BBS training has been imparted to GAIL Employee – 08.07.2015. Outside emergency call attended. (02 Nos.)

Agartala:

Awareness programme on pipeline safety & encroachment removal was held on 16.07.2015 among the villagers in association with District Administration at Khayerpur.

Fire & safety training imparted to 11 Gail employees & 8 contract technicians.

Bengaluru:

Safety Awareness Work Shop Conducted for the Deputy Commissioner, Dharward- Hubli Dist and other Dist. officials like S.P, ADC, VDA & PDO of Dharward district at collector’s office on 06.07.2015.

Mutual Aid agreement signed with M/S BHEL, Bengaluru and also Safety Awareness Training Conducted to the Consumer M/S BHEL at Bangalore on 14.07.2015.

BBS Regional Steering Committee for the Southern Region Conducted on 17.07.2015. BBS follow-up meeting and observer training imparted from 24.07.2015 to

25.07.2015. IMS audit for second stage certification held on from 27.07.2015 to 31.07.2015. New incident reporting system (Revision - 2) training given to GAIL employee of

Dump test of CO2 flooding system carried out at two HVJ RR Stations (RR 13 &14). Kandla:

Safety training & awareness program carried out. Kochi:

Loni:

A meeting with mutual aid members of Piyala and Faridabad region was organised by JLPL-NCR region at RT- Piyala.

OISD audit conducted at JLPL-NCR (Loni region). Training session to develop BBS observers conducted.

Mansarampura:

Replacement of defective pipe through Hot tapping & Stoppling at pinhole leakage location i.e. chainage 1018.31 Km of JLPL.

Installation of ONGRID Solar System completed at SV- 46. Mumbai:

Behaviour Based Safety first follow up workshop has been conducted at Mumbai on 16th – 17th July, 2015.

Behaviour based training is imparted to various observers in DUPL, DPPL and trombay region in the month of July, 2015.

NCR:

F&S trainings /awareness program including BBS Training & Life Saver was conducted at NTPC Terminal Faridabad & IP Chhainsa for GAIL Employees, Contract workers & security guards.

Nasirabad:

OISD phase II audit was conducted on 28.07.2015. Training on “BBS” and “Life saver topic” was given to Employees on 10.07.2015.

Pata:

Behaviour Based Safety Lead Trainer workshop was organised wef 28-31st July, 2015. Total 22 GAIL Employees from Pata & Other installation were trained as a BBS Lead Trainer.

4th BBS follow up visit carried out by M/s. OCC from 28th to 1st July, 2015 On site Emergency Action Plan (Revision 11) updated with revised role and

responsibilities and circulated to all Pata employees during the month Electrical Safety Committee meeting was conducted on 09.07.2015 TS-103 hydro testing and statutory inspection done

Puducherry:

BBS Training session taken for Cauvery Basin Employees. BBS audit carried out by BBS implementor.

Rajahmundry:

Training program on Basic Fire Fighting has been conducted on 29-07-2015 and 33 Nos. Contract employees attended the program.

LSS Training program on the 3'd quarter topic "Know SOP - Get to work" has been conducted on 31-07-2015 at NFCL Kakinada along with BBS Training Program.

Community Awareness program on Natural Gas Pipelines safety and Dos & Don'ts in case of leakage are being conducting by hiring local focus group and arranging in PL ROU Villages. In 2nd phase 45 villages covered so far.

T4S Audit for new pipelines i.e. KGVPL 20" x 41.1 KM from K-Cheruvu to GMR Vemagiri and 24" x 20 KM PL from Oduru to KJ Point has been carried out by M/s BVQI as per PNGRB guidelines.

Samakhiali:

Training to GAIL employees & Contract Workers on “Behaviour Based Safety”. “Pipeline Safety Awareness” conducted at Lakadiya village on 17.07.2015. BBS workshop and observers training programme conducted on 09.07.2015 and

10.07.2015.

A training on “BBS Awareness program” for contract employees was carried out on 10.07.2015.

