WHEREAS, Robert Layton Wright, 7th child of Walter and Johnnie Pearl Wright was born on January 7, 1933 in Palatka, Florida; graduated from Central Academy and served his country in the U.S. Marine Corp in Korea to be honorably discharged in 1955; and

. WHEREAS, 46 years ago on May 25, 1969, he married Ethel Wilcox Wright, and has 12 children, 25 grandchildren, and 10 great-grandchildren; and

WHEREAS, he has been involved in this community in many ways as an employee at R.C. Bannerman Learning Center, the City of Green Cove Springs and Challenge Enterprises, a sponsor of the Food Pantry, organizer in starting day care programs, a volunteer with the former Green Cove Springs "Family Frolics" and "Take Back The Night", the 3rd Vice Moderator for East Florida Bethany Association in Jacksonville, Grand Marshal of the Soul Food Festival Parade, and a member of the City of Green Cove Springs Visioning Committee; and

WHEREAS, he has many passions such as baseball, mentoring and being a role model for local teens, a hobby as a master builder and craftsman, teaches bible study, and holds the longest position as Pastor of Mt. Pleasant Missionary Baptist Church for 40 years as of this August 2015.

NOW, THEREFORE, WE, THE MAYOR AND THE CI1Y COUNCIL OF THE CI1Y OF GREEN COVE SPRINGS DO HEREBY PROCLAIM:

1. Every April 21st to be "Reverend Robert L. Wright Day" in Green Cove Springs and urge citizens to take time to recognize and share their appreciation for his many years of community service, to include holding the position of the longest serving Pastor for 40 years at Mt. Pleasant Missionary Baptist Church.

WHEREAS, Florida's future prosperity depends on nurturing the healthy development of over 4 million children currently living, growing and learning within our many diverse communities; and

WHEREAS, research shows that safe and nurturing relationships and stimulating and stable environments improve brain development and child well-being, while neglectful or abusive experiences and unstable or stressful environments increase the odds of poor childhood outcomes; and

WHEREAS, the abuse and neglect of children can cause severe, costly, and lifelong problems affecting all of society, including physical, and mental health problems, school failure, and criminal behavior; and

WHEREAS, research also shows that parents and caregivers who have social networks and know how to seek help in times of trouble are more resilient and better able to provide safe environments and nurturing experiences for tl1eir children; and

WHEREAS, individuals, businesses, schools, and faith-based and community organizations must make children a top priority and take action to support the physical, social, emotional and educational development and competency of all children; and

WHEREAS, during tl1e month of April, Prevent Child Abuse Florida, in collaboration with the Florida Department of Children and Families, will be engaging communities throughout Florida in a coordinated effort to prevent child abuse and neglect by promoting awareness of healthy child development, positive parenting practices and the types of concrete support families need within their communities.

Gentlemen:

The City of Green Cove Springs wishes to express its sincere appreciation to you for completion of the electrical installation for Magnolia Point, Unit VIII-A. Your hard work and dedication not only allowed home construction to begin in a timely manner, but it also saved the City approximately $30,000 in anticipated contractor costs.

Development of this new area of Magnolia Point had progressed to the point that builders were delayed in obtaining permits because the developer had not yet installed their portion of the electric system. Originally, the developer's electrician was to install the underground conduits, a contractor was to pull in the wires, and the City crew was to set transformers and make everything hot. Because the developer's electrician was late in installing the conduits, you, along with help from Public Works, took over the contractor's portion of the work as well as your own and completed the system within just a few days.

CITY OF GREEN COVE SPRINGS, FLORIDA STAFF REPORT

FOR MEETING OF APRIL 21, 2015

SUBJECT: SECOND AND FINAL READING OF ORDINANCE NO. 0-05-2015, AN ORDINANCE OF THE CITY OF GREEN COVE SPRINGS, FLORIDA, VOLUNTARILY ANNEXING APPROXIMATELY 140 ACRES OF REAL PROPERTY INTO THE CORPORATE LIMITS OF THE CITY

BACKGROUND: In 2011, the City, with the assistance of Florida Communities Trust (FCT) grant funds, purchased approximately 140 acres for Phase Ill of the Green Cove Springs Nature Preserve - Ed Gustafson Regional Park. The approximately 140 acres parcel proposed for annexation is part of the Phase Ill acquisition and is identified by the Clay County Property Appraiser as Parcels # 016515-002-00, 016594-000-00, 016598-000-00, 016598-001-00, 016598-002-00, and 016600-000-00.

One of the requirements of the FCT grant agreement was to amend the Future Land Use Map (FLUM) designation for the acquired property to Recreation and/or Conservation. After the property is annexed into the City, the property will be designated with the City's Recreation FLUM designation.

On March 3, 2015, the City Council authorized the annexation of Parcels# 016515-002-00, 016594-000-00, 016598-000-00, 016598-001-00, 016598-002-00, and 016600-000-00, and directed the City Attorney to prepare the Ordinance to annex the parcel into the City. On March 5th, the Clerk sent the letter required by State Statutes to the Chairperson of the County Commission notifying the County of the proposed annexation. The required advertisements were published in the Clay Today on March 19th and 25th, notifying the public of the proposed annexation. In addition, a sign has been posted on the property and letters were sent out to property owner within 300 feet of the property noticing the annexation.

FISCAL IMPACT I FUNDING SOURCE: The cost of advertising and notice requirements for the annexation required by State Statutes.

STAFF RECOMMENDATION: Staff recommends the City Council approve Ordinance No. 0-02-2015 on second and final reading.

ORDINANCE NO. 0-05-2015

AN ORDINANCE OF THE CITY OF GREEN COVE SPRINGS, FLORIDA, VOLUNTARILY ANNEXING APPROXIMATELY 140 ACRES OF REAL PROPERTY INTO THE CORPORATE LIMITS OF THE CITY; SAID PROPERTY BEING GENERALLY LOCATED ADJACENT TO THE EXISTING SOUTHERN CITY LIMITS, EASTERLY OF OAKRIDGE A VENUE/COUNTY ROAD lSA AND SOUTHERLY OF GREEN COVE A VENUE; DESCRIBING SAID PROPERTY BY METES AND BOUNDS; FINDING THAT THE OWNERS OF SAID PROPERTY HAVE PETITIONED THE CITY PURSUANT TO CHAPTER 171.044, FLORIDA STATUTES, TO VOLUNTARILY ANNEX SAME; FINDING THAT THE PROPERTY IS CONTIGUOUS TO THE EXISTING CITY LIMITS AND REASONABLY COMPACT; PROVIDING FOR REPEALER, SEVERABILITY, AND SETTING AN EFFECTIVE DATE.

WHEREAS, .all owners (City of Green Cove Springs) of the property subject hereof have petitioned the City to have their property described in Exhibit "A" and as also depicted in the parcel map attached hereto as Exhibit "B", to be annexed into the City limits pursuant to Chapter 171.044, Florida Statutes; and

WHEREAS, the City has determined that the property conforms to the requirements of Chapter 171.044, Florida Statutes, for real property to be voluntarily annexed; and

WHEREAS, said property is contiguous to the existing City limits and is reasonably compact; and

WHEREAS, the Clay County Board of County Commissioners has been given due notice as required in Florida Statute 171.044(6); and

WHEREAS, all other notices required by law have been given.

NOW, THEREFORE, BE IT ENACTED BY THE CITY COUNCIL OF GREEN COVE SPRINGS AS FOLLOWS:

Section 1. That pursuant to the provision of Chapter 171.044, Florida Statutes, the City Council does hereby voluntarily annex the real property described in Exhibit "A" and depicted on Exhibit "B" into the corporate limits of the City of Green Cove Springs, Florida.

Section 2. Repealer. Any ordinances or parts thereof in conflict with the provisions of this ordinance are hereby repealed to the extent of such conflict.

Per Florida Statutes 171.044(6), notice is hereby given to the Clay County Board of County Commissioners that the City Council of the City of Green Cove Springs has authorized the voluntary annexation of a City owned parcel located located east of Oakridge Avenue/ County Road 15A and south of Green Cove Avenue, known as the Phase Ill of the Green Cove Springs Nature Preserve - Ed Gustafson Regional Park into corporate limits of the City of Green Cove Springs. The 140 acres are identified by the Property Appraiser as of Parcels# 016515-002-00, 016594-000-00, 016598-000-00, 016598-001-00, 016598-002-00, and 016600-000-00.

A copy of Ordinance No.0-05-20'15, including a map identifying the parcel, is attached herewith. Also attached is a copy of the legal notice that will be published on March 19 and March 26, 2015, in the Clay Today. The final hearing and adoption of the above referenced Ordinance is scheduled to be heard by the Green Cove Springs City Council on Tuesday, April 21, 2015 at 7:00 p.m. in the Green Cove Springs City Council Chambers, 321 Walnut Street, Green Cove Springs, Florida.

