1. Lodging: This Get Up & Go! Certificate entitles the recipient to two nights of accommo-dations in a standard hotel room at one of the locations described. Accommodations are for two adults 25 years of age or older and up to two children under age 18. Sorry, no pets.

2. Availability: Accommodations are assigned subject to availability and certain restric-tions listed. Availability may be limited one week before and after major US holidays (i.e. Easter, Christmas, 4th of July) and local special events (i.e. Daytona 500, Mardi Gras) and at beach front destinations during the summer months (i.e. Daytona, Myrtle Beach).

3. Activation (STEP I): You must activate within 45 days of receiving this Certificate, at which time you will select your destination. There is no cost to activate. An Activation Acknowledgment Letter will be mailed to you within 72 hours that will provide you with your unique toll-free 800 number to central reservations for you to call when you are ready to make your reservation.

4. Travel Window: Your activated Certificate will be valid for 12 months from the Acti-vation Date or no later than December 31, 2015, whichever comes sooner. Travel must be completed before the expiration date.

5. Reservation (Step 2): Reservations must be made 30 days prior to travel. At the time the reservation is confirmed, a $50 refundable credit card deposit is required at the time of reservation confirmation and will be refunded 30 days after check-in automatically to the same credit card used to make the deposit. A Reservation Confirmation Letter, including check-in information and directions, will be mailed to you within 72 hours. Your reservation confirmation letter, a major credit card and a valid driver’s license will be required upon check-in.

6. Incidental Charges: This Certificate is for hotel accommodations only and does not include transportation, parking fees, airfare, room taxes ($4-$19 per night), meals and any miscellaneous expenses. Upon check-in, hotels will require an imprint of a major credit card to cover such expenses.

7. Cancellations: Cancellation or changes to an active reservation must be received at least 72 hours prior to your arrival date. Failure to cancel or reschedule at least 72 hours prior, or simply not showing up, will result in forfeiture of your Certificate and reservation processing fee or deposit.

8. Usage Limitations: Only one Certificate per household per year may be used. This Certificate is intended for promotional use only by the issuer and cannot be sold to the recipient. This Certificate is nontransferable and may only be used by the original recipient. This Certificate may not be used for conventions or group travel.

9. Liability Limitations: Get Up & Go! Travel, Inc., or it’s assigned agent, will arrange the hotel accommodations acting only as an agent and will not assume any responsibility for loss, delay, accident, injury or damage to persons, property or any liability whatsoever arising from or in conjunction with services provided.

10. Retail Value: The approximate retail value of the accommodations is $98 to $704 depend-ing on location, hotel availability and travel dates.

11. State Disclosures: This offer is not valid where restricted or prohibited by law. You may cancel this contract [this Certificate] without any penalty or obligation within 30 days from the date of purchase or receipt of this Certificate, whichever occurs later. You may also cancel this contract if accommodations or facilities are not available pursuant to a request for use as provided in the contract. If you decide to cancel, you must notify in writing of your intent to cancel by returning the Certificate and sending notice to: Get Up & Go!