Training on Incident reporting system for GAIL employees was carried out on 10.07.2015.

Vaghodia LPG & CS:

BBS Implementation activities carried out as follows:- Meeting of all committees carried out as per BBS Schedule, Steering committee reviewed the progress of implementation of BBS at Vaghodia on 21.07.2015, Awards for BBS Person for the month of June - 2015 in different categories were conferred.

HAZOP study of Vaghodia complex (GPU/Compressor station) commenced on 22.07.2015 through third party

The team of M/s. Earnest & Young under the supervision of CO-RM started Risk assessment of the plant on 27.07.2015 onwards.

Training imparted to Students of Sub Officer’s course from National Fire Academy, Vadodara.

Vijaipur LPG & CS:

For successful stream lining of operations of C2/C2 petrochemical complex-II with adequate supervision & safety checks without any incident, F&S crew deputed at site round the clock for close supervision of activities in safe manner.

Annual Shut down of LPH PH-1 & 2 was carried out safely & under proper supervision.

Pre-start up safety audit of LPH Ph-1 & 2 and its compliance before start-up. Vizag:

Onsite Mock Drill was conducted on 8th July 2015 in presence of Joint Chief Inspector of Factories on 8th July 2015

Vadodara:

Reviewed ERDMP of all six base stations under RPNHQ, Vadodara region & sent to ED (O&M) Central Region for approval.

On 20.07.2015 reviewed fire and safety system of consignment stockist under Ahmedabad Zonal office.

Reviewed ERDMP of all the six base stations under RPNHQ.

Near-miss Incidents Reported by Work Centres: Near miss incidents have been reported from all sites. Notable near-miss incidents are listed below: Abu Road: On 22.07.2015 at 1030 hrs, Sh. M K Mughdhum, Mgr. (Ops) was performing his morning shift duty at MCR (Main Control Room) while he was moving toward the instrumentation panel, his leg was struck in the false flooring cover, which was not properly placed, and he somehow managed to escape from fall injury. Agartala: On 06.07.2015 at 11:30 hrs, a team of GAIL engineers and technicians were carrying out PCV maintenance at Khayerpur terminal. While venting of gas was carried out the gas exhaust with pressure was almost hitting the face of the technician as the vent port was not sufficiently high. Vent port needs to beat higher level as this could cause fatal accident. Fortunately, the technicians were saved in this case; however, the incident is a wakeup call for causing serious damages while carrying out gas venting activities. Bengaluru: On 14.07.2015 at 11:00 hrs, while entering in to SV -22 for routine checking of equipment, Mr. Prasana of Contract Technician lost his control while walking; suddenly Mr. A Manjunatha supported him to escape the accident. Finally found that wet mud is lying on the Road. Chhainsa: On 29.07.2015 at 1745 hrs, Sr. Officer (C&P) Sh. Devraj was leaving from his desk after completing the office job for the day, when the whole unit of vanishing blind installed just adjacent to his workstation came out of its fixtures and collapsed on the floor. Luckily the officer was not available at his desk; otherwise an injury might have occurred to him. Cherlapalli: On 02.07.2015 at 1700hrs, ROV maintenance was being carried out at GAIL Terminal in BPCL Bottling Plant and Motor was being removed using hydra, when sudden jerk was received and the hand of technician holding the motor valve struck the cover assembly. However no injuries were inflicted and the hydra operator was advised to always ensure smooth, gradual movement of loads without jerks. Dibiyapur: On 18.07.2015, Sh. Pramod Kumar (Dy Mgr, Opr.) reported that he may fall in open Pit near Gangotri guest house while going from guest house to park but he escaped the injury and immediately informed to civil dept. for covering the open pit. G Konduru: On 16.07.2015 at 11:00 hrs, replacement of MOV 1901 valve was carried out by mechanical team at process area. Suddenly spanner slipped from his hand. Other technicians were working at same site but spanner fell at a safe distance. There was chance for incident at site but narrowly missed from injury at site.