AN ORDINANCE OF THE CITY OF GREEN COVE SPRINGS, FLORIDA, VOLUNTARILY ANNEXING APPROXIMATELY 140 ACRES OF REAL PROPERTY INTO THE CORPORATE LIMITS OF THE CITY; SAID PROPERTY BEING GENERALLY LOCATED ADJACENT TO THE EXISTING SOUTHERN CITY LIMITS, EASTERLY OF OAKIUDGE A VENUE/COUNTY ROAD 15A AND SOUTHERLY OF GREEN COVE AVENUE; DESCRIBING SAID PROPERTY BY METES AND BOUNDS; FINDING THAT THE OWNERS OF SAID PROPERTY HAVE PETITIONED THE CITY PURSUANT TO CHAPTER 171.044, FLORIDA STATUTES, TO VOLUNTARILY ANNEX SAME; FINDING THAT THE PROPERTY IS CONTIGUOUS TO THE EXISTING CITY LIMITS AND REASONABLY COMPACT; PROVIDING FOR REPEALER, SEVERABILITY, AND SETTING AN EFFECTIVE DATE.

WHEREAS, all owners (City of Green Cove Springs) of the propetty subject hereof have petitioned the City to have their property described in Exhibit "A" and as also depicted in the parcel map attached hereto as Exhibit HB", to be annexed into the City limits pursuant to Chapter 171.044, Florida Statutes; and

WHEREAS, the City has determined that the property confo1ms to the requirements of Chapter 171.044, Florida Statutes, for real property to be voluntarily annexed; and

WHEREAS, said property is contiguous to the existing City limits and is reasonably compact; and

WHEREAS, the Clay County Board of County Commissioners has been given due notice as required in Florida Statute 171.044(6); and

WHEREAS, all other notices required by law have been given.

NOW, THEREFORE, BE IT ENACTED BY THE CITY COUNCIL OF GREEN COVE SPRINGS AS FOLLOWS:

Section 1. That pursuant to the provision of Chapter 171.044, Florida Statutes, the City Council does hereby voluntarily alU1ex the real property described in Exhibit "A" and depicted on Exhibit "B" into the corporate limits of the City of Green Cove Springs, Florida.

Section 2. Repealer, Any ordinances or parts thereof in conflict with the provisions of this ordinance are hereby repealed to the extent of such conflic~ .

Section 4. Effective Date. This Ordinance shall take effect immediately upon passage.

NOTICE OF PROPOSED ANNEXATION NOTICE OF PUBLIC HEARING

The City Council of the City of Green Cove Springs has scheduled a public hearing to be held at 7:00 p.m., Tuesday, April 21, 2015, in the City Council Chambers, 321 Walnut Street, Green Cove Springs, Florida, to consider adoption of an ordinance. The ordinance title is as follows:

AN ORDINANCE OF THE CITY OF GREEN COVE SPRINGS, FLOIUDA, VOLUNTARILY ANNEXING APPROXIMATELY 140 ACRES OF REAL PROPERTY INTO THE CORPORATE LIMITS OF THE CITY; SAJD PROPERTY BEING GENERALLY LOCATED ADJACENT TO THE EXISTING SOUTHERN CITY LIMITS, EASTERLY OF OAKRIDGE AVENUE/COUNTY ROAD 15A AND SOUTHERLY OF GREEN COVE AVENUE; DESCRIBING SAID PROPERTY BY METES AND BOUNDS; FINDING THAT THE OWNERS OF SAID PROPERTY HAVE PETITIONED THE CITY PURSUANT TO CHAPTER 171.044, FLORIDA STATUTES, TO VOLUNTARILY ANNEX SAME; FINDING THAT THE PROPERTY IS CONTIGUOUS TO THE EXISTING CITY LIMITS AND REASONABLY COMPACT; PROVIDING FOR REPEALER, SEVERABILITY, AND SETTING AN EFFECTIVE DATE.

The proposed annexation includes six (6) parcels of real property totaling approximately 140 acres located east of Oakridge Avenue/County Road 15A and south of Green Cove Avenue in Green Cove Springs. A complete legal description of the property by metes and bounds and a copy of Ordinance 0-06-2015 can be obtained from the office of the City Clerk, second floor of City Hall, 321 Walnut Street, Green Cove Springs, Florida. The 140 acres for the proposed annexation is identified by the Clay County Property Appraiser by RE Parcels# 016515-002-00, 016594-000-00, 016598-000-00, Ol6598-001-00, 016598-002-00, and 016600-000-00.

BACKGROUND: In 2003, the FDOT started a program to compensate all local maintaining agencies for their costs associated with the operation and maintenance of lighting systems located on state owned roadways. The City received $4,780 for FY 02/03. The City received $21,770.64 for FY 12/13, $26,352.00 for FY 13/14, and $27,142.56 for FY 14/15.

MOTION: Approve the renewal of the FDOT State Highway System Lighting, Maintenance, and Compensation Agreement - Contract No. AN491 - for the period of July 1, 2015 through June 30, 2016 in the amount of $27,956.88 and the City invoice requesting reimbursement of $27,142.56 for F/Y 14/15.

1.0 PURPOSE

This exhibit defines the method and limits of compensation to be made to the Maintaining Agency for the services described in this Agreement and method by which payments will be made.

2.0 COMPENSATION

For the satisfactory completion of all services detailed in this Agreement, the Department will pay the Maintaining Agency the Total Lump Sum amount indicated in Section 2 of the Agreement. The Maintaining Agency will receive one lump sum payment at the end of each fiscal year for satisfactory completion of service.

Total Lumps.um Amount for each Fiscal Year is calculated by taking all of the actual number of qualifying lights times 90% times the FY Unit Rate.

Example: 330 lights x 0.90 x $201.58 = $59,869.26

FY 12-13 $201.58 13-14 $244:00 14-15 $251.32

Beginning FY 14-15, the Unit Rate for each fiscal year will be 3% more than the Unit Rate from the previous fiscal year, unless otherwise specified in an Amendment to this Agreement r changed through an update of this exhibit.

SUBJECT: City Council approval of Amendment No. 3 to the Professional Services Agreement with Hawkins, Inc.

BACKGROUND: The current agreement between the City of Green Cove Springs and Hawkins, Inc. allows for the renewal of up to four ( 4) successive one year terms. This will be the third (3) renewal term.

FISCAL IMPACT I FUNDING SOURCE: None.

RECOMMENDATION: Staff recommends approval of Amendment No. 3 to the Professional Service Agreement with Hawkins, Inc. agreeing to renew under the same terms, conditions and specifications for the period retroactive to April 15, 2015 through April 14, 2016 and authorizing the Mayor to execute the documents on behalf of the City.

CITY OF GREEN COVE SPRINGS, FLORIDA STAFF REPORT

FOR MEETING OF APRIL 21, 2015

SUBJECT: Approval of Resolution No. R-07-2015, a Resolution authorizing the City Manager or her Designee to apply for a Florida Boating Improvement Program Grant and serve as Project Manager

BACKGROUND: Staff has applied for a Florida Boating Improvement Program Grant in the amount of $150,000. The Florida Boating Improvement Program provides grants to local governments for boating access. The purpose of this grant will be to construct improvements to the existing City Pier to make it more accessible to larger ships (up to 150 feet in length) and to make it safer for smaller recreational boats who dock at the pier. The improvements should also help the environment by reducing the impact of northeast winds on the grasses along the shoreline that provide both a safe haven for fish and food for manatees.

The project budget is $200,000, which includes matching funds of $50,000 designated in the FY 15 CIP Budget. The project components include:

1. Structural and utility repairs to the north and south floating docks 2. Installation of "dolphins" (separate pilings for large ships to moor to) 3. Installation of a wave break to reduce the impact ofNor'easters 4. Repairs to the "Mclnemy" dock which include a kayak launch.

Staff is requesting ratification of the grant application and approval of Resolution R-07-2015 as the deadline for submission of the grant application was April 9, 2015.

FISCAL IMP ACT: The proposed matching cash funds of $50,000 are appropriated and available in the approved FY 15 Capital Improvements Budget.

RECOMMENDATION: Staff recommends approval of Resolution No. R-07-2015, a Resolution authorizing the City Manager or her Designee to serve as the project manager and apply for a Florida Boating Improvement Program Grant in the amount of $150,000.

A RESOLUTION OF THE CITY COUNCIL OF GREEN COVE SPRINGS, FLORIDA, AUTHORIZING THE CITY MANAGER, OR HER DESIGNEE, TO APPLY FOR A FLORIDA BOATING IMPROVEMENT PROGRAM GRANT AND SERVE AS PROJECT MANAGER.

The grant requires a 50:50 match. The time the City staff works on the grant will be the in-kind match for the grant. No additional City funds will be required.

Florida Urban and Community Forestry Grant Program As part of the U.S. Forest Service Urban and Community Forestry Matching Grant Program, funds will

be available to organizations to develop or enhance their urban and community forestry programs.

Awards are made as 50-50 matching grants (50 percent federal, 50 percent applicant) to Local

governments, educational institutions, Native-American tribal governments, and Legally organized

nonprofit (volunteer) organizations in the five grant categories Listed below.

tree maintenance projects. Only 20% of funds will be used for this

purpose.

Nonprofit Administration

This is for temporary personnel costs only and is meant to help

nonprofit groups become more effective in their ability to support and

promote Local tree management programs.

Information and Education

This category includes educational programs, Arbor Day programs, Workshops/Training sessions, Youth

programs, PSA (public service announcements) development, volunteer training, purchasing or

developing web pages, brochures and exhibits.

