Guidelines in availing the

GRADUATION PHOTOPACKAGE

Students who will avail the Graduation Photo

Package will AUTOMATICALLY BE INCLUDEDin the Vicissitude 2012 (DLSU-D yearbook).

Students who are considered part of thegraduating batch but failed to meet the

requirements for graduation may still avail the

Graduation Photo Package, and consequently

be included in the yearbook.

The Graduation Photo Package (GPP) Fee must be paidonly at the Accounting Office where an Official Receipt

(OR) must be issued. Graduating students, however,have to secure payment slips which will be issuedduring the pictorial registration

The OR must be presented on the pictorial schedule forproper accommodation. A stub indicating that thestudent understands and agrees with the guidelines setby the Editorial Board (EB) which is issued by the

Student Publications Office at GMH115, and thePictorial Stub issued during the pictorial registrationmust also be presented.

The pictorial schedule set by Vicissitude 2012 EB

shall be strictly followed. However, if a student

fails to report on the indicated schedule,supporting document shall be presented to SPO /

Vicissitude 2012 EB; otherwise, the student shall

be subjected to a penalty of three hundred(P300.00) pesos per reschedule.

The graduating students must first fill out and

submit an online student data form before going

to his/her scheduled pictorial. The GPP will not

be released to those who will not comply.

Graduating students are advised to bring upper

garment for the formal and casual shots.

However, they may choose to borrow from the

limited apparel provided by the photo studio.

Immediately after the pictorial, graduatingstudents shall be the ones to choose shots to be

printed. Chosen shots will be written on stubs

provided by the Photo Studio and these shouldbe kept by students for future verification.

Request for group shots shall NOT beentertained.

SPO shall be the designated unit to receiveand store the GPP. The Vicissitude 2012 EB /

Photo Studio Staff / SPO Staff shall be

designated to distribute graduation photo

packages.

Storage of graduation photo package,

graduation rites photos and relevant

documents shall be kept and managed by the

SPO for a period of one (1) year only.

Unclaimed photos after the given period shall

be disposed of without prior notice.

Distribution of GRADUATION PHOTO

PACKAGES

Graduation Photo Packages shall be distributed per collegeon dates that SPO/Vicissitude 2012 EB shall announcethrough posters, tarpaulins, University website, email:[email protected], and facebook account:Vicissitude Mmxii. Graduation Rites Photos, on the other

hand, shall be released six weeks after the Graduationceremonies.

The Vicissitude 2012 EB / Photo Studio Staff / SPO Staff

shall be the designated persons to release GraduationPhoto Packages / Graduation Rites Photos at the GMH115or at other venues which will be announced.

In claiming the Graduation Photo Package / GraduationRites Photos, the owners ID must be presented to the

Vicissitude 2012 EB / Photo Studio / SPO staff. The GPPOfficial Receipt however may be requested to bepresented, only to validate payment.

Graduating students with complaints / concerns (poorquality, missing, etc.) must promptly inform theVicissitude 2012 EB / SPO Staff for appropriate action.Complaints pertaining to Photo Packages shall only be

entertained up to the third month from thedistribution of graduation rites pictures.

A representative may claim the Graduation

Photo Package / Graduation Rites Photos on

behalf of a graduate, provided that an

authorization letter, the owners school ID,and the representatives ID are presented to

SPO.

YEARBOOK GUIDELINES

The yearbook shall include those who have their

pictorials on dates scheduled by SPO /Vicissitude.

In case of siblings, parent and children, letter ofrequest for single subscription shall be submitted

to SPO before January 15, 2012.

Yearbook requirements, such as the onlinestudent data form, submitted beyond thedeadline set by the Vicissitude Editorial Board /SPO shall not be accepted. This may lead to ones

incomplete profile in the yearbook.

The Yearbook primarily serves as memento

and is not an official proof of graduation.

Yearbook fee must be settled before the final

exam of the second semester. The Vicissitude

Staff / SPO reserves the right to pull out a

graduating students profile from the

yearbook, if payment for subscription fee is

not settled before the printing of the saidmemento.

Graduating students must send their information

/ write-up online via URL that will be provided by

SPO. No verification of data will be done as theinteractive file will be the basis for all information

which will be used as reference in the even of

errors discovered once the copies of VICISSITUDE

2012 is printed.

The target release of the Vicissitude 2012 is one

(1) year from the graduation of batch 2012.

A representative may claim the Yearbook onbehalf of a graduate, provided that anauthorization letter along with the ownersschool ID, is presented to SPO. The representativemust also present a valid ID.

DLSU-D shall keep remaining yearbook 2012stocks only for a maximum of two (2) years fromthe day of its launching. Copies that are notclaimed two (2) years after the launching of theyearbook shall be disposed at the option of the

Vicissitude EB / Student Publications Office,without prior notice.
