I Add or Remove my Spouse during the Year?

If you add your covered spouse with a qualified family status change (e.g. marriage, loss of coverage),

your covered spouse will have 60 days to complete a Biometric Health Screening and Well-Being

Assessment, to continue your wellness discount for the rest of the year (provided a wellness discount

was already earned).

If your spouse completes

If your spouse is no longer eligible for a Perrigo medical plan: and you experienced a qualified

family status change (e.g. divorce, gains other coverage) after you have earned a wellness discount, the

wellness discount will remain.

If you met the program requirements in the prior year but your spouse did not meet the requirements, you

will be awarded a wellness discount as soon as administratively possible once your spouse is removed

from the medical plan. Please contact HEALTHYyou you you you customer service to add the wellness discount.

What if I am a New Hire? New hires have the opportunity to earn points towards a

wellness discount as noted in the table on the next page.

Notice Regarding Wellness Program

HEALTHYyou you you you is a voluntary wellness program available to all

eligible employees and spouses who are enrolled in a Perrigo medical plan. The program is administered according to federal rules permitting employer-sponsored wellness programs that seek to improve employee health or prevent disease, including the Americans with Disabilities Act of 1990, the Genetic Information Nondiscrimination Act of 2008, and the Health Insurance Portability and Accountability Act, as applicable, among others. If you choose to participate in the wellness program you will be asked to complete a voluntary well-being assessment or "WBA" that asks a series of questions about your health-related activities and behaviors and whether you have or had certain medical conditions (e.g., cancer, diabetes, or heart disease). You will also be asked to complete a biometric screening, which will include a blood test; testing blood sugar, triglycerides, cholesterol, and a cotinine test to evaluate tobacco use through urine, blood or saliva. The biometric screening and cotinine tests are used to identify potential indicators that a participant is at risk for conditions such as diabetes, metabolic syndrome, or heart disease. You are not required to complete the WBA or to participate in the blood test or other medical examinations.

However, employees who choose to participate in the wellness program will receive a medical premium discount (see the Employee Benefits Guide on the myHR portal through Inside Perrigo for discount details) for completion of Level I and Level II program guidelines. Although you are not required to complete the WBA or participate in the biometric screening, it is a component of the wellness program, for those who wish to participate. You are eligible to receive a Partial Wellness Discount (Level I) or a Full Wellness Discount (Level II) based on the completion of program guidelines.

Incentives on your medical premiums are available for employees who participate in certain health-related activities (see the HEALTHYyou you you you guide on the myHR portal through

Inside Perrigo for program details) or achieve certain health outcomes (Tobacco Free and meeting 2 of 4 Healthy Standards). If you are unable to participate in any of the health-related activities or achieve any of the health outcomes required to earn an incentive, you may be entitled to a reasonable accommodation or an alternative standard. You may request a reasonable accommodation or an alternative standard by contacting Limeade (HEALTHYyou you you you administrator)

The information from your WBA and the results from your biometric screening will be used to provide you with information to help you understand your current health and potential risks, and may also be used to offer you services through the wellness program, such as health coaching. You are encouraged to share your results or concerns with your own doctor.

Protections from Disclosure of Medical Information

We are required by law to maintain the privacy and security of your personal identifiable health information. Although the wellness program and Perrigo, Inc. may use aggregate information it collects to design a program based on identified health risks in the workplace, HEALTHYyou you you you will never

disclose any of your personal information either publicly or to the employer, except as necessary to respond to a request from you for a reasonable accommodation needed to participate in the wellness program, or as expressly permitted by law. Medical information that personally identifies you, that is provided in connection with the wellness program will not be provided to your supervisors or managers and may never be used to make decisions regarding your employment.

Your health information will not be sold, exchanged, transferred, or otherwise disclosed except to the extent permitted by law to carry out specific activities related to the wellness program, and you will not be asked or required to waive the confidentiality of your health information as a condition of participating in the wellness program or receiving an incentive. Anyone who receives your information for purposes of providing you services as part of the wellness program will abide by the same confidentiality requirements. The only individual(s) who will receive your personal identifiable health information are the wellness program administrator (Limeade), the biometric and health coaching administrator (Aduro), and other third-party vendors as deemed necessary by the employer in order to provide you with services under the wellness program.

In addition, all medical information obtained through the wellness program will be maintained separate from your personnel records, information stored electronically will be encrypted, and no information you provide as part of the wellness program will be used in making any employment decision. Additionally, we comply with HIPAA and Privacy requirements with respect to such information. Appropriate precautions will be taken to avoid any data breach, and in the event a data breach occurs involving information you provide in connection with the wellness program, we will notify you immediately.

You may not be discriminated against in employment because of the medical information you provide as part of participating in the wellness program, nor may you be subjected to retaliation if you choose not to participate.

Common HEALTHYyou you you you Terms

ADURO: A Limeade coaching partner that manages the HEALTHYyouyouyouyou coaching programs.

Biometric Health Screening: A mini-physical completed onsite with a Wellness Program clinician or offsite through

a LabCorp Patient Service Center or as part of your annual physical with your own doctor (use Physician Fax Form).

The Perrigo Medical Center may be used by employees and spouses in Michigan.

Breathe Easy: A six-week smoking cessation program available as a reasonable alternative to the Tobacco Free

requirement.

Consumer Directed Health Plan (CDHP): A plan that combines a qualified high-deductible health plan with a

Health Savings Account used to fund out-of-pocket costs with pre-tax dollars contributed through payroll deductions.

Cotinine Test: A tobacco-use detection test that measures cotinine (a byproduct of nicotine) during a Biometric

Health Screening, using saliva (cheek swab) if conducted during a Perrigo onsite screening. If conducted offsite,

blood, or urine may be used.

Covered Spouse: A spouse of a Perrigo employee who is enrolled as a dependent in a Perrigo medical plan.

Employee: Perrigo full-time or part-time benefits eligible (not including non-benefits eligible employees).

Healthy Standards: Four health standards (BMI—Body Mass Index, Blood Pressure, Total Cholesterol, Glucose

Level) measured during a Biometric Health Screening. Participants need to be within a healthy range in at least 2 of

the 4 Healthy Standards requirements to be eligible for a wellness discount.

Limeade: The administrator of the HEALTHYyouyouyouyou Wellness Program.

Metabolic Syndrome: The name for a group of factors that raise the risk for heart disease and other health

problems, such as diabetes and stroke.

My Health Matters: A six-week program available as a reasonable alternative to meeting 2 of the 4 Healthy

Standards requirement.

Non-Benefits Enrolled: Employee’s who are benefits-eligible but not enrolled in a Perrigo Medical Plan who will be

able to participate in HEALTHYyouyouyouyou (no wellness discount will apply).

Tobacco Free: In the past six months, you or your covered spouse did not at one time, regularly, or occasionally

use cigarettes, cigars, pipes, smokeless tobacco (chew, snuff, etc.) or did not use nicotine electronic delivery

systems (e-cigarettes) used with nicotine or nicotine-free liquids.

Well-Being Assessment: A set of questions focusing on 6 life areas that helps participants understand more about

their well-being and health risks to assist in improving or maintaining their health and overall well-being.

Wellness Discount: Refers to the discounts available that reduces an employee’s medical contribution.