Urban Forestry or Arboricultural Training

This category is intended to provide cost-share funding for the development of new or additional

continuing education courses or degree track educational courses in urban forestry or arboriculture.

A maximum of $10,000 will be awarded to successful applicants for demonstration and for

information and education projects. Staffing grants will be limited to three years, and the applicant

will have to reapply on an annual basis. Otherwise, the maximum award is $30,000 for each

applicant, and $30,000 for individual practices. Approximately $400,000 in funds will be available

during this enrollment period.

CITY OF GREEN COVE SPRINGS, FLORIDA STAFF REPORT

FOR MEETING OF APRIL 21, 2015

SUBJECT: Augusta Savage Arts & Community Center (ASACC) Master Plan Update

BACKGROUND: At the March 3rd Council session, the Administration was directed to report back with an update to a Master Plan for the ASACC Campus that would include cost estimates for the Industrial Arts building footprint and renovation of the Library building. Staff has been able to accomplish some of these items. This staff report does accomplish updating the Master Conceptual Site Plan and provides further recommendations for Council consideration.

In 2008, a conceptual site plan (attached) was done for ASACC campus. Since that time, the City has made various strides on the campus. Some level of effort has occurred on every building on the campus, except for the Industrial Arts building. The least amount of effort thus far has occurred on the Library. The only improvements made on the Library were the installation of a foam roof in 2008 which has a life expectancy between five to seven years; it is now seven years later. In 2011, new roofs were placed over the gym, classroom, and cafeteria buildings. We also 'dried-in' the Cafeteria building in 2011, formerly known as the T. R. Marie Auditorium. Improvements were also made to the interior of the gymnasium in 2011 such that this building is fully functional. In 2014 the City partnered with Head Start to accelerate additional improvements on classroom spaces. The degree of effort on each building has been commensurate to the building condition. Every effort has been made to "dry-in" facilities to limit water damage and deterioration subject to budget limitations.

The following pages are excerpts from a February 15, 2011 staff report that summarize each building condition in 2011 and outline a plan of action to address those needs. It also notes any buildings that require environmental remediation.

EXCERPT FROM FEBRUARY 15, 2011 COUNCIL AGENDA ITEM:

The prioritization of buildings on the campus was based on these key assumptions: 1) Maintaining the City's physical assets. 2) Safeguarding our cultural heritage. 3) Implementing a Goal in the area of Recreation and Open Space from our 2005

Vision Statement found on page 12 titled "Expand recreational opportunities"; Action step "Investigate developing a Community Center."

4) Utilizing Surtax funds for this capital project. 5) Renovating facilities at the ASACC in priority order based upon their physical

condition. 6) Determining which facilities must be completed in the current fiscal year based

upon their physical condition. 7) Completing facilities at the ASACC that are in a productive use and have the

capability to generate revenues.

A detailed discussion on the condition of each building follows directly below which formulates the basis for our recommendation on this capital project. The order in which each building appears coincides with the bid packet that was released based on the condition of each facility at theASACC.

BUILDING# 3 CAFETERIA, 3,989 S.F.:

The roof on the main portion of the cafeteria has catastrophically failed. The kitchen area of the cafeteria is actually in decent shape. There is a bathroom section immediately behind the stage and also a walk-in refrigerator that have been added onto the main building. All of these roof leaks have lead to standing water in the building and mold growing on the walls. In addition to reroofing the main portion of the building and the kitchen, the bathrooms and walk-in cooler addition should be demolished to prevent more water from entering the building in the future.

There are no asbestos concerns in this building. The only lead concerns are the paint on the doors.

BUILDING# 5 GYMNASIUM, 6,257 S.F.:

This building has 10-12 leaks throughout the roof over the gym floor. It also has several leaks along the front edge where the flat roof over the restrooms joins the front face of the building.

The rear portion of the building is currently not up to code. There is no insulation or ceiling. The air conditioning duct work and electrical wiring is all exposed. The electrical panels are also open to the rest of the room which could allow unauthorized personnel, or worse children, to access these panels (which are currently locked at all times). The lack of a ceiling and insulation in this area again leads to unnecessarily high utility bills. The electrical panels need to be segregated away from the rest of the area which will allow the rest of the rear area to be used as an office and study area for the children in the community.

There are no lead and asbestos concerns in this building.

BUILDING# 1 CLASSROOMS, 10,580 S.F.:

The classroom building currently does not leak over any of the conditioned spaces (includes all classrooms and restrooms). The walkway around the interior of the building has failed in numerous places.

The shingled roof that covers the existing building has reached the end of its useful life.

The only asbestos concern in this building is an old heat duct that runs between the old concrete flat roof and the newer trussed roof As long as this duct remains intact, there is no immediate concern.

The only lead concern in this building is the paint on the brick columns supporting the walkway roof If we provide a fresh coat of paint over this paint, then there is no immediate concern until we demolish the brick columns.

BUILDING# 4 INDUSTRIAL ARTS, 4,800 S.F.:

Currently, the only leaks in this building are in the small wooden addition on the south side of the building. As long as these doors remain closed, there is no immediate threat to the remainder of the building. There are actually two roofs on this building. There is a low-pitched roof constructed over an old flat roof. The shingles on the pitched roof are beyond their useful life. They are brittle arid the edges are curling up. The roof has failed in a couple of places around the outer edge. Given the condition of the shingle roof and the uncertainty of the condition of the flat roof, it is entirely possible that this roof could leak at any time. This building is used for storage and staff will continue to monitor this building for leaks.

The only asbestos concerns in this building are a large kiln that was used for making pottery at some point in the past. There are no asbestos concerns with the structure itself

BUILDING# 2 LIBRARY, 2,267 S.F.:

This building currently does not leak. About three years ago, the City had a 4" thick foam roof sprayed over the entire building. However, while this building is dry now, this foam product only has a life expectancy of 5-7 years.

There are asbestos concerns in the floor tile and adhesive. There are no lead concerns in this building.

FUTURE DIRECTIONS:

Neighborhood Meeting on February 26, 2015

The City conducted a neighborhood meeting at the ASACC on February 261h. The meeting

concluded with two recommendations; one dealing with the Industrial Arts Building, another with the Library building. The first step is to declare the Industrial Arts Building as surplus, demolish and clear the site, and place that footprint area into public green space for a playground. Next step is to renovate the Library building to serve as a location for a student resource center, dance studio, various instructional programs and other community uses.

During the meeting, the City Manager also advised the group that demolition of the site would cost approximately $36,000 of which about $10,000 was for asbestos abatement. A cost estimate to place a new roof on the Industrial Arts Building was estimated at $60,000. Clearly demolition

on the site is a better use of resources. These estimates along with comments received during the February 26th community meeting were reported to the Council on March 3rd. The Friends of Augusta Savage, a not-for-profit corporation, also held a meeting and reported these recommendations on March 1 gth to the City Manager. The excerpts from their e-mail follow below:

"1) A new playground with an interactive track around the playground (to be positioned in the area of the future building removal). Research is being done to draw up plans for this project and costs estimate. This project will promote health and fitness for the young to the elderly.

2) 2400 sf building - 1200 sf to be transformed into a library with a computer lab and display cabinets for historical exhibits. This will aid the community in learning and give them the resources they may need for study, research and even job aide. This will benefit the community as a whole, whether school age or adult. The remaining 1200 sf would be transformed into a dance studio with mirrored walls for community dance groups. This is a current interest of the community and should be considered. This 1200 sf could also be used for music and the arts as a whole."

Partners

The City was approached on March 26th by a faith-based organization (Heart to Heart Christian Academy) that desires to lease some of the vacant classrooms. They understand that any and all vacant space in this building will require renovation and repair. They are considering putting together a proposal for Council consideration to partner with the City to place classrooms currently vacant into productive use in exchange for leased space in the Classroom building. The City exercised a similar arrangement with Head Start that would allow them to expand their leased space. As of today (April 16th) the Head Start plans were finally approved for this improvement. A building permit will be issued as soon as permit fees are remitted to the City.

The other tenant in our Classroom building is the Food Pantry which occupies 1,400 s.f. and continues to provide a critical service to our community. We have opened up a dialogue with the Food Pantry to determine their willingness to relocate to another area within the ASACC campus. This would then allow us to utilize the entire Classroom space for instructional training and uses. On the attached 2008 conceptual plan, an area adjacent to the Industrial Arts Building was designated for that purpose. This is a longer term strategy, but one that the Council should consider in collaboration with the Food Pantry. The City Manager held a meeting with the Food Pantry Director and she will be present at the April 21st Council session. She was most appreciative of our efforts to include her in our master site planning.

Moving forward, it should be noted that physical improvements are still needed at the ASACC Classroom, Industrial Arts and Library facilities. Once those improvements are done in total, then every building will be complete on the campus, save the Auditorium. This in itself is a significant accomplishment. Therefore, we will recommend during the upcoming budget development process that further discussion on these items occur to coincide with the next five­year Capital Improvement Program (CIP) planning horizon.