Gandhar: On 06.07.2015 at around 0331 hrs, Near Hydrocarbon Pump in unit area, one welding machine which was connected with another machine through cable. Suddenly spark generated in cable joint and it starts burning. Standby fireman immediately use DCP extinguisher and fire was extinguished. Instructions given to electrical dept. to ensure that all cable joint are properly joint with insulation tape and checked before using it. On 10.07.2015 at around 1920 hrs, Operation dept. informed that LPG leakage occurred in LPG line near Naphtha column. F&S team takes immediate action. Meanwhile it was observed that Mech. Dept. team tries to attend the LPG leakage using normal sparking tools. Immediately work stopped & Mech. Dept. was advised to use only non- sparking tools in gas probe area. Later on non- sparking tools made available at site & job safely completed. Due to quick & attentive action of F&S personnel, a chance of major incident averted at site. Hazira: On 25.07.2015 at 1100hrs, Plant Employee Mr. H S Gautam SI (F&S) was doing a work of dismantling of sprinkler system line provided on Train –A strainer. For calling firemen from fire station he entered in telephone booth after opening the telephone booth he suddenly found 4 nails protruding from base plate on which telephone was kept. He merely escaped from getting injury to his forearm from those protruding nails. Incident was immediately reported to F&S shift in charge & Telecom dept. F&S & Telecom Dept. investigated the incident. The protruding nails was immediately removed by telecom dept. to prevent reoccurrence of such incidents in future. Jhabua: On 30.07.2015, Painting job was under progress in DVPL I filter area. During the job Sh Ram Bahadur, painter, who was working at the top of the filter line, slipped & fell but as he was wearing the safety belt, so due its support he controlled himself and with the help of fellow employee he got down safely. Had he not worn the safety belt there was chance of serious injury. Jamnagar: On 30.07.2015, it was reported that, Mr. Bhagat Gareja (Fire-tech) of M/s. NICE International while taking field round near pump- 101B, his right foot got stumbled in a gap created due to improperly placed drain cover plates on drain channel and he lost his balance. He somehow managed himself from falling and escaped unhurt. Kandla: On 09.07.2015 at 1815 hrs, while Mr. Ashok Nana Patel was proceeding to home after completion of his duty, he tried to sit in the vehicle & he could not take his seat properly suddenly driver of the Vehicle started vehicle & moved his vehicle towards Gandhidam without insuring that Mr. Patel not seated in the vehicle properly. At the same time someone asked the driver to stop vehicle immediately, then the driver stopped vehicle & after insuring that Mr Patel taken his seat properly he moved the Vehicle to the right direction. Kailaras: On 01.07.2015 Agra CGS, It was reported by Security Supervisor at Agra CGS, that while unloading Helium cylinders from delivery vehicle, Mr. Tulsi Ram, one of the contract worker, got minor injury on his ring finger of left hand while handling filled cylinders. He was given immediate first aid.

