1. Salary

The salary offered is a Band 5, which currently has an hourly rate of Band 5 Level A $32.53 to Band 5 Level D $37.28. Salary and conditions of appointment will be in accordance with the Glenelg Shire Council Enterprise Agreement.

2. Superannuation

The Shire is required to make a statutory contribution to the scheme for the employee’s benefit. The employee may also make a contribution. From 1 July 2014 the statutory contribution is 9.5% of the annual salary. Under the Enterprise Agreement the Local Authorities Superannuation Fund, known as Vision Super will be the Employer Default Fund. Employees may choose to have contributions directed to any complying superannuation fund under the Superannuation Industry (Supervision) Act 1993. All employer superannuation contributions will be made by the employer to Vision Super, or to any other complying fund of the employee’s choice.

3. Eligibility to Apply

To be eligible to apply for any position with the Glenelg Shire Council applicants must be an Australian or New Zealand citizen or have Permanent Resident Status or have a valid Working Visa.

4. Normal Hours & Days of Work

The terms of engagement will be on a casual basis, where the successful applicants may be engaged in intermittent or relieving work of a casual and/or unexpected nature. There are no regular or guaranteed hours or work.

5. Medical, Police Check & Working with Children Check

Applicants may be required to have a pre-employment medical, by a medical practitioner nominated and paid for by the Council and will be required to have a satisfactory Police Check and a valid Working with Children Check card. These are ongoing requirements of the role.

6. Annual Leave & Sick Leave

Employees will be paid 25% loading in lieu of annual leave, sick leave and public holidays.

The aim of assessment is to maximise the ability of an individual to remain living independently at home in the community, by identifying Client’s goals and developing a support plan to suit those needs. 1. Position Objectives

a) To assist the frail, aged and people with a disability to remain in their own homes and to assist

carers and families assessed as ‘in need of support’, through the provision of a range of direct and practical assistance in the users home.

2. Key Responsibilities & Performance Standards

The key responsibilities and performance standards may be modified from time to time to ensure that the desired outcomes are in accordance with the Annual Plans which seek to achieve Council’s stated goals as identified in its Council Plan. Without limiting the above, the key responsibilities and the performance standards of the Officer will include:

Assessment a) Carry out a holistic assessment of an individual’s needs for Regional Assessment Service

(RAS) – known as Home Support Assessment and Department of Health & Human Services, HACC Program for younger people – known as Living at Home Assessment and non-HACC services to ensure independence is maximised. Council provides a client centred service that meets the needs of clients and also considers service provision from a Wellness and Reablement and active service model perspective of “doing with not doing for”.

b) Assessment Officers ensure goal directed planning is developed for support plan.

Key Responsibilities & Performance Standards continued Assessment continued c) Work closely with Senior Assessment Officer to achieve effective collection of statistical data. d) Undertake OHS assessment of client homes. e) Required to report to and liaise with the Regional Assessment Service (RAS) Coordinator. Council Services a) Liaise with hospitals, district nurses and other service providers to ensure effective

partnerships. Provide support to the families of clients. Information, Referral & Advice a) Provide general and specific information on services available, both those provided by council

and others available in the community, as required and provide advice and assistance to clients, potential clients and their families and refer to other agencies if required.

General a) Assessment and care management has two sub components:

- Holistic assessment of Client’s needs with Home Support Assessment and Living at Home Assessment

- Support plan representing Client’s goals. b) Assist in meeting corporate objectives and section requirements by carrying out a variety of

tasks as directed from time to time which relate generally to the position responsibilities but which may not be specifically designated within the scope of this position description.

c) To communicate results of assessments and reassessments, and any other relevant comments to the Senior Assessment Officer

d) To communicate to other relevant agencies within 48 hours, the results of any assessments/reassessments which may indicate a need for services other than Council services.

e) To attend Hospital Discharge meetings and Extended Care Assessment Service (ECAS) meetings for the purpose of exchanging information relating to Assessment.

f) To maintain up to date relevant customer records. g) Attend quarterly Regional Assessment Meetings with other Assessment Agencies. Competent

computer skills to be able to use relevant aged care software. The Manager may direct the Officer to carry out such duties as are within the limits of the employee’s skill, competence and training, provided such duties do not promote a narrowing of the employee’s skill base. The position will require the achievement of performance standards and indicators determined on an annual basis and subject to ongoing review. 3. Confidentiality

c) Accountable for appropriate assessment of client needs. d) Accountable for ensuring current knowledge of services available. e) Accountable for ensuring documentation on clients is monitored and updated. 6. Judgement & Decision Making

a) The Manager or their nominated Officer must first approve changes to Council Policy or

specific targets set for the position. b) Application of appropriate procedures to tasks undertaken. c) Providing support to the Manager in implementation of Section/Corporate objectives. d) Guidance and advice is usually available within time to make a choice. 7. Specialist Skills & Knowledge a) Demonstrated experience and ability in sensitively dealing with frail aged and people with a

disability and their carers. b) Knowledge of the relevant National Screening & Assessment Form (NSAF) , Living at Home

Assessment Tool and service coordination tool and the Wellness and Re-ablement approach and the active service model.

c) Sound knowledge of services available to residents within health and welfare system. d) Sound knowledge of needs of aged and people with a disability. e) Experience in care management. f) Knowledge of client assessment techniques and goal setting including clients with more

complex needs. g) Proficient in software used within Council, Microsoft Office software including Word, Excel

(including graphs), Outlook, Power Point etc. Training will be provided on council’s record management system.

b) Knowledge and proficiency in the use of the aged care software program ‘Xpedite’ is desirable. c) Substantial experience in assessment is essential. d) Previous experience aimed at meeting the needs of aged people and people with disabilities. e) Capacity for independent travel is essential. f) Demonstrated knowledge of information technology and keyboarding skills is essential. g) A current driver’s licence is essential. 11.