FISCAL IMPACT I FUNDING SOURCE: In the FY 2014/2015 Capital Improvement Plan (CIP) $97,551 has been earmarked from Infrastructure Surtax Proceeds and designated for work on the "Industrial Arts Building." In the CIP approved in September 2014 it was contemplated that those funds should be directed towards reroofing that building, since it is the only building on the site that does not have a new roof. We have attached an excerpt from the Five-Year CIP covering FY 2015 - FY 2019 with regards to the entire ASACC Campus.

However, given the physical condition of the building and comments from the neighborhood meeting, we now believe those dollars would be better re-directed to do these items in this order. We did obtain quotes on the Industrial Arts Building last fall. No quotations have been obtained for work on the Library building.

Key priority steps could include:

~ Demolish Industrial Arts Building - $36,000 ~Utilize and redirect this open space - TBD ~ Renovate Library - TBD

Redirecting our efforts to wisely allocate the $97 ,551 in the current year would be the most prudent course of action. It would also follow previous recommendations from the Administration that led us to "complete" one building at a time.

To recap, the five (5) buildings on the site and their current condition and use in 2015 follow below:

1. Gymnasium - complete and in FY 2015 now offering recreational programs with a set schedule

2. Industrial Arts Building - incomplete; not available for public use. 3. Classrooms - Two tenants (Head Start and Food Pantry). Balance of building is not up to

building code and cannot be placed into public use. 4. Auditorium - partially complete; interior is not up to building code and cannot be placed

into public use. 5. Library - incomplete; not available for public use.

As the City plans its future CIP, it must also be noted that there are inherent operational needs that will occur as buildings are put into productive use. These will be annual recurring costs. At present, we have two tenants: Food Pantry and Head Start. That relationship will continue as these key community partners provide essential services and lease payments.

Furthermore, grants must be explored as another viable funding source to support this capital project in conjunction with infrastructure surtax proceeds.

Designate the Industrial Arts Building footprint for public open space. 4. Obtain cost estimates for a Playground. 5. Renovate the Library building. 6. Obtain cost estimates for the Library. 7. Continue dialogue with potential partners for collaboration in Classroom facility.

CITY OF GREEN COVE SPRINGS, FLORIDA STAFF REPORT

SUBJECT: Review and Acceptance of 30% design documents for Historic Spring Park Pool Improvements.

SUMMARY:

In August 2014, Council was advised by staff that proposed improvements to Spring Park would involve a phased approach. The phases were outlined as shown below from an excerpt from a August 5, 2014 agenda item.

EXCERPT FROM AUGUST 5, 2014 COUNCIL AGENDA ITEM

The first phase involves the de-construction of the structure itsel:fl and any underlying residual structures. The reason for "de-construction" as opposed to demolition is to protect the roadway (Walnut St) as well as the structural integrity of the pool. The proposed task order to JAMSE Engineering is presented to accomplish the engineering and design necessary to accommodate the de-construction phase. As you can see in the task order, the City is providing some services in­house and some background documentation in order to both speed up the engineering process as well as minimize the cost of engineering. Two key components to the engineering portion of the project are a soil study and site survey of the immediate area. The City is requisitioning these items directly and work is already underway. John Miller with JAMSE Engineering will be available Tuesday evening to answer any questions.

The second phase will be to develop a plan or design a concept for what the finished product should look like and what amenities it should offer. As mentioned at the July 29, 2014 Council Meeting under City Manager announcements, a community meeting I visioning session tentatively scheduled for Sept 13, 2014 will be a critical component of this plan.

Once the plan or concept is fully developed, it will need to be engineered. Out of this process will come the plans and specifications needed to bid the construction phase. It will also provide a more detailed cost estimate of the construction phase.

The final phase will be construction. Once the funds are programmed and the plans and specifications are completed. The project can be bid. We will then be able to select a contractor to complete the construction of the community's vision for Spring Park.

At that time, Council authorized the design of the de-construction of the Old City Hall building. This authorization placed the first phase into motion and de-construction is approximately 50% complete.. Simultaneously, staff has been working with consultants to complete the phase II conceptual site plan. Using the original design concept and input from the community meeting that was held on September 13, 2014, CHW was tasked with the completion of 30% design drawings. These drawings have now been completed to the point where we are ready for a second

community meeting which is scheduled to be held on April 18, 2015. Based on the plans as they currently stand and community input from this Saturday's meeting, staff will then be ready Tuesday evening to receive acceptance of the plan from Council.

The current plans show the footprint of a new building, accessory pavilions, an interactive water feature, and improvements to the pool, spring and sidewalks. It is important to keep in mind that this site plan is at 30% and certainly anything is subject to change based on the desires of the community or Council.

As we finalize phase II and move into the next phase, funding relies completely upon the $700,000 commitment from the County. Another $700,000 is under consideration from the State Legislative Delegation. We should know in the next few months with regards to that allocation. Staff has already advised the County that we must have an executed agreement in hand within 60 days so that we can keep our project on schedule and complete all work by Summer 2016. The next consultant that will be brought into the team for the next phase will be an aquatic engineer. An RFQ (copy of scope attached) has been released as of April 16, 2015 with plans to bring a recommendation back to Council in late June. This aquatic engineer will be responsible for providing input and designing the requirements for the pool, equipment rooms, interactive water feature and the Spring flow. Again, funding for this engineer and all future expenses are fully dependent upon the funding from the County and our Legislative Delegation.

At this time, staff is asking that Council accept the 30% plans with any modifications that may result from the Community Meeting on April 18, 2015.

FISCAL IMPACT: Preliminary total project costs are currently estimated at $2, 100,000. The City expects to expend its $350,000 commitment in the FY 15 CIP budget by September 30. The County has committed $700,000 and we are seeking $700,000 from our State Legislative Delegation. Prior to releasing bids, the City must receive the BOCC pledge. We have been assured that this will occur within the next sixty ( 60) days. The current year CIP has allocated only $350,000 from Surtax.

RECOMMENDATION: Staff recommends acceptance of the 30% plans with any modifications that may result from the Community Meeting on April 18, 2015

MOTION: Accept the 30% plans as submitted [with any modifications that result from the April 18 Community Meeting], and authorize capital project to proceed to bidding phase subject to receipt of BOCC funding.

PROJECT DESCRIPTION:

The City of Green Cove Springs seeks proposals for an Aquatic Engineer for Historic Spring Park

Pool & Interactive Water Feature. Because of the unique site limitations the City recommends that all

perspective parties attend a site visit meeting scheduled for April 30, 2015, at 10:00 a.m. at 229

Walnut St, Green Cove Springs, FL 32043. This is not a mandatory meeting. This is a design project

and therefore must meet the requirements for such projects as specified by Florida State Statutes.

I. BACKGROUND:

The City of Green Cove Springs, Florida requires the professional services of a Florida licensed

engineer specializing in aquatics for assistance in the redesign process of our public swimming pool.

The existing swimming pool is approximately 90' x 40' and holds 135,000 gallons. It is a naturally

spring feed pool, a cultural icon. Therefore the City will require that the historic integrity of the pool

be maintained throughout the design process. The location of the Spring Park Pool and related

facilities is shown on the attached plan.

The City has started a major redesign of the area around the swimming pool and would like to

upgrade the Pool to meet some of the new criteria or codes for swimming pools. We have completed

a conceptual design; copies attached, and are in the process of completing 20-30% design drawings

during the month of April, 2015. The successful respondent will then work with other members of

the City design team that is comprised of City staff along with other disciplines, such as but not

limited to, structural engineers, civil engineers, mechanical engineers, and so forth.

Our intention will be 1st review by staff on May 13th, then we anticipate awarding a contract services

during June 2nd Council Meeting based on recommendations of the Selection Committee. We are

planning on completing all engineering and construction bidding in June 2015, construction work

completed by 5/1/2016.

II. PROJECT SCOPE ELEMENTS

III: QUALIFICATION OF SUBMITTALS:

1. Licensed engineer with a minimum of 10 years' experienc,e in Pool or Aquatic designs and features.

2. Preferred local professional within 100 miles of City limits.

3. A list of at least three (3) completed projects of similar size and nature completed within the last five (5) years must be included for reference checks. Photos of the structures are recommended but not required.

IV. INQUIRIES AND INTERPRETATIONS:

Responses to inquiries which directly affect an interpretation or change to this RFQ will be issued in

writing by addendum and mailed or faxed to all paiiies recorded by the City as having received a copy

of the RFQ. All information regarding this RFQ shall be placed on the City's website at

interpretation or changes to this RFQ must be received by the City's contact person listed. All such

addenda issued by the City prior to the last date that submittals are required to be received shall be

considered part of the RFQ, and the Respondent shall be required to consider and acknowledge receipt

of such in its response. Firms receiving this RFQ other than directly from the City are responsible for

notifying the Finance Department that they are in receipt of a submittal package and are to provide a

name and address in the event an amendment is issued. It is the obligation of the Respondent to make

sure that it has received all addenda prior to submission of its response. Respondents may obtain

information on all addenda issued to the date of inquiry from the City's contact person or at

Only those responses to inquiries, which are made by formal written addenda, shall be binding. Oral

and other interpretations or clarifications will be without legal effect, and shall not be binding on the

City. The Respondent must acknowledge receipt of all addenda in its response.

