Tap into our network. We understand that choice, flexibility and affordability are top priorities. That’s why we have several networks available with our Oxford products. The Freedom Network is available in New York, New Jersey and Connecticut, giving you access to more than 103,000 providers in the tristate area. In New York and New Jersey, we also have the Liberty Network, a subset of the Freedom Network, which offers access to more than 76,000 providers1. We also have our smaller Oxford Metro NetworkSM, for groups in New York, and Oxford Garden State Network, for groups in New Jersey.

Plus, the UnitedHealthcare Choice Plus Network offers many2 members access to over 930,000 physicians and other health care professionals at 5,663 hospitals3 across the United States if you need medical attention when outside the tristate area.4

Find a network provider in just five steps.Step 1: Log into myuhc.com. You can create an account by clicking on “Register Now.”

Step 2: Once logged in, go to the “Find a doctor” search.

Step 3: Your ZIP code will automatically be used – you can change this by clicking “Change location.” Then you have two choices:

• Enter the provider name, facility or medical group in the search box.

OR

• Click on the icon that reflects the type of provider you are looking for and follow the prompts.

Step 4: Click “Search.”

Step 5: View your results.

If you have access* to the Choice Plus network outside of the New York, New Jersey and Connecticut tristate area:

1. On the myuhc.com home page, under Links and Tools, click “Find a Physician, Laboratory or Facility.”

2. Choose “All UnitedHealthcare Plans,” then “Choice Plus.”

Keep in mind that some UnitedHealthcare Choice Plus doctors (e.g., certain Mayo Clinic Arizona doctors) are not participating in all Oxford plan networks, but may appear in results from a Choice Plus search.

See the enclosed fliers for more information about searching for a participating Oxford network provider. Also, you can call the toll-free member phone number on your health plan ID card for help finding a provider in your plan.

Please note: The provider search experience is not compatible with Windows Internet Explorer 8 (IE8). If your system is using the IE8 web browser, you’ll automatically be directed to an earlier version of the UnitedHealthcare provider search tool.

Preventive health. To help you stay healthier.Regular checkups and tests may help you avoid, or find, serious health problems. Then you and your doctor can work as a team for your overall health. Visit uhc.com/preventivecare to get useful suggestions based on your age and gender that are designed to help you maintain and improve your health.

Preventive care is covered 100 percent5 and includes immunizations, preventive exams and health screenings.

Keep in mind that procedures for an already chronic condition are considered diagnostic. They are not preventive and are not covered at 100 percent. You can view the complete list of preventive care guidelines at uhc.com/preventivecare.

Easier access to plan information.Visit your member website, myuhc.com, where you can:

• Take a quick view of your plan benefits, including your copayments, coinsurances, deductibles and out-of-pocket maximum.6

• Print a temporary health plan ID card.

• Order up to a 90-day supply of medications.

• Search for network primary care physicians and other health care providers and specialists.

• Search for a network hospital or other health care facility, including pharmacies.7

• Chat live with a registered nurse 24 hours a day, seven days a week.

• View a claim and print an Explanation of Benefits (EOB).

• View your Certificate of Coverage (COC).

• Take advantage of health and wellness resources, discount offers and more.

Access your pharmacy information If you log in to myuhc.com and click on “Manage My Prescriptions,” you can access optumrx.com. There, you can:

• View your Prescription Drug List (PDL) and check your costs for commonly prescribed medications.

• Find our prescription mail-in order form for a convenient and cost-effective way to order up to a 90-day supply of medications.

• View a list of drugs that need precertification.

• Compare drug alternatives with our PharmAdvisor™ tool.

• Learn about the Step Therapy8 program to help you get less expensive, but equally effective, medications.

• Access a list of preventive medications available at no additional cost.* Please ask your benefits administrator or check your health plan documents to verify if you have access to the Choice Plus network.
