IMPORTANT MEDICAL INFORMATION Each School year a parent consent form to treat and pro- vide medical services must be filled out. This is a State of Indiana and Perry Township Schools policy. Please go into your child's SKYWARD account and complete the form. If you have any questions, please contact your schools office. Parking in Designated Areas Parents, please park in our designated parking areas in the north lot. Please do not park along the curb. This is our fire lane and we need to keep it clear for emergency vehi- cles. Thank you for your cooperation Dont forget Wednesdays is early dismissal! Car riders may be picked up at 3:30. Buses should arrive 25 minutes earlier than normal. MEDICATION POLICY Medication can be administered at school. The following rules apply to prescription and non-prescription medications: 1. Medication must be provided by the student's parent or guardian and must be in the original container as supplied by the pharmacist or, in the case of OCT (over the counter) medications, brought in with the manufacturer's original label with the ingredients listed and the student's name affixed to the origi- nal package. 2. Written parent permission and instruction regarding dosage, time and frequency of administration. Par- ent permission shall be renewed at the beginning of each school year. 3. The parent or guardian is responsible at the end of the treatment regime for removing from the school any unused medication which was prescribed for his/her child. If the parent does not pick up the medi- cation by the end of the school year, the school nurse will dispose of and document that medication was discarded. 4. Students may not have medications in their possession while on school premises without written per- mission of a parent or guardian, written authorization of the student's physician, and, in the case of a prescription drug, a written prescription of a licensed medical practitioner authorized to prescribe the particular drug. Specific medication questions should be directed to Mrs. Keller at 789-2841 Background Checks Volunteers will need to go through the Community Resources tab on the Perry Township Website to complete a background check or use the link below. Background checks are good for one school year. The system will reset each July 1st.