V. SELECTION PROCESS:

The selection process will be conducted in accordance with Florida Statutes, Chapter 287. A Selection Committee will evaluate and rank the proposals based on Criteria outlined within the RFQ.

Review of Written Proposals

~ Using the criteria given in items A through G described in the "Proposal Evaluation Criteria" section of this RFQ and with emphasis on professional competence, the City of

Green Cove Springs selection committee shall review all proposals received to determine those individuals, who are fully qualified, responsible and suitable to provide the professional services set forth by this Request for Qualifications.

~ Discussion and Interviews

The City of Green Cove Springs Selection Committee may then hold an interview with one or more of the firms who have been deemed by the committee to be fully qualified, responsible and suitable to provide the services set forth by this Request for Qualifications.

~ Evaluation

Using all of the information developed during the proposal review and interview stages, the City of Green Cove Springs shall then select in order of preference whose professional qualifications are deemed most meritorious. The City Manager will be the tie breaker if such a tie develops in the reviewing stage. At that time City will then present to Council.

PROPOSAL EVALUATION CRITERIA

The City of Green Cove Springs Evaluation criteria shall include the following factors which shall be

considered in the evaluation and ranking process.

A. Past performance [25 points] Past record of professional accomplislunent. A summary of the firrn' s experience in providing the services required in the request for qualifications. Provide a minimum of three (3) references relating to work you have done for clients with full name, title, address, phone and fax numbers.

B. Recent, current, and projected workloads of the Firm [5 points] Staff availability and capability to meet deadlines. Include a statement of interest in providing the services, including a description of the unique abilities of the firm.

C. The ability of the professional personnel [25 points] General description of the firm, including size and number of employees. Provide qualifications of the firm's project manager and project teams and overall qualifications and experience of the firm.

D. Volume of work previously awarded [10 points] Volume of work previously awarded to each firm by the City, with the object of effecting an equitable distribution of contracts among qualified firms, provided such distribution does not violate the principle of selection of the most qualified finns. Firms which have not worked with the City of Green Cove Springs before will be awarded more points.

E. Willingness to meet time and budget requirements [15 points] Financial responsibility and insurance. The form of business of the prime consultant, i.e., proprietorship, partnership, corporation; years in business; changesin ownership; bank reference; and any other infonnation the applicant may wish to supply to verify financial responsibility. The

amount of professional liability insurance currently in force and the amount of any claims against firms insurance.

F. Location [15 points] The location of the firm's office that will have the responsibility for providing the services within the 100 mile radius and the ability of the firm to respond within two hours for emergencies to requests and requirements of the City.

G. Certified Minority Business Enterprise [5 points] Ce1iified minority business enterprise as defined by the Florida Small and Minority Business Assistance Act. Ce1iification must accompany proposal.

The City of Green Cove Springs reserves the right to negotiate and contract with any engineering firm or individual suited to provide the services required for this project.

Interpretation of the Land Development Regulations

Interpretation # - 2015-04 Subject - Banners

Findings of Fact 1. Section 110-1.1. - Definitions of the Land Development Code defines Banners Signs as:

"Any sign composed of lightweight material enclosed or not enclosed in a rigid frame, . secured or mounted so as to allow movement of the sign caused by movement of the atmosphere, possessing characters, letters, Wustrations, or ornamentations applied to paper, flexible plastic, or fabric of any kind. National flags and flags of political subdivisions shall not be considered banners for the purpose of this code." The banner definition would include "wind signs."

2. Section 110-1 .1. - Definitions of the Land Development Code defines Ground Signs, Building Signs, and Monument Signs as: "Building sign: A sign displayed upon or attached to any part of the exterior of a building, including walls, windows, doors, parapets, marquees, and roof slopes. Ground sign: A sign that is movable or permanently erected on a free-standing frame, mast, or pole and not attached to any building with a surface area of less than 100 square feet per side, exclusive of base, but including ornamentation. Monument sign: Any free-standing, essentially solid monument structure containing a sign face which is supported solely by its own ground-mounted base and which is not attached or affixed in any way to a building, fence, or other structure."

3. Section 110-8 - Permitted permanent signs (attached) - addresses the requirements for both building and ground signs.

4. Section 102-581 contains the special signage requirements of properties in the Gateway Corridor "Signage along and within the gateway corridor shall meet the requirements of chapter 110 of this Code as amended with the exception that all ground signs be monument signs and that no more than one ground sign shall be permitted for each 200 feet of road, with ·a maximum of two ground signs for projects with over 400 feet of right-of-way."

5. The City Council passed Resolution No. R-32-2014 on November 18, 2014 to allow banners for the first 90 days of a business.

6. Resolution No. R-32-2014 defined the size of a banner as: "The banner size shall not exceed twenty (24) square feet, a wind sign shall not exceed 30" x 96" and shall have a maximum above ground height of nine (9) feet and sandwich board signs shall not be larger than twelve (12) square feet and may contain two (2) sign fronts."

7. Other municipalities in Northeast Florida have allowed banners and wind sign less than 24 square feet, if permitted, to be counted toward the building or ground sign calculations.

Conclusion 1. The banner definition -included Section 110-1.1 would include "wind signs" and would be

considered a "monument sign." 2. Banners under 24 square feet in size are permitted and are considered signs. 3. Banners under 24 square feet in size must meet the sign requirements of Section 110-8 and

Section 102-581 of the Land Development Code and will be counted with the overall permitted sign calculations for a business/property.

4. Banners must be permitted as a sign according to Chapter 14 of the City Code.

A RESOLUTION OF THE CITY OF G:Q.EEN COVE SPRINGS, FLORIDA, ESTABLISHING A "WELCOME NEW BUSINESSES TO THE CITY CELEBRATION" BY ALLOWING NEW BUSINESSES SPECIAL ADVERTISING OPTIONS DURING THEIR FIRST 90 DAYS AFfER OP~NING, AND SE'ITING AN EFFECTIVE DATE.

WHEREAS, City Code Section 110-5 (4) and other provisions of Chapter no allows the City Council to authorize special flags and banners during celebrations; and

· WHEREAS, the City recognizes the first go days of a new business is critical to its success and hereby establishes· a "Welcome New Businesses to the City Celebration" during such time.

NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF GREEN COvE SPRINGS, FLORIDA THAT:

Section 1. The City does hereby temporarily authorizes new businesses during the first go days after receiving a Local Business Tax Receipt to erect and maintain certain flags, banners, sandwich board signs and wind signs and the like (hereinafter called "banners", "sandwich board signs" and "wind signs") provided they comply with the following conditions: ·

a. The banner size· shall not exceed twenty (24) square feet, a wind sign shall not exceed 30" x g6" and shall have a maximum above ground height of nine (g) feet and sandwich board signs shall not be larger than twelve (12) square feet and may co~tain two (2) sign fronts. ·

b. The banner, sandwich board sign or wind sign shall not be located so as to be a hazard to pedestrian or automotive traffic.

c. Businesses shall have the following options to choose per street front:

(1) Banner and sandwich board sign. (2) Banner and wind sign.

d. The banner, sandwich board signs and wind signs must be safely secured so as to be safe in the event of high winds. In the event of an approaching tropical storm or hurricane, the banner, sandwich board signs and wind signs shall be removed for public safety.

e. Banners and wind signs shall not be located on any public rights-of-way and if found there shall constitute a Code Violation, subject to immediate removal and shall be subject to assessment of Code Enforcement Fine.

f. Banners, sandwich boards and wind signs may only promote the business upon the land where the business is located.

g. In the sole but reasonable discretion of the City Development Services Department, such banners, sandwich boards and wind signs must be located, erected, and maintained in a safe location and condition.

h. Sandwich board signs and '\i\rind signs are allowed to be displayed during normal operating hours for the business establishment for the first 90 days of operation and shall be removed after the 90 day period and a·t the close of business each day and on weekends, if business is not open for business.

Section 3. The City Council of Green Cove Springs reserves the right to suspend or terminate the celebration in its sole judgment.

Section 4. Effective Date. This Resolution shall take effect immediately upon passage and remain in effect unless suspended, terminated or extended by the City Council.

Sec. 110-8. - Permitted permanent signs.

(a) Sign types allowed. A permanent sign may be a ground or building sign. A permanent sign may not be a roof sign .

(b) Content. A permanent sign may display any message so long as it is not harmful to minors as defined by F.S. § 847.001 (6).

a. Subject to the design criteria in this chapter, the maximum height of a building sign shall be 18 feet, except that on a building of more than two stories, a single building sign is allowed above 18 feet on each side of the building.

b. Each multiple occupancy complex may display one permanent accessory building sign on each side of the principal building or buildings in which the complex is located, not to exceed a sign area of up to ten percent of the facade area of each building side or 200 square feet, whichever is smaller.

c. Each occupant of a multiple occupancy complex may display three permanent accessory building signs on any exterior portion of the complex that is part of the occupant's unit (not including a common or jointly owned area). The total combined sign area shall not exceed 15 percent of the facade area or 200 square feet, whichever is smaller.

d. Each occupant not located in a multiple occupancy complex may display three permanent accessory building signs on each side of the principal building in which the occupancy is located, not to exceed a total combined sign area for each building side of 20 percent of the facade of the building side or 200 square feet, whichever is smaller.

e. Building address numbers for residential structures shall be at least four inches tall, be a contrasting color of building and visible from the street and commercial structures shall be at least six inches tall, and be visible from the street for emergency services identification.