Khera: On 07.07.2015, one of the roof sheets provided on GTC-C shed found loose & could have fallen down resulting in injury/damage. On 17.07.2015, handles not provided for trench cover plates in GEG area which could have resulted in injury while lifting. Loni: On 08.07.2015 at 1130hrs, Mr. Kumar, Contract Technician, was going for attending the maintenance job in the process area. While approaching towards the job, his feet slipped and he was just to fall on the ground but fortunately he managed his body balance and narrowly escaped being injured. After investigation, it was observed that some grease and lubricant were spread over the ground since previous day as previous day some valve maintenance job was undertaken and maintenance crew did not clean the working area after finishing their job. On 12.07.2015 at 1410 hrs, Mr G R Chouhan, DGM (O&M)-OIC was on scheduled safety visit at RT-Gurgaon. While approaching towards process area through internal approach road, the foot of Mr. Chouhan got slipped due to algae formation in the middle of approach road. Mr. Chouhan was luckily escaped being fall on the road as one of the Technician and Security Guard hold him. He was narrowly escaped being injured. Mumbai: On 27.076.2015, at about 1530 hrs a civil work was in progress in the pathway of the main entrance at the New GAIL Bldg., Belapur. 02 nos. of drains on the pathway was kept open and were unattended. Another contract personnel unknown of the open drain in the pathway were just about to pass through the same. But due to immediate attention of SM (F&S) who was passing nearby, the contract personnel were stopped immediately else they might have fell into the drain leading to injury/ mishap. On investigation it was found that the contract personnel carrying out the work was assigned another job hence the work site was unattended. The contract personnel was called and directed to be at the work site for and till the completion of work and cover the drain at the earliest and also all the work to be carried under proper and continuous supervision. On 28/7/2015, at about 1830 hrs, CM (RGMC) was sitting on a chair. Suddenly the back rest of the chair got misplaces. However due to the partition wall of cubical available back side; he was able to get his balance and avoided from falling down. The adjustable rod of the chair was then checked. It was found that adjusting rod of chair has got some problem and that chair has been replaced. The office boy has been asked to check all the chairs to prevent recurrence of such events. On 29.07.2015 at 1230 hrs, a carbondioxide extinguisher kept in the office of the GAIL, MTNL Bldg. fell down suddenly releasing the C02 gas from the discharge horn. Immediately the C02 extinguisher was closed & placed. The same if not closed immediately might result to panic and also might have resulted to an injury/ mishap to any of the employee. On investigation it was found that an employee mistakenly dash with the extinguisher placed. Mansarampura: On 18.07.2015, a tractor loaded with trolley was passing through diverted portion of the road at a location in Kaushalpur village near IP-04 at 1018.31 Km chainage where the job of defective pipe replacement through hot tapping was under progress. sudden when the tractor reached near the end of diverted portion of road, due to uneven surface the tractor got tilted, wheels got jammed in the soil & was about to meet an accident. But luckily we were having Hydra available with us at site. The tractor was safely pulled out with the help of Hydra, no damage & injury reported roper insertion of locking pin and placed properly. NCR: On 20.07.2015, during the shifting of P/L job of DESU Bawana P/L, venting was being done at DESU terminal. While going for venting operation through pathway for opening the vent valve, the person on duty could not see the cable marker grouted in between the pathway due to darkness in the area. He was about to trip due the marker and escape without hurt as he was wearing the safety shoes. Nasirabad: On 31.07.2015, a foam Trolley was received from the vendor packed in a wooden box. Mr. Anubhav Jain (Contract staff, C&P) along with F&S team was trying to open the wooden box with tools. During the course of action his hand hit the box corner where iron nails were fixed. Fortunately no injury was reported. Pata: On 09.07.2015, Sh. Abhay Kumar Gupta (CPF No. 2525) reported that while moving into corridor of SB#1 his right foot hit a table kept in pathway. Since he was wearing safety shoes no injury occurred. On 19.07.2015, Sh.Amar Bajaj (CPF no.8611) reported that while coming towards GC 201 his leg get trapped in protruded earth strip and he narrowly escaped tumbling. On 19.07.2015, Sh.Saurav Anand (CPF no.16208) reported that during flange lifting at Adsorber area (100) by EOT crane, the wire rope sling broken. No one was injured as no one was standing near or below the work area. On 29.07.2015, there is open drain in-front of newly installed Kirloskar compressor panel. Mr. V K Tyagi, DM (Instrumentation) escaped from falling into drain, while working in the area. On 31.07.2015, Sh. Sunil Kumar (CPF no. 16284) reported that while taking safety field round near EP-3 strainer he accidently kept his right hand on an exposed hot steam tracing line running through a cable tray near EP-3 strainer and narrowly escaped with minor burn in right hand palm. On 22.07.2015, Mr. Avishek Anand (CPF no.15209) reported that while doing preventive maintenance of motor (47-PAM -113 D & D) near CT-3, he tripped on a protruding GI pipe and fell down but luckily escaped without any injury.