(3) Multiple frontages. If a building has frontage on two or more streets, each frontage shall be separately considered for the purposes of determining compliance with the provisions of these regulations, but the permitted sign area for one frontage may not be combined with that permitted on another frontage to increase the permitted sign area on one frontage.

( d) Signs at entrances to residential developments.

(1) Generally. A permanent sign may be displayed at the entrance to residential developments.

(2) Restrictions. One sign is permitted at each entrance into the development from each abutting street. The sign may be a single sign with two faces of equal size or may be two single-faced structures of equal size located on each side of the entrance. No face of the sign shall exceed 32 square feet in size, and may be illuminated in a steady light only.

(e) Off-site signs.

PLANNING

A Future Land Use Map (FLUM) Amendment and an accompanying text amendment to the Comprehensive Plan submitted by Magnolia Cove Development, LLC was reviewed by the Planning and Zoning Board. The text amendment is to reduce the minimum acreage required in the Residential Riverfront - RRF FLUM category from 10 acres to 7 acres. The FLUM amendment is to change the existing designation on the property from Residential High Density - RHD, allowing up to 12 dwelling units per acre to Residential Riverfront - RRF, allowing up to 20 dwelling units per acre.

The FLUM amendment and rezoning for the 140 acres, known as the Green Cove Spring Nature Preserve Phase III/Ed Gustafson Regional Park, located west of Oakridge Avenue/C.R. 15A and south of Green Cove Avenue was recommended for approval by the Planning and Zoning Board. The FLUM amendment will be required to be reviewed by the Department of Economic Opportunity prior to adoption.

I listened to a webinar presented by Smart Growth America that was titled "Safer Streets, Stronger Economies". The webinar presented new research from the National Complete Streets Coalition on the outcomes of Complete Streets projects across the country. It shows that when communities invest in their streetscapes, businesses thrive.

A Business License was issued for Mack Daddy Smokers to be located at 965 Worthingtpn Avenue in Reynolds Park. The business relocated from Jacksonville and will manufacture smokers.

Interviews were conducted with potential candidates for the new Clerk-Typist position in Development Services. It is anticipated that the new employee could begin in April.

Revenues for planning related fees for March were $3,843.

BUILDING

Building permit activity continues to increase. The total number of building permits issued in March increased from 35 permits issued in February to 42 permits in March, and the revenues for the month also increased. Building permits were issued for 13 new houses; 3 in Magnolia West and 10 in Magnolia Point. A Certificate of Occupancy was issued for 9 dwelling units; 8 in Magnolia West, and 1 in Magnolia Point. For the month of March, Paul Felker completed 306 inspections of various types.

Revenues for building related fees for March were $35,261.

CODE ENFORCEMENT

Marcy James, the new part-time Code Enforcement Officer, started her position in March. In March, 5 new Code Enforcement violations were filed.

• Installed 400 amp service for light at runway on 1027 Bunker Ave. • Investigated 301 and 305 N. Palmetto Ave. for possible power theft or tampering. None

could be found. • The Electric Department welcomed new employee, Melvin Mercado, our Apprentice

Lineman II. This was a good start to hiring people for the Department but we are still short experienced Linemen.

During the month of March, the Electric Division responded to the following outages:

03/19/15 - Between 5:00 p.m. - 6:15 p.m., reworked the top of the riser pole at Gustafson's Dairy after being hit by lightning.

03/20/15 - Between 11 :55 p.m. - 1 :45 a.m., disconnected service and replaced blown transformer fuse at 204 Clay St. after a tree tore the meter can from the house. One customer was affected. Re­attached riser and meter can damaged by fallen tree on 24 March 2015.

03/20/15 - Between 5:55 p.m. - 7:15 p.m., removed tree off lines and repaired transformer at 305 Sunset Ave. to restore power.

03/23/15 - Between 10:45 a.m. - 11 :30 a.m., removed tree from line at St. Johns Landing, behind the primary metering. Replaced lateral line fuse and re-energized the transformer. About 75 customers were affected.

03/30/15 - Between 2:30 a.m. - 3:30 a.m., replaced blown fuses and checked the voltage for the traffic light at U.S. 17 N. and C.R. 315. Squirrel caused the problem that affected 3 customers.

EXECUTIVE SUMMARY GREEN COVE SPRINGS POLICE DEPARTMENT

CHIEF ROBERT A. MUSCO

During the month of March 2015, Officers of the Green Cove Springs Police Department handled 428 citizen's calls for police service. Overall, Officers responded to and handled 3,608 police related service or enforcement calls, including traffic and self-initiated activity. As a result of these activities, there were 17 arrests affected, 136 Uniform Traffic Citations, and 68 Traffic Warnings issued. Additionally, Officers responded to 242 back-up calls, conducted 506 building security checks, and provided 1,646 extra patrols throughout the City. Response times for calls for service were as follows: Priority 1 = 02:59, Priority z = 01:07, Priority J = 03:17. Additionally, the Communications Section received and answered 291 telephone calls to our 911 emergency lines and 1,819 telephone calls to our standard telephone lines.

Officers continue to proactively address "Quality of Life" issues throughout the City by providing quality police services for our community. Over the past month, your police officers addressed the following issues in their continued efforts to provide security and safety for our community:

Sergeant Buchanan:· Traffic Selective: Performed traffic selective 4 times at 4 locations; Martin Luther King, Jr. Blvd. at Oakridge Avenue, Oakridge Avenue at Wayne Street, Houston Street at Leonard Street and South Roberts Street at Roberts Circle; resulting in 3 Uniform Traffic Citations and 4 Traffic Warnings being issued. Pop Project: (1) In response to speeding complaints on Medinah Lane in the morning hours, my shift has been conducting traffic assignments on Medinah Lane and on Colonial Drive. Numerous citations and warnings have been given for violations observed. Problem Solving: (1) Complaints have been received regarding construction vehicles staging in Magnolia Park before 0700 hours waiting to enter Magnolia Point. Officers have been assigned to prevent the vehicles from parking in and damaging the very small community park. I made contact with a driver in the park who advised he was being instructed to park there by the guards at Magnolia Point. I met with the guards and informed them they cannot authorize construction vehicles to stage in the Magnolia Park. Since then, we have had fewer problems. Community Involvement: (1) I attended the Swearing In Ceremony for Officer Braswell at City Hall. (2) I worked the Railroad Festival. (3) I attended the Easter Egg Hunt at the Thomas Hogan Gymnasium. Other Significant Accomplishments: (1) I completed an online recertification course for my Taser Instructor Certification. (2) I attended training to be recertified as a Taser Instructor. (3) I have Trainee Officer Braswell in training for Phase 3 of the Field Training Program.

Officer Robinson: Traffic Selective: Performed traffic selective 7 times at 5 locations: Walnut Street at West Street, Walnut Street at Oakridge Avenue, Palmer Street at Orange Avenue, 1401 Harbor Road and the 700 Block of Houston Street; resulting in 12 Uniform Traffic Citations and 7 Traffic Warnings being issued.

Pop Project: I spoke to several Charles E. Bennett Elementary School students about Bike Safety. I asked the bikers that did not have helmets if they chose not to wear them or if they did not have them at all. More than half of them stated they did not have a helmet. I will do a Bike Safety Program during the month of May due to the fact it is National Bike Safety Month. Problem Solving: Extra patrols during early morning hours at Magnolia Park due to concerns of construction vehicles parking there prior to entering Magnolia Point. Community Involvement: (1) On 03/09/15, I conducted a monthly Camp Cadet board meeting at the Orange Park Police Department. (2) On 03/10/15, I conducted a quarterly Neighborhood Watch Meeting at the Green Cove Springs Police Department. (3) On 03/12/15, Detective Luedtke and I spoke to the 5th grade class at Charles E. Bennett Elementary School about bullying. (4) On 03/20/15, I attended the Clay Action Coalition Meeting held at the Fleming Island Golf and Country Club. Other Significant Accomplishments: (1) On 03/09/15, I attended a Media Relations class held by Commander Mulligan at the St. Johns County Sheriff's Office. (2) I attended a Bike Safety Certification Class with Officer Braswell in Gainesville, FL. (3) I received the Citizenship of the Year Award at the Elk's Lodge. (4) I conducted a Felony Traffic Stop In-Service Training on 03/27 /15 at the Green Cove Springs Police Department.

Officer Lazo: Traffic Selective: No specific location listed; resulting in 11 Uniform Traffic Citations and 12 Traffic Warnings being issued. Pop Project: (1) During the month of March, I assisted in ensuring construction vehicles were not illegally parking· at Magnolia Park before gaining access into Magnolia Point Golf and Country Club. (2) I conducted traffic assignments at Magnolia Point Golf and Country Club due to several reported dangerous speeding and reckless driving. Problem Solving: (1) I continued to focus on property checks on the security watch list as well as businesses to ensure police presence and security in the community. (2) I conducted traffic assignments at Charles E. Bennett Elementary and Green Cove Springs Junior High school zones to ensure student safety and speed enforcement. Community Involvement: (1) I accepted the task and honor of being the Incident Commander for the 2015 Memorial Day RiverFest. (2) I participated in the Easter egg hunt held at Thomas Hogan Gym. Other Significant: I enrolled into a Biology class at Florida State College at Jacksonville to further my college education.

Sergeant Hess: Traffic Selective: Performed traffic selective 5 times at 2 different locations: Orange Avenue at Oak Street and 800 Block of Leonard C. Taylor Parkway; resulting in 1 Uniform Traffic Citation and 15 Traffic Warnings being issued. Pop Project: (1) While on routine patrol, I observed the light above the bathrooms in Vera Francis Hall Park was turning on and off. I had dispatch send a problem report to the Electric Department to have the problem corrected. (2) I made a custom photo backdrop in the booking room at the Police Department. Problem Solving: (1) I received information from a Sergeant at the Clay County Sheriff's Office in reference to several burglaries occurring just outside the city limits. The stores were being burglarized in the early morning hours by breaking the front door glass. The thieves were filling garbage cans with cartons of cigarettes and dragging them behind the stores where their car was waiting. I advised my squad of the information and extra patrols were performed

at closed business locations. (2) I repaired and programmed the reader board sign in preparation for the Railroad and History Festival. Community Involvement: (1) I attended the Swearing In Ceremony for Officer Braswell at City Hall. (2) Sergeant Perry and I assisted a stranded motorist in Magnolia Layne Shopping Center. His starter broke and he could not get the vehicle started. I showed him how to "pop start" the vehicle and we got it started for him so he could get home. Other Significant Accomplishments: (1) I attended training held by Officer Robinson covering felony stops and related tactics. (2) I trained Officer Braswell in her second phase of training in the FTO program and she successfully completed the Phase and has been advanced to Phase Three with Sergeant Buchanan.

Officer Checo: Traffic Selective: Performed traffic selective 9 times at 6 different locations: 923 Leonard C. Taylor Parkway, South Orange Avenue at Leonard C. Taylor Parkway, Idlewild Avenue at Melrose Avenue, Thomas Street at Oakridge Avenue, the 700 block of Houston Street and Governor Street at North Orange Avenue; resulting in 5 Uniform Traffic Citations and 14 Traffic Warnings being issued. Pop Project: Generators were run and checked for serviceability. Problem Solving: Extra patrols were conducted in the Magnolia West subdivision due to the recent construction of new houses. The patrols were in an attempt to deter any possible burglaries, theft, or vandalism. Community Involvement: (1) I attended the Swearing In Ceremony for Officer Braswell at City Hall. (2) I participated in the City of Green Cove Springs Easter Egg Hunt. (3) I attended Officer Robinson's award ceremony at the Elk's Lodge. Other Significant Accomplishments: (1) I attended training held by Officer Robinson covering felony stops and related tactics. (2) I completed Taser instructor re-certification class held at Daytona Florida at the Volusia County Sheriff's Office. (3) I completed the instructor certification on the new Phazzer conductive electronic weapon.

Officer Padgett: Traffic Selective: No specific location listed; resulting in 13 Uniform Traffic Citations and 24 Traffic Warnings being issued. Pop Project: I continued preparing a presentation for Commercial Motor Vehicle training for the Police Department. Problem Solving: (1) I enhanced my efforts in traffic enforcement in attempts to deter reckless and erratic drivers by being more visible on the roadways. (2) I continued my efforts in proactive patrols resulting in multiple arrests involving drugs and active warrants. Community Involvement: (1) I attended the Swearing In Ceremony for Officer Braswell at City Hall. (2) In the month of March, I met with the property manager of Reynolds Park, John Brown. During my conversation with Brown, I advised him a map and more information for Reynold's Park would benefit not only the Police Department in handling calls, but also public safety when responding to medical emergencies. Mr. Brown stated there has been multiple route changes in the complex that have hindered pubiic safety from · locating and responding to medical emergencies because of the changes. Mr. Brown advised me he is going to develop a map of Reynolds Park and forward it to the Police Department. (3) In the month of March, I supervised a community service worker completing community service hours at the Police Department.

Sergeant Perrv: Traffic Selective: .Performed traffic selective 8 times at 7 locations: 1100 Block of North Orange Avenue, 923 Leonard C. Taylor Parkway, 1300 block of Idlewild Avenue, 429 North Orange Avenue, 800 Block Leonard C. Taylor Parkway, and 410 South Orange Avenue; resulting in 5 Uniform Traffic Citations and 18 Traffic Warnings being issued. Pop Project: (1) Street light out on Houston Avenue and Calico Jack Way - north side of road (no number listed on it). (2) Street light out on Houston Avenue beside R.C. Bannerman (#9 on pole). (3) No parking sign on Spring Street just west of parking lot has sticker covering up bottom portion of sign. (#1-3 sent to Public Works). Problem Solving: (1) Complaints in the 200 Block of Walburg Avenue continue and we have been conducting area checks and I provided photos to the Lieutenant. (2) Complaints of heavy traffic and possible drug activity in the 900 block of St. Johns Avenue, extra patrols are being conducted. (3) Complaints of heavy traffic and possible drug activity in the 700 block of South Highland Avenue, extra patrols are being conducted. Community Involvement: (1) I spent some time at the Thomas Hogan Gym with Officer Jett and spoke with the youth in the community. (2) Officer Robinson and I are making changes to the Neighborhood Watch Program and will getting different Officers involved with this program. Other Significant Accomplishments: (1) Field Training Officer for Officer Jett who successfully completed Phase II and moved forward to Phase III. (2) I assisted Officer Jett on training involving taking sworn statements from subjects that cannot read or write and dealing with juvenile statements. (3) I reviewed two policies in DMS.

Officer Kennison: Traffic Selective: Performed traffic selective 3 times at 3 locations: 1 South Oakridge Avenue, 1300 Block of Idlewild Avenue and the 600 Block of North Orange Avenue; resulting in 6 Traffic Warnings being issued. Pop Project: Recently reassigned to cover patrol shift due to shortage of manpower. Problem Solving: Working on correcting driver issues with the Egis Tech biometric reader to on the Getac laptops to comply with CJIS security policy and the second authentication guidelines. Community Involvement: (1) I assisted Magnolia Point bible study group throughout the month by setting up the EOC/Meeting room for audio/video presentation. SIGNIFICANT OFFICER ACTIVITY D.E.A. TASK FORCE

Officer Graham: I have opened up 2 new cases on Doctors in Jacksonville who are prescribing pain medication without a valid medical reason. (2) We have purchased numerous ounces of cocaine from 5 different distributers in Jacksonville and are awaiting arrest warrants. (3) I have identified 2 new illegal steroid distributers located in Clay County and I am conducting surveillances in an attempt to identify their source of supply. (4) I arrested a steroid distributer located on the southside of Jacksonville. (5) During a joint investigation with the IRS, a search warrant was conducted in Clay County where 27 lb. (valued around $450,000) of gold and a Cadillac Escalade were seized.

SIGNIFICANT OFFICER ACTIVITY CLAY COUNTY DRUG TASK FORCE

Officer Guzman: I had or assisted fellow Detectives with the following activity inside of the City limits: (1) I conducted 2 controlled purchases of Crack Cocaine (total 0.6 grams), at 2 separate locations in the City's westside. (2) I am conducting an on-going narcotics investigation on North Cypress Avenue. (3) I conducted a "Knock and Talk" investigation on St. Johns Avenue in reference to a citizen complaint I received about the home. (4) On 3 separate occasions, I met with a local apartment manager in reference to suspected drug activity on the property. (5) I conducted a controlled purchase of Crack Cocaine (0.2 grams) in the westside of the City. (6) I conducted 2 controlled purchases of Crack Cocaine (total 0.3 grams) at 2 separate locations in the City. (7) I arrested a male at the intersection of Sunset Drive and Idlewild Avenue in reference to Possession of Drug Paraphernalia. The arrest stemmed from a drug complaint I received about the male earlier in the month. I had or assisted fellow Detectives with the following activity outside of the City limits: (1) I assisted fellow Detectives with meth-lab investigations in Middleburg and Fleming Island. (2) I conducted a controlled purchase of Marijuana (1.89 grams) in the Orange Park area. (3) I assisted fellow Narcotics Detectives with controlled purchases in different areas of the county. (4) I assisted my partner with surveillance on a home in Lake Asbury. (5) I assisted a fellow Detective with marijuana related search warrant in Oak Leaf. I also did the following things this month: (1) The Strategic Response Division and I assisted the Jacksonville Field Office of the Federal Bureau of Investigations in an on-going murder investigation. (2) I disseminated my monthly Narcotics Intelligence Report to the Green Cove Springs Police Officers and Supervisors. (3) I met with D.E.A. Task Force Officer Graham in reference to drug suspects inside of Jacksonville related to Green Cove Springs.