Puducherry: On 04.07.2015, Operator was sitting in Kamalapuram Terminal and suddenly ceiling fan fell down near him. Fortunately he escaped unhurt as he just away from the perpendicular line of the fan. Reported by Shri Prajeesh SO (F&S) On 28.07.2015, during night checking of the duty posts, while entering Kamalapuram post, got tripped by the ONGC pipeline going across our gate. I recovered without getting hurt. Rajahmundry: On 15.07.2015, while shifting gas cylinders on Xylo Vehicle from GAIL Bhavan, the vehicle crossed on the drain covered with cemented blocks. But due to aging and weight of the vehicle, the cemented block on drain developed cracks and supposed to be breakdown. On seeing the cracks with sound, the driver immediately moved the vehicle out of the drain; otherwise the vehicle might have fall down in the drain and may cause damage to vehicle as well as injury to the driver. The same is avoided with the alertness of the driver. Samakhiali: On 12.07.2015 at 16:10 hrs, Mr. Haneef along with two contract workers were doing housekeeping job inside the material store. One worker was engaged in shifting of damaged glasses. The glass having sharp edge, slipped from hand during shifting. The worker has wearing hand gloves, safety shoe and narrowly escaped from an injury. Usar: On 15.07.2015 at 1400 hrs, For entering HR dept. MS. Niketa, Sr. PA was pulling glass door, controlled by Access control system after punching ID card. Suddenly the door got cracked in its upper half portion. She instantly removed her hand from the door and moved to a safe distance. Vaghodia CS: On 04.07.2015 at 1100 hrs, Mr. JN Dave was performing standby duty at HRSG-B for structure modification job. He was taking safety round near GTC-A where Air intake filter replacement job was going on. One contract person removing filter from height and passing it to the next person, who was standing on ground floor. After removal of filter, first person was throwing the filter towards second person who was catching the filter. The activity was going on likewise. Second person who was catching the filter taking break for drinking water. First person was not aware that the second person was not ready. He threw the filter but second person not in position to catch the filter and filter fell down on the ground near the person. If he would not have out of line movement of the filter, it might have struck to his head or body part, causing direct injury or indirect injury to him Vaghodia GPU: On 25.07.2015 at 0915 hrs, In the morning, while general shift bus are entering into the plant premises through Gate No. 1, there is general practice that CISF personnel has to check the vehicle physically. After checking and clearance of entry, the barrier is opened for entry of the vehicle into plant. On that day, the CISF personnel raised the barrier, but due to some logic failure/ repeated command, the barrier started lowering automatically while the vehicle was coming inside the plant. Bus driver observed it, immediately applied brake and stopped the vehicle just before reaching up to the barrier, otherwise barrier may hit the bus or vice versa and barrier and /or bus might got damaged.

Vijaipur CS: On 17.07.2015, Mr. R Rathwa, Fireman was taking safety inspection round at VDPL compressor area, noticed that grinding work was being carried out at fabrication yard of M/s. B&R and 02 persons were standing in the vicinity of the same. The spark from grinding machine could have harmed or injured them. The work was immediately stopped & area was barricaded and subsequently job was started. Vijaipur GPU: On 23.07.2015, a near miss incident was reported by Mr. Ram Charan Ojha, Engineer (Elect) wherein it was informed during nut bolt tightening activity, pneumatic line of pneumatic machine got detached from joint. The same was not coupled properly. Immediately the line was restored & coupled properly. Vizag: On 16.07.2015 at 14:35 hrs, Personnel were opening nut bolts of a flange joint of the recirculation line of Booster Pump 103 A. Suddenly the spanner slipped and fell on ground. All the personnel were wearing Safety Shoes, Safety Helmet, Eye Protection and Hand protection, the slipped and fallen spanner does not cause injury to any person. Vadodara: On 21.07.2015 at 20:00 hrs, Incident – At Ankhi IP Stn, pipeline modification job was being carried out. Generator of Aska Inflatable Light was started for lighting purpose & the generator was running smoothly with boom inflated. Suddenly the Inflatable Tower tilted slowly due to opening of Zip of Inflatable Tower. .