SIGNIFICANT DETECTIVE ACTIVITY

Detective Acres: Significant Cases and Arrests: (1) Case# 2015000169 (death investigation), final Medical Examiner report pending. (2) Case# 2015000531 Criminal Mischief, Charles E. Bennett Elementary School. Minor damage to a gate, no suspect information at this time. (3) Case# 2015000659 Fraud (credit card). Investigation continues. Significant Accomplishments: (1) Officer Robinson and I appeared on "The Morning Show" on Channel 4 to help promote this year's upcoming Camp Cadet. (2) I assisted Officer Santiago with a Theft Case by placing holds on some pawn transactions at 2 pawn shops in

Sergeant Whiddon: Traffic Selective: Performed traffic selective 4 times at 3 locations: Palmetto Avenue and Oak Street, Orange Avenue and Park Street, and Idlewild Avenue and West Street; resulting in 9 Uniform Traffic Citations and 1 Traffic Warnings being issued. Pop Project: I worked the area of South Palmetto Avenue at Cove Street in reference to reports of speeders in the area. Problem Solving: A Clay County Sheriff's Office Deputy had a vehicle flee from him, which entered the City. The deputy estimated the speed in excess of 100 MPH when the vehicle turned into a residential area and we were asked to assist in locating the vehicle. The deputy was able to provide a Georgia tag number, which came back to a car lot in Waycross. I contacted the Waycross Police Department in reference to the vehicle. They contacted the owner of the car lot and learned the vehicle was rented by a female who alleged the vehicle was stolen. I was able to identify a possible suspect who may have been delivering illicit drugs to the area. I subsequently located the vehicle at an abandoned house with the keys in the ignition and the doors unlocked. The vehicle was processed for evidence and subsequently towed to be returned to the owner. Community Involvement: (1) I continue to interact with the children at Thomas Hogan Gym on a daily basis. (2) I attended the award ceremony for Officer Robinson held at the Elks Lodge. Other Significant Accomplishments: (1) I completed the monthly work schedule. (2) I completed vehicle and weapons inspections for the squad.

Part time Officer O'Daniel: Traffic Selective: Performed traffic selective 9 times at 6 locations: Idlewild Avenue at South West Street, North Orange Avenue at Governor Street, Oakridge Avenue at Thomas Street, Idlewild Avenue at Oakridge Avenue, Oak Street at Palmetto Avenue, and South Orange Avenue at Oak Street; resulting in 56 Uniform Traffic Citations and 5 Traffic Warnings being issued. Pop Project: I continue to work traffic complaints in several areas of the City, answering questions from residents. Community Involvement: I attended the award ceremony for Officer Robinson held at the Elks Lodge. Other Significant: (1) I took Car 139 to the city garage for service. The police radio was not working and the driver's door actuator was replaced. (2) I took Car 136 to the city garage for annual service.

Part time Officer Faro: Traffic Selective: (1) I reviewed 805 Red Light violations. (2) I attended Red Light Traffic Court. (3) I attended Red Light Traffic Magistrate Hearing. Problem Solving: I managed scheduling for the Part Time Officers. (2) I answered many questions and assisted citizens on Red Light violations. (3) Worked on implementing new ATS UTC procedures. Other Significant Accomplishments: (1) I prepared the evidence package for County Traffic Court. (2) I prepared the evidence package for City Magistrate Traffic Court.

Detective Luedtke: Significant Cases and Arrests: (1) Case# 2015000598 - Officer Padgett conducted the complete investigation. He was able to obtain a confession from the suspect. The suspect returned the following day and was arrested. (2) Case# 2013001687 - I learned that the suspect was incarcerated and is due to be released in 2024. I contacted the victim and advised of the new information and completed a supplemental report. (3) Case# 2015000458 - I received video from inside the bus. I presented the investigation to the SAO and am waiting on a response. ( 4) Case# 201500349 - I contacted the Internal Revenue Service and provided with information. The investigation was transferred to IRS Fraud Division. (5) Case# 2014000036 - The victim in this investigation is currently incarcerated and is due to be released at the end of this month. I spoke with the victim's father in regards to the allegations and completed a supplemental report. (6) Case# 2015000612 - I am investigating a theft of a bank deposit. I am in the process of obtaining a subpoena to obtain video. Significant Accomplishments: (1) Detective Acres and I have also been covering a patrol shift including working weekends and due to manpower issues, while continuing to do my assigned duties as an investigator. (2) Officer Robinson and I spoke with Charles E Bennett Elementary Students about bullying and had them sign a pledge to stop bullying . (3) I completed the monthly Crime Prevention Tip. (4) I attended an award that Officer Robinson received for Civilian of the Year given by the Elks Lodge. (5) I obtained my Bachelor's Degree in Criminal Justice through Columbia College.

SIGNIFICANT EVIDENCE CUSTODIAN ACTIVITY

Part-time Evidence Custodian Cunningham: (1) I processed 21 cases with items requiring storage in the Green Cove Springs Police Department property/evidence room. (2) I processed 9 items and forwarded them to the Florida Department of Law Enforcement Crime Lab for analysis. (3) 42 items were returned by the Florida Department of Law Enforcement Crime Lab to be re-entered into the Green Cove Springs Police Department property room. (4) I assisted the State Attorney's Office with information and/or property for 7 separate cases.

Street Department

Public Works Monthly Executive Summary

March 2015

During the month of March, The Street Department has been busy repairing and replacing sidewalks throughout the City. Additional activities included:

• Preparation for the Railroad Festival. • Started the Walnut Alley improvements.

• Finished the Cypress Street storm drain improvements.

They completed 15 additional work orders that pertained to street issues.

Parks Department

The Parks Department did their daily mowing and landscaping trying to keep the city looking great. They

mowed, weed-eated, and edged all areas one time including the DOT right-of-ways, City Parks, and FCT

property owned by the City. Additional activities included for the month of March:

• Magnolia Point Park, trimmed trees back.

• Before and after preparation for Railroad Festival.

• Cleaned the Pool once.

• Before and after preparation for Sunrise service in Spring Parl<.

• Employee (Jason Lima) turned in lost wallet to the Police Department.

• Monthly playground equipment inspection and made necessary repairs.

They completed 10 additional work orders outside of their normal daily work schedules.

Equipment Maintenance

The Equipment Maintenance Shop, as always, does its best to keep the equipment and vehicles going so the City can continue to provide the citizens with the exceptional service they are accustomed to. During the month of March, David and Danny completed 94 work orders.

Tradesworker

During the month of March, Mike Kelley completed 25 work orders.

Solid Waste Department

On Saturday March 21, 2015 the City of Green Cove Springs participated in the St. Johns River Celebration.

Mike Worley and Mike Williams worked the event this year. The City of Green Cove Springs has been

participating in this event for about eighteen years. Volunteers came from the City of Green Cove Springs

and Clay County. This year the volunteers included Mike Null and his two daughters. They collected trash

from the banks of the St. Johns River and Governors Creek which included Spring Park, Vera Francis Hall Park

and the Mount Olive Cemetery. They placed small items in plastic bags and large items in piles. They had

the option of taking their trash to the Clay County disposal site at Knights Boat Ramp or placing it in piles for

the city crew to pick up. The city provided trash drop off locations at Spring Park and Vera Francis Park.

They also picked up a large pile collected at the Cemetery. The city crew picked up the piles and large items

and took them to Knights Boat Ramp for disposal. An old large piling had been stuck next to the Spring Park

pier in about five feet of water. The Clay County Sheriff's Department tied a rope to it and broke it free.

Mike Williams and Mike Worley pulled it to shore. With the assistance of Mike Null it was dragged on the

bank and cut up for disposal. It was a rewarding and fun event for all that participated. Over the years this

program has been a great success. The St. Johns River and Governors Creek are much cleaner than they

were in the past. The first few years that the city participated the amount of trash collected was

tremendous. The claw truck was utilized and picked up about two 20 yard truckloads of trash and one 20

yard truckload of tires. This year the city collected one pickup truck load of trash and a few tires. Clay

County reported that a total of 1.05 tons were collected on this day and brought to their disposal site at

Knights Boat Ramp.

Cove Life, Litter Cleanup Program:

• Pick up everything around the rollout cans including large piles of furniture and trash.

• Pick up as much loose trash, palm fronds, and sticks as possible on the streets, along the R.0.W. and

ditches regardless of if it is near a container or not.

This month the city collected:

• 242.86 of class I garbage

• 9.89 tons of recycling

• 192.41 tons of yard waste

\Nater/\Naste\Nater

• W/WW crews/management had personal interaction with many citizens over water/sewer issues.

• Completed Wastewater Master Plan

• Conducting spring hydrant flushing program.

• Continued remediation of malfunctioning or out of service hydrants.

• Routinely inspected water, sewer and reclaimed water construction in Magnolia Point Phase VIII.

• Line Crew continued major lift station maintenance and cleaning schedule for 32 lift stations.

• Operated and maintained the Harbor Rd. and South Wastewater Treatment Plants as well as the Harbor Rd. and Reynolds Water Treatment Plants to meet the requirements of Regulatory Agencies.

• Continued to make process control adjustments at the Harbor Rd. and South WWTP in efforts to further lower Total Nitrogen and Total Phosphorus levels being discharged to the St. Johns River.

• Continued grass cutting and weed eating at Water and Wastewater Treatment Facilities as well as lift stations.

• Completed 10 new services.

• Completed 32 meter replacements.

• Completed 65 water related work orders.

• Completed 24 sewer related work orders.

• Responded to 100 locate requests.
